Mercuric chloride 유발 신부전에서 산소유리기에 미치는 Sodium selenite의 영향 (Influence of sodium selenite on oxygen free radical in mercuric chloride induced renal failure)

  • Oxygen free radical have recently been found to mediate cell injury after ischemia in the kidney. The purpose of our study was to determine whether selenium had an effect on damge mediated by oxygen free radical in inorganic mercury induced renal failure, toxic model of renal failure. Toxic renal failure model was produced by subcutaneous injection of mercuric chloride (4mg/kg) once a day for 7 consecutive days. In additionally, coadministration of sodium selenite (1mg/kg) was performed by the same condition. As a consequence of this study, we were able to detect partially unequivocal role of selenium as below dipicted. The combination of sodium selenite showed that markedly inhibited production of superoxide radical in mercuric chloride alone. On the other hand, combined sodium selenite was unable to enhance against significantly lowered superoxide dismutase activity after mercuric chloride insult. However, simultaneous administration of sodium selenite was inclined to induce mitochondrial superoxide dismutase and catalase.

Deleterious Effects of Hyperoxemic Extracorporeal Circulation during Cardiovascular Surgery

  • Although extracorporeal circulation (ECC) has been routinely used for cardiovascular surgery, hyperoxemia during ECC may produce oxygen toxicity and cellular injury. This study was performed to investigate the clinical influences of hyperoxemic ECC during cardiovascular operation. 40 adult patients scheduled for elective cardiovascular surgery were classified into normoxemic (arterial oxygen tension: 115 mmHg, n=20) and hyperoxemic (arterial oxygen tension: 380 mmHg, n=20) ECC. At preoperative and postoperative period, total leukocyte and neutrophil counts, platelet counts, iron, glucose, aspartate aminotransferase (AST), alanine aminotransferase (ALT), blood urea nitrogen (BUN), and creatinine in peripheral arterial blood, malondialdehyde (MDA) and troponin-T concentration (TnT) in coronary sinus blood, pulmonary vascular resistance (PVR), and postoperative blood loss volume (BLS) were measured and compared between groups. Hyperoxemic group had postoperatively higher total leukocyte and neutrophil counts, MDA, TnT, PVR total BLS, iron, glucose, AST, ALT, BUN, and creatinine than normoxemic group (p<0.05).0 conclusion, hyperoxemic ECC results in greater inflammatory response and oxidative damaging effects on the heart lung, liver and kidney, probably being adverse to postoperative patient recovery. For reducing these deleterious effects and improving postoperative outcomes, management lowering oxygen tension during ECC is recommended.

Effect of atorvastatin on dendritic cells of tubulointerstitium in diabetic rats

  • Inflammatory reactology has become increasingly important in diabetic kidney disease. In this study, we estabilished STZ-induced diabetic rat model to investigate whether dendritic cells (DCs) mediated tubulointerstitial damages, and whether the effects by DCs were mediated by P-selectin expression and can be inhibited by atorvastatin. The study demonstrated that there was an accumulation of DCs in diabetic rats mediated by P-selectin. It also showed the accumulation of DCs and expression of P-selectin was closely correlated with the degree of renal tubulointerstitial injury. These effects were markedly attenuated by atorvastatin. Thus, DCs play a role in tubulointerstitial damages, atorvasttin can prevent renal tubulointerstitium from damage by inhibiting the P-selectin expression and DCs migration.

  • Objectives : Developmental disorder include every disorder that obstruct functional developments. For example, Mental Retardation, Autism, Developmental Academic Skill Disorder, Developmental Language Disorder, Cerebral Palsy, Tic Disorder(Tourette's Disorder), Attention Deficit Hyperactivity Disorder, Brain injury etc. Methods : Chinese medical circles study herbs, acupuncture and cooperate Western medicine for treat the Developmental disorder variety. So, we research Chinese and Korean Medical Journal from 1990 to 2003, choose the Acupuncture Therapy. Results : Acupuncture Therapy include head needling, body acupuncture, ear-acupuncture therapy, therapy of point injection. By these ways control brain, the brain's marrow, liver, kidney, heart and treat the developmental disorder effective.

Pathology of C3 Glomerulopathy

  • C3 glomerulopathy is a renal disorder involving dysregulation of alternative pathway complement activation. In most instances, a membranoproliferative pattern of glomerular injury with a prevalence of C3 deposition is observed by immunofluorescence microscopy. Dense deposit disease (DDD) and C3 glomerulonephritis (C3GN) are subclasses of C3 glomerulopathy that are distinguishable by electron microscopy. Highly electron-dense transformation of glomerular basement membrane is characteristic of DDD. C3GN should be differentiated from post-infectious glomerulonephritis and other immune complex-mediated glomerulonephritides showing C3 deposits.

  • Dyslipidemia in nephrotic syndrome (NS) is often characterized by marked increases in the levels of total cholesterol, triglycerides, low-density lipoprotein cholesterol, and other lipoproteins, such as very low-density lipoprotein, intermediate-density lipoprotein, and lipoprotein(a). It has been suggested that impaired catabolism of lipoproteins and cholesterol is mainly due to decreased lipoprotein lipase and hepatic lipase activity, and increased biosynthesis of lipoproteins in the liver. The management strategies for dyslipidemia in patients with NS consist of lifestyle modification, lipid-lowering agents represented by statins, second-line agents such as fibrates and bile acid sequestrants, and lipid apheresis. Compared with dyslipidemia in adult NS patients, whose risks of atherosclerotic disease and progressive renal injury are considered high, clinical data on dyslipidemia in pediatric NS patients are limited. Therefore, it is necessary to pay more attention to the evaluation and management of dyslipidemia in pediatric patients with NS in clinical practice.

소아에서 조혈모세포이식 후 급성 신질환의 분석 (Analysis of Acute Kidney Injury in Pediatric Patients with Stem Cell Transplantation)

  • 소아 악성종양환아들의 완치를 위한 조혈모세포이식술이 최근 널리 이루어지고 있다. 하지만 높은 치료율과 더불어 이의 부작용 또한 많이 보고되고 있다. 본 연구에서는 소아에서 조혈모세포이식술 이후의 급성 신질환의 임상적인 특성과 결과에 대해 알아보고자 하였다. 1995년 9월부터 2009년 4월까지 영남대학교 부속병원 소아청소년과에서 조혈모세포이식을 시행한 53명의 환자들을 대상으로 하였다. 조혈모세포이식 3개월 후의 사구체여과율을 계산하여 신장의 기능이 25% 이상 감소한 '조기 신기능 감소군'과 25% 미만 감소한 '조기 신기능 비감소군' 두 군으로 나누어 의무기록을 후향적으로 분석하였다. 환아의 원인 질환은 급성 백혈병과 신경모세포종이 각각 23명과 13명으로 가장 많았고, 그 외 뇌종양 및 기타 고형 종양이 6명, 재생불량성 빈혈이 4명, 악성 림프종이 4명, 골수이형성 증후군이 2명, 선천성 면역결핍질환이 1명이었다. 조혈모세포이식 3개월 후 25% 이상의 사구체여과율이 감소한 환아는 18명이었다. '조기 신기능 감소군'과 '조기 신기능 비감소군'은 각각 18명과 35명이었고, 이식세포 공급원은 제대혈(n=11)이 4명과 7명, 골수(n=16)는 8명과 8명, 자가 말초혈액(n=26)은 6명과 20명으로 차이는 없었다. 골수 및 제대혈 이식을 시행한 27명의 환아에서 급성 이식편대숙주반응은 신기능 감소군과 비감소군에서 차이를 보이지 않았다. 모든 환아는 전처치법 중 1가지 이상의 신독성 항암제를 투여받았으며 전신방사선조사 및 항흉선세포글로불린을 포함한 전처치법에 따른 두 군의 차이는 없었다. 사망한 환아는 '조기 신기능 감소군'과 '조기 신기능 비감소군'이 각각 12명과 10명이었고, '조기 신기능 감소군'에서 사망환아의 비율이 더 많았고, 사망의 원인으로는 감염, 급성 신부전, 재발 등이었고 '조기 신기능 비감소군'에 비해서 '조기 신기능 감소군'에서 급성 신부전이 많았다. 하지만 신장기능의 저하정도에 따라서는 사망한 환아는 차이를 보이지 않았다. 이번 연구에서는 소아에서 조혈모세포이식 3개월 후의 조기 신기능 저하는 18명이 있었으며 조혈모세포이식 후 발생하는 합병증 중 하나인 급성 이식편대숙주반응과 관련이 없었으며 이식세포 공급원과도 관련이 없었다. 조기 신기능 저하를 보인 환아에서 사망한 환아가 많았고, 신장기능 저하가 중요한 사망 원인이었다. 생존한 조기 신기능 저하군의 환아는 모두 신기능을 회복하였으며, 이식 12개월 후 후기 신기능 저하는 1명으로 이식의 합병증과는 관련이 없었으나 이후 이식 6년째에 재발로 사망하였다.

줄기세포 연구를 위한 급성신장손상 모델 (Acute Kidney Injury Models: Focus on the Therapeutic Effects of Stem Cell in Preclinical Approach)

  • 최근 줄기세포를 이용한 재생의학분야의 세포치료 연구가 활발히 진행되고 있으나 상대적으로 복잡한 구조를 갖는 신장에서는 난치성 질환인 급성신부전에 대한 줄기세포 치료의 효과는 아직도 논란의 여지가 많다. 이는 연구자마다 다양한 동물모델을 사용하는 것이 한 원인으로 지적되고 있다. 본 연구에서는 줄기세포의 치료효과 분석을 위한 최적의 동물모델 조건을 확립하고자 한다. 쥐를 사용하여 세 종류의 급성신장손상모델을 다음과 같이 다양한 농도와 조건에서 분석하였다: glycerol (대조군, 8 ml/kg, 10 ml/kg), cisplatin (대조군, 5 mg/kg, 10 mg/kg)과 허혈/재관류 (대조군, 35 분 허혈, 45 분 허혈). 줄기세포의 재생능력 평가를 위한 모델의 적합성 정도를 신장손상과 회복의 형태적, 기능적 패턴과 생존률 분석을 통하여 실시하였다. 독성모델인 glycerol과 cisplatin은 각각 10 ml/kg과 7.5 mg/kg 농도에서, 그리고 허혈모델은 45 분 모델에서 대조군 및 다른 비교모델에 비해 장기간의 관찰 기간 동안 신기능의 유의적 차이를 보였다. 또한 45 분 허혈 모델에서 50%의 사망률을 보인 것을 제외하고는 모든 군에서 관찰기간 동안 100% 생존율을 보였다. 조직소견에서 허혈과 glycerol 모델에서는 2일째, cisplatin 모델에서는 5일째 각각 유의적인 조직변화가 관찰되었다. 따라서, 관찰기간 동안 급격한 생존율 감소 없이 안전하고, 조직학적 평가와 신기능에서 대조군과 유의적인 차이를 보이는 cisplatin 7.5 mg/kg, glycerol 10 ml/kg, 35 분 허혈 모델이 치료효과를 평가하기에 적합한 모델로 사료되며, 줄기세포의 세포치료효과 분석에 적합한 급성신장손상 모델로 추천된다.

  • In oriental medicine, Artemisia Iwayomogi(Compositae) has been used clinically for jaundice, hepatitis, liver cirrhosis etc. The purposes of present study were to examine pharmacological effects of Artemisia lwayomogi water extract(AIWE) on weights of body, liver, kidney, spleen and adrenal, and on biochemical parameters (activities of AST, ALT and LDH, contents of cholesterol and triacylglycerol, and levels of hepatic lipid peroxide) against hepatic injury by carbon tetrachloride($CCl_4$) in rats. The results were as follow; 1. Body weights were reduced by $CCl_4$. In AIWE pretreatment groups, reduction of body weights was inhibited at 48 hours. Increased liver weights by $CCl_4$ were reduced in proportion to numbers of treatment of AIWE in AIWE pre- and posttreatment groups. Increased kidney weights by $CCl_4$ were reduced in AIWE pretreatment groups at 72 hours. Increased weights of spleen and adrenal by $CCl_4$ were not affected by AIWE treament. 2. Increased AST activities by $CCl_4$ were significantly (p<0.05) decreased in AIWE posttreatment groups at 48 and 72 hours. Increased ALT activities by $CCl_4$ were significantly(p<0.05) decreased in AIWE posttreatment groups at 48 hours. Increased LDH activities by $CCl_4$ were very significantly (p<0.01, p<0.001) decreased in AIWE posttreatment groups at 48 and 72 hours, respectively. 3. Increased cholesterol contents by $CCl_4$ were significantly (p<0.05) decreased in AIWE posttreatment groups at 24 and 48 hours. Decreased triacylglycerol contents by $CCl_4$ were significantly (p<0.05) increased in AIWE posttreatment at 48 and 72 hours. 4. Increased hepatic lipid peroxide levels by $CCl_4$ were significantly (p<0.05, p<0.01) decreased in AIWE posttreatment groups at 48 and 72 hours, respectively. In conclusion, AIWE did not affect normal liver function and had property of antioxidant, due to reduced lipid peroxidation by $CCl_4$. AIWE seems to have hepatoprotective effects rather than direct preventive effects to $CCl_4$-induced necrotic degeneration of liver cell, cholestasis and damages in metabolism of lipid.

Protective Effect of Phosphatidylcholine on Lipopolysaccharide-Induced Acute Inflammation in Multiple Organ Injury

  • Soybean polyunsaturated phosphatidylcholine (PC) is thought to exert anti-inflammatory activities and has potent effects in attenuating acute renal failure and liver dysfunction. The aim of this study was to investigate the effects of PC in protecting multiple organ injury (MOI) from lipopolysaccharide (LPS). Six groups of rats (N=8) were used in this study. Three groups acted as controls and received only saline, hydrocortisone (HC, 6 mg/kg, i.v.) or PC (600 mg/kg, i.p.) without LPS (15 mg/kg, i.p.) injections. Other 3 groups, as the test groups, were administered saline, HC or PC in the presence of LPS. Six hours after the LPS injection, blood and organs (lung, liver and kidney) were collected from each group to measure inflammatory cytokines and perform histopathology and myeloperoxidase (MPO) assessment. Serum cytokines (TNF-${\alpha}$, IL-6 and IL-10) and MPO activities were significantly increased, and significant histopathological changes in the organs were observed by LPS challenge. These findings were significantly attenuated by PC or HC. The treatment with PC or HC resulted in a significant attenuation on the increase in serum levels of TNF-${\alpha}$ and IL-6, pro-inflammatory cytokines, while neither PC nor HC significantly attenuated serum levels of IL-10, anti-inflammatory cytokine. In the organs, the enhanced infiltration of neutrophils and expression of ED2 positive macrophage were attenuated by PC or HC. Inductions of MPO activity were also significantly attenuated by PC or HC. From the findings, we suggest that PC may be a functional material for its use as an anti-inflammatory agent.
