정적 스트레칭과 에비안스-함베르크 스트레칭이 슬관절의 관절가동범위의 변화에 미치는 영향 (The Effects of Static Stretching and Evjenth-Hamberg Stretching on Range of Motion of Knee Joint)

  • The purpose of this study was to examine the effects of static stretching and Evjenth-Hamberg stretching the on range of motion (ROM) of the knee joint. The subjects were composed of twenty healthy males without weight-training experience. The ROM of the knee joint was measured by using an En-Knee. Three tests (performed on the 1st week, 4th week, and 8th week) were conducted to examine the change of each variable. Data were analyzed with a $2{\times}3$ analysis of variance (group${\times}$test) for repeated measures on last factor through SPSS 10.0. The data analysis revealed the change in the ROM was dependent on the stretching method. The results were as follows: The ROM was improved in both methods by each time, but the E-HS was more improved than the SS. In conclusion, this study indicated that the E-HS is more efficient than the SS on the ROM of the knee joint.

반복 회전운동에서 코발트 크롬 합금과 미끄럼 접촉하는 돼지 무릎 관절연골의 접촉압력이 마찰계수 변화에 미치는 영향 (Effect of Contact Pressure on the Variations in Coefficients of Friction Between Porcine Knee Joint Cartilage and Co-Cr Alloy in a Repeat Pass Sliding Motion)

  • In this study, the influence of contact pressure on the variation in coefficients of friction between porcine knee joint cartilage and Co-Cr alloy in a repeat pass sliding motion was investigated. Flat-ended cartilage pin specimens(9 mm diameter, 8 mm long) were prepared from porcine(6 months old) knee joints by a drill-type punch. Friction tests were conducted by using a pin-on-disk type friction tester for an hour in PBS lubricated condition under the contact pressures of 0.5, 1 and 2 MPa with 50 mm distance per a cycle at ambient condition. As a result, coefficients of friction increased as the test duration increased for all contact pressures. The maximum coefficients of friction were 0.082, 0.06 and 0.098 for 0.5, 1, and 2 MPa, respectively. It showed that coefficients of friction of porcine knee joint cartilage against Co-Cr alloy depended on the level of contact pressure and related to squeeze film lubrication mechanism.

슬관절 골관절염환자에서 도수 관절가동술이 통증, 관절가동범위, 신체기능과 균형능력에 미치는 효과 (The Effect of Manual Joint Mobilization on Pain, ROM, Body Function and Balance in Patients with Knee Osteoarthritis)

  • PURPOSE: The purpose of this study was to investigate effect of the manual joint mobilization to the patients with knee osteoarthritis and to determine the effect of pain, range of motion, body function and balance after applying it. METHODS: The thirty participants who complained the knee pain were randomly assigned to control (Con) group (n=15) that received the general physical therapy and experimental (Exp) group (n=15) that received the applied the manual joint mobilization and the general physical therapy three times per week, 30 minutes per day for four weeks. It measured the visual analogue scale (VAS), the range of motion (ROM), body function (WOMAC) and balance (TUG). RESULTS: It showed the significantly different between the control group and experiment group in VAS, ROM and WOMAC. After 4 weeks, the experiment group was significantly different from other group in VAS, ROM and WOMAC. But the measurement of balance did not show the significantly difference within group and between groups. CONCLUSION: This results suggest that Manual joint mobilization was effective in pain, ROM, function in patient with knee osteoarthritis.

무릎 골관절염 환자의 보행속도에 따른 하지 관절 강성 변화 (Changes of Lower Limb Joints Stiffness with Gait Speed in Knee Osteoarthritis)

  • Spring-like leg models have been employed to explain various dynamic characteristics in human walking. However, this leg stiffness model has limitations to represent complex motion of actual human gait, especially the behaviors of each lower limb joint. The purpose of this research was to determine changes of total leg stiffness and lower limb joint stiffness with gait speed in knee osteoarthritis. Joint stiffness defined as the ratio of the joint torque change to the angular displacement change. Eight subjects with knee osteoarthritis participated to this study. The subject walked on a 12 m long and 1 m wide walkway with three sets of four different randomly ordered gait speeds, ranging from their self-selected speed to maximum speed. Kinetic and kinematic data were measured using three force plates and an optical marker system, respectively. Joint torques of lower limb joints calculated by a multi-segment inverse dynamics model. Total leg and each lower limb joint had constant stiffness during single support phase. The leg and hip joint stiffness increased with gait speed. The correlation between knee joint angles and torques had significant changed by the degree of severity of knee osteoarthritis.

  • The aim of this study was to analyze the characteristics of knee joint sound in frequency domain and classify the knee joint diseases. The spectral analysis of knee joint sounds was performed using LPC(Linear Predictive Coding) and Wigner-Ville distribution. Ten normal subjects and 5 patients with meniscal tearing were enrolled. Each subject was seated on a chair and underwent active knee flexion and extension for 60 seconds. Sampling frequency was 10kHz and electronic stethoscope and electro-goniometer were applied during the knee motion for data collection. The spectral analysis showed 3 peaks in both groups and the difference energy distribution in time-frequency domain. These results suggest that the diagnosis of knee joint pathology using the auscultation could be easier and more correct.

Effect of Motion Taping in a Rehabilitation Exercise Program on Quadricep Muscle Activity and WOMAC (Pain, Stiffness, Physical Function) in Elderly People with Knee Osteoarthritis

  • Purpose: This study was conducted to determine the effects of motion taping on muscle activity and the WOMAC in a rehabilitation exercise program for elderly women with knee joint osteoarthritis. Methods: The subjects were 28 elderly women aged over 65 years with knee joint osteoarthritis. The subjects were divided into two groups: one experimental and one control group. During the study period, a total of 26 patients completed the experiment, with one drop each from the experimental group and control group. The experimental group applied motion taping and conducted a rehabilitation exercise program. The control group experienced a rehabilitation exercise program without motion taping. The rehabilitation exercise program consisted of warm-up exercises, maximum isometric exercises, the range of motion of the joints, and leg stretching exercises. The intervention was conducted three times a week for six weeks. To investigate the effects of the intervention, muscle activity and the WOMAC were measured. The WOMAC is a tool that can be used to evaluate the pain, stiffness, and physical function of osteoarthritis patients; it has 29 items in three areas. Results: The change in muscle activity according to the intervention showed a statistically significant increase in both the experimental group and control group. The WOMAC also showed statistically significant changes in terms of pain, stiffness, and physical function in both the experimental and control groups. The experimental group showed a greater functional improvement than the control group. Conclusion: For older women with osteoarthritis of the knee, a rehabilitation exercise program is a good intervention. When motion-taping is applied, it is considered to be an intervention program that can be expected to have a better effect on knee joint osteoarthritis.

무릎관절 전치환술에 따른 하지의 관절 각도 변화 (Changes in Lower Extremity Joint Angles after Total Knee Replacement)

  • Purpose : The purpose of present study is to evaluate the joint angles of legs in the standing posture for six patients with unilateral knee osteoarthritis. Methods : The participants underwent unilateral total knee replacement. A motion analysis was used to measure the joint angles of the hip, knee, and ankle. The measurements were taken before the surgery, one week and two weeks after the surgery. Both sides of the legs were evaluated. Results : This result showed that after a certain healing period, both hip joint angles showed a significant difference while there was no significant difference in the knee and ankle joint angles. After surgery, the angle of ankle dorsiflexion was smaller on the operated side than the opposite side compared to the pre-surgery measurements. Conclusion : After surgery, the asymmetry in a standing position left unchanged due to contracture of the knee joint and tightness in the hamstring muscle. Therefore, when physiotherapists plan an initial exercise programs for TKR patients, it is essential to apply adequate exercises which consider the contracture of the leg joints.

Pedicled Perforator Flaps for Reconstruction of Bilateral Knee Defects: A Case Report

  • Reconstruction of soft tissue defects of the knee has always been a challenging task for plastic surgeons. Various reconstructive choices are available depending on the location, size, and depth of the defect relative to the knee joint. Defects on the knee joint have several characteristic features. The use of a free flap is preferred for reconstructions involving obliteration of large-cavity defects, but recipient pedicle isolation can be difficult because of the extent of the injury zone. Furthermore, the true defect during knee joint flexion is larger than during knee joint extension, and a durable flap is necessary for joint movement. We report for the first time on the use of pedicled perforator flaps for reconstruction of bilateral knee defects in a 76-year-old woman. The operative procedure required skeletonizing the perforators of an antero-lateral thigh flap and antero-medial thigh flap and rotating the flap in the defect. The patient returned to normal daily activity and had a full range of motion two months after the accident. The shorter operating time with decreased donor site morbidity and its durability make this flap a valuable alternative for soft tissue reconstruction of the knee.

몸통 운동의 제약이 최대 수직점프의 수행에 미치는 영향 (The Effects of Restricted Trunk Motion on the Performance of Maximum Vertical Jump)

  • 본 연구에서는 수직점프시 몸통운동의 제약이 수직점프의 수행에 미치는 영향을 알아보기 위해 10명의 성인 남성을 대상으로 몸통을 자유롭게 사용한 일반적인 형태와 몸통동작을 제한한 형태의 수직점프를 비교, 분석하였다. 분석 결과 몸통동작을 제한한 경우 비 제약점프에 비해 10% 정도의 수행 손실이 있었는데, 이는 이지 순간의 중심 높이보다는 이 지속도의 차이에 의해 발생하였다. 몸통의 운동은 엉덩관절의 모멘트 파워를 증가시켜 추진의 초기 시점부터 지면반력의 증가에 기여하였는데 이러한 요인이 수행력 증가의 주요한 요인으로 작용하였다. 반면 몸통운동이 제한된 점프에서는 엉덩관절에서의 역학적 출력의 감소를 보상하는 측면에서 무릎관절의 역할이 증가하였으나 충분치 못하였다. 또한 몸통동작의 제한으로 엉덩관절 무릎관절 발목관절의 순차적인 신전패턴과는 상이하게 추진시점 직후 엉덩관절과 무릎관절이 동시에 신전하는 형태의 점프가 이루어져 협응패턴의 변화가 나타났다. 결국 본 연구의 결과 몸통의 적절한 사용은 수직점프의 수행 향상에 효과적으로 기여하는 것으로 나타났다.

The Effects of Visual Biofeedback Information on Hyperextended Knee Control

  • Purpose: A hyperextended knee is described as knee pain associated with an impaired knee extensor mechanism. Additionally, a hyperextended knee may involve reduced position sense of the knee joint that decreases the individual's ability to control end-range knee extension movement. The purpose of this study was to investigate the effects of visual biofeedback information for plantar pressure distribution on knee joint angle and lower extremity muscle activities in participants with hyperextended knees. Methods: Twenty-three participants with hyperextended knees were recruited for the study. Surface electromyography signals were recorded for the biceps femoris, rectus femoris, gastrocnemius, and tibialis anterior muscle activities. The plantar pressure distribution was displayed and measured using a pressure distribution measuring plate. Knee joint angle kinematic parameters were recorded using a motion analysis system. The visual biofeedback condition was the point at which the difference between the forefoot and backfoot plantar foot pressure on the monitor was minimized. The Wilcoxon signed-rank test was used to determine the significance between the visual biofeedback condition and the preferred condition. Results: The knee joint angle was significantly decreased in the visual biofeedback condition compared to that in the preferred condition (p<0.05). The rectus femoris and gastrocnemius muscle activities were significantly different between the visual biofeedback and preferred conditions (p<0.05). Conclusion: The results of this study showed that visual biofeedback of information about plantar pressure distribution is effective for correcting hyperextended knees.
