  • Degloving injuries result from the tangential force against the skin surface, with resultant separation of the skin and the subcutaneous tissue from the rigid underlying muscle and fascia. These injuries are associated with extensive soft tissue loss and occasionally with exposure of bone, and they require reconstructive modality for resurfacing and successful rehabilitation that considers the vascular anatomy and the timing of the operation. A 19-year-old male patient was transferred to our facility with degloving injury extending from the lower third of the right thigh to the malleolar area. The tibial bone was exposed to a size of $2{\times}3.5cm^2$ on the upper third of the lower leg at the posttraumatic third day. The exposed soft tissue was healthy, and the patient did not have any other associated disease. At the posttraumatic sixth day, one-stage resurfacing was performed with a medial gastrocnemius muscle flap transposition for the denuded bone and a split-thickness skin graft for the entire raw surface. The transposed gastrocnemius muscle attained its anatomical shape quickly, and the operating time was relatively short. No transfusion was needed. This early reconstruction prevented the accumulation of chronic granulation tissue, which leads to contracture of the wound and joint. The early correction of the gastrocnemius muscle flap transposition made early rehabilitation possible, and the patient recovered a nearly full range of motion at the injured knee joint. The leg contour was almost symmetric at one month postoperatively.

  • Objective: This study aimed to investigate how the movements occurring during the follow-through phase after releasing an arrow among elite compound archers, are associated with the arrow impact points on the target. Method: Nine elite archers performed consecutive compound archery shooting under conditions identical to actual competitions using their own bows and equipment. Motion capture system and force platform were utilized to record the changes in joint positions and center of pressure, respectively. Principal component analysis was employed to identify the patterns in which multidimensional joint positions and COP changes were organized with horizontal and vertical coordinates of arrow impact points. Subsequently, correlation analysis quantified the relationship between individual variables and the coordinates of arrow impacts on the target. Results: We found a common organizational pattern in which the two axes of the impact point coordinates were grouped into the first two principal components. The movements of the upper and lower limbs following release exhibited opposite patterns in the anterior-posterior axis, with significant correlations observed between the arrow impact points of the horizontal axis and the left shoulder, right elbow, left hip, and both knees. Additionally, the lateral movements induced by the reaction force upon arrow release showed significant associations with the vertical coordinates of the impact points. Particularly, the correlations between the movements of the left shoulder and elbow, as well as the bilateral hip and right knee, were consistently observed among all participants. Conclusion: These findings implied that the post-release movements could significantly influence the trajectory and impact points of the arrows in compound archery. We suggest that a consistent and controlled movement during the follow-through phase may be more beneficial for optimizing shooting accuracy and precision rather than minimizing movements.

  • Objective: The purpose of this study was to investigate kinematic comparisons of Tsukahara vault in gymnastics between a top-level athlete and sublevel collegiate athletes in order to obtain information on key biomechanical points for successful Tsukahara vaults. Methods: An Olympic gold medalist (height, 160 cm; weight, 52 kg; age, 25 years) and five sublevel collegiate gymnasts (height, $168.2{\pm}3.4cm$; weight, $59.6{\pm}3.1kg$; age, $23.2{\pm}1.6years$) participated in this study. They repeatedly performed Tsukahara vaults including one somersault. Fourteen motion-capturing cameras were used to collect the trajectories of 26 body markers during Tsukahara vaults. Event time, displacement and velocity of the center of mass, joint angles, the distance between the two hands on the horse, and averaged horizontal and vertical impact forces were calculated and compared. Results: The top-level athlete showed a larger range of motion (ROM) of the hip and knee joints compared to sublevel collegiate athletes during board contact. During horse contact, the top-level athlete had a narrow distance between the two hands with extended elbows and shoulders in order to produce a strong blocking force from the horse with a shorter contact time. At the moment of horse take-off, reactive hip extension of the top-level athlete enhanced propulsive take-off velocity and hip posture during post-flight phase. Conclusion: Even though a high velocity of the center of mass is important, the posture and interactive action during horse contact is crucial to post-flight performance and the advanced performance of Tsukahara vaults.

  • This study was investigated the kinematic factors of the last approach strides and. take off motion for the skill improving of BKH elite male athlete. 'The subjects chosen for the study were BKH and. KASZCZYK Emillian male athletes who were participated in 2003 Dae-Gu Universiad Games. Three high speed video cameras set in 60frames/s setting were used. for recording from the last approach strides to the apex position. After digitizing motion, the Direct Linear Transformation(DLT) technique was employed to obtain 3-D position coordinates, The kinematic factors of the distance, velocity and angle variable were calculated for Kwon3D 3.1. The following conclusions were drawn; 1. It showed longer stride length, as well as faster horizontal and lateral velocity than the success trial during the approach phase. For consistent of the approach rhythm, it appeared that the subject should a short length for obtain the breaking force by the lower COG during the approach phase. 2. The body lean angle showed a small angle by a high COG during the take-off phase. For obtain the vertical displacement of the COG and a enough space form the bar after take-off, it appeared that the subject should increase the body lean angle. 3. For obtain the vertical force during the takeoff phase, it appeared that the subject should keep straight as possible the knee joint. Therefor, the subject can be obtain a enough breaking force at the approach landing.

  • The purpose of this study was to analyze the kinematic factors and throwing variables for the 3-turn and 4-turn techniques and for release as well as to provide technical advice for improving athletic performance in hammer throwing. Data analysis led to the following conclusions: To increase the rotation speed for the 3-turn and 4-turn techniques, the time elapsed during the 1-foot support period should be decreased the distance between the rotating foot and the rotation axis should be small and the height of the hip joint should be increased at the times of release The throwing angle at the moment of release should be more than 40 degrees, and the throwing position should be taken vertically high at the shoulder joints. To accelerate the motion of the hammer, the speed should not be reduced during the 1-foot support period but should be increased during the 2-foot support period for much greater acceleration. In the 3-turn technique, the angles of the shoulder axis and hummer string should be dragged angle at the maximum point and lead angle at the minimum point, and dragged angle at the maximum and minimum points in the 4-turn at the time of relase The upper body should be quickly bent backward, the knee angle should be extended, and the angles of the shoulder axis and hammer string should be dragged angle close to 90 degrees.

  • Objective: The purpose of this study was to assess the consistency of the gliding and push-off motion for single leg skating from the first to fourteenth steps. We hypothesized that: 1) there would be no difference in stroke trajectory, step rate, and cycle rate between the left and right steps of gliding; and 2) there would be a difference in the resultant velocity of toe push-off and the horizontal velocity of the center of mass after six step push-offs. Method: The study included five male 500-m speed skaters (mean height, $1.80{\pm}0.02m$; mean weight, $76.8{\pm}3.96kg$; record, $35.83{\pm}0.30sec$; 100-m record, <9.97 sec). Data were collected from the first to fourteenth steps (40 m) and recorded using five digital JVC GR-HD1KR video cameras (Victor Co., Japan) operating at a sampling frequency of 60 fields/sec and shutter speed of 1/500 sec. For each film frame, the joint positions were digitized using the KWON3D motion analyzer. Position data were filtered with low-pass Butterworth $4^{th}$ order at the cut-off frequency of 7.4 Hz. Results: The right toe of the skating trajectories at $2^{nd}$, $5^{th}$, and $7^{th}$ strokes differed from those of the left toe. The angles of the right and left knee demonstrated unbalanced patterns from the flexion and extension legs. The step and cycle rates of the right and left leg differed from the start until 20 m. The resultant velocities of the toe at the push-off phase and of the body mass center diverged before the six push-offs. Conclusion: This study's findings indicate that the toe of skating trajectory on left and right sliding after push-off should maintain a symmetrical trajectory. The resultant velocity of toe push-off and horizontal velocity from the center of body need to be separated after about six step push-offs.

  • This study aimed to determine the effects of the blockage of visual feedback on joint dynamics of the lower extremity. Fifteen healthy male subjects(age: $24.1{\pm}2.3\;yr$, height: $178.7{\pm}5.2\;cm$, weight: $73.6{\pm}6.6\;kg$) participated in this study. Each subject performed single-legged landing from a 45 cm-platform with the eyes open or closed. During the landing performance, three-dimensional kinematics of the lower extremity and ground reaction force(GRF) were recorded using a 8 infrared camera motion analysis system (Vicon MX-F20, Oxford Metric Ltd, Oxford, UK) with a force platform(ORG-6, AMTI, Watertown, MA). The results showed that at 50 ms prior to foot contact and at the time of foot contact, ankle plantar-flexion angle was smaller(p<.05) but the knee joint valgus and the hip flexion angles were greater with the eyes closed as compared to with the eyes open(p<.05). An increase in anterior GRF was observed during single-legged landing with the eyes closed as compared to with the eyes open(p<.05). Time to peak GRF in the medial, vertical and posterior directions occurred significantly earlier when the eyes were closed as compared to when the eyes were open(p<.05). Landing with the eyes closed resulted in a higher peak vertical loading rate(p<.05). In addition, the shock-absorbing power decreased at the ankle joint(p<.05) but increased at the hip joints when landing with the eyes closed(p<.05). When the eyes were closed, landing could be characterized by a less plantarflexed ankle joint and more flexed hip joint, with a faster time to peak GRF. These results imply that subjects are able to adapt the control of landing to different feedback conditions. Therefore, we suggest that training programs be introduced to reduce these injury risk factors.

  • The purpose of this study was to investigate the effect of combined exercise on injury risk factors of lower extremity during landing. Ten sports talented athletes participated in this study. Sports talented athletes participated in a combined exercise (sports talented exercise, coordination) for 16 weeks. A three-dimensional motion analysis was performed using eight infrared cameras (sampling rate of 100 Hz), one force plate, and electromyography system (sampling rate of 1000 Hz) during landing. Kinetic, and kinematics analysis including average impulsive force, angle of lower extremity, vertical stiffness, onset of muscle activation were calculated by Matlab2009a software. Paired t-test was performed at alpha=.05. The average impulsive force in landing phase was not statistically significant (t=-.748, p=.474). The hip joint angle was more decreased in post test compared to pre test (E1: t=2.682, p=.025, E2: t=5.609, p=.000, E3: t=2.538, p=.032). The knee joint (E1: t=-.343, p=.739, E2: t=1.319, p=.220, E3: t=.589, p=.570) and ankle joint (E1: t=.081, p=.937, E2: t=.784, p=.453, E3: t=.392, p=.704) angle were tended to decrease after combined exercise. The vertical stiffness was tended to decrease after combined exercise (t=1.972, p=.080). Onset of quadriceps femoris (t=.698, p=.503) and medial gastocnemius (t=1.858, p=.096) were tended to be faster than biceps femoris (t=-.333, p=.747) after combined exercise. Although thses findings were not statistically significant except on a hip joint angle, risk factors of lower extremity such as joint angle, vertical stiffness and onset of quadriceps femoris, medial gastrocnemius were positively changed after the combined exercise but an additional training for improved onset of biceps femoris would be required in the future.

  • Objectives : To determine whether the pragmatic acupuncture treatment provides more effective pain relief than treatment using the same acupuncture point to the all patients. Methods : We randomly allocated participants to treatment group 1 and 2. The group 1 is the pragmatic treatment group and the group 2 is using the same acupuncture point to the all patients. Primary outcomes were measured by the Western Ontario and McMaster Universities Osteoarthritis index(WOMAC) pain and function scores at 4, 8, and 14 weeks. Secondary outcomes were measured by 100mm VAS(Visual Analog Scale), ROM(Range of Motion) using Goniometer, and pain threshold using pressure algometer. Results : When patients were extension of the knee, they were statistically significant in improvement of the ROM in 14 weeks. Whole body condition and pain rate through VAS measurement were improved significantly in 14 weeks. Also pain score and function score of WOMAC were improved significantly in 14 weeks. We could get difference in pain score of two acupuncture groups significantly in 14 weeks. But we could not get difference in whole score of two acupuncture groups significantly. Local temperature using T.C thermometer was changed significantly in 14 weeks. But we could not get difference in whole score of two acupuncture groups significantly. Excluding above item, DITI, pain threshold, and ROM of the knee flexion were no difference in before and after treatment.
