Coronavirus disease 2019 (COVID-19) is a highly infectious respiratory disease caused by a severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2). SARS-CoV-2 infection may cause clinical manifestations of multiple organ damage, including various neurological syndromes. There are currently two oral antiviral drugs-Paxlovid and molnupiravir-that are recognized to treat COVID-19, but there are still no drugs that can specifically fight the challenges of SARS-CoV-2 variants. Nucleotide-binding oligomerization domain-like receptor pyrin domain-containing-3 (NLRP3) inflammasome is a multimolecular complex that can sense heterogeneous pathogen-associated molecular patterns associated with neurological disorders. The NLRP3 activation stimulates the production of caspase-1-mediated interleukin (IL)-1β, IL-18, and other cytokines in immune cells. Panax (P.) ginseng is a medicinal plant that has traditionally been widely used to boost immunity and treat various pathological conditions in the nervous system due to its safety and anti-inflammatory/oxidant/viral activities. Several recent reports have indicated that P. ginseng and its active ingredients may regulate NLRP3 inflammasome activation in the nervous system. Therefore, this review article discusses the current knowledge regarding the pathogenesis of neurological disorders related to COVID-19 and NLRP3 inflammasome activation and the possibility of using P. ginseng in a strategy targeting this pathway to treat neurological disorders.
높은 감염성과 치사율을 지닌 급성 호흡기 질환 메르스는 2015년 5월 국내에 잠입한 후 218일 간 186명의 감염자를 산출하면서 전국적 공포 분위기를 조성하였다. 다행히 7개월 후 감염자가 끊기자 12월 24일 메르스 종식이 선언되었지만, 메르스 사태는 우리 의료계나 사회 전반에 맹성하고 혁신해야 많은 과제를 남겼다. 본 논문은 (1) 2015년 5월 20일부터 12월까지의 국내에서 벌어진 메르스 사태에 대한 간략한 복기로부터 시작해, (2) 해당 사태를 단순한 역학적(疫學的) 차원을 넘어선 범사회적 재해로 재해석하고, (3) 메르스가 우리 사회에 그토록 큰 파문을 초래했던 이유와 향후 실천과제를 사회의료학적 관점에서 탐지한 후, (4) 사회의료학의 비전과 전망에 관한 소견으로 끝을 맺고자 한다.
Severe acute respiratory syndrome-coronavirus 2 (SARS-CoV-2) is a novel virus that causes coronavirus disease 2019 (COVID-19). To understand the identity, functional characteristics and therapeutic targets of the virus and the diseases, appropriate infection models that recapitulate the in vivo pathophysiology of the viral infection are necessary. This article reviews the various infection models, including Vero cells, human cell lines, organoids, and animal models, and discusses their advantages and disadvantages. This knowledge will be helpful for establishing an efficient system for defense against emerging infectious diseases.
Background : Early in November 2002, severe acute respiratory syndrome (SARS) began to spread throughout thirty-two countries around the world. A total of 5,327 cases were reported in China, including 1,755 cases in Hong Kong and 655 in Taiwan. The total number of cases reported from Canada and Singapore was more than 200. The total number of SARS cases world-wide reached 8,437 with incidences. Mortality from SARS is estimated at around 11%. Under the guidance of the World Health Organization (WHO), clinical studies on the use of integrated herbal medicine and western medicine for treating SARS were carried out in China and Hong Kong. The official report suggested that integrated treatment was more effective than western medicine alone in clinical symptoms, lung inflammation, blood oxygen saturation, immunological functions and others. Objective : To overview the treatment principle and the prescriptions according to the regimens and the stages for SARS of the reports, and further to broaden our knowledge of treatment of acute infectious diseases using natural herbal medicine. Methods : We reviewed nine of WHO SARS reports that comprehensively described the principles and methods of treatment and summarized them into eight treatment methods. We analyzed the herbal formulae on the basis of their treatment principles, evaluated them in accordance with warm disease study, listed frequently used herbs, and assessed patent prescriptions and herbal injections that were mentioned in the reports. Results and Conclusion : The reports divided the course of SARS into 3 to 6 stages such as high fever stage, fastigium stage, and convalescent stage. Frequently used herbs were Radix Scutellariae, Radix Paeoniae, Radix Astragali, Semen Armeniacae, Radix Pseudostellariae, Radix Ophiopogonis, and Fructus Schizandrae. Herbal decoctions were the primary method of treatment, while extracts or injections were secondary. Prescriptions were mainly based on the warm disease study. SARS is a viral disease caused by corona virus, and herbal medicine is proven to be effective against it. We believe that evidences and experiences from SARS cases can be a good reference to further researches on acute infectious diseases.
Bordetella (B.) bronchiseptica, Mycoplasma (M.) cynos, and M. canis are the major bacterial pathogens that cause canine infectious respiratory disease complex (CIRDC). In this study, we developed a triplex real-time polymerase chain reaction (tqPCR) assay for the differential detection of these bacteria in a single reaction. The assay specifically amplified three bacterial genes with a detection limit of below 10 copies/reaction. The assay showed high repeatability and reproducibility, with coefficients of intra- and inter-assay variations of less than 1%. The diagnostic results of the assay using 94 clinical samples from household dogs with CIRDC clinical signs, the prevalence of B. bronchiseptica, M. cynos, and M. canis was 22.3%, 18.1%, and 20.2%, respectively, indicating that the diagnostic sensitivity was comparable to those of previously reported qPCR assays. The dual infection rate of B. bronchiseptica and M. cynos, B. bronchiseptica and M. canis, and M. cynos and M. canis was 5.3%, 7.4%, and 3.1%, respectively. Moreover, the triple infection rate of B. bronchiseptica, M. cynos, and M. canis was 2.1%. These results indicate that coinfections with B. bronchiseptica, M. cynos, and M. canis have frequently occurred in the Korean dog population. The newly developed tqPCR assay in the present study will be a useful tool for etiological and epidemiological studies on these three CIRDC-associated bacterial pathogens. The prevalence and coinfection data revealed through this study will contribute to expanding knowledge on the epidemiology of CIRDC in the recent Korean dog population.
본 연구는 간호사를 대상으로 COVID-19에 대한 호흡기 감염관리 수행도에 영향을 미치는 요인을 파악하고자 시도되었다. 구조화된 설문지를 이용하여 191명의 간호사로부터 일반적 특성, COVID-19 감염관리 지식, 불안, 피로도, 수행도를 파악하였다. 자료분석은 IBM SPSS 21.0 프로그램을 이용하여 기술통계, t-test, one way ANOVA, 상관분석, 다중회귀분석을 하였다. 본 연구에서 지식과 피로도는 양의 상관관계, 불안과 수행도는 음의 상관관계가 있었으며 COVID-19 호흡기 감염관리 수행도에 영향을 미치는 요인은 불안으로 나타났다. 간호사의 COVID-19 호흡기 감염관리 수행도를 향상시키기 위한 교육프로그램 개발 시 신종감염병에 대한 불안 정도를 관리할 수 있는 내용을 포함해야 할 것으로 보인다. 또한 간호사가 신종감염병 환자를 미리 간호해볼 수 있도록 역할극이나 시뮬레이션 교육방안 적용을 고려해 볼 필요가 있다.
본 연구는 보건계열 대학생의 메르스에 대한 지식, 태도 및 예방행위 실천도를 파악하고, 메르스 및 호흡기 감염예방과 교육을 위한 기초자료를 제공하고자 하였다. 자료수집은 2015년 6월 8일부터 6월 13일까지 전주시 소재 일개대학교 보건계열 대학생 251명을 대상으로 설문조사하였다. 연구 대상자는 치위생과 46.6%, 임상병리과 30.7%, 응급구조과 22.7%로, 메르스 감염예방 교육은 69.7%가 받았다. 메르스에 대한 지식은 메르스 감염예방 교육을 받은 경우(t=3.457, p=0.001), 여학생이 높았으며(t=-2.945, p=0.005), 메르스에 대한 태도는 치위생과(F=8.048, p<0.001), 3학년 학생(F=3.978, p=0.020)이 높았다. 메르스에 관한 지식, 태도 및 예방행위 실천도, 감염에 대한 두려움간의 상관관계는 지식(r=0.133, p=0.036)이 높고, 태도(r=0.158, p=0.012)가 긍정적이고, 감염에 대한 두려움(r=0.312, p<0.001)이 높을수록 예방행위 실천도가 높았다. 이상의 연구결과를 종합해볼 때 보건계열 대학생이 신종감염병인 메르스 감염을 예방하기 위해서는 메르스에 관한 지식을 향상시키고, 의료기관에서 메르스 감염관리에 대해 긍정적인 태도를 갖도록 하여 메르스 감염 예방행위를 실천할 수 있도록 해야할 것이다. 또한 의료기관 임상실습 전 신종감염병을 포함한 감염병에 대한 감염원인, 전파 기전, 예방법 등의 감염관리 교육 프로그램을 구체적으로 개발하여야 할 것이다.
본 연구는 메르스 대유행시 요양병원 종사자의 메르스에 대한 지식, 태도 및 감염예방행위 실천도와의 관계를 파악하기 위하여 시도되었다. 연구대상자는 J도 소재 요양병원 종사자 211명으로 구조화된 설문지를 이용하였으며, 자료수집기간은 2015년 6월 15일부터 7월 15일까지였다. 자료 분석은 SPSS/WIN 24.0을 이용하여 independent t-test, ANOVA, Welch test로 분석하였고, 사후 검정은 scheff test, Grames-hawell test 방법으로 시행하였다. 요양병원 종사자의 메르스에 대한 지식점수는 간호사와 간호조무사, 감염관리 교육경험이 있는 경우, 지난 해 인플루엔자 예방접종을 한 경우, 손소독제 사용이 용이한 경우에 높았으며, 메르스에 대한 태도는 간호사가 간호조무사와 간병인보다 긍정적이었다. 메르스 감염예방행위 실천도는 일상생활에서는 지난 해 인플루엔자 예방접종을 한 경우, 환자 간호 시에는 지난 해 인플루엔자 예방접종을 한 경우, 손소독제 사용이 용이한 경우에 높았다. 요양병원 종사자의 메르스에 관한 지식, 태도 및 감염예방행위 실천도와의 관계는 지식이 높고, 태도가 긍정적일수록 감염예방행위 실천도가 높게 나타났다. 이에 메르스와 같은 신종감염병이 유행할 때, 요양병원에서는 메르스 질병 특성 및 전파, 예방법이 포함된 감염관리 교육을 실시해 메르스에 대한 지식수준을 높이고, 태도를 긍정적으로 유도하여, 감염예방행위 실천도를 향상시킬 필요가 있을 것이다.
특발성 세기관지중심성 간질성 폐렴은 염증 및 섬유화가 주로 소기도 및 주변을 침범하고, 부분적으로 폐포로 퍼져나가는 특징을 보이는 질환으로 아직까지 국내에서는 발표된 증례가 없다. 저자들은 한 달간의 호흡곤란으로 내원한 남자에서 방사선학적으로 망상형 병변과 중심소엽성 결절을 보이면서 조직 검사상 세기관지에 집중된 병리 소견을 보여 세기관지중심성 간질성 폐렴을 진단한 예를 경험하였기에 보고하는 바이다.
This study was conducted to investigate the infection of livestock diseases using 500 blood samples from wild boars captured in six cities and one county in northern Gyeonggi province, South Korea. We examined 239 cases of classical swine fever virus (CSFV), and each of 500 cases of foot and mouth disease virus (FMDV), porcine reproductive and respiratory syndrome virus (PRRSV), porcine circovirus type 2 (PCV2), Mycoplasma hyopneumoniae (MH), Actinobacillus pleuropneumoniae (APP), Pasteurella multocida type A (PMA), Hemophilus parasuis (HP), Salmonella (Sal.) spp. infections. Antibodies were detected against CSFV (23.4%), PRRSV (4.0%), PCV2 (60.4%), MH (3.0%), APP (69.2%), PMA (52.8%), HP (11.8%), and Sal. spp. infections (37.2%). No antibodies were detected against FMDV. As a result of antigenic analysis of 68 positive cases (13.6%) out of 500 PRRS antigen tests, 61 North American cases, 6 European cases, 1 North American-European complex case. PCV2 has 158 positive cases (31.6%) out of 500 antigen tests, and the results indicate that a considerable number of individuals are infected. To our knowledge, this is the first seroprevalence report of MH, APP, PMA, HP, and Sal. spp. infections in wild boars in South Korea.
