The effect of hip joint strengthening exercise using proprioceptive neuromuscular facilitation on balance, sit to stand and walking ability in a person with traumatic brain injury: a case report

  • Objective: The purpose of this study was to investigate the effect of the hip joint strengthening exercises using proprioceptive neuromuscular facilitation (PNF) on the clinical symptoms and the treatment effects in balance, sit to stand, and gait abilities in patients with TBI. Design: A single case study. Methods: A 13-year-old adolescent with quadriplegia and hip joint control impairment participated in this four-week training intervention. The patient, diagnosed with TBI, wastreated with hip joint strengthening exercises using PNF. In the first week, we focused on strengthening the body, relaxing the hip flexors and activating the hip extensor muscles in order to solve the patient's physical function and body structure. From the 2nd and 4th week, we improved the motivation through the task-oriented method, and then weight-bearing training of the right lower extremity was proceeded by kicking a soccor ball with the left lower extremity. The exercises were performed for 4 weeks, 5 days a week, for 60 minutes with the exercise intensity gradually increased according to the subject's physical abilities. Results: As a result of the study, the patient demonstrated improvements in the physical examination, which were evaluated before and after intervention and included the manual muscle test, modified Ashworth scale, sensory assessment, coordination assessment, Berg balance scale, 5-time sit to stand test, and the 10 meters walk test. Conclusions: The results of this case suggest that a hip joint strengthening exercise program using PNF may improve hip control ability, balance, sit to stand and gait ability in a patient with TBI.

The Effect of Neck and Shoulder Self-Stretching Exercise Using Audiovisual Media on Neck Pain, Postural Alignment, and Joint Position Error in Women with Chronic Neck Pain (시청각 매체를 활용한 목, 어깨 자가신장운동이 만성 목통증 여성의 통증, 자세정렬과 관절위치감각에 미치는 영향)

  • Background: The purpose of this study is to investigate the effect of neck and shoulder self-stretching exercise using audiovisual media on neck pain, postural alignment, and joint position error in women with chronic neck pain. Methods: The subjects included 20 women that gave consent to participate in the study voluntarily. They performed the self-stretching exercises using audiovisual media was carried out 20 minutes 5 times a week during 3 weeks. Neck disability index (NDI) and visual analogue scale (VAS) were used to measure the functional disability and pain, A pressure pain threshold was measured using an algometer, and a cervical range of motion (CROM) measurement tool was used to measure the range of motion and error of proprioceptive position sense of the cervical spine. To assess posture alignment, forward head angle (FHA), forward shoulder angle (FSA) were measured using image J software. Results: The neck pain intensity was statistically significantly within group (p<.05). Neck and shoulder functional disability were a statistically significant difference within group (p<.05). Splenius capitis and upper trapezius pressure pain threshold were statistically significant difference in within group (p<.05). The postural alignment was statistically significantly within group (p<.05). The cervical range of motion in neck extension, right and left lateral flexion were statistically significantly within group (p<.05). The joint position error in neck flexion, extension, right and left lateral flexion decreased statistically significantly within group (p<.05). Conclusion: Self-stretching exercise using audiovisual media increased the mobility of the neck, decreased neck pain and joint position error, and improved posture alignment. As a result, there was a positive effect by applying the self-stretching exercise using audiovisual media to people with neck pain. Based on this, it is thought that it can be used as the basis for research related to home training programs for healthy self-management.

  • Background: This study aimed to investigate the effects of a complex manual therapy program on hip and knee pain occurring together with menstrual pain and iliotibial band tightness in a woman with sacroiliac joint dysfunction. Methods: This study was carried out over a period of 3 months by dividing the treatment process into three sequential stages. Intensities of menstrual pain, hip pain, and knee pain, and hip range of motion (ROM) in the participant were assessed at baseline and at the end of each stage, and a modified intervention was applied according to results of assessments after each stage. Results: There was a decrease in the intensity of menstrual pain after the first and third stages of treatment, and there were reductions in the intensities of hip and knee pain after the third stage. Hip ROM was rather decreased after the second stage, but it was increased after the third stage. Conclusion: The findings of this study indicate that treatment of the instability and positional fault of sacroiliac joint can relieve hip and knee pain, improve hip ROM, and alleviate primary dysmenorrhea in patients with sacroiliac joint dysfunction.

Evaluation of Magnetization Transfer Ratio Imaging by Phase Sensitive Method in Knee Joint (슬관절 부위에서 자화전이 위상감도법에 의한 자화전이율 영상 평가)

  • Although MR imaging is generally applicable to depict knee joint deterioration it, is sometimes occurred to mis-read and mis-diagnose the common knee joint diseases. In this study, we employed magnetization transfer ratio (MTR) method to improve the diagnosis of the various knee joint diseases. Spin-echo (SE) T2-weighted images (TR/TE 3,400-3,500/90-100 ms) were obtained in seven cases of knee joint deterioration, FSE T2-weighted images (TR/TE 4,500-5,000/100-108 ms) were obtained in seven cases of knee joint deterioration, gradient-echo (GRE) T2-weighted images (TR/TE 9/4.56/$50^{\circ}$ flip angle, NEX 1) were obtained in 3 cases of knee joint deterioration, In six cases of knee joint deterioration, fat suppression was performed using a T2-weighted short T1/tau inverse recovery (STIR) sequence (TR/TE =2,894-3,215 ms/70 ms, NEX 3, ETL 9). Calculation of MTR for individual pixels was performed on registration of unsaturated and saturated images. After processing to make MTR images, the images were displayed in gray color. For improving diagnosis, three-dimensional isotropic volume images, the MR tristimulus color mapping and the MTR map was employed. MTR images showed diagnostic images quality to assess the patients' pathologies. The intensity difference between MTR images and conventional MRI was seen on the color bar. The profile graph on MTR imaging effect showed a quantitative measure of the relative decrease in signal intensity due to the MT pulse. To diagnose the pathologies of the knee joint, the profile graph data was shown on the image as a small cross. The present study indicated that MTR images in the knee joint were feasible. Investigation of physical change on MTR imaging enables to provide us more insight in the physical and technical basis of MTR imaging. MTR images could be useful for rapid assessment of diseases that we examine unambiguous contrast in MT images of knee disorder patients.

Bioluminescence Imaging of Chondrocytes in Rabbits by Intraarticular Injection of D-Luciferin (토끼에서 D-luciferin의 관절강 주입에 의한 연골세포의 자연발광 영상)

  • Purpose: Luciferase is one of the most commonly used reporter enzymes in the field of in vivo optical imaging. D-luciferin, the substrate for firefly luciferase has very high cost that allows this kind of experiment limited to small animals such as mice and rats. In this current study, we validated local injection of D-luciferin in the articular capsule for bioluminescence imaging in rabbits. Materials and Methods: Chondrocytes were cultured and infected by replication-defective adenoviral vector encoding firefly luciferase (Fluc). Chondrocytes expressing Fluc were injected or implanted in the left knee joint. The rabbits underwent optical imaging studies after local injection of D-luciferin at 1, 5, 7, 9 days after cellular administration. We sought whether optimal imaging signals was could be by a cooled CCD camera after local injection of D-luciferin. Results: Imaging signal was not observed from the left knee joint after intraperitoneal injection of D-luciferin (15 mg/kg), whereas it was observed after intraarticular injection. Photon intensity from the left knee joint of rabbits was compared between cell injected and implanted groups after intraarticular injection of D-luciferin. During the period of imaging studies, photon intensity of the cell implanted group was 5-10 times higher than that of the cell injected group. Conclusion: We successfully imaged chondrocytes expressing Fluc after intraarticular injection of D-luciferin. This technique may be further applied to develop new drugs for knee joint disease.

Factors Influencing Quality of Life in the Community Dwelling Vulnerable Older women with Chronic Joint Pain (만성 관절통증이 있는 취약계층 재가 여성노인의 삶의 질에 영향을 미치는 요인)

  • The purpose of this study was to identify the predictors of the quality of life in community dwelling vulnerable older women with chronic joint pain. A cross-sectional study was conducted on 234 participants registered at a visiting health care service in the public health center of C-city. The structured questionnaire were used to collect data on the general characteristics, pain rating index, pain intensity, perceived health status, functional independence, sleep pattern, depression, and quality of life from February 16 to March 13, 2017. The data were analyzed using a t-test, ANOVA, Pearson's correlation coefficients, and hierarchical multiple regression analysis. The significant factors of the quality of life including general characteristics were depression (${\beta}=-.60$, p<.001), pain intensity scale (${\beta}=-.15$, p=.007), health insurance type (${\beta}=.15$, p=.001), perceived health status (${\beta}=.14$, p=.007), duration of pain (${\beta}=-.10$, p=.019), marital status (${\beta}=.10$, p=.024), and functional independence (${\beta}=.09$, p=.036). These factors accounted for 63.1% of the total variance in the quality of life. The findings suggest that a public healthcare program on managing depression is effective in improving the quality of life in community dwelling older women with chronic joint pain. Public health nurses should intervene in nursing care for economically vulnerable aged and pain management based on a precise assessment from the beginning of pain.

Evaluation of image quality for metal artifact using protocol parameters in the MRI (자기공명영상에서 프로토콜 변화를 이용한 금속인공물의 영상평가)

  • In the aging society, TKRA is steadily increased because of joint diseases. Artificial joint used in TKRA generates metal artifacts in the MRI. Metal artifact may affect diagnosis. In study, We are going to minimize the effect of metal artifact to improve the value of diagnosis by changing the sequence and the type of artificial joint(Co-Cr, Ni-Ti). 1.5T AVANTO, plastic containers and each of the artificial joint (Normal, Co-Cr, NiTi) were used. After the artificial joints fixed in a paper cup was inserted in a plastic container of cylindrical, Signal intensity was measured. To obtain strong and uniform signal intensity, the plastic container was filled with water. We changed Sequences(T1 TSE, T2 TSE, PD TSE) and obtained an Axial image. After excepting the maximum and minimum values, We calculated the average of SNR, CNR and PSNR. Consequently, The SNR, CNR value of PD TSE are measured higher than these of T1 TSE, T2 TSE and The PSNR of Co-Cr is higher than this of Ni-Ti. The SNR of Co-Cr is similar to the SNR of normal comparing this of Ni-Ti. As a result, Using sequence of PD Tse and Co-Cr alloy is considered to be useful.

Fatigue Behavior of the Single Spot Welded Joint of Zinc Galvanized Steel Sheets (아연도금 강판의 점용접재의 피로균형에 관한 연구)

  • The behavior of fatigue crack growth in the single spot welded joint of zinc galvanized steel sheets was studied experimentally and analytically based on fracture mechanics. Axial tension fatigue tests were carried out with the BSxGAB specimen that the bare plane(GAB) of monogalvanized steel sheet was spot welded to the double thickness bare steel sheet(BS), and with the GAxGAB specimen that the galvanized plane (GA) was spot welded to the equal thickness bare plane (GAB) 1. The relation between maximum stress intensity factor, K sub(max) and the number of cycles to failure, N sub(f) has shown a linear relation on log-log plot in the spot weld of the zinc galvanized steel sheet. 2. The fatigue strength of BSxGAB specimens is about 23% higher than that of GAxGAB specimens at the fatigue strength of $1\times10^6$ cycles. And the fatigue life of BSxGAB specimens at the same load range increases 6~9 times higher than that of GAxGAB specimens. 3. The general tendency at the angle of bending($\theta$) in an applied load has changed rapidly at the initial 20% of its life. After then, it has changed slowly. The change at the angle of bending has increased linearly as the load range increases. 4. It has shown a linear relation between the location ratio of initiation ${\gamma}$ and fatigue life $N_f$ on the semi-log graph paper. Here $\gamma$ means that the crack distance between main crack and sub-crack, 2L is divided by the nugget diameter, 2r. $\gamma=a{\cdot}log N_f+n$ (where a and n are material constant.)

A Study on the Optical Pattern Recognition Using pSDF and Binary Joint Transform Correlator (pSDF와 이진 결합 변환 상관기를 이용한 광 패턴 인식에 관한 연구)

  • In this paper, pSDF-based referance image is realized. Using BJTC (binary joint transform correlator) as the spatial plane correlator, optical pattern recognition for interclass identification and interclass discrimination is performed. Computer simulation shows that the correlation performance of BJTC is superior to that of JTC. Experimental results using BJTC reveal that correlation peak intensity is constant within the error rang from $4.1{\%}\to\9.6{\%}$ in interclass identification and correlation peak intensity of one class is two times higher than that of the other class in interclass discrimination, which indicates its superiority in discrimination sensitivity.

Correlation between pain and degenerative bony changes on cone-beam computed tomography images of temporomandibular joints

  • Background: The aim of this study was to assess correlation between pain and degenerative bony changes on cone-beam computed tomography (CBCT) images of temporomandibular joints (TMJs). Methods: Two hundred eighty-three temporomandibular joints with degenerative bony changes were evaluated. Pain intensity (numeric rating scale, NRS) and pain duration in patients with degenerative joint disease (DJD) were also analyzed. We classified condylar bony changes on CBCT into five types: osteophyte (Osp), erosion (Ero), flattening (Fla), subchondral sclerosis (Scl), and pseudocyst (Pse). Results: Degenerative bony changes were the most frequent in the age groups of 10~19, 20-29, and 50~59 years. The most frequent pain intensity was "none" (NRS 0, 34.6%) followed by "annoying" (NRS 3-5, 29.7%). The most frequent condylar bony change was Fla (219 joints, 77.4%) followed by Ero (169 joints, 59.7%). "Ero + Fla" was the most common combination of the bony changes (12.7%). The frequency of erosion was directly proportional to NRS, but the frequency of osteophyte was inversely proportional. The prevalence of Ero increased from onset until 2 years and gradually decreased thereafter. The prevalence of Osp, Ero, and Pse increased with age. Conclusions: Osp and Ero can be pain-related variables in degenerative joint disease (DJD) patients. "Six months to 2 years" may be a meaningful time point from the active, unstable phase to the stabilized late phase of DJD.
