Implications for the Analysis of Human Error in Railway Accidents (철도 인적오류 사고분석에 대한 시사점)

  • The basic concept of analysis of human error that induced railway accident is that errors are consequences, not causes. But in most cases, it is likely that direct causes of the accident could be concluded as real causes, which make it difficult to find out root causes. Design, training, staffing, culture and condition are general category applied to investigate human error. In order to examine how those approach could help accident analysis, this paper studied accident investigation reports of UK RAIB(Rail Accident Investigation Branch). Rather than consider specific investigation method, we focus investigation result on how to describe causal factors and how to indicate recommendations to prevent similar accident. The reports show that they try to find out causes more in organizational, environmental and job factors, which implies the necessity to improve investigation process of human error accident in Korea.

The Improvement of Investigation Method and System through the Review of Musculoskeletal Risk Factor Investigation Reports (근골격계 유해요인 조사 보고서 검토를 통한 조사 방법 및 제도 개선 방안)

  • Objective: The objective of this study was to suggest improvement methods for investigations and systems through a review of musculoskeletal risk factor investigation reports conducted in the workplace. Methods: A total of 34 reports on musculoskeletal risk factor investigations were reviewed. To confirm the appropriateness of the evaluation, reevaluation was conducted through a field investigation using other evaluation tools. In order to understand the effectiveness of the improvement plan, a survey was conducted with those in charge of tasks with ergonomic burdens. Results: As a result of performing a field survey using the work sampling method recommended by the KOSHA (Korea Occupational Safety and Health Agency) for ten tasks, eight tasks were determined to pose an ergonomic burden. When 30 tasks were evaluated with RULA (rapid entire body assessment), 90% of them were evaluated at a higher level than the results of the workplace survey. Among the improvement measures, only 36% of the field workers affirmed them, and in only 19% of the workplaces were actual improvements made. Conclusions: In order to improve the ergonomic investigation system for risk factors in the ergonomic burden task, it is necessary to secure the objectivity of the evaluation performed by the institution and enable the active participation of workers in improvement activities.

Investigation of Terminology Coverage in Radiology Reporting Templates and Free-text Reports

  • The Radiological Society of North America (RSNA) is improving reporting practices by developing an online library of clear and consistent report templates. To compare term occurrences in free-text radiology reports and RSNA reporting templates, the Wilcoxon signed-rank test method was applied to investigate how much of the content of conventional narrative reports is covered by the terms included in the RSNA reporting templates. The results show that the RSNA reporting templates cover most terms that appear in actual radiology reports. The Wilcoxon test may be helpful in evaluatingexisting templates and guiding the enhancement of reporting templates.

  • Intuitive learning method was applied at the beginning of each lecture to induce students to draw their interests. Avoiding simple explanation of equations and problem solving by using them, we repeated theoretical concepts verbally and applied physical meanings when we developed and wrote equations. By these methods we expected to find a way to increase students' learning effects. We also took a group investigation on pursuing term projects. Students choose their own subjects individually and submitted reports according to the time schedule. The reports included the contents that they learned during classes. After choosing best reports for each group by instructor, students at each group divided roles and prepared presentations. Thorough these methods they increased their scores from mid-term to final exams, and got aquatinted with responsibilities among group and organizations. They also experienced physical meanings from the usual daily life phenomena which could be connected to the engineering concepts and improved abilities as junior engineers.

  • This paper reports the performance results of hydration heat control of mass concrete walls with pipe cooling system. The thickness of walls ranged from 0.9 to 2.2m. In order to investigate the effect of pipe cooling on the thermal and thermal crack characteristics, the pipe cooling was conducted for 42 walls, and the investigation of thermal cracks was conducted for 14 walls. Based on the investigation, the pipe cooling method decreased the peak temperature of about 13-2$0^{\circ}C$ and the thermal crack width of about 30% for mass concrete walls.

  • Hydrogen cyanide is one of the toxic agents with carbon monooxide in fire victims and is released by combustion of nitrogen-containing organic material such as plastic and wool. Until now there are few reports about blood cyanide concentrations in fatalities by fire in Korea. So in this study we examined blood cyanide concentration in 12 cases of fire fatalities. (omitted)

Practice in Application of the Index of Streetscape Greenness on Environmental Impact Assessment - Case Study of Street in Seoul (서울시 환경영향평가에서 가로 녹시율 지표의 적용 실태)

  • The purpose of this research is to analysis the application of the ISG(Index of Streetscape Greenness) in EIA through case study of Seoul. ISG is a new index which directly expresses human's perceptions of plants in a street and is defined as the area ratio of which leaves of plants occupy an eye-level view of a person standing on the center line of a street. In this research the 42 cases of 3 step reports such as plans, drafts, and final reports which had been passed through the EIA procedure of Seoul were analyzed. The results are as follows: In the drafts frequent mistakes or errors such as unsuitable picture composition, unsuitable sample points, inconsistency of the simulation result with the planting plan, and insufficient numbers of analysis were found, but almost of them were corrected in the final reports. In the most of final reports, the 30% of minimum requirement was satisfied. At all cases the changes of the ISG values were clearly positive. Finally the average current ISG was 19.6%, meanwhile the average proposed ISG was 35.4%. It was verified that the ISG is very practical index to promote green streetscape, but in order to guarantee an achievement a thorough investigation of reports are needed.

  • We are faced with water pollutions on a population explosion. Considering the importance, we research European rivers based on OECD reports. Observations in the reports have defects that make evaluation of environmental situations be difficult. By using interpolations in the compensation quantitative structure-activity relation ships (CQSAR), we complement the defects in the water quality of rivers through big cities. Thus, we get complete data set for dissolved oxygen, biochemical oxygen demand, and total phosphorus. Using the data set, we examine re-naturalization of the Rhein and the Donau in Germany. We investigate the effect of dams between Slovakia and Hungary, by using reconstructions of neural networks in CQSAR. The reconstructions have functions to extract a principal relation. On the investigation, we examine assertions of conservation groups. As the result, we confirm the re-naturalization is effective, and find a negative effect of the dam construction on changes of dissolved oxygens in the Hungary Donau. We investigate the Seine and the Thames, too.

Report on the trends of the Drug Abuse and the Mortalities related to Intoxication of Drug-Toxic Substances in the Central Area of Korea in 2001

  • This presentation reports the trends of the drug abuses(DA) and the mortalities related to drug-toxicants(MDT) in the Central area of Korea in 2001. We surveyed the DA cases and MDT. which were requested to analyze the drug-toxicants in the Central district office of National Institute of Scientific Investigation. he detected drugs on DA cases were methamphetamine. marihuana. opiates. inhalants(toluene. butane. ropane). dextromethorphane. carisoprodol. benzodiazepines, nalbuphine. fenfluamine. and iscellaneous in order of cases. Men are more liable to drug abuses than women. and the most common age group was 30s. Surveys of MDT shows that the defected toxicants are paraquat(sedative). methomy(insecticide). dicholrvos (insecticide). benxodiaxeqines(anxiolytic), and miscellaneous in order do cases. Men's intoxications by the drug-toxicants are more occured than woman's And most common intoxicated age group was 40s. These trends of the DA cases and the MDT in Central Area fo Korea. can help the forensic toxicologists and government to plan the prevention policy of the DA cases and MDT as well as its future estimation.

A Study Comparing Korean, Chinese, and Japanese Fire Investigation Operating System (한.중.일 화재조사 운영체제 비교연구)

  • When enforcing fire investigation, duty for basic principles, responsibility of investigation, report deadline, and all sorts of standard forms are based on fire investigation report regulation which is internal instructions. This study has a purpose to prepare alternatives that are absent in Korea or have to be strengthened by comparing and examining Japanese and Chinese fire investigation report regulations with Korean one. As a result, it appears that Korea needs to standardize the report deadline of urgency fire and general fire into 30 days when additional investigation is needed, have investigators draw delays reports up when they exceed the deadline not to make work vacuum, and found and organize forms of certificate of custody and return for all fire related materials when investigators require data from fire-related people, to improve whole systems.
