후두보정기법이 노인성 음성장애 환자의 음성개선에 미치는 효과 (Effects of SKLCT® for Voice Improvement in Patients with Presbyphonia)

  • This study evaluated the effect of the therapeutic methods between vocal function exercise(VFE), which has been used for the patients with presbyphonia in the precedent studies and laryngeal calibration technique($SKLCT^{(R)}$), which is designed by the author. We identified 58 patients who was been diagnosed as presbylaryngis by laryngoscopic examinations. 21 patients were underwent voice therapy using $SKLCT^{(R)}$, 20 patients were taken VFE, and the control group of 17 patients were not taken any voice therapy. All subjects received the therapy, ranging from seven to nine sessions, and were evaluated the voice change on pretherapy and posttherapy. The grade of hoarseness, roughness, and breathiness voice were reduced on perceptual judgments after $SKLCT^{(R)}$, but only grade of hoarseness was reduced after the VFE. Jitter, Shimmer, NHR were reduced and MPT were increased after the $SKLCT^{(R)}$(p<.05), while Jitter and SFF were reduced after the VFE. Frequency and intensity range were increased significantly on the posttest performance after taking voice therapy by the $SKLCT^{(R)}$, on the other hand only intensity range was increased after VFE. Especially, we can find the significant change that glottic gap and supraglottic compressions was reduced in most of patients after the $SKLCT^{(R)}$, but there's no changes in the group of VFE and control group. In the study, we can suggest that the $SKLCT^{(R)}$ may be useful in improving the voice qualities and laryngeal function of presbyphonia.

흰쥐 후두에 분포된 신경에서 Substance P 및 Calcitonin Gene-Related Peptides의 발현양상 (The Distribution of Substance P and Calcitonin Gene-Related Peptides Immunoreactive Fibers in the Rat′s Larynx)

  • The larynx has three major functions such as protective reflex, respiration and phonation, and is richly innervated by sensory, sympathetic and parasympathetic nerves. The sensory innervation of the laryngeal mucosa, which is involved in the perception of pain, mechanical and chemical irritation, prtects the airway via various laryngeal reflexes. We studied the distribution of Substance P (SP) and Calcitonin Gene-Related Peptide (CGRP) sensory fibers in the rat's larynx using the immuno-histochemical methods. Many SP and CGRP immunoreactive fibers were found in all regions of the laryngeal mucosa except the vocal cords. SP immunoreactive fibers showed a very similar distribution to the CGRP fibers in the epithelium and submucosa. But SP immunoreactive fibers were sparser than CGRP immunoreactive fibers in distribution density. Both reactive fibers were denser in the supraglottic region than subglottic region. Especially, intraepithelial fibers displayed the densest innervation to the laryngeal surface of the epiglottis. h the subepithelium, SP and CGRP immunoreactive fibers were distributed along the wall of vessels and around the glands. The present results suggest that the regional distribution of SP and CGRP immunoreactivity may be responsible for the protective reflex function of the laryngeal inlet.

신경후두학 (Neurolaryngology)

  • Over the last 30 years or so, it has been recognized that neurologic disorders could impair laryngeal function, and that neurogenic etiology could be discerned frequently. This has led collaborations between laryngologists and neurologists and focus on the management of neurogenic dysfunction of the larynx and pharynx, including central and peripheral disorder. The author introduced anatomical structure, nerve distribution and neurophysiology of the larynx for understanding its basic functions. The symptoms, diseases from laryngeal dysfunction and the development of diagnosis and management were also discussed.

정상인과 식도발성 음성에서의 공기역학적 비교 연구 (The Aerodynamic Analysis between Normal Voice and Esophageal Voice)

  • Voice rehabilitation is very important concerning in laryngectomees. Esophageal speech is a common and widely used method of voice restoration. But, until now there is no reliable data which shows the aerodynamic characteristics of esophageal speech. In order to evaluate the vocal quality of normal laryngeal and esophageal speech, several aerodynamic parameters were measured in 13 adults with normal laryngeal voice and 2 excellent esophageal speakers using Aerophone II voice function analyzer. The examined parameters were maximal flow rate, mean airflow rate, subglottic pressure, vocal efficiency, glottic resistance, maximal phonation time and mean sound pressure level. In vocal efficiency, there is no difference between two groups, but in other parameters, marked differences were showed in esophageal speakers, especially mean resistance. Results indicates that esophageal speakers make the efficient voices with poor aerodynamic condition, comparing with normal laryngeal speakers.

T4 병기 성문상암에서 상윤상후두부분적출술 1례 (A Case of Supracricoid Partial Laryngectomy in T4 Supraglottic Carcinoma)

  • Supracricoid partial laryngectomy (SCPL) has been performed in selected patients with transglottic carcinoma of larynx who are not amenable to classically conservative Partial laryngectomy. We have applied this procedure for the complete removal of carcinoma and the satisfactory recovery of function in 74 supraglottic laryngeal cancer Patient with thyroid cartilage invasion. The 65-year-old man with supraglottic squamous carcinoma with thyroid cartilage invasion (74 stage) was treated by SCPL with bilateral lateral neck dissection. The part of larynx and whole thyroid cartilage with tumor invasion was completely removed with covering thyrohyoid muscle and fascia. External perichondrium of the thyroid cartilage containing carcinoma was not exposed from the overlying muscle and fascia on postoperative pathological examination. We confirmed the invasion of thyroid cartilage histopathologically. Tumor was confined in the specimen and the safety margin was proved in all direction. Postoperative course and functional results were uneventful. SCPL can be applied in selected cases of locally advanced 74 laryngeal cancer.

Vein Wrapping Technique for Nerve Reconstruction in Patients with Thyroid Cancer Invading the Recurrent Laryngeal Nerve

  • Recurrent laryngeal nerve paralysis is the most common and serious complication after thyroid cancer surgery. The objective of this study was to report the advantages of the vein wrapping technique for nerve reconstruction in patients with thyroid cancer invading the recurrent laryngeal nerve and its effects on postoperative phonatory function. The subjects were three patients who underwent resection of the recurrent laryngeal nerve during surgical extirpation of papillary thyroid cancer. Free ansa cervicalis nerve graft or direct neurorrhaphy with a vein wrapping technique was used to facilitate nerve regeneration, protect the anastomosed nerve site mechanically, and prevent neuroma formation. One-year postoperative laryngoscopic examination revealed good vocal cord mobility. Maximum phonation time ($19.5{\pm}0.3$ sec) was longer than a previously-reported value in conventional reconstruction patients ($18.8{\pm}6.6$ sec). The present phonation efficiency index ($7.88{\pm}0.78$) was higher than that previously calculated in conventional reconstruction ($7.59{\pm}2.82$). The mean value of the Voice Handicap Index-10 was 6, which was within the normal range. This study demonstrates improvement in phonation indices measured 1 year after recurrent laryngeal nerve reconstruction. Our results confirm that the vein wrapping technique has theoretical advantages and could be favored over conventional reconstruction techniques for invenerate nerve injuries.

Executive function and Korean children's stop production

  • Previous studies have established a role for cognitive differences in explaining variability in speech processing across individuals. In the case of perceptual cue weighting in the context of a sound change, studies have produced conflicting results regarding the relationship between executive function and the use of redundant cues. The current study aimed to explore this relationship in acoustic cue weighting during speech production. Forty-one Korean-speaking children read a list of stop-initial words and completed two tests that assess executive function, i.e., Dimensional Change Card Sorting (DCCS) and digit n-back. Voice onset time (VOT) and fundamental frequency (F0) were measured in each word, and analyses were carried out to determine the extent to which children's executive function predicted their use of both informative and less informative cues to the three pairs comprising the Korean three-way stop laryngeal contrast. No evidence was found for a relationship between cognitive ability and acoustic cue weighting in production, which is at odds with previous, albeit conflicting, results for speech perception. While this result may be due to the lack of task demands in the production task used here, it nevertheless expands the empirical ground upon which future work in this area may proceed.

연축성 발성장애(Spasmodic Dysphonia)에 대한 보톡스 주입치료 (Botox Injection for the Management of Spasmodic Dysphonia)

  • Spasmodic dysphonia is a chronic, focal, movement-induced, action-specific dystonia of the laryngeal musculature during speech. It can have a profound effect on quality of life, severely limiting people's communication, especially via telephone and in noisy backgrounds. Spasmodic dysphonia (SD) is usually of the adductor type characterized by glottic contractions causing tightness and voice breaks with forced-strangled voice, but it may also be abductor type or, much less commonly, mixed. Treatment options for adductor spasmodic dysphonia (ADSD) include voice therapy, surgical procedures, and botulinum toxin injections (Botox). The use of Botox injected into the laryngeal muscles remains the "gold standard" treatment for reducing the vocal symptoms of ADSD and Botox induces a temporary paresis of the laryngeal muscles and provides short-term relief of symptoms. Repeated injections of the laryngeal muscles, generally every 3-4 months, are required for continuous relief of symptoms. Improvement in vocal function has been reported after use of Botox injections, though a completely normal voice is rarely achieved. In this hospital, 1,030 patients have been enrolled for Botox injection therapy so far (May, 2012). In this review article, I'd like to present my personal experience of management of spasmodic dysphonia mainly by Botox injection.

  • 후두외상의 손상은 그 정도나 범위에 따라 차이는 인지만 주요 후유증으로는 기도폐쇄, 부종, 주위조직의 봉와직염 및 농양, 누공, 후두연골 및 연골지막염, 만성 후두협착, 성대마비, 기관발거곤란증, 성음장애 등을 들 수 있고, 일반적인 후두외상의 치료방법은 일차적으로 신속한 기도유지를 위한 처치를 한 다음 상기각 후유증에 따르는 이차 시술을 시행하는 것이 보통이다. 최근 저자들은 교통사고로 인한 후두부 및 경부의 폐쇄적 외상으로 갑상연골 골절과 좌측 성대마비, 연하장애 및 우측 쇄골 골절을 보인 환자에게서 갑상연골 정복술을 시행 후 술후 2개월에 상기 증세의 호전을 보인 예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

언어습득기 이전 청각장애인의 후두소견 및 음성학적 특성 (Laryngeal Findings and Phonetic Characteristics in Prelingually Deaf Patients)

  • Background and Objectives : There are few studies reported that specifically examine the laryngeal function in patients with profound hearing loss or deafness, This study was designed to examine videostroboscopic findings and phonetic characteristics in adult patients with prelingually deaf. Materials and Method: Sixteen patients (seven males, nine females) diagnosed as prelingually deaf aged from 19 to 54 years, and were compared with a 20 normal control group with no laryngeal pathology and normal hearing group, Videostroboscopic evaluations were rated by experienced judges on various parameters describing the structure and function of the laryngeal mechanism during comfortable pitch and loudness phonations. Acoustic analysis test were done, and a nasalance test performed to measure rabbit, baby, and mother passage. CSL were measured to determine the first and two formant frequencies of vowels /a/, /i/, /u/, Statistical analysis was done using Mann-Whitney U or Wilcoxon signed ranks test. Results: Videostroboscopic findings showed phase symmetry but significantly more occurrences decrement in the amplitude of vibration, mucosal wave, irregularity of the vibration and increased glottal gap size during the closed phase of phonation, In addition, group of prelingually deaf patients were observed to have significantly more occurrences of abnormal supraglottic activities during phonation. The percentage of shimmer in the group of prelingually deaf patients were higher than in the control group. Characteristics of vowels were lower of the second formant of the vowel /i/. Nasalance in prelingually deaf patients showed normal nasality for all passages, Conclusion: Prelingually deaf patients show stroboscopic abnormal findings without any mucosal lesion, suggesting that they have considerable functional voice disorder. We suggest that prelingually deaf adults should perform vocal training for normalized laryngeal function after cochlear implantation.
