Physiological Studies on Adaptation of Tilapia(Oreochromis miloticus) in the Various Salinities II. Serum Components Levels and Electrophoretic Patterns (틸라피아의 해수순치에 관한 생리학적 연구 II. 혈청성분과 전기영동상의 변화)

  • This study was taken to examine serum components concentrations and electrophoretic patterns of female tilapia(Oreochromis niloticus) living in 0$\textperthousand$, 10$\textperthousand$, 20$\textperthousand$, and 30$\textperthousand$ salt concentrations, respectively. The results obtained in these experiments were summarized as follows. The level of albumin and total protein showed changes in each salinity, but didn't significantly(P<0.05) change in Oreochromis niloticus. The level of BUN didn't significantly(P<0.05) change. When fish were adapted from 0$\textperthousand$ to 10$\textperthousand$, 20$\textperthousand$ and 30$\textperthousand$, each calcium level in every salinity groups showed less than that of control, and didn't significantly change in 10$\textperthousand$, 20$\textperthousand$, 30$\textperthousand$ salinity. The level of calcium didn't significantly(P<0.05) change in each salinity. In 20$\textperthousand$ salinity, the level of cholesterol was at the highest peak. When fish were adapted from 0$\textperthousand$ to 10$\textperthousand$, 20$\textperthousand$ and 30$\textperthousand$, each glucose level gradually decreased. When fish were adapted from 0$\textperthousand$ to 10$\textperthousand$, 20$\textperthousand$ and 30$\textperthousand$, each glucose level gradually decreased. When fish were adapted from 0$\textperthousand$ to 10$\textperthousand$, 20$\textperthousand$ and 30$\textperthousand$. In 30$\textperthousand$ salinity, the level of alkaline phosphatase was at the highest peak. The level of serum enzyme such as SGOT and SGPT was higher in seawater-adapted group than in freshwater group. The level of phosphorus chnage significantly(P<0.05) in each salinity. Correlation coefficient between serum albumin and glucose in 0$\textperthousand$ was +0.924. Correlation coefficient between serum SGOT and SGPT of individuals in 0$\textperthousand$ was +0.917. Fraction 1 of transferrin patterns of tilapia(Oreochromis niloticus) adapted in seawater was much thicker than that of transferrin patterns of individuals adapted in freshwater. Also fraction No. a wasn't observed in some individuals adapted in freshwater. These results showed that transferrin adapted in seawater relatively increased. Slight differences, that is, showed to be observed in total iron binding capacityand iron saturatin rate between tilapia adapted in freshwater and in seawater. The increase in total iron binding capacity was attributed to a rise in transferrin pressent in the first fraction of serum protein adapted in seawater. Accordingly, the serum iron levles seemed to be related to salinity($\textperthousand$).

The Effect of Nutrition Education and Iron Supplementation on Iron Status of High School Girls (여고생의 철영양상태 개선을 위한 영양교육과 철보충제 효과 연구)

  • This study was designed to investigate the effects of iron supplementation and nutrition education on the iron status and anemia of high school girls. The subjects resided in Ulsan city in Korea and were already diagnosed as having anemia or iron deficiency. Over a period of three months, one iron tablet (80 mg Fe as ferrous sulfate/day) was administered to the iron deficient subjects and two tablets (160 mg Fe as ferrous sulfate/day) were administered to the anemia subjects. The average height and weight of anemia subjects were 161.24 $\pm$ 4.50 cm and 50.87 $\pm$ 5.86 kg, respectively. The average BMI (kg/$m^2$ )was 19.58 $\pm$ 2.03 and the PIBW(percent ideal body weight) were 92.52 $\pm$ 9.84%. Except for vitamin A and vitamin C intakes, the intake levels of all other nutrients were below the RDA. Total calorie intakes of anemia subjects were 73.5% of RDA. The iron intakes of subjects from food were 69. 1% of RDA and the Ca intakes were 59.1% of RDA. The basal hemoglobin(Hb) concentration of anemia subjects averaged 10.77 $\pm$ 1.33 g/dl, and this increased significantly (p < 0.001) to 12.12 $\pm$ 1.08 g/dl, after iron supplementation. The basal ferritin, and transferrin saturations {TS (%)}of anemia subjects were 12.51 $\pm$ 15.19 ng/$m\ell$ and 8.43 $\pm$ 7.56%, respectively, and these significantly increased to 20.59 $\pm$ 22.39 ng/$m\ell$ and 15.56 $\pm$ 12.87%, respectively. The level of total iron binding protein (TIBC) significantly decreased from the initial 486.80 $\pm$ 70.16 $\mu\textrm{g}$/dl to 417.86 $\pm$ 67.73 $\mu\textrm{g}$/dl (p < 0.001) after iron supplementation. For the iron deficiency subjects, the ferritin, iron and TS(%) levels were increased significantly (p < 0.001) and the TIBC levels were significantly (p <0.001) decreased after iron supplementation. Anemia symptoms such as 'Feeling blue (p<0.05)', 'Decreased ability to concentrate (p<0.001)' and 'Poor memory (p<0.05)' improved significantly after iron supplementation in the anemia subjects. The number of tablets administered was positively correlated with changes in serum hemoglobin (t=0.194, p< 0.01), serum ferritin (t=0.181, p<0.01), TS(%) (t=0.141, p<0.05), and hematocrit (t=0.254, p<0.01), and was negatively correlated with changes in TIBC (t=-0.143. p<0.05) and red cell distribution width (RDW, t=-0.140, p<0.05). In conclusion, daily iron supplementation was effective in improving the iron status and reducing symptoms of anemia in high school girls. (Korean J Nutrition 35 (9) : 943~951,2002)

  • The ferrokinetics and red cell life spans of the patients with chronic glomerulonephritis were investigated by the double tracing method using radioactive iron ($^{59}Fe$) and chromium ($^{51}Cr$). According to the serum NPN levels, the patients were subdivided into 3 groups: Group 1. 6 patients, had the levels below 40 mg/dl Group 2. 6 patients, had the levels between 41 mg/dl to 80 mg/dl Group 3. 10 patients, had the levels above 80 mg/dl The results were as follows: 1) Red blood cell-, hematocrit- and hemoglobin values were moderately reduced in patients with normal serum NPN levels, while markedly reduced in patients with elevated serum NPN levels. 2) The plasma volume was increased, while the red cell volume was decreased in patients with elevated serum NPN levels, hence, total blood volume was unchanged. 3) The serum iron level was slightly reduced h patients of groups 1 and 2, while was within the normal ranges in patients of group 3. 4) i) In patients with normal serum NPN levels, the plasma iron disappearance rate, red cell iron utilization rate, red cell iron turnover rate, daily red cell iron renewal rate, circulating red cell iron and red cell iron concentration were within the normal ranges, while the plasma iron turnover rate was slightly reduced. ii) In patients with elevated serum NPN levels, the plasma iron disappearance rate was delayed, while the plasma iron turnover rate was within the normal ranges. The red cell iron utilization rate, red cell iron turnover rate and circulating red cell iron were decreased and the period in which the red cell iron utilization rate reachd its peak was delayed in Group 3 patients. The daily red cell iron renewal rate and the red cell iron concentration were unchanged. iii) The mean red cell life span was within the normal ranges in patients with normal serum NPN levels, while was shortened in patients with elevated serum NPN levels.

The Relationship between Helicobacter pylori Infection and Iron-Deficiency: Seroprevalence Study in 937 Pubescent Children (Helicobacter pylori 감염과 철 결핍의 관계: 937명의 사춘기 환아의 혈청 유병률 연구)

  • Purpose: The purpose of this study was to investigate the relationship between Helicobacter pylori (H. pylori) infection and iron-deficiency anemia in pubescent children, susceptible to iron deficiency due to the high iron requirements for growth. Methods: Hemoglobin, serum iron, total iron-binding capacity, serum ferritin, and serum IgG antibodies to H. pylori were measured in 937 children (475 boys and 462 girls). Their ages ranged from 10 to 18 years. The prevalences of H. pylori infection were compared between groups, based on the presence or absence of anemia, hypoferritinemia, iron deficiency, and iron-deficiency anemia. The levels of hemoglobin, serum iron, total iron-binding capacity, transferrin saturation, and serum ferritin were obtained according to the presence or absence of H. pylori infection. Results: The prevalences of anemia, iron deficiency, iron-deficiency anemia, and H. pylori infection were 8.1%, 9.1%, 3.1%, and 20.8%, respectively. The H. pylori-positive rates in anemia, hypoferritinemia, and iron-deficiency group were 34.2%, 29.5%, and 35.3%, respectively, compared to 19.6% in the non-anemia group, 19.2% in the non-hypoferritinemia group, and 19.4% in the non-iron deficiency group. The H. pylori-positive rate in the iron-deficiency anemia group was 44.8% in comparison with 20.0% in the non-iron-deficiency anemia group. Hemoglobin and iron levels did not show any significant differences between the H. pylori-positive and -negative groups, whereas the serum ferritin level decreased significantly in the H. pylori-infected group. Conclusion: H. pylori infection is thought to be associated with iron deficiency in pubescent children.

Relationship between Serum pro-hepcidin Concentration and Body Iron Status in Female College Students (여대생에서 혈청 pro-hepcidin 농도와 철분 영양 상태와의 관계)

  • Hepcidin, a peptide hormone synthesized mainly by the liver, has been implicated as a key regulator of iron homeostasis. Results from studies with experimental animal models suggested that hepcidin levels are related with body iron status, but little data is available in human subjects. This study was conducted to determine the relationship between serum pro-hepcidin levels, blood indexes of anemia, and dietary iron intake in female college students. Serum pro-hepcidin concentrations were measured by enzyme-linked immunosorbent assay in eighty-two women with $22.1\pm0.2$ years old. Dietary intake data were collected by using the 24-hour recall method for 3 days. Mean concentrations of serum pro-hepcidin were 85.1 ng/ml$\pm$6.1(s.d.) with the range of 13.6-295.7 ng/ml. The median value of serum pro-hepcidin in the study subjects was 70.3 ng/ml. Serum pro-hepcidin concentrations were positively correlated with hemoglobin concentrations (r=0.273, p=0.013), and also with hematocrit (r=0.291, p=0.008). To examine whether the level of dietary iron intake affects serum pro-hepcidin levels, study subjects were divided into two groups according to the amounts of daily iron intake. Serum pro-hepcidin concentrations were $22\%$ lower in groups with low iron intake (${\leq}10.1$ mg/day), compared to high-iron intake group (>10.1 mg/day) . In conclusion, these data, as in agreement with findings in mice, suggest that hepcidin plays an important role in regulating iron metabolism in the human body.

Action Mechanism of LB10522, a New Catechol-Substituted Cephalosporin (카테콜 치환체를 가진 세파로스포린계 항생제 LB10522의 작용기전)

  • LB10522 is a new parenteral broad spectrum cephalosporin with a catechol moiety at C-7 position of beta-lactam ring. This compound can utilize tonB-dependent iron transp ort system in addition to porin proteins to enter bacterial periplasmic space and access to penicillin-binding proteins (PBPs) which are the lethal targets of ${\beta}$-lactam antibiotics. The chelating activity of LB10522 to metal iron was measured by spectrophotometrically scanning the absorbance from 200 to 900nm. When $FeCl_3$ was added, optical density was increased between 450 and 800nm. LB10522 was more active against gram-negative strains in iron-depleted media than in iron-replete media. This is due to the increased expression of iron transport channels in iron-depleted condition. LB10522 showed a similar activity against E. coli DC2 (permeability mutant) and E. coli DCO (wild type strain) in both iron-depleted and iron-replete media, indicating a minimal permeaility barrier for LB10522 uptake. LB10522 had high affinities to PBP 3 and PBP 1A, 1B of E. coli. By blocking these proteins, LB10522 caused inhibition of cell division and the eventual death of cells. This result was correlated well with the morphological changes in E. coli exposed to LB10522. Although the in vitro MIC of LB10522 against P. aeruginosa 1912E mutant (tonB) was 8-times higher than that of the P. aeruginosa 1912E parent strain, LB10522 showed a similar in vivo protection efficacy against both strains in the mouse systemic infection model. This result suggested that tonB mutant, which requires a high level of iron for normal growth, might have a difficulty in surviving in their host with an iron-limited environment.

The Use of Iron Supplements of Pregnant Women and Pregnancy Outcome (임신부의 철분 보충제 사용과 임신결과)

  • It is known that Korean pregnant women take iron supplements at a higher than the recommended level. This study was designed to provide data on current iron intake levels both from food sources and supplement to better guide iron supplement use during pregnancy. We also explored associations of iron supplement intake levels with various sociocultural factors and pregnancy outcomes. Dietary intakes of 510 pregnant women were assessed by a validated 102-item food frequency questionnaire, and information on types and amounts of nutritional supplement intakes were also attained. While dietary intake levels of most nutrients exceeded the KDRIs (Korea Dietary Reference Intakes: EAR: Estimated Average Requirements), folate fell short of the KDRIs. A total of 428 women (83.9%) reported to take iron supplement. The pregnant women were divided into the three groups (group I: Fe supplement intake ${\le}$ EAR, group II: EAR < Fe supplement intake ${\le}$ 3 times of EAR, group III: 3 times of EAR < Fe supplement intake). The mean dietary intake of iron was 24% of the total iron intake for pregnant women. Iron intake from food was not significantly different among I, II, and III. In case of iron intake from supplements, the most frequent dose (34.1%) was 90-100 mg/day, and the mean iron supplement intake was 362% of the EAR. The study findings showed that those with higher levels of iron supplements had better meal quality measured by NAR (Nutrient Adequacy Ratio) and INQ (Index of Nutrient Quality). In addition iron supplement intake levels were significantly related to age (20s: 66.5 ${\pm}$ 38.6 mg/day, 30s: 77.3 ${\pm}$ 47.8 mg/day, p < 0.0116) and experience of childbirth (1st pregnancy: 70.9 ${\pm}$ 41.2 mg/day, 2nd pregnancy: 64.5 ${\pm}$ 39.5 mg/day, ${\ge}$ 3rd pregnancy: 94.4 ${\pm}$ 63.8 mg/day, p < 0.005). However, no significant difference was found between iron supplement intake levels and various pregnancy outcomes including birth weight, birth height, gestational age, weight gain during pregnancy, and jaundice. It is worrisome that iron intake by supplement use greatly exceeded the EAR, suggesting the need of appropriate guidelines for iron supplement intake during pregnancy. Thus iron overdose from supplements in pregnancy should be considered as a serious condition.

A Cross-sectional Study of Biochemical Analysis and Assessment of Iron Deficiency by Gestational Age(II) (임신 시기별 생화학적 철분 분석 및 철분 결핍상태에 대한 횡적 조사 연구(II))

  • The purpose of this research is to assess hematological and biochemical status and the prevalence of iron deficiency of pregnant women by gestational age to provide the primary data about iron nutritional status of pregnant women. Pregnant women visiting public health centers in Ulsan participated in study and were divided into 3 trimester by last menstrual period(LMP). Hemoglobin (Hgb), hematocrit(Hct)and mean corpuscular volume(MCV) among iron status indices were not statistically different from normal distribution, however total iron binding capacity(TIBC) and serum ferritin were skewed to left and serum iron and transferrin saturation(TS) were skewed to right. Hgb was positively correlated with Hct(r=0.93, p<0.001) but TIBC was negatively correlated with all indices. Serum ferritin was also correlated with all indices, especially in 3rd trimester but not reached to 1st trimester level. Mean corpuscular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC), Red cell distribution width(RDW), serum iron and TS were not significantly different by trimester, however when serum serum iron was adjusted with hematocrit to correct the hemodilution, it significantly decreased in 2nd trimester. MCV increased in 2nd trimester and was maintained until late pregnancy, TIBC continued to increase throughout the trimester. The prevalence of anemic by CDC(Centers for Disease Control) Hgb criteria(Hgb <11.0g/dl in 1st and 3nd trimester, Hgb<10.5g/dl in 2nd trimester) was 2.8% in 1st trimester, 22.5% in 2nd trimester, 27.1% in 3rd trimester and was similar with prevalence by CDC Hct criteria(Hct < 33% in 1st and 3rd, Hct < 32% in 2nd). The prevalence of anemic of total subjects was 32.7% by WHO criteria(Hgb < 11.0g/dl). Although almost iron status indices increased in 3rd trimester, the prevalence of anemia by different criteria of all indices increased throughout the trimester, so iron nutritional status was considered as serious during late pregnancy. However, since factors other than iron deficiency, such as infection, infection, inflammation, other nutrient deficiency may also play a significant role, to differentiate the anemia due to mainly iron deficiency from the anemia due to other factors, serum ferritin is among the more useful indices in distinguishing the two conditions because it is depressed only in iron deficiency. Hgb<11.0g/dl and serum ferritin<12.0ug/L as the criteria of iron deficiency was suggested by CDC. 17.8% of all subjects were classified as iron deficient anemia, 14.9% as anemic from other reasons, 21.2% as iron deficiency any only 46.2% were in normal iron status.

A Comparative Study of the Iron Nutritional Status of Female College Women according to Bone Mineral Density

  • The purpose of this study was to investigate the relationship between nutritional status of iron and bone minernl density in premenopausal women. In the study, we classified the subjects into osteopenia (-2.5-I, n=29) groups according to their lumbar spine bone mineral density. Anthropometric measurements, dietary intake analysis and blood biochemistry measurements were performed on the subjects. The average ages of those in the osteopenia and normal groups were 22.2 yrs and 23.0 yrs, respectively, with no significant difference. The average body mass index (p<0.05) of those in the osteopenia group (19.6) was significantly lower than that of the normal group (21.3). The mean protein intake of those in the osteopenia group was significantly lower than that (p<0.05) the subjects in the normal group. The osteopenia group consumed a significantly lower amount of iron (p<0.05) and non-heme iron (p<0.05) compared to the normal group. The intakes of total food, vegetables and milk of those in the osteopenia group were significantly lower than those of the subjects in the normal group. The serum ferritin (p<0.001) level of those in the osteopenia group was significantly lower than those of the subjects in the normal group. In conclusion, a balance of iron status may be helpful in the prevention of bone mass loss in premenopausal young women.

  • We surveyed Korean pregnant women who participated in the Maeil mothers' club and factory tour from March to April, 2004 and assessed volunteers' hemoglobin (Hb) levels. The average Hb level of 747 subjects was $11.5{\pm}1.5g/dl$ and anemic subjects were 268 women, 35.9% of total subjects. The ratio of anemic subjects was 42.9% in the first trimester, 43.6% in the second trimester and 29.1 % in the third trimester. About 87.9% of subjects began to take health foods after conception. Seventy nine percent of subjects took iron supplement as one of health foods and 73.8% of them began it in the second trimester. The results of this study showed that health foods were more effective than iron supplement alone in improving the iron status of Korean pregnant women. Therefore, the better improvement effects are expected when pregnant women begin health food-based approaches as early as possible before pregnancy to improve pre-pregnancy iron reserve.
