The purpose of this study was to find the relationship between standing posture biomechanics and physical fitness in the elderly. Physical fitness variables and postural variables for 227 (140 women and 87 men) elderly individuals were tested. Physical fitness tests (Korean Institute of Sports Science, 2012) included 3m sit, walk, and return, grip test, 30 second chair sit and stand, sit and reach, figure 8 walks, and 2 minute stationary march. Postural biomechanics variables included resting calcaneal stance position (RCSP), shoulder slope, pelvic slope, knee flexion angle, leg length difference, thoracic angle, and upper body slope. In statistical analysis, multiple regression was conducted by using stepwise selection method via SAS (version 9.2). Analysis for both men and women revealed significant relationships between physical fitness and age, upper body slope, knee flexion angle, leg length difference. Pelvic and thoracic angle were only related to figure 8 walking and sit and reach in women, while RCSP and shoulder slope had no relationship with any physical fitness variables.
Background: Total knee arthroplasty (TKA) recovers the alignment of the knee joint, but fails to automatically restore the alignment and function of the hip and ankle joints. It may affect the alignment and stability of the knee joint, therefore therapeutic intervention in hip and ankle joint is necessary for the rehabilitation process after TKA. Objects: The aim of this study was to comparison of the effects of the two exercise methods on the coronal plane alignment after TKA. This study conducted an experiment by dividing subjects into a lower extremity isometric co-contraction group (LEIC) and a quadriceps isolated isometric contraction (QIIC) group. Methods: A total of 37 subjects were randomly assigned to the LEIC ($n_1$=19) or the QIIC ($n_2$=18). Exercise was applied to five times per week for three weeks, starting on the eighth day after surgery. Range of motion exercises were performed as a common intervention and then each group performed quadriceps isometric contraction exercises with 10 sets of 5 repetitions. Radiological imaging was performed prior to surgery, one month and six months after surgery. In addition, the hip-knee-ankle angle (HKA) and tibiotalar angle (TTA) were measured. Results: The HKA was close to neutral in the LEIC rather than the QIIC (p<.05). The LEIC showed varus and the QIIC exhibited valgus TTA (p<.05). In a comparison of HKA and TTA over time, there was no significant change in either group (p>.05). According to the comparison of the TTA before surgery, the LEIC showed significant changes in the varus direction (p<.05), while there was no significant change in the QIIC (p>.05). Conclusion: The LEIC method triggered changes in the TTA and brought the HKA close to the neutral. Thus, LEIC is more effective than QIIC in creating stability in the coronal plane alignment of the knee and ankle joints after TKA.

Purpose: To evaluate the reliability and validity of the femorotibial mechanical axis angle from radiographs in the weight bearing (WB) and supine positions compared with navigation-measured values. Materials and Methods: Sixty-eight cases of navigation-assisted total knee arthroplasty (TKA) were included. The pre- and postoperative whole leg radiographs (WLRs) in WB and supine positions were compared with the initial and final navigation values. Results: The mean mechanical axis angle from the preoperative WBWLR and navigation were not statistically different (p=0.079) and were correlated strongly with each other (intraclass correlation [ICC], 0.818). However, on postoperative measurements, although the WBWLR and navigation values were not different (p=0.098), they were not correlated with each other (ICC, 0.093). The standard error of measurement was $1.8^{\circ}{\pm}3.6^{\circ}$ for the preoperative WBWLR and $2.5^{\circ}{\pm}4.8^{\circ}$ for the postoperative WBWLR. The validity that was determined by the Bland-Altman plot was not acceptable for both pre- and postoperative measurements. Conclusions: The preoperative WBWLR could provide accurate but not precise measurement value of the femorotibial mechanical axis angle for navigation-assisted TKA, and postoperative measurements in navigation were not comparable with radiographic measurements. The lack of agreement was found between the radiographic and navigation measurements of the coronal alignment regardless of pre- or postoperative evaluation although the accuracy was found acceptable. 무릎 과다 폄을 가진 대상자에게 비대칭적 선 자세와 같은 외부적 요소가 추가되게 되면 무릎 관절에 가해지는 체중 지지가 더 증가하게 되며, 무릎 뒤쪽 구조들의 과신장이 유발된다. 이는 무릎 과다 폄 변형을 증가시킬 뿐만 아니라, 앞 십자인대와 같은 무릎 주변 연부조직의 변형 및 손상을 유발하는 요인이 될 수 있다. 따라서, 비대칭적 선 자세가 무릎 관절의 위치와 하지 근육의 활성도에 어떠한 영향을 미치는지 알아보는 것이 이 연구의 목적이다. 33명의 자발적 참여대상자들이 연구에 참여하였으며 총 60개의 다리에 대하여 무릎 과다 폄군, 정상군으로 나누어 하지근육의 근 활성도 및 무릎 폄 각도를 측정하였다. 비대칭적 선 자세 시, 무릎 관절 폄 각도와 장딴지근의 근 활성도 값은 정상군과 무릎 과다 폄 군 사이의 통계학적 유의한 차이가 나타났다. 결과를 토대로 비대칭적인 선 자세가 무릎의 과다 폄을 증가시키는 요인이 됨을 확인하였다. 따라서, 무릎 정렬에 이상을 가진 대상자들에게는 정상적인 정렬 회복과 함께 대칭적인 선 자세를 유지할 수 있도록 하는 치료적 중재 수립에 도움을 줄 것이며, 추후 엉덩관절과 발목관절의 영향을 포함한 조사연구가 필요하겠다.
The quadriceps angle (Q angle) has been used to reflect the quadriceps muscle's force on the patella in the frontal plane. Previous investigations of the Q angle and it's relationship to knee disorders have yield equivocal results. The purpose of this study was to analyze the factors related to the Q angle and it's relation to other variables such as leg length, body weight, CTA (calcaneus to tibia angle), TOA (toe out angle), and pelvic width in normal subjects. The participants were 60 students (30 men and 30 women) who had no orthopedic and neurological impairments, aged from 20 to 29 years of age, with an average age of 22.1 years. Prior to participation, each subject was informed of the procedures of the experiment from a researcher and assistant researchers. The equipment used in this study were modified standard goniometer, ruler, marking pen, and Martin apparatus for pelvic width. In order to determine the statistical significance of the experiment, regression analysis, independent t-test, and Pearson correlation were used at the 0.05 level. The results were as follows: 1) It was found that the Q angle of women is greater than that of men's from both knees. 2) There was no significant difference between right and left quadriceps angle. 3) The Q angle decreased as the body weight (leg length) shifted from low to high. 4) It seems that factors related to the Q angle were body weight, CTA, and pelvic width, but there was no significant difference at the 0.05 level.
The purpose of this study was to identify the saddle to pedal length contributing to successful performance in kayak using a kayak ergometer. Ten male elite kayak players participated in this study. players were tested on the kayak ergometer which was varied saddle to pedal length by the knee flexion angle(90deg; 120deg; 150deg) to measure stroke frequency, paddling amplitude, joint angle, RoM and angular velocity, foot pressure and force, iEMG using the 3D motion system, foot pressure system and EMG wireless system. At a results, rowing at 120deg on knee flexion angle showed higher stroke frequency and paddling amplitude than other knee flexion angles. RoM at upper extremity showed not significant difference between knee flexion angles. But there were significant differences in thorax and pelvis rotation RoM, knee flexion-extension RoM in each condition. In addition, foot pressure, force and iEMG were significantly different in knee flexion angles. Study showed that changed of saddle to pedal length affected rowing performance kinds of stroke frequency, paddling amplitude. The most important thing, increased range of motion in pelvic and thorax has occurred by force that generated foot-bar to seat. Not only that, but it seems to be attributed to a technical adaptation developed to maximum rowing performance.
Purpose:This study was designed to investigate difference in isokinetic muscle strength in the knee extensor muscle and characteristic differences in muscle strength between males and females through the ankle joint angles. Methods: Seventy-four subjects participated in this study. There were two groups: 36 males and 38 females. The mean age of the men was 24.58 years and women was 23.74 years. Subjects were seated on a CON-TREX LP (leg press) lean to back of chair, and there bodies were fixed by straps with the hip joint at an angle of $130^{\circ}$. After randomly fixing the ankle joint at $0^{\circ}$, $20^{\circ}$, and $40^{\circ}$ of plantar flexion (PF) in range of full extension of knee joint. We studied force max average, force max average/kg, power average, and total work through the angle of the ankle joint when the knee joint was extended from $90^{\circ}$ to $180^{\circ}$. Results: In the male group, all maximum measured value showed at the ankle joint $0^{\circ}$, all minimum measured value showed at $40^{\circ}$ PF (p<0.01). In the female group, all maximum measured value showed at the ankle joint $20^{\circ}$ PF, especially the power average increased significantly. All minimum measured value showed $40^{\circ}$ PF (p<0.01). Conclusion: There are differences between males and females in isokinetic muscle strength of the knee extensor through ankle joint angles in healthy adults. Males and Females have different characteristics of muscle strength through the ankle joint angles.
Purpose: This study examined the effects of squatting with different foot positions on the muscle activation of the vastus medialis oblique (VMO) and vastus lateralis (VL) muscles in subjects with genu varum. Methods: Thirty four subjects participated in this study. Surface electromyography was used to measure the muscle activation of the VMO and VL muscles at the knee angles ($15^{\circ}$, $60^{\circ}$) at three foot positions (internal rotation, neutral position, external rotation) during squatting. Results: Muscle activation differences at different knee angles and foot positions differed significantly between the VMO and VL muscles in both the varus and normal groups. In addition, there was a significant difference according to the knee angles with the foot in external rotation in the VMO and VL ratio. In the varus group, however, the VMO and VL ratio were significantly different only with the feet in internal rotation. In the muscle activation changes of the knee angle differences in the foot position, there was no significant difference in the varus group, but both the VMO and VL muscles were significant different in the normal group. Conclusion: In both groups, regardless of the foot position, muscle activation of the VMO and VL muscles increased with increasing knee flexion angle. In the normal group, when squatting with the feet in external rotation, the VMO and VL muscles activations increased with increasing knee angle. In the varus group, however, the foot position did not affect the VMO or VL muscle activation. This study shows that subjects with genu varum and normal subjects have different VMO and VL muscle activation patterns during squat exercises.
Purpose : The purpose of this study was to assess the effects of the lower extremity muscle activity on bridging exercise according to the knee joint angle. Methods : Twenty-five healthy adults volunteered to participate in this study. Subjects were required to complete following four bridging exercises; knee joint flexion $120^{\circ}$, $90^{\circ}$, $60^{\circ}$, $45^{\circ}$. Surface electromyography from selected lower extremity muscles was normalized to maximum voluntary isometric contraction. Muscle activity was measured by QEMG-4 system (LXM 3204, Laxtha Korea). A repeated measures of one-way ANOVA was used to determine the influence of bridging exercise on muscle activity for each muscle and descriptive statistics was used to determine muscle ratio. Results : The biceps femoris of all bridging exercises showed significantly(p<.05). The vastus medialis and lateralis of all bridging exercises showed significant excepted $120^{\circ}$(p<.05). The rectus femoris of all bridging exercises showed no significant. Median of vastus medialis/rectus femoris ratio of $120^{\circ}$ was 2.03, $90^{\circ}$ was 2.16, $60^{\circ}$ was 2.67, $45^{\circ}$ was 4.10. Median of vastus lateralis/rectus femoris ratio of $120^{\circ}$ was 1.70, $90^{\circ}$ was 1.70, $60^{\circ}$ was 2.08, $45^{\circ}$ was 2.58. Median of vastus medialis/vastus lateralis ratio of $120^{\circ}$ was 1.26, $90^{\circ}$ was 1.50, $60^{\circ}$ was 1.52, $45^{\circ}$ was 1.47. Conclusion : Angular motion decreasing with knee joint flexion made increase biceps femoris and vastus medialis activation. This result will be use knee joint stabilizing exercises during bridging or unstable surface training and biceps femoris strength training.
Background: Individuals with spinal cord injury (SCI) rely on their upper limbs for body-lifting activity (BLA). While studies have examined the electromyography (EMG) and kinematics of the shoulder joints during BLA, no studies have considered foot position during BLA. Objects: This study compared the effects of different foot positions during BLA on the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion in individuals with SCI. Methods: The study enrolled 13 mens with motor-complete paraplegic SCI, ASIA (American Spinal Injury Association) A or B. All subjects performed BLA with the feet positioned on the wheelchair footrest and on the floor independently. Surface EMG was used to collect data from the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii. The peak plantar pressure was measured using pedar-X and the knee flexion angle with Image J. Borg's rating perceived exertion scale was used to measure the physical activity intensity level. The paired t-test was used to compare the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion between the two feet positions during BLA. Results: The activity of the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii and rating perceived exertion decreased significantly and the peak plantar pressure and knee flexion angle increased significantly when performing BLA with the feet positioned on the wheelchair footrest compared with on the floor (p<.05). Conclusion: These findings suggest that individuals with SCI may perform BLA with the feet positioned on the wheelchair footrest for weight-relief lifting to decrease the shoulder muscle activities and the rating perceived exertion and to increase the peak plantar pressure and the knee flexion angle.
