A Study on the Consciousness of Fare-fighting Officers for the Establishment and Pevitalization of Integrated Disaster Management System (통합재난관리체계 구축 및 활성화를 위한 소방공무원 의식조사 연구)

  • In this study, the problems in the domestic disaster management system were identified through the examination of the system, relevant implications were drawn through the present status of domestic integrated disaster management system, and the consciousness survey for the integrated disaster management system targeting fire-fighting officers in the front line of the disaster was carried out to identify the measures to improve the system. Based on the findings, the measures to establish and revitalize the integrated disaster management system was presented, and the contents of the measures are as follows. In order to establish and revitalize the integrated disaster management system, the following measures should be prepared for each item. In the organizational aspect, the coordination system between government agencies should be established properly and the relevant information should he shared and delivered smoothly. In the legal and institutional aspect, the improvement is required to prevent any miscommunications in the coordination and joint operation plan through the collaboration between government agencies should be established in consideration of characteristics of each government agency. Also, in the aspect of system, the equipment should be maintained and updated continuously in order to respond to rapidly changing disasters and the organization, laws, institutions and system should be improved systematically and harmoniously. And, the detailed measures appropriate for the characteristics of domestic disasters should be prepared through the examination of advantages and disadvantages of integrated disaster management system in advanced countries in details and the detailed plans for the establishment of integrated situation management system in relevant organizations and the unification of situation room through the establishment of efficient integrated management system should be established.

Treatment of soft Tissue Sarcoma by General Orthopedic Surgeon Rather than Orthopedic Oncologist (골연부조직 종양 전공자가 아닌 일반 정형외과 전문의에 의해 치료된 연부조직육종)

  • Purpose: To investigate the quality of training hospital based treatment, we evaluated the soft tissue sarcoma treatments afforded by general orthopedic surgeon rather than orthopedic oncologist. Materials and Methods: We reviewed the details of 25 patients with pathologically confirmed soft tissue sarcoma who registered in our hospital between July 1997 and 2006 September. We evaluated initial diagnoses, the surgical treatment (including adjuvant therapy) and the follow up method used and related these to the principles of soft tissue sarcoma treatment. Results: The study cohort comprised 16 men and 9 women of mean age of 50.2 years. A diagnostic biopsy was performed in 9(36%) cases before definitive surgical treatment. Wide excision was performed in 13(52%) cases. For the 12 cases in which the grade of sarcoma was estimated, adequate surgical treatment with adjuvant therapy was performed only in 4(33.3%) cases. In addition, an adequate follow up schedule was adopted in only 4(16%) of the 25 study subjects. Conclusion: Unexpectedly, many cases of soft tissue sarcoma were treated inadequately even in a training hospital. An intensive education program on the treatment of soft tissue sarcoma is necessary for all orthopedic surgeons.

Effect of Aspect Ratio and Diagonal Reinforcement on Shear Performance of Concrete Coupling Beams Reinforced with High-Strength Steel Bars (세장비 및 대각철근 유무에 따른 고강도 철근보강 콘크리트 연결보의 전단성능)

  • As per current seismic design codes, diagonally reinforced coupling beams are restricted to coupling beams having aspect ratio below 4. However, a grouped diagonally reinforcement detail makes distribution of steel bars in the beam much harder, furthermore it may result in poor construction quality. This paper describes the experimental results of concrete coupling beam reinforced with high-strength steel bars (SD500 & SD600 grades). In order to improve workability for fabricating coupling beams, a headed large diameter steel bar was used in this study. Two full-scale coupling beams were fabricated and tested with variables of reinforcement details and aspect ratio. To reflect real behavior characteristic of the beam coupling shear walls, a rigid steel frame system with linked joints was set on the reaction floor. As a test result, it was noted that cracking and yielding of reinforcement were initially progressed at the coupling beam-to-shear wall joint, and were progressed to the mid-span of the coupling beam, based on the steel strain and failure modes. It was found that the coupling beams have sufficient deformation capacity for drift ratio of shear wall corresponding to the design displacement in FEMA 450-1. In this study, the headed horizontal steel bar was also efficient for coupling beams to exhibit shear performance required by seismic design codes. For detailed design for coupling beam reinforced with high-strength steel, however, research about the effect of variable aspect ratios on the structural behavior of coupling beam is suggested.

Dispute Issues and Improvement of Inter-layer Joints in Apartment Houses (공동주택 층간이음부의 분쟁 쟁점 및 개선 방안)

  • Recent rise in the supply rate of new public apartment houses leads to an increasement in disputes regarding the construction quality of the apartments between the residents and the construction companies. According to the dispute cases filed for claiming the collective defect repair fees, inter-layer concrete joints turned out to be the most frequently disputed item. For this reason, this study selects the inter-layer concrete joints to further analyze the primary causes and details of each dispute case. From the results of this study, three primary causes of the disputes are found, which are 1) the absence of standard specifications for construction quality control and management after construction; 2) the absence of established standards for repair when construction defects are found; and 3) the fact that the court grants generous compensation for disputes concerning the apartment houses. In order to prevent construction defects in inter-layer concrete joints, this study provides three suggestions including 1) the current standard specifications for inter-layer concrete joints should be further specified by the Ministry of Land, Infrastructure and Transport; 2) a construction defect should be judged according to the compliance to the standard specifications; and 3) a clear and institutional protocol needs to be established for defect repair in cases that new public apartment houses have been judged to have defects.

Evaluation of Structural Performance of RC Beam with Different Depths to Lap Splice Detail of SD700 Headed Bar (SD700 확대머리 철근의 겹침이음 상세를 적용한 단차가 있는 RC 보의 구조성능 평가)

  • This paper conducts an evaluation of the structural performance of the lap splice detail of SD700 headed bar experiment for developing an RC beam with different depths joint details. The experiment variable is lap splice length, yield strength, and end anchorage of main reinforcements. For all specimens, a headed bar was applied to the main reinforcement of the beam with low depth (B2), and the beam with high depth (B1) was applied to the main reinforcement with two splice methods: straight headed bar and 90° hooked-headed bar. The experimental results were that specimens of applying SD500 and SD600 had the results of flexural fracture at the lap splice location, which maximum load was similar. For specimens of appling SD500, the 90° hooked-headed bar of B1, suppressed horizontal cracks in the lap splice section compared to the straight headed bar. Specimens of applying an SD 700 headed bar had the results of brittle anchorage failure. In addition, maximum load was increased with the lap splice length increasing. For specimens of applying SD700 headed bar, test for test maximum load/theoretical load for test development length/design development length were estimated to be 1.30~1.48 for the ACI 318-19 equation, and 1.14~1.30 for the KDS-2021 equation. Thus, ACI 318-19 equation had conservatively greater safety factors as estimated development lengththened.

Functional Result of Limb Salvage Surgery with Tumor Prosthesis for Osteosarcoma of Proximal Tibia (근위 경골 골육종의 종양대치물을 이용한 사지 구제술 후의 기능 평가)

  • Purpose : Limb salvage for osteosarcoma of proximal tibia is challenging problem due to difficulties in mobilizing or retracting the main neurovascular structure, inadequate soft tissue coverage, and unsolved problem of patellar tendon reattachment to endoprosthesis. The authors analyzed the functional result of limb salvage using tumor prosthesis with medial gastrocnemius rotation plasty for osteosarcama of the proximal tibia. Materials and Methods : Eleven patients with histologically proven osteosarcoma of the proximal tibia, treated with adjuvant and neoadjuvant chemotherapy and limb salvage operation with tumor prosthesis between January 1992 and December 1998 at our Medical Center, were selected. There were 6 male and 5 female. Age ranged from 15 years to 23.7 years with an average of 23.7 years. Follow-up period ranged from 1 year to 4.5 years with an average of 2.5 years. The final functional result was evaluated using the method by ISOLS, 1993. The factors include pain, functional activities, emotional acceptance, use of external supports, walking ability and gait. Each of the factors has been scored from 0 to 5 depending on the appropriate description or data. The rating score is determined by dividing the individual factor scores into the total score and indicates percentage of normal function. Results : The overall functional result ranged from 53,3% to 86.7% with an average of 68.3% of normal function. In details, the averages were 82.5% for pain, 62.5% for functional activities, 67.5% for emotional acceptance, 77.5% for use of external supports, 62.5% for walking ability, and 57.5% for gait. The average range of motion of the knee joint was $5^{\circ}$ extension and $85^{\circ}$ flexion. Five patients have extension lag ranged from $5^{\circ}$ to $15^{\circ}$ with an average of $10^{\circ}$. Two patients suffered postoperative infection. One was treated with antibiotics injection only, but the other needed removal of the prosthesis and knee fusion. Both of them showed unsatisfactory result. C o n c l u s i o n : The overall functional result after limb salvage using tumor prosthesis with medial gastrocnemius rotational flap for osteosarcoma of the proximal tibia was relatively satisfactory in case of no postoperative infection. The patients were less satisfactory in functional activities, emotional acceptance and gait than pain, use of external supports due to limitation of motion and extension lag. More aggressive postoperative physical therapy and protection with brace for 6~9 months as well as surgical technique is mandatory for more satisfactory result.

The 1998, 1999 Patterns of Care Study for Breast Irradiation after Mastectomy in Korea (1998, 1999년도 우리나라에서 시행된 근치적 유방 전절제술 후 방사선치료 현황 조사)

  • [ $\underline{Purpose}$ ]: To determine the patterns of evaluation and treatment in patients with breast cancer after mastectomy and treated with radiotherapy. A nationwide study was performed with the goal of improving radiotherapy treatment. $\underline{Materials\;and\;Methods}$: A web- based database system for the Korean Patterns of Care Study (PCS) for 6 common cancers was developed. Randomly selected records of 286 eligible patients treated between 1998 and 1999 from 17 hospitals were reviewed. $\underline{Results}$: The ages of the study patients ranged from 20 to 80 years (median age 44 years). The pathologic T stage by the AJCC was T1 in 9.7% of the cases, T2 in 59.2% of the cases, T3 in 25.6% of the cases, and T4 in 5.3% of the cases. For analysis of nodal involvement, N0 was 7.3%, N1 was 14%, N2 was 38.8%, and N3 was 38.5% of the cases. The AJCC stage was stage I in 0.7% of the cases, stage IIa in 3.8% of the cases, stage IIb in 9.8% of the cases, stage IIIa in 43% of the cases, stage IIIb in 2.8% of the cases, and IIIc in 38.5% of the cases. There were various sequences of chemotherapy and radiotherapy after mastectomy. Mastectomy and chemotherapy followed by radiotherapy was the most commonly performed sequence in 47% of the cases. Mastectomy, chemotherapy, and radiotherapy followed by additional chemotherapy was performed in 35% of the cases, and neoadjuvant chemoradiotherapy was performed in 12.5% of the cases. The radiotherapy volume was chest wall only in 5.6% of the cases. The volume was chest wall and supraclavicular fossa (SCL) in 20.3% of the cases; chest wall, SCL and internal mammary lymph node (IMN) in 27.6% of the cases; chest wall, SCL and posterior axillary lymph node in 25.9% of the cases; chest wall, SCL, IMN, and posterior axillary lymph node in 19.9% of the cases. Two patients received IMN only. The method of chest wall irradiation was tangential field in 57.3% of the cases and electron beam in 42% of the cases. A bolus for the chest wall was used in 54.8% of the tangential field cases and 52.5% of the electron beam cases. The radiation dose to the chest wall was $45{\sim}59.4\;Gy$ (median 50.4 Gy), to the SCL was $45{\sim}59.4\;Gy$ (median 50.4 Gy), and to the PAB was $4.8{\sim}38.8\;Gy$, (median 9 Gy) $\underline{Conclusion}$: Different and various treatment methods were used for radiotherapy of the breast cancer patients after mastectomy in each hospital. Most of treatment methods varied in the irradiation of the chest wall. A separate analysis for the details of radiotherapy planning also needs to be followed and the outcome of treatment is needed in order to evaluate the different processes.
