Postoperative Deep Infection after Arthroscopic Knee Surgery (슬관절 관절경 수술 후 발생한 심부 감염)

  • Purpose: The purpose of this study is to evaluate characteristics of a deep infection after arthroscopic knee surgery. Materials and Methods: We selected 894 patients who underwent arthroscopic knee surgery between February 1994 and August 2006. We analyzed the results of the patients with definite infection. Results: Seven cases out of the 894 knee arthroscopic surgery which was performed by one surgeon during 12 years were diagnosed as postoperative deep infection (0.9%). Infection developed in one repair case among the meniscal surgeries (1419=0.2%). There were six infection cases in intraarticular ligament reconstruction (6/343=2%); 3 in ACL surgeries (3/152), 2 in PCL surgeries (2/70) and 1 in combined cruciate ligament surgery and extra-articular reconstruction (l/26). Conclusion: Postoperative infection rate of arthroscopic knee surgery was relatively low. However an attention for the prevention of postoperative deep infection should be paid in intraarticular ligament reconstruction because of its relatively high risk of infection.

CLINICAL STUDY ON THE ETIOLOGY, DIFFERENTIAL DIAGNOSIS AND TREATMENT OF TRISMUS (개구장애 환자의 병인, 감별진단 및 치료방식에 대한 임상연구)

  • Trismus is a common problem to most people experiencing at once in his or her life and to most dental practitioners experiencing frequently. It has a number of potential causes which are single factor or complex factors. Its treatment will depend on the cause. The purpose of this study was to discuss the causes of trismus condition and the various treatments available. This study was made by reviewing of collected data from 86 patients complained of trismus among patients who were diagnosed by TMD, tumor, infection including tetanus, soft tissue anomalies, bony fracture and ankylosis from Jan 2002 to Dec 2004 on department of oral and maxillofacial surgery at Pusan National University Hospital, South Korea. The clinical reviews regarding chief complaints, clinical characteristics, diagnostic examination, treatments and the results on the patients were given as follows. 1. The etiology of trismus commonly were derived from temporomandibular joint(TMJ) disorder, TMJ ankylosis, TMJ tumor, odontogenic maxillofacial infection, mandibular condylar fracture, tetanus. 2. The chief complaints of trismus patients were progressive mouth opening limitation, TMJ pain, malocclusion, facial asymmetry, retrognathic state. 3. Especially, for the differential diagnosis between the fibrous ankylosis and true bony ankylosis, computed tomogram (CT) was useful. Surgical gap arthroplasty on bony ankylosis patients was applied and the gain of mouth opening after operation was average 35.8 mm during 19 months. 4. The tetanus, rarely, also induced the trismus with the range of mouth opening less than 10 mm. The average serum level of tetanus anti-toxin was 0.02-0.04 IU/mL. The limitation of mouth opening was improved into average 38 mm on 4 weeks after injection of 10,000 units of tetanus immune globulin. 5. In the treatment of osteochondroma, TMD, odontogenic infection and fracture, and the others inducing trismus, to obtian the maximum result and decreased inadequate time and effort, it is important to finding the causes from the exact clinical examination and diagnosis.

The Elderly Health and Dietary Management in Gyeonggi Province - Comparison with Gender Difference - (경기지역 노인의 건강과 식생활관리 I - 노인의 성별 비교 -)

  • The purpose of this study is to search how to promote health and improve nutrition and health care of the elderly people in rural area. Behaviors for health promotion and habits against health risk were surveyed. Dietary management was analyzed for surveyed nutrient intake by 24 hr -recall method. 242 subjects were collected in 12 cities or counties in Gyeonggi Province and 20 elderly people (10 male and 10 female) were selected out of 1 village in each district. Questionnaire for health behavior and dietary management was carried out by trained interviewers. Statistical analyses were made by SAS (version 8.1) and Chi-square tests and General Linear Models were used. Characteristics of the elderly people were 61-74 year-olds (68.2%), elementary school educated (78.4%), with spouse (51.7%), monthly living cost of 500-1,000 thousand won(43.4%), and monthly pocket money of 50-100 or 100-200 thousand won(33.5%, 26.5%). 41.4% of the subjects checked up medical examination regularly. The alcohol drinking status was significantly different according to gender: high no-drink rate of female (52.5%) and low no-drink rate of male (25.6%). Kinds of disease were different according to gender: higher proportion of cardiovascular disease(46.3%) and diabetes mellitus(8.1 %) in male and joint lumbago neuralgia(44.4%) and osteoporosis(8.6%) in female. Gastrointestinal complaints were nausea (69.0%) and chronic indigestion (17.8%). Constipation (12.0%) and vomiting (4.3%) were more frequent in female. Dietary management was good (3 meals per day: 93.4%., dining with family: 72.4%, regular mealtime: 72.4%, and 3-4 times of snacks per week: 44.9%) except side dish taking of 3-4 kinds only. However, almost one-third of the female elderly ate alone (30.6%) which was well compared with one-fifth of the male (19.7%). And food and nutrients intake were not significantly different according to gender except that male elderly's intake of energy and protein was lower than that of female's. The surveyed subjects had no difficulties in Activities of Daily Living (ADL), but some female elderly had some difficulties with Instrumental Activities of Daily Living (IADL) like working at home, using transport, and going shopping. These results suggest that low quality of life linked with low economic status of the rural elderly and that congregate meal at village hall would be required for the female elderly eating alone. For the undernourished male elderly, it would be needed to provide snacks and to establish nutrition and health surveillance system.

Type I immune-mediated polyarthritis with azathioprine therapy in a Shih-tzu dog

  • A 2-month-old female Shih-tzu dog was referred because of lameness, exercise intolerance, depression, elbow and stifle joint swelling. Physical examination, complete blood counts, serum-chemistry, radiography, synovial fluid analysis, antinuclear antibody test, and rheumatoid factor measurement were initiated. On radiography, soft tissue swelling of elbow and stifle joints without erosiveness were founded. The results of synovial fluid analysis revealed severe neutrophilic pleocytosis (nondegenerative), decreased viscosity, increased turbidity, positive on mucin-clot test, and negative on bacterial culture. The results of rheumatoid factor measurement and antinuclear antibody test were negative and below 1 : 40, respectively. Based on all tests, we diagnosed this case as juvenile onset type I immune-mediated polyarthritis. Azathioprine (1 mg/kg body weight, per os q 24 h, for 4 weeks) was then administered and clinical signs improved gradually. Four weeks after azathioprine administration, clinical signs were disappeared. This report describes the clinical findings, imaging characteristics, synovial fluid findings, and other laboratory results of type I immune-mediated polyarthritis and successful management with azathioprine therapy.

Petrology of the Volcanic Rocks in the Paekrogdam Crater area, Mt. Halla, Jeju Island (제주도 한라산 백록담 분화구 일대 화산암류의 암석학적 연구)

  • The Paekrogdam summit crater area, Mt. Halla, Jeju Island, Korea, composed of Paekrogdam trachyte, Paekrogdam trachybasalt, and Manseidongsan conglomerate in ascending order. Joint systems show concentric and radial patterns around the summit crate wall. The Paekrogdam crater is a summit crater lake which erupted the tuffs, scorias and lava flows of Paekrogdam trachybasalt after the emplaceent of Paekrogdam trachyte dome. SiO$_2$ contents of mafic and felsic lavas are respectively, 48.0∼53.7 wt.% and 60.7∼67.4 wt.%, reflecting bimodal volcanism. And lavas with SiO$_2$ between 53.7 wt.% and 60.7 wt.% are not found. According to TAS diagram and K$_2$O-Na$_2$O diagram, the volcanic rocks belong to the normal alkaline rock series of alkali basalt-trachybasalt-basaltic trachyandesite and trachyte association. Oxide vs. MgO diagrams represent that the mafic lavas fractionated with crystallization of olivine, clinopyroxene, magnetite and ilmenite and felsic trachyte of plagioclase and apatite. The characteristics of trace elements and REEs shows that primary magma for the trachybasalt magma would have been derived from partial melting of garnet peridotite mantle. In the discrimination diagrams, the volcanic rocks are plotted at the region of within plate basalt (WPB).

Biomechanical Testing and Evaluation for Korean Badminton Shoes Project(I) (한국형 배드민턴화 개발을 위한 생체역학적 성능평가(I))

  • The purpose of this project was to determine biomechanical differences between Type A(Korean brand) and Type B(world top brand) badminton shoes and to make recommendations to improve the fit and function of Type A badminton shoes. Measurements of shoe shape and dimensions, foot movement within the shoe, cushioning of ground reaction forces, in-shoe pressure and outsole traction were performed. In addition, subjective feedback of the fit and function of the shoes was quantified for 17 recreational badminton players. Type A shoe had a much higher heel and shallower heel cup, so the heel was not secured well in the shoe and the ankle joint was higher off the ground. Foot slippage was up to 40% greater in Type A shoe than Type B shoe. Impact forces and peak pressures under the foot were generally higher with Type A shoe compared to Type B shoe. The flexion axis of Type A shoe occurred in the midfoot, not at the ball of the foot like Type B shoe, where you would want the shoe flexion to occur. In summary, there are several characteristics where A Type shoe and B Type differ. Therefore, a few recommendations are provided to help improve the fit and function of A Type shoe.

Development of a Data Reference Model for Joint Utilization of Biological Resource Research Data (생물자원 연구데이터의 공동 활용을 위한 데이터 참조모델 개발)

  • The biological resources research data around the world are not only very critical themselves but should be shared and utilized. Up to now, the biological resources have been compiled and managed individually depending on the purpose and characteristics of the study without any clear standard. So, in this study, the data reference model would be suggested which is applicable in the phase ranging from the start of the construction of the information system and which can be commonly used. For this purpose, the data model of the related information system would be expanded based on the domestic and foreign standards and data control policy so that the data reference model which can be commonly applicable to individual information system would be developed and its application procedure would be suggested. In addition, for the purpose of proving the excellence of the suggested data reference model, the quality level would be verified by applying the Korgstie's data model evaluation model and its level of data sharing with the domestic and foreign standards would be compared. The test results of this model showed that this model is better than the conventional data model in classifying the data into 4 levels of resources, target, activities and performances and that it has higher quality and sharing level of data in the data reference model which defines the derivation and relation of entity.

Deterioration Assessment and Consolidation Effect of Ethylsilicate Consolidants for Samneunggyeseongakyukjonbul(Rock-carved Yukjonbul Buddha in Samneung Valley) in Namsan, Gyeongju (경주 남산 삼릉계곡 선각육존불의 훼손도 평가와 표면 강화처리제 적용 효과)

  • This study demonstrates the consolidation effect of ethylsilicate consolidants considering material characteristics and weathering degree of Samneunggyeseongakyukjonbul(rock-carved Yukjonbul Buddha in Samneung Valley) in Namsan, Gyeongju. The buddha statue is composed of alkali feldspar granite and contains numerous sets of joint with exfoliation and granular disintegration, therefore the statue is necessary to be treated for surface strength. The laboratory and in-situ tests of consolidation effect showed more increase of ultrasonic velocity that KSE 300, a relatively highly concentrated consolidant, performed more increase of ultrasonic velocity and decrease of porosity than others after treatments in weathered granite. And the consolidated rock with OH 100 was more resistant to salt weathering. For the buddha statues, KSE 300 is more applicable to enhance surface strength because it showed higher consolidation effect for long term than OH 100 and the statues has not been weathered by salts.

A Discussion of the Two Alternative Methods for Quantifying Changes : by Pixel Values Versus by Thematic Categories (변화의 정량화 방법에 관한 고찰 : 픽셀값 대 분류항목별)

  • In a number of areas, there are important benefits to be gained when we bring both the detection and monitoring abilities of remote sensing as well as the philosophical approach and analytic capabilities of a geographic information system to bear on a problem. A key area in the joint applications of remote sensing technology and GIS is to identify change. Whether this change is of interest for its own sake, or because the change causes us to act (for example, to update a map), remote sensing provides an excellent suite of tools for detecting change. At the same time, a GIS is perhaps the best analytic toot for quantifying the process of change. There are two alternative methods for quantifying changes. The conceptually simple approach is to un the pixel values in each of the images. This method is practical but may be too simple to identify the variety of changes in a complex scene. The common alternative is called symbolic change detection. The analyst first decides on a set of thematic categories that are important to distinguish for the application. This approach is useful only if accurate landuse/cover classifications can be obtained. Persons conducting digital change detection must be intimately familiar with the environment under study, the quality of the data set and the characteristics of change detection algorithms. Also, much work remains to identify optimum change detection algorithms for specific geographic areas and problems.

Framework for Technology Valuation of Early Stage Technologies (초기단계 기술의 가치평가 방법론 적용 프레임워크)

  • Early stages of technology valuation have been often overlooked or under-represented. The early stage technologies are even riskier due to their inadequacy of commercial development and market applicability. More than 95% of patents fail to earn any revenues so that the majority of patents were valueless. Technology transfers from laboratories at universities and research institutes to industrial firms have increased to acquire value from invented technologies. Technology transfer, a process of transferring discoveries and innovations resulted from research to commercial sectors, typically comprises several steps: disclosing the discoveries and innovations, i.e., intellectual property (IP), evaluating the IP's economic prospects, securing a patent, copyright or trademark for the IP, commercializing the technology through licensing, forming a joint venture, or selling. At each of those stages in the research and development of technology, the value of technology would play a very important role of making decision on the movement toward the next step, however, the financial value of technology is not easy to determine due to a great amount of uncertainty in the course of research and development, and commercialization. This paper refers to technology embodied as devices, equipment, software or processes primarily developed at public research institutions such as universities. Sometimes it is also as the result of externally financed projects contracted with industry. Nearly always technology developed at public research entities results in laboratory prototypes. When it is required to define the technology transfer contract terms for the license of the university patrimonial rights to external funding companies or other interested parties, a question arises: what is the monetary value? In this paper, we present a method for technology valuation based on the identification of specific value points related to its development. The final technology value must be within previously defined value limits. This paper consists of the review of issues related to technology transfer and commercialization, the identification of characteristics of technologies in the early stage of technology development, the formulation of framework of methods to value the early stage technologies, and the conclusion and implication of the previous review.
