Effects of Visual Information on Joint Angular Velocity of Trunk and Lower Extremities in Sitting and Squat Motion

  • Purpose: The purpose of this study is to determine the effects of visual information on movement time and each angular velocity of trunk and lower extremity joints while healthy adults are in sitting and squat motion. Methods: Participants consisted of 20 healthy male and female adults; movement time and each angular velocity of trunk, pelvis, hip, knee and ankle of sitting and squat motion according to common vision, visual task and visual block were analyzed using a three dimensional motion analysis system. Results: Each angular velocity of the trunk, pelvis, hip, knee and ankle in phase 2 of the sitting showed significant difference according to the types of visual information (p<0.05). Movement time and each angular velocity of pelvis and hip in phase 2 of squat motion showed significant difference according to the types of visual information (p<0.05). According to the common vision, each angular velocity of knee and ankle in phase 1 was significantly fast in sitting (p<0.05). According to the common vision, each angular velocity of trunk, pelvis, hip, knee, and ankle in phase 2 was significantly fast in sitting (p<0.05). Conclusion: Visual information affects the angular velocity of the motion in a simple action such as sitting, and that in more complicated squat motion affects both the angular velocity and the movement time. In addition, according to the common vision, visual task and visual block, as angular velocities of all joints were faster in sitting than squat motion.

비복 동맥 피판을 이용한 슬관절 주위의 연부 조직 결손의 치료 (Sural Artery Flap for the Treatment of Soft Tissue Defects around Knee Joint)

  • Purpose: This report presents the authors' experience of twelve patients with sural artery flap for soft tissue defects around the knee joint. Materials and Methods: The patients' age ranged from 25 to 80 years; seven of the patients were male and five were female. The cause of soft-tissue defects involved wide excision for malignant soft tissue tumor, tumor prosthesis related infection, infection after total knee arthroplasty and chronic osteomyelitis. Postoperative range of motion was checked. The sensibility of flap was evaluated by Semmes-Weinstein monofilaments and two-point discrimination. Results: All flaps survived and provided satisfactory coverage of the defect. There was no complication except one delayed skin graft incorporation at donor site. Seven knee joints which had been stiff previously gained average 58 degrees of ROM postoperatively. All flaps retained sensibility and showed no significant increase in sensory thresholds comparing with contralateral side. Conclusion: Sural artery flap not only shows high survival rate and broad coverage ability, but also offers improvement in range of motion and preservation of sensation. We speculate that sural artery flap is valuable for the reconstruction of the soft tissue defects around knee joint.

퇴행성 슬관절염 환자에서 슬관절방(膝關節方) 1호(號)의 유효성 연구 (The Clinical Study on the Effect of Sulguanjul-bang No.1 on Osteoarthritis of Knee Joint)

  • Objective : This study is designed to find out the effects of Sulguanjul-Bang No.1 on osteoarthritis in knee joint. Methods : 45 patients with symptomatic osteoarthritis of the knee joint were enrolled in a short term 8-week open clinical trial. The 45 patients were taken Sulguanjul-bang No.1 two times a day before meals for 8 week. After 8 week treatment period, the following parameters were analyzed ; Visual analogue Scale(VAS) as first effective parameter, Lysholm index score, patient's global assessment, passive movement range of the knee joint. Results : Sulguanjul-bang No.1 treatment led to significant improvement in the pain and symptoms of osteoarthritis as determined by all efficacy measures. After 8 weeks of therapy, there was significant improvement in VAS and Lysholm index; squat, sit down and up, crepitaion, swelling. The range of motion was improved in 6 of 9 cases. The improvement in patient's global assessment was seen in 84%. Conclusion : Sulguanjul-bang No.1 was very beneficial effect in pain relief and improvement functions on osteoarthritis.

Three-Dimensional Contact Dynamic Model of the Human Knee Joint During Walking

  • It is well known that the geometry of the articular surface has a major role in determining the position of articular contact and the lines of action for the contact forces. The contact force calculation of the knee joint under the effect of sliding and rolling is one of the most challenging issues in this field. We present a 3-D human knee joint model including sliding and rolling motions and major ligaments to calculate the lateral and medial condyle contact forces from the recovered total internal reaction force using inverse dynamic contact modeling and the Least-Square method. As results, it is believed that the patella, muscles and tendon affect a lot for the internal reaction forces at the initial heel contact stage. With increasing flexion angles during gait, the decreasing contact area is progressively shifted to the posterior direction on the tibia plateau. In addition, the medial side contact force is larger than the lateral side contact force in the knee joint during normal human walking. The total internal forces of the knee joint are reasonable compared to previous studies.

근골격 모델을 이용한 대퇴절단환자의 계단보행에 대한 동역학 해석 (Dynamic Analysis of Stair Climbing for the Above-knee Amputee with Musculoskeletal Models)

  • It is important to understand the characteristics of amputee gait to develop more advanced prostheses. The aim of this study was quantitatively to analyze the stair climbing task for the above-knee amputee with a prosthesis and to predict muscle forces and joint moments at musculoskeletal joints by dynamic analysis. The three-dimensional musculoskeletal model of lower extremities was constructed by gait analysis and transformation software for one above-knee amputee and ten healthy people. The measured ground reaction forces and kinematical data of each joint by gait analysis were used as input data during inverse dynamic analysis. Lastly, dynamic analysis of above-knee amputee during stair climbing were performed using musculoskeletal models. The results showed that summed muscle farces of hip extensor of amputated leg were greater than those of sound leg but the opposite results were revealed at hip abductor and knee flexor of amputated leg. We could also find that the higher moments at hip and knee joint of sound leg were needed to overcome the flexion moment caused by body weight and amputated leg. In conclusion, dynamic analysis using musculoskeletal models may be a useful mean to predict muscle forces and joint moments for specific motion tasks related to rehacilitation therapy..

스탠스 유형에 따른 테니스 포핸드 스트로크의 하지관절각도와 회전각도 분석 (Analysis of Lower Limb Joint Angle and Rotation Angle of Tennis Forehand Stroke by Stance Pattern)

  • The purpose of this study was to analyze the kinematics variables of during forehand stroke by stance patterns. Eight high school tennis players were chosen for the study, who have never been injured for last six months, in Busan. They performed horizontal swing and vertical swing that it was done each five consecutive trial in the condition of square, open and semi-open stance. It was filmed by 6 video camera and used with 3-dimensional motion analyzer system. The following kinematic variables were analyzed in relation to angle of segment( shoulder, hip and knee joint). The conclusion were as follow: 1. The angle of hip joint represented at impact that horizontal swing was not significant difference by stance patterns but vertical swing was increased in open stance than square and semi-open stance. 2. The angle of both knee was not significant difference between all stance types and swing patterns. 3. The angle of shoulder, hip and knee joint rotation showed that open stance was increased than square and semi-open stance in all swing types and event.

한방 치료를 병행한 슬관절 재활 CPM 증례보고 3례 (Case Report : 3 Cases of CPM Combined with Korean Medical Treatment for Knee Joint Rehabilitation)

  • Objectives : The aim of this study was to report the cases of knee injury that are treated with both CPM and Korean medicine. Methods : We treated 3 patients who had knee joint pain and gait disturbance by knee injuries with Korean medicine and CPM treatment. We evaluated the outcomes by checking NRS, ROM, Lysholm knee score, walking state and other symptoms. Results : The NRS, ROM, Lysholm knee score, and other symptoms of 3 patients were improved before they. These results show possibilities of simultaneous treatment of Korean medicine and CPM. But there is a limit that the number of cases is not sufficient, and there was no long-term follow-up after discharge. Conclusions : Simultaneous treatment of Korean medicine and CPM has an effect on knee joint rehabilitation. Further studies are needed to prove the effectiveness of Korean medicine combined with CPM.

슬관절 전치환술 후 세라밴드 운동프로그램이 수술 후 통증, 슬관절 굴곡각도 및 심리적 지수에 미치는 효과 (Effects of a Thera-Band Exercise Program on Pain, Knee Flexion ROM, and Psychological Parameters Following Total Knee Arthroplasty)

  • Purpose: The purpose of the study was to investigate the effects of the Thera-Band exercise program following total knee arthroplasty. Methods: The research design for this study was a nonequivalent control group non-synchronized design. Participants were 30 patients for the experimental group and 30 patients for the control group. The experimental group participated in the Thera-Band exercise program in addition to conventional CPM (continuous passive motion) exercise. The control group received conventional CPM exercise only. Outcome measures were pain, knee flexion range of motion, CRP, and psychological parameters (self-efficacy and fear of falling). Data were analyzed using ${\chi}^2$-test, Fisher's exact test, t-test, and repeated measure ANOVA with SPSS/PC version 21.0. Results: There were significant improvement in self-efficacy, and decreases in pain, and fear of falling in the experimental group compared to the control group. However, no significant differences were found between the two groups for CRP and knee flexion ROM. Conclusion: The Thera-Band exercise program gave an additional benefit over the conventional CPM exercise for patients following total knee arthroplasty, and is recommended for use as an effective nursing intervention for patients after total knee arthroplasty.

Change in Kinetics and Kinematics during 1-Footed Drop Landing with an Increase in Upper Body Weight

  • The purpose of this study was to investigate changes in kinetic and kinematic variables associated with an increase in upper body weight. Eighteen healthy male university students($175.96{\pm}4.19\;cm$, $70.79{\pm}8.26\;kg$) participated. Eight motion analysis cameras(Qualysis Oqus 500) and 2 force AMTI platforms(Advanced Mechanical Technologies Inc. OR6-7, US) were used to record motion and forces during the drop landing at a frequency of 120 Hz and 1200 Hz, respectively. QTM software(Qualisys Track Manager) was used to record the data, and the variables were analyzed with Visual 3D and Matlab 2009. For the drop landing, a box of $4{\times}2{\times}0.46\;m$ was constructed from wood. Knee and ankle maximum flexion angle, knee flexion angle, knee and ankle angle at landing, time for maximum ankle flexion after landing, and time for maximum knee flexion after landing were calculated. There was a significant change in the time for maximum and minimum ground force reaction and the time for maximum dorsal flexion after landing(p<.05) with increasing weight. There was no significant change for the hip, knee, and ankle ROM, whereas there was an increase in the angle ROM as the weight increased, in the order of ankle, knee, and hip ROM. This result shows that the ankle joint ROM increased with increasing weight for shock attenuation during the drop landing. There was a trend for greater ankle ROM than knee ROM, but there was no clear change in the ROM of the hip joint with increasing weight. In conclusion, this study shows the importance of ankle joint flexibility and strength for safe drop landing.

노인여성의 계단 내려오기 시 무릎 관절 테이핑이 슬개대퇴 압박력에 미치는 영향에 관한 연구 (Study of Effects on Taping of Knee Joint for Patellofemoral Compressive Force During Stair Descent in Elderly Women)

  • The purpose of this study was to investigate the effect of taping on knee joint for patellofemoral compressive force (PCF) during stair descent for elderly women. Ten healthy elderly women voluntarily participated in this study. A three-dimensional motion analysis system and force plates were used to analyze the movements of the joints for the lower extremities. The results were as follows: There were no significant differences for the maximum PCF, maximum quadriceps contraction force and maximum knee extension moment (p>.05) but, there was a pattern decreasing all values with the taping during stair descent. There were significant differences for the knee and ankle angle on the event of maximum PCF (p<.05) and there was a pattern decreasing all values with the taping during stair descent. Therefore, taping on the knee would be effective to relieve the pain like patellofemoral pain syndrome in the knee joint.
