  • The restraint force is required for the accurate measurement and analysis to protect weldment from residual stress. Also, this residual stress caused by cracks in weldments is often observed in the weldments of large size nozzles or radial tanks after welding. This paper is preformed to evaluate the welding restraint forces using load cell with STS304 thin plate which is used as the pressure vessel steel in the industry field. As a result, as the welding currents are higher and the welding speeds are more slowly, the magnitude of restraint force in process of the flat plate welding shows to be more large.

  • For decreasing the maritime pollution accident of the large material(crude oil, ore and so on) carrier, as we construct the Virtual AtoN AIS utilizing the existing AIS Base Station in the area of major route, passage way and danger or in case of emergency danger, it can help the normal vessel to pass the area without any obstacles and inform the danger elements to AIS. Intralobar Pulmonary Sequestration: Report Of One Case (폐내엽형 격절부 1례 보고)

  • Intralobar pulmonary sequestration is a rare congenital malformation characterized by a cystic portion of the lung that derives its arterial blood supply through aberrant vessel directly of systemic circulation. Intralobar pulmonary sequestration Is usually contained within the visceral pleura of a pulmonary lobe and its venous drainage to the pulmonary venous system. We experienced a case of pulmonary sequestration preoperatively confirmed. The patient was 17 year old female whose complaints were mild fever and profuse purulent sputum. Chest film showed a large thin walled cystic lesion with air-fluid level at the left lower posterior basal lung field. Aortogram revealed an aberrant artery originated from thoracic aorta just above the diaphragm and that drained via pulmonary vein into the left atrium. At time of operation, a large abscess cavity measuring 9x8x3 cm in dimension at the left lower lobe was noted. And the aberrant artery, measuring 0.5 cm in diameter and 2 cm in length, arising from thoracic aorta just above the diaphragm was noted. After division and ligation of the aberrant artery, a left lower lobectomy was performed and the patient`s postoperative course was uneventful.

Retreatment of a recurrent giant aneurysm of the internal carotid artery after treatment with a flow-diverting stent

  • Flow-diverting stents (FDSs) have proven advantageous for the treatment of large, fusiform, and dissecting aneurysms that are otherwise difficult to treat. Retreatment strategies for recurrent large or giant aneurysms after FDSs are limited to overlapping implantation of an additional FDS or definitive occlusion of the parent vessel. We report a recurrent giant aneurysm that was initially treated with an FDS with coils and was successfully treated with an additional FDS. Visual symptoms due to the mass effect of the recurrent aneurysm were completely resolved, and follow-up digital subtraction angiography revealed complete obliteration of the aneurysm. Additional FDS implantation for the retreatment of incompletely occluded aneurysms after the initial FDS treatment may be feasible and safe. Further studies are required to validate these results.

Clinical Significance of Detecting Lymphatic and Blood Vessel Invasion in Stage II Colon Cancer Using Markers D2-40 and CD34 in Combination

  • This research was conducted to compare differences in colon cancer lymphatic vessel invasion (LVI) with D2-40 antibody labeling and regular HE staining, blood vessel invasion (BVI) with CD34 antibody labeling and HE staining and to assess the possibility of using D2-40-LVI/CD34-BVI in combination for predicting stage II colon cancer prognosis and guiding adjuvant chemotherapy.Anti-D2-40 and anti-CD34 antibodies were applied to tissue samples of 220 cases of stage II colon cancer to label lymphatic vessels and small blood vessels, respectively. LVI and BVI were assessed and multivariate COX regression analysis was performed for associations with colon cancer prognosis. Regular HE staining proved unable to differentiate lymphatic vessels from blood vessels, while D2-40 selectively labeled lymphatic endothelial cell cytosol and CD34 was widely expressed in large and small blood vessels of tumors as well as normal tissues. Compared to regular HE staining, D2-40-labeling for LVI and CD34-labeling for BVI significantly increased positive rate (22.3% vs 10.0% for LVI, and 19.1% vs 9.1% for BVI). Multivariate analysis indicated that TNM stage, pathology tissue type, post-surgery adjuvant chemotherapy, D2-40-LVI, and CD34-BVI were independent factors affecting whole group colon cancer prognosis, while HE staining-BVI, HE staining-LVI were not significantly related. When CD34-BVI/D2-40-LVI were used in combination for detection, the risk of death for patients with two or one positive results was 5.003 times that in the LVI(-)&BVI(-) group (95% CI 2.365 - 9.679). D2-40 antibody LVI labeling and CD34 antibody BVI labeling have higher specificity and accuracy than regular HE staining and can be used as molecular biological indicators for prognosis prediction and guidance of adjuvant chemotherapy for stage II colon cancer.

Immunohistochemical Study for the Angiogenesis Factors and Vascular Wall Matrix Proteins in Intracranial Aneurysms (뇌동맥류에서 혈관형성 인자와 혈관벽 기질 단백에 대한 면역조직화학적 연구)

  • Objective : Until now, it has been little known about the biological mechanisms associated with the genesis, growth, and rupture of intracranial aneurysm. This study was performed to investigate and understand a part of these mechanisms. Materials and Methods : Immunohistochemical stains for angiogenesis growth factors(basic fibroblast growth factor (bFGF) and vascular endothelial growth factor(VEGF)) and selected vascular wall matrix proteins(alpha smooth muscle actin(${\alpha}SMA$) and collagen Type IV) were performed in fixed sections from a normal circle of Willis artery which was taken from the autopsy specimen as a control vessel and 17 aneurysmal wall specimens which was taken during surgical clipping of aneurysms. The staining intensity and distribution of immunoreactivity to angiogenesis growth factors and selected wall matrix proteins in control vessel and aneurysmal wall were examined and compared with each other. The difference of staining intensity according to the size of aneurysm was also investigated. Results : There was no immunoreactivity to bFGF and VEGF in the control vessel. bFGF immunoreactivity was exhibited in 15 of 17 aneurysm specimens around smooth muscle cells within the media of aneurysm. VEGF immunoreactivity was also exhibited in all aneurysm specimens in patches or diffusely affecting all layers of the aneurysmal wall. The degrees of intensity of bFGF and VEGF immunoexpression were proportionate roughly to the size of aneurysm. Strong immunoexpression of both factors were noticed in large aneurysm. A regularly arranged and defined band of immunoreactivity of ${\alpha}SMA$ was noticed in the media of the control vessel, whereas diffuse, faint, irregularly arranged ${\alpha}SMA$ was noticed in the aneurysmal wall. A regularly defined band of collagen Type IV immunoreactivity was also noticed in the subendothelium of the control vessel, whereas diffuse disorganized immunoreactivity of collagen Type IV was noticed in the entire wall of the aneurysm. Conclusion : These results indicate substantial evidences of abnormal expression of angiogenesis factors and changes of selected vascular wall matrix proteins in the wall of intracranial aneurysm. The unbalanced changes of angiogenesis factors and vascular wall matrix proteins in the wall of aneurysm may be one of the biological mechanisms for the growth and rupture of aneurysm.

An Empirical Study to Improve Vessel Departure Control Regulations for Restricted Visibility, Focused on Pyeongtaek Port (시계제한시 선박 출항통제규정 개선에 관한 실증적 연구 -평택항을 중심으로-)

  • The purpose of this study is to improve the current vessel departure control regulations to prevent traffic congestion after the lifting of vessel departure controls due to restricted visibility. AIS data was collected to analyze the traffic volume of normal traffic flow and after departure control. A statistical test was conducted to confirm the difference in traffic volume at peak hours according to whether or not departure control was used. The results of the t-test showed that there was a significant difference in traffic volume among groups of less than 10,000 tons in gross tonnage. However, the Mann-Whitney test showed no difference in traffic volume regardless of vessel control. Small and medium-sized vessels of less than 10,000 tons after departure control increased in traffic volume by 142% over normal traffic, and it was concluded that traffic congestion resulted as these small and medium-sized vessels were departing at the same time as large vessels of more than 10,000 tons. In order to prevent vessel collision accidents due to traffic congestion, it is suggested that the navigability of vessels less than 160 m or less than 10,000 tons should be improved.

The Protective Effects of Statins towards Vessel Wall Injury Caused by a Stent Retrieving Mechanical Thrombectomy Device : A Histological Analysis of the Rabbit Carotid Artery Model

  • Objective : Endovascular mechanical thrombectomy (MT) has been regarded as one of the standard treatments for acute ischemic stroke caused by large vessel occlusion. Despite the wide use of stent retrievers for MT, arterial intimal damage caused when deployed stent is pulled has been a certain disadvantage. We hypothesized that statin could protect and stabilize vessel damage after endovascular MT using a stent retriever. In this animal study, we observed the protective effects of the statins towards MT-induced vessel wall injury. Methods : Twenty-eight carotid arteries of fourteen rabbits were used in the experiments with MT using stent retriever. We divided the rabbits into four groups as follows : group 1, negative control; group 2, positive control; group 3, statin before MT; and group 4, statin after MT. After MT procedures, we harvested the carotid arteries and performed histomorphological and immunohistochemical analyses. Results : In histomorphological analysis with hematoxylin and eosin and Masson's trichrome stain, significant intimal thickening (p<0.05) was observed in the positive control (group 2), compared to in the negative control (group 1). Intimal thickening was improved in the statin-administered groups (groups 3 and 4 vs. group 2, p<0.05). We also observed that statin administration after MT (group 4) resulted in a more effective decrease in intimal thickness than statin administration before MT (group 3) (p<0.05). We performed immunohistochemical analysis with the antibodies for tumor necrosis factor-alpha (TNF-α), cluster of differentiation (CD)11b, and CD163. In contrast to the negative control (group 1), the stained percentage areas of all immunological markers were markedly increased in the positive control (group 2) (p<0.05). Based on statin administration, the percentage area of TNF-α staining was significantly reduced (p<0.05) in group 3, compared to the positive control group (group 2). However, significant differences were not observed for CD11b and CD163 staining. In group 4, no significant differences were observed for TNF-α, CD11b, and CD163 staining (p≥0.05). The differences in the percentage areas of the different markers between the statin-administered groups (groups 3 and 4) were also not revealed. Conclusion : We presented that statin administration before and after MT exerted protective effects towards vessel wall injury. The efficacy of statins was greater post-administration than pre-administration. Thus, statin administration in routine prescriptions in the peri-procedural period is strongly advised.

  • As interest increases related to the development of eco-friendly energy, the offshore wind turbine market is growing at an increasing rate every year. In line with this, the demand for an installation vessel with large scaled capacity is also increasing rapidly. The wind turbine installation vessel (WTIV) is a fixed penetration of the spudcan in the sea-bed to install the wind turbine. At this time, a review of the spudcan is an important issue regarding structural safety in the entire structure system. In the study, we analyzed the current procedure suggested by classification of societies and new procedures reflect the new loading scenarios based on reasonable operating conditions; which is also verified through FE-analysis. The current procedure shows that the maximum stress is less than the allowable criteria because it does not consider the effect of the sea-bed slope, the leg bending moment, and the spudcan shape. However, results of some load conditions as defined by the new procedure confirm that it is necessary to reinforce the structure to required levels under actual pre-load conditions. Therefore, the new procedure considers additional actual operating conditions and the possible problems were verified through detailed FE-analysis.

Acute cerebral infarction associated with thrombocytopenia in primary Sjogren's syndrome : A Case Report (저혈소판증을 동반한 급성 대뇌경색을 보인 원발성 쇼그렌 증후군 1례)

  • Sjogren's syndrome is an autoimmune disease characterized by dry mouth and neutropenia. Although it does not commonly involve the central nervous system, Sjogren's syndrome sometimes affects small vessels through microangiopathic alterations. A 34-year-old woman was hospitalized for left upper quadrantanopia and a tingling sensation in the left hemibody. Brain magnetic resonance imaging revealed acute infarction in the right posterior cerebral artery territory. In laboratory tests, antinuclear (FANA2+) and anti-DNA antibodies (anti-SS-A (Ro)) were detected. Salivary gland scintigraphy revealed moderately decreasedexcretion of saliva. Based on these findings, we concluded she had Sjogren's syndrome. As in this patient, large vessel involvement in Sjogren's syndrome is far less common. Furthermore, it is difficult to administer antiplatelet drugsto patients with thrombocytopenia in Sjogren's syndrome. This is a case of the patient with Sjogren's syndrome that involved thrombocytopenia and large vessel invasion who was treated with antiplatelet drugs and hydroxychloroquine.
