십자인대 재건술 후 슬관절 신전근의 근기능 향상을 위한 대퇴사두근 운동의 유용성 (The Usefulness of Quadriceps Exercise for Improvement of Knee Extensor Muscular Function after Cruciate Ligament Reconstruction)

  • Background: The purpose of this study was to investigate the usefulness of quadriceps exercise for improvement of knee extensor muscular function after cruciate ligament reconstruction. Methods: Participate in 18 of patients with cruciate ligament reconstruction, ACL & PCL reconstruction was 9 of patients, respectively. All subjects instructed that quadriceps exercise repeated daily, composed to QSE(quadriceps femoris setting exercise) and SLR(straight leg raises). Knee extensor muscular function was measured before and after quadriceps exercise. Initial test was measured before cruciate ligament reconstruction, and retest was measured 6month after cruciate ligament reconstruction. Results: Knee extensor muscular function significantly increased in both ACL and PCL reconstruction(P<0.05). Conclusion: The results of this study indicated that quadriceps exercise useful for improve knee extensor muscular function in patients with cruciate ligament reconstruction.

Three-dimensional Reconstruction of the Knee for Ligament Reconstruction

  • As computer technology develops and this is applied to medical image field, three dimensional image reconstruction technology using computer simulation is utilized in various categories that include anatomical study and biomechanics study of human body. Especially orthopedic surgeons are able to investigate biomechanical function and be provided information for operations with this technology in terms of ligament reconstruction of knee. And this technology can be utilized in preparing preoperative planning and instructions and training. This review is about three dimensional image reconstruction technology which is utilized in ligament reconstruction of knee.

Pedicled Perforator Flaps for Reconstruction of Bilateral Knee Defects: A Case Report

  • Reconstruction of soft tissue defects of the knee has always been a challenging task for plastic surgeons. Various reconstructive choices are available depending on the location, size, and depth of the defect relative to the knee joint. Defects on the knee joint have several characteristic features. The use of a free flap is preferred for reconstructions involving obliteration of large-cavity defects, but recipient pedicle isolation can be difficult because of the extent of the injury zone. Furthermore, the true defect during knee joint flexion is larger than during knee joint extension, and a durable flap is necessary for joint movement. We report for the first time on the use of pedicled perforator flaps for reconstruction of bilateral knee defects in a 76-year-old woman. The operative procedure required skeletonizing the perforators of an antero-lateral thigh flap and antero-medial thigh flap and rotating the flap in the defect. The patient returned to normal daily activity and had a full range of motion two months after the accident. The shorter operating time with decreased donor site morbidity and its durability make this flap a valuable alternative for soft tissue reconstruction of the knee.

슬관절 전치환술 후 발생한 피부 괴사부의 재건 (Reconstruction of Necrosis Following Total Knee Replacement Arthroplasty)

  • In spite of proper maneuver of total knee replacement arthroplasty, some patients suffer from skin necrosis just above the implant. From Mar. 2000 to Jan. 2004, the authors performed reconstruction of knee skin defects after total knee replacement athroplasty. Total 6 cases of flap surgery were performed and patients ranged between 43-years-old to 82-years-old. Rectus femoris perforator based reversed adipofascial flaps were used in 2 cases, medial gastrocnemius muscular island flaps were used in 2 cases and sural artery based on adipofascial rotation flap was used in 1 case. One patient with extended necrosis underwent reconstruction with dual flaps of sural artery based adipofascial rotation flap and medial gastrocnemius muscular island flap. There were no distinctive complication needing additional procedure in all cases during the long term follow up. Reconstruction of necrosis following total knee replacement arthroplasty had several characteristics different from simple knee defect. The patients might have the history of long term steroid usages, excessive skin tension due to implants, underlying disease such as diabetes, rheumatoid disease, and etc. In addition, the early ambulation is mandatory in these patients of total knee replacement arthroplasty. With regards to these special considerations, a single stage and reliable operation must be needed. The authors introduce various reconstruction methods and algorithm that may aid easy decision making.

  • Reconstruction of soft tissue defect of knee joint area has been remained a challenging task for plastic surgeons. The earlier the normal tissue saved and the necrotic tissue removed, the less the patients had complications and functional disability. But such defects are difficult to manage for its poor vascularity, rigid tissue distensibility, easy infectability and a relatively long healing period. The goal of flap coverage in the knee joint should not only be satisfactory wound coverage, but also acceptable appearance and minimal donor site morbidity. We have treated five cases using the anterolateral thigh perforator flaps for reconstruction successfully. In conclusion, we believe that in cases of knee joint area soft tissue defects, flaps like anterolateral thigh perforator flap should be considered as the first line of treatment.

역혈류성 전외측대퇴 혈관경피판을 이용한 무릎 주위 결손의 재건 (Distally Based Anterolateral thigh Pedicled Flap in the Reconstruction of Defect Around Knee)

  • Purpose: As the soft tissue defect around the knee is difficult to reconstruct, local flap or free flap is used. Distally based anterolateral thigh pedicled flap introduced by Zhang uses sufficient reverse flow supplied from the vascular network around the knee. We report successful reconstruction of defect around knee by this method. Methods: Four patients with skin & soft tissue defect around knee have been treated for reconstruction using the distally based anterolateral thigh pedicled flap. First, the doppler was used to check the perforator flap of the descending branch of the lateral circumflex femoral artery and to draw and dissect the perforator flap as much as needed. After the dissection, the proximal of the descending branch was clamped and checked for sufficient supply of blood flow from the reverse flow and then ligated. It was dissected along the descending branch and in order to prevent damage to the joined parts of the descending branch and the lateral superior geniculate artery, a more careful ligation was done starting from 10 cm superior to the knee. The defect was reconstructed after securing enough vascular pedicle to cover all the damaged parts. Results: Not all patients suffered from flap necrosis. In case of the patient with chronic osteomyelitis, slight venous congestion was observed right after the surgery but it disappeared the following day. All three patients had no occurences of additional complications. Conclusion: Distally based anterolateral thigh pedicled flap was enough to provide large flap for knee reconstruction. It had sufficient blood flow and vascular pedicle. It also had taken short operation time compared to the free flap operation. The distally based anterolateral thigh pedicled flap used by the authors is a very useful way of reconstructing the area around knee.

  • The anterior cruciate ligament(ACL) is an important stabilizer of knee joint. The ACL injury of knee is common and a serious ACL injury leads to ligament reconstruction surgery. Gait analysis is essential to identify knee condition of patients who display abnormal gait. The purpose of this study is to evaluate and classify knee condition of ACL deficient patients using a nonlinear dynamic method. The nonlinear method focuses on understanding how variations in the gait pattern change over time. The experiments were carried out for 17 subjects(l2 healthy subjects and five subjects with unilateral deficiency) walking on a motorized treadmill for 100 seconds. Three dimensional kinematics of the lower extremity were collected by using four cameras and KWON 3D motion analysis system. The largest Lyapunov exponent calculated from knee joint flexion-extension time series was used to quantify knee stability. The results revealed the difference between healthy subjects and patients. The deficient knee was significantly unstable compared with the contralateral knee. This study suggests an evaluation scheme of the severity of injury and the level of recovery. The proposed Lyapunov exponent can be used in rehabilitation and diagnosis of recoverable patients.

후방 십자 인대 재건술에서 동종 이식건의 역할 (The Role of Allograft for Posterior Cruciate Ligament Reconstruction)

  • The use of autogenous tissues is preferred for knee ligament reconstruction. However allografts play a role in major ligament reconstructive procedures in which multiple substitutions or revisions are required. In the dislocated knee, allografts may offer an advantage in reconstructing the PCL. But allografts in knee ligament surgery must be considered in terms of biomechanical and regenerative properties, disease transmission and immunogenecity, and methods of preservation and sterilization. Also only a few authors have described the use of allograft for reconstruction of a ruptured PCL, either a single procedure, or in combination with ACL repair following knee dislocation. Furthermore, the problems that the clinician faces with use of allografts is the necessity for supervision to ensure that the grafts are correctly processed, secondarily sterilized, and free of transmissible diseases. For these reasons, the routine use of allograft materials in the treatment of ligament deficiencies should be avoid and provide with meaningful outcome studies, including longterm follow-up.

Role of the Gastrocnemius Musculocutaneous with a Propeller Style Skin Flap in Knee Region Reconstruction: Indications and Pitfalls

  • Background Soft tissue reconstruction around the knee area is still an open question, particularly in persistent infections and multiple reoperations scenario. Flap coverage should guarantee jointmobility and protection, even when foreign materials are implanted. The chimeric harvesting of the musculocutaneous gastrocnemius flap, based on the sural artery perforators, can extend its applicability in soft tissue reconstruction of the upper leg, overcoming the drawbacks of the alternative pedicled flaps. Methods A multicenter retrospective study was conducted enrolling patients who underwent to a pedicled, chimeric gastrocnemius musculocutaneous-medial sural artery perforator (GM-MSAP) or lateral sural artery perforator (GM-LSAP) flap for knee coverage in total knee arthroplasty (TKA) recurrent infections and oncological or traumatic defects of the upper leg from 2018 to 2021. Outcomes evaluated were the successful soft tissue reconstruction and flap complications. Surgical timing, reconstruction planning, technique, and rehabilitation protocols were discussed. Results Twenty-one patients were included in the study. Nineteen GM-MSAPs and 2 GM-LSAPs were performed (soft tissue reconstruction in infected TKA [12], in infected hardware [4], and in oncological patients [5]). Donor site was closed primarily in 9 cases, whereas a skin graft was required in 12. Flap wound dehiscence (1), distal flap necrosis (1), distal necrosis of the skin paddle (1), and donor site infection (1) were the encountered complications. Flap reraise associated to implant exchange or extensive debridement was successful without requiring any further flap surgery. Conclusion The propeller-perforator GM-MSAP offers qualitative defect coverage and easiness of multiple flap reraise due to skin availability and its laxity.

Effects of Sensorimotor Training Volume on Recovery of Knee Joint Stability in Patients following Anterior Cruciate Ligament Reconstruction

  • Purpose: The purpose of this study is to examine the effects of sensorimotor training on knee joint stability after anterior cruciate ligament reconstruction. Methods: The subjects were sixteen 16 adults who received anterior cruciate reconstruction by arthroscopy, and underwent sensorimotor training for which was to have them maintenanceain of a standing position with a step Balance ball on the affected side over 30 degrees knee flexion with 100% weight bearing for 15-20 seconds. Before the genuine experiment commenced, the Lysholm scale was had been used to assess functional disorders on the affected knee joint. KT-2000 Arthrometer measurement equipment was used to measure anterior displacement of tibia against to femur before and after the sensorimotor training. Results: There was significant relaxation on the affected side in tibia anterior displacement of the affected and sound sides on in supine position before the sensorimotor training. There was little significant difference in tibia anterior displacement of the affected knee joints on in the supine position before and after the sensorimotor training. The results also showed that there was a reduction in the difference of tibia anterior displacement of the affected knee joints on in the standing position. These results suggest that the effects of sensorimotor training on knee joint stability after anterior cruciate ligament reconstruction is to induce the change of tibia anterior displacement against femur and the variation of muscles activation. Conclusion: The sensorimotor training may contribute to the improvement of joint functional stability in people who are in post-operation state and with orthopedic musculoskelectal injuries.
