The Effects of Bee Venom Acupunture Therapy on Shoulder Pain Patients in Stroke Sequelae (봉약침이 중풍후유증으로 인한 견비통에 미치는 치료 효과에 관한 연구)

  • Objective : The aim of this study is to investigate the curative effect of Bee Venom Acupuncture Therapy for pain and limited R.O.M (range of motion) of shoulder in stroke patients. Methods : The subjects of this study were 6 patients with shoulder pain in stroke sequelae. Routine Oriental Medical programs (Acupunture, moxibustion, herbal medicine and physical therapy) were maintained for each subject throughout this study. Single subject ABABAB design was adopted. Each period was 4 days as a rule. Only during the treatment period, Bee Venom Acupuncture Therapy was provided as intervention at the acupoints of LI15(Gyeonu), TE14(Gyeollyo), GB21(Gyeonjeong), LI14(Bino). The change of pain was measured with a Visual Analogue Scale(VAS). The pain threshold was measured using pressure algometer at the same acupoints where Bee Venom Acupuncture Therapy was provided. And the R.O.M of shoulder joint (flexion, extension, abduction, adduction, external rotation, internal rotation) was measured as well. Analysis was performed by Bayesian analysis using WinBUGS for the comparison of treatment(Bee Venom Acupuncture Therapy) and non-treatment. Results : The median overall improvement for difference in VAS was -2.219(-3.213, -1.175), for difference in external rotation of shoulder R.O.M was 9.992(-2.298, 18.49), for difference in tenderness score of LI14(Bino) by pressure algometer was 5.05(0.6283, 7.762). 95% credibility intervals being shown in brackets. However, the median overall improvement for difference in the other measurements was not significant. Conclusion : This study suggests that Bee Venom Therapy may be applicable to decrease pain and improve R.O.M of shoulder in hemiplegia patients with stroke. Further elaborated single subject designs need to be accumulated to confirm the effects of Bee Venom Acupunture Therapy on shoulder pain in patients with stroke sequelae.

Behavior of Concrete Columns Confined by Carbon Fiber Sheets under a Constant Axial Force with Reversed Cyclic Lateral Loading (일정축력하의 탄소섬유쉬트 보강기둥의 횡가력시 거동특성에 관한 실험연구)

  • An investigation was conducted into the flexural behavior of earthquake damaged reinforced concrete columns repaired with carbon fiber sheets. Six column specimens were tested to failure under reversed cyclic loading. Two columns were specimens for control with no sheets and tested. These columns were repaired with carbon fiber sheets and retested to evaluate the effect of the confinement of the carbon fiber on the damaged column. Another two columns were repaired and tested with no pre-cyclic loading. The test specimens were designed to model single bent under a constant axial force with reversed cyclic lateral loading. Carbon fiber sheets were used to repair damaged concrete columns in the critically stressed areas near the column footing joint and the physical, mechanical properties of carbon fiber sheets are described. The performance of repaired columns in terms of their hysteretic response is evaluated and compared to those of the original columns. The results indicate that the repaire technique with carbon fiber sheets is highly effective. Both flexural strength and displacement ductility of repaired columns were higher than those of the original columns.

Design Considerations and Pull-Out Behavior of Mechanical Anchor of Reinforcement (철근 기계적 정착장치의 설계 고려사항과 인발특성)

  • In RC structure, sufficient anchorage of reinforcement is necessary for the member to produce the full strength. Generally, conventional standard hook is used for the reinforcement's anchorage. However, the use of standard hook results in steel congestion, making fabrication and construction difficult. Mechanical anchor offers a potential solution to these problems and may also ease fabrication, construction and concrete placement. In this paper, the required characteristics and the design considerations of mechanical anchor were studied. Also, the mechanical anchor was designed according to the requirements. To investigate the pull-out behavior and properness of mechanical anchorage, pull-out tests were performed. The parameters of tests were embedment length, diameter of reinforcement, concrete compressive strength, and spacing of reinforcements. The strengths of mechanical anchor were consistent with the predictions by CCD method. The slip between mechanical anchor and concrete could be controlled under 0.2mm. Therefore, the mechanical anchor with adequate embedment could be used for reinforcement's anchorage. However, it was observed that the strength of mechanical anchors with short spacing of reinforcements was greatly reduced. To apply the mechanical anchor in practice (e.g. anchorage of the beams reinforcements in beam-column joint), other effects that affect the mechanical anchor mechanism, such as confinement effect of adjacent member from frame action or effects of shear reinforcement, should be considered.

Effect of Therapeutic Gymnastic Ball Exercise in Patients With Chronic Low Back Pain (만성 요통환자의 치료를 위한 치료용 볼 운동의 효과)

  • This study was designed 10 investigate the effects of therapeutic gymnastic ball exercise on pain, flexibility, lumbar disability level and daily activity levels in male patients of the armed forces medical hospital who complain of chronic low back pain. Twenty-three males were placed in the experimental group and twenty-nine males were placed in the control group. All of the subjects were chosen on the basis of availability among in-patients who were diagnosed with low back pain. The control patients were matched to the experimental group and they were selected considering gender, pain duration and age. Gymnastic ball exercise therapy was developed by the author with the assistance of a rehabilitation specialist. Gymnastic ball exercise therapy includes muscle relaxation, flexibility, muscle strength and posture development exercises. The gymnastic ball exercise therapy was carried out by the experimental group three times a week for eight weeks. Before and after the experiments, the intensity of pain, the lumbar joint mobility (flexibility), the lumbar disability levels, and the daily activity levels of the subjects were measured, respectively. The intensity of pain and the lumbar disability levels were measured by the Visual Analogue Scale, the level of flexibility by a measurement ruler, and the level of disability by the Oswestry Low Back Pain Disability Scale. Data were analysed using a t-test, a paired t-test and an unpaired t-test. The results were as follows: 1. The intensity of pain in the lumbar spine in the experimental group was significantly decreased compared with that of the control group during the 4th week and 8th week. 2. The flexibility of the lumbar spine in the experimental group was significantly increased compared with that of the control group during the 4th week and 8 week. 3. The level of pain caused by anterior, posterior, left lateral and right lateral bending and by rotation in experimental group was significantly decreased compared with that of the control group. 4. The Oswestry Disability score of the experimental group was significantly increased compared with that of control group. These findings indicate that gymnastic ball exercise therapy could be effective in decreasing pain and lumbar disability, and increasing the daily activity levels and lumbar flexibility in patients with chronic low back pain. The study also suggests that gymnastic ball exercise therapy could be an essential factor for effective nursing intervention for patients suffering from chronic low back pain.

Effect of Adhesive Shear Strength of CFRP/Ni-Cr Alloy Under Severe Environmental Condition (가혹 환경이 복합재/Ni-Cr 합금 접착전단강도에 미치는 영향 연구)

  • The mechanical property related to lap shear strength of the joint structure between carbon fiber reinforced polymer (CFRP) composite and metal (Ni-Cr Alloy) under varying environmental conditions (temperature and humidity) was studied in order to apply to the aircraft fan blade. Room temperature dry (RTD), elevated temperature wet (ETW), and cold temperature dry (CTD) environmental conditions were chosen for investigation based on the flight conditions of aircraft. Lap shear strength tests were conducted according to ASTM Standard D3528 to evaluate the shear strength. The microstructure characteristic of failure zone was analyzed by SEM images to check the adhesive shear strength with the three environmental conditions. In comparison with shear strength for the RTD condition, the shear strength in the ETW condition was reduced by 72.8% while those for the CTD condition increased by 56.5%. The moisture absorption and high temperature in ETW condition strongly had an affect on mechanical property of adhesive, while cold temperature could enhance the adhesive shear strength due to the higher brittleness.

  • The leading surgical method for correcting the misalignment of the varus and valgus in the knee joint is the high tibial osteotomy (HTO). In the opening wedge HTO (OWHTO), there is no concern about damaging the peroneal nerve on the lateral tibia of the proximal fibula. OWHTO has been the preferred choice, as the opening of the correction angle can be modulated during the operation. The correction of the varus and valgus on the coronal plane are performed adroitly. Nevertheless, there have been numerous reports of unintended changes in the medial tibial plateau and posterior slope angle (PSA). The authors have developed an HTO method using computer-assisted surgery with the aim of addressing the abovementioned problems from an engineer's perspective. CT images of the high tibia were reconstructed three-dimensionally, and a virtual osteotomy was performed on a computer. In addition, this study recommends a surgical method that does not cause changes in the PSA after OWHTO. The results of the study are expected to suggest a clear relationship between the anteromedial cortex oblique angle of each patient and the PSA, and an optimal PSA selection method for individuals.

Effects of Heme Oxygenase-1 on VEGF Expression in Rheumatoid Arthritis (류마티스 관절염에서 HO-1에 의한 VEGF 발현 유도에 대한 연구)

  • Heme oxygenase-1 (HO-1), an inducible heme-degrading enzyme, is expressed by macrophages and endothelial cells in response to various stresses and mediators of inflammation. HO-1 has been recently implicated in regulation of angiogenesis via expression of VEGF. The purpose of this study was to determine the effects of HO-1 modulation on the collagen-induced arthritis (CIA) model and on angiogenesis via up- regulation of VEGF expression in human synovial fibroblast. DBA/1J mice were treated with an inhibitor of HO-1, tin protoporphyrin IX (SnPP), or with an inducer of HO-1, cobalt protoporphyrin IX (CoPP), from day 1 to day 35 after CIA induction. The clinical evolution of disease was monitored visually. At the end of the experiment, histopathologic changes were examined on the joints. VEGF expression in paws were measured by immunohistochemical stain. mRNA expression of HO-1 and VEGF stimulated with various concentration of $TNF-{\alpha}$, CoPP accessed on human synovial fibroblast by RT-PCR. Effects of pretreatment with SnPP on mRNA expression of HO-1 and VEGF in the presence of CoPP and $TNF-{\alpha}$ in synovial fibroblast was accessed by Real-time RT-PCR. Administration of cobalt protoporphyrin IX significantly induced the inflammatory response, with increased arthritis index and expression of VEGF in the paws of the arthritis models. Treatment with SnPP significantly reduced the severity of CIA through inhibition of joint inflammation and cartilage destruction. The expression of VEGF were also significantly reduced by SnPP treatment in the paw. CoPPIX as inducer of HO-1, increased HO-1 and VEGF expression dose dependently in synovial fibroblast. In contrast, inhibition of HO-1 activity by SnPPIX abrogated CoPPIX-induced HO-1 and VEGF production in synovial fibroblast. Stimulation with $TNF-{\alpha}$ increased HO-1 and VEGF expression itself and showed additive effect on HO-1 and VEGF expression when it treated with CoPP. When SnPP was treated with CoPP and $TNF-{\alpha}$, it abrogated the CoPP induced HO-1 and VEGF expression and also abrogated $TNF-{\alpha}$ induced HO-1 and VEGF expression in synovial fibroblast. The effects of HO-1 induction in rheumatoid arthritis results in aggravation of arthritis via up-regulation of VEGF. I concluded that inhibition of the expression or activity of HO-1 could be a therapeutic target of rheumatoid arthritis.

Study on the Movement of New Qi-gong "WuQinXi" Exercise for Lumbar Spinal Disease : Based on 10 Mode, 15 Mode, 18 Mode (요추 질환에 대한 신기공 오금희의 동작연구 -10식, 15식, 18식을 중심으로-)

  • Hua-Tuo(145-208) created five "WuQinXi" exercise by imitating the movements of a tiger, a deer, a bear, a monkey and a bird. The "WuQinXi" exercise, one of the medical Qi-gongs, is an exercise maximizing human's self healing power and has been effective significantly at several modern researches. There are many exercise therapies in western medcine, such as Willamss flexion exercise, Mckenzie's extension exercise, vertebral stabilization exercise and so on. However, there isn't a special exercise therapy which can be applied for medical practice in oriental medicine. So We selected some motions from "WuQinXi" exercise, which are suitable for lumbar spinal disease, and analyzed them. After that, We assorted them by kinds of lumbar spinal disease. First, We selected 22 motions which are related with lumbar movements from 3 type "WuQinXi" exercises ; 10 mode, 15 mode, 18 mode. And then, We classified them according to lumbar movements as flexion, extention, lateral bending and rotation, and also functions as stabilization and rubbing. Next, with these classifications, We assorted those motions by kinds of lumbar spinal disease as HIVD(herniation of intervertebral disc), spinal stenosis, spondylolysis and spondylolisthesis, facet joint syndrome, compression fracture and spondylosis. We expect that trying "WuQinXi" exercise at clinic in this way, the particular exercise therapy of oriental medicine, "WuQinXi" exercise will become more popular. And Oriental medical doctors will be able to teach patients "WuQinXi" exercise's motions easily at clinic, depending on kinds of lumbar spinal disease each patient suffers from. We plan to study more about 20 mode, 30 mode, 40 mode and the effect of "WuQinXi" exercise by comparing patients who do the "WuQinXi" exercise with the patients who do the western medical exercise therapy.

Loop Suture Technique for Flexor Digitorum Profundus Tendon Repair in the Insertion Site (고리 봉합법을 이용한 심부 수지 굴건 종지부에서의 건봉합)

  • Purpose: In the case of repair for far distal parts of FDP (Flexor digitorum profundus) division, the method of either pull-out suture or fixation of tendon to the distal phalanx is preferred. In this paper, the results of a modified loop suture technique used for the complete division of FDP from both zone 1a and distal parts of zone 1b in Moiemen classification are presented. Methods: From July 2006 to July 2009, the modified loop suture technique was used for the 10 cases of FDP in complete division from zone 1a and distal parts of zone 1b, especially where insertion sites were less than 1 cm apart from a tendon of a stump. In a suture technique, a loop is applied to each distal and proximal parts of tendon respectively. Core suture of 2-strand and epitendinous suture are done with PDS 4-0. Out of 10 patients, the study was done on 6 patients who were available for the followup. The average age of the patients was 49.1 years (in the range from 26 to 67). 5 males and 1 female patients were involved in this study. There were 3 cases with zone 1a and distal parts of zone 1b. The average distance to the distal tendon end was 0.6 cm. There were 5 cases underwent microsurgical repair where both artery and nerve divided. One case of only tendon displacement was presented. The dorsal protective splint was kept for 5 weeks on average. The results of the following tests were measured: active & passive range of motion, grip strength test, key pinch and pulp pinch test. Results: The follow-up period on average was 11 months, in the range from 2 to 20 months. There was no case of re-rupture, but tenolysis was performed in 1 cases. In all 6 cases, the average active range of motion of distal interphalangeal joint was 50.8 degree. The grip strength (ipsilateral/contralateral) was measured as 88.7% and the pulp pinch test was 79.2% as those of contralateral side. Flexion contracture was presented in 2 cases (15 degree on average) and there was no quadrigia effect found. Conclusion: Despite short length of tendon from the insertion site in FDS rupture in zone 1a and distal parts of zone 1b, sufficient functional recovery could be expected with the tendon to tendon repair using the modified loop suture technique.

Effect of Groove Conditions on the Mechanical Properties of Welds Produced by the Combined Welding Process of Flux Cored Arc and Electro Gas in EH36 TMCP Steel Plate for Hull Structures (선체구조용 EH36 TMCP 후판의 FCAW 및 EGW 조합 용접부 기계적 성질에 미치는 개선조건의 영향)

  • Characterization of microstructures and mechanical properties of 83mm thickness EH36-TM welds produced by the combined flux cored arc (FCA) and electro gas (EG) welding processes has been studied with the two different groove conditions, single-V (SV) and double-V (DV) bevels. The welding consumables used for FCA and EG welding processes were ASME/AWS A5.29 (E81T1-K2) and A5.26 (EG72T), respectively. Experimental results showed that all the mechanical properties of welds such as tensile property, CVN toughness and Vickers hardness met IACS requirements. The tensile strength of EG welded plates were reduced by approximately 4% (DV: 3.8%, SV: 4.2%) compared to the base metal. The hardness value of SV-beveled weld metal was slightly lower than that of DV-beveled one. There were no significant differences as per welding groove conditions except for the weld metal. In addition, at the fusion line, the toughness of SV condition was 20J lower and the weld metal was 40J lower than DV condition, respectively. On the basis of microstructural analysis, grain boundary ferrite (GBF) structures for SV condition were 2 times higher volume fraction than for DV condition and their packet sizes were coarsened to almost double. It was thus suggested that the GBF volume fractions and packet sizes in the weld metal of EH36-TM steel plates are the most important factors affecting the mechanical properties of the combined FCA and EG welded joint. Nevertheless, all the results of welds with both DV and SV conditions were found to be excellent.
