  • Background: TNF-$\alpha$ plays a major role in producing left ventricular dysfunction cardio-myopathy pulmonary edema and inhibits the compensatory mechanism of congestive heart failure. IL-6 is an acute reactant of immune reaction and also known to control immune reaction but its function in the myocyte was not clearly investigated. Author's performed this experiment to investigate the contents of TNF-$\alpha$ and IL-6 on the assumption that TNF-$\alpha$ and IL-6 may reside in nonfailing heart that has gone cardiac surgery and play some role in cardiac function. Material and Method : Right auricular tissues were sampled from 12 patients who had undergone total corrective surgery for both congenital and acquired heart diseases from January 1998 to June 1998 in Kosin Universcfy Gospel hospital. The quantitive analysis of TNF-$\alpha$ and IL-6 were assessed by ELISA method in right auricular tissue. Hemodynamic values about the pressure of ventricle atrium aorta pulmonary artery and cardiac index pulmonary and systemic vascular resistance and cardiac output were measured by echocardiography and cardiac catheterization and biochemical analyses of LDH & AST were done before operation. statistical analysis was by Paired Student t-test. Patients were divided into children(under 15 years olds) and adults groups and the data was compared beween two groups. Conclusion: Mild pulmonary hypertension and increased pulmonary vascular resistance were existed in both group. The contents of tissue TNF-$\alpha$ IL-6 in each group were independent of each data.

The Effect of Ultrasound on Epidermal Growth Factor Expression in Rat Wound Tissue (초음파가 창상조직내 표피성장인자의 발현에 미치는 영향)

  • This study was performed to investigate the effect of ultrasound irradiation on epidermal growth factor(EGF) expression in rat wound tissue. Skin wounds were created below 5mm to both sides of scapular inferior angle. The right wound was used experimental side and left was used control side. Ultrasound was irradiated pulse rate $20\%$, frequency 1MHz, intensity $0.5W/cm^2$ for 5 minutes during 3days. After sonication during 3 days, rats were sacrified. The expression of epidermal growth factor evaluated immunohistochemistry on mouse anti-EGF. In the control side, a little expression of EGF was observed at epidermis and dermis. In the experimental side, A strong immunostaining was seen at epidermis and dermis. This study suggests that ultrasound irradiation is effective on EGF expression in wound tissue.

  • Hemophilia A is a hemorrhagic disease caused by coagulation factor VIII deficiency. In head and neck cancer surgery, especially during a reconstructive one, complications can occur. These include hematomas due to bleeding which can then lead to flap ischemia, necrosis, and impaired wound healing. There are fewer cases of reconstructive surgery in patients with hemophilia A. Here in we report, a reconstructive surgery that involved mass resection, partial glossectomy (right), selective neck dissection (right, Levels I, II, III, IV), and reconstruction at the lateral arm free flap (left) in a 25-year-old man with hemophilia A. The surgery was successfully performed without any complications after pretreatment with Factor VIII concentrate, which has not been reported earlier.

  • In this study, the correlation analysis between total system error and environmental factor variables was performed to confirm the effect on the performance of the integrated navigation system by various environmental factors. To collect flight data of hybrid vertical take-off and landing UAVs, scenarios including various turning sections and straight sections such as left turn, right turn, turning rate, and path change angle were selected, and environmental data of wind direction, wind speed, temperature, air pressure, and humidity were collected in real time through weather station. As a result of the correlation analysis between the collected flight data and environmental data, it was concluded that the performance of the integrated navigation system by environmental factors within the collected data was not significant affected and was robust.

  • This study establishes the initial data to develop a well-fitted underwear pattern by categorizing and analyzing torso types based on body measurements of women in their 50s and 60s. The results are as follows: First, the statistical assessment on the body measurements showed meaningful differences among age groups in twenty seven items (except for bust breadth, hip width armscye depth, hip depth, neck base circumference, armscye circumference, chest circumference, hip circumference, bishoulder length, shoulder length, front interscye, back interscye, weight and inclined angle of left shoulder). Women in their early 50s and late 60s (respectively) showed the highest values in height and depth. Second, there are five body factors according to the results of the factor analysis: Factor 1 (circumference, width, and depth of upper body measurements) - the degree of body depth and obesity, Factor 2 (height and vertical length) - The vertical torso length, Factor 3 - the size of shoulder, Factor 4 - the vertical upper body length, and Factor 5 - the size of shoulder angle. Third, the results of the cluster analysis showed that there are four distinctive body types. The largest number of the study subjects was related to Type 3 (30.69%), followed by Type 2 (26.78%), Type 1 (25.84%), and Type 4 (16.69%), respectively. For distribution of age groups by body type, Type 3 was the most common among the 60s group while Type 2 appeared most frequently among the 50s.

  • The purpose of this study was to analyze the difference in reaction patterns during the support phase after perturbation in gait according to different age. A total of 12 subjects participated; 5 elderly and 7 adults(control), to investigate the differences between normal and perturbed gait. The step length didn't change during normal and perturbed gait but was longer in the control group. There was no difference in the step width. When the right foot was perturbed, the control group's left foot speed was faster than the elderly group's which was to maintain stability. The elderly flexed both right and left knees more than the control group. After the perturbation, the elderly group had a larger trunk anterior flexion. With the larger flexion of both legs of the elderly group it shows that the lack of knee flexion strength is a factor, that could cause falling and so a prevention program should focus on strengthening the quadriceps. With the excessive trunk flexion after the perturbation by the elderly group observed, it is suggested that while walking everyday a good routine of walking with an upright posture should be developed.

A Vestibular Schwannoma Associated with Massive Intratumoral Hemorrhage (종양내 출혈을 동반한 청신경 초종)

  • The report of massive intratumoral hemorrhage from vestibular schwannoma is rare. A 66-year-old female who had suffered from disturbance of hearing for one year developed severe headache and dizziness. Brain MRI showed crescent shaped mass in the left cerebellopontine angle. A left suboccipital approach revealed an $3{\times}3cm$-sized encapsulated mass. The tumor was totally extirpated together with clot. Histologically the tumor was schwannoma with massive hemorrhage. Postoperative course was uneventful. The authors report the rare case of vestibular schwannoma presenting with intratumoral hemorrhage with review of possible pathophysiology and associated factor.

Destructive Radiologic Development of Intravascular Papillary Endothelial Hyperplasia on Skull Bone

  • Intravascular papillary endothelial hyperplasia (IPEH) is a rare vascular benign lesion that rarely involves the central nervous system with or without skull invasion. We report a rare case of IPEH on the skull bone, which displayed destructive radiologic development associated with hemorrhage. A 14-year-old male presented with an incidentally detected a small enhancing, left frontal osteolytic lesion. Previously, he underwent operation and received adjuvant chemoradiation therapy for cerebellar medulloblastoma. Follow-up magnetic resonance imaging revealed a left frontal bone lesion, which expanded to an approximately 2 cm-sized well-circumscribed osteolytic lesion associated with hemorrhage for 20 months. Frontal craniectomy and cranioplasty were performed. Destructive change was detected on the inner table and diploic space of the skull. The mass had a cystic feature with hemorrhagic content without dural attachment. Pathologic examination showed the capsule consisted of parallel collagen lamellae representing a vascular wall, vascular lumen, which was pathognomonic for IPEH. Immunohistochemical staining revealed that the capsule was positive for CD34 and factor VIII, which favor the final diagnosis of IPEH. This was the first case of intracalvarial IPEH.

  • In this paper, a new split ring resonator using left-handed meta-material adapted in a bandpass filter with 2-stages is suggested. In this proposed bandpass filter, the size of the novel resonator can be easily controlled. Also, the bandwidth can be adjusted with the position of the tapped-line. The proposed resonator and filter has the center frequency of 10 GHz for I-band in military-satellite communication system with the Qe value of 184. The experimental results of the filter show that the insertion and return losses are 1.43 dB and 16.8 dB with bandwidth of 10 %, respectively.

Successful Left-Heart Decompression during Extracorporeal Membrane Oxygenation in an Adult Patient by Percutaneous Transaortic Catheter Venting

  • Venoarterial extracorporeal membrane oxygenation (VA ECMO) is widely used in patients with cardiogenic shock. Insufficient decompression of the left ventricle (LV) is considered a major factor preventing adequate LV recovery. A 40-year-old male was diagnosed with acute myocardial infarction, and revascularization was performed using percutaneous stenting. However, cardiogenic shock occurred, and VA ECMO was initiated. Severe LV failure developed, and percutaneous transaortic catheter venting (TACV) was incorporated into the venous circuit of VA ECMO under transthoracic echocardiography guidance. The patient was successfully weaned from VA ECMO. Percutaneous TACV is an effective, relatively noninvasive, and rapid method of LV decompression in patients undergoing VA ECMO.
