A Case of Korean Medicine Treatment for a Patient with Somatic Symptom Disorder Hospitalized after a Traffic Accident

  • Objectives: The purpose of this study was to report the effect of Korean medicine treatment on a patient with somatic symptom disorder hospitalized after a traffic accident. Methods: The subject for this study was a patient diagnosed with somatic symptom disorder that worsened after a traffic accident. During hospitalization, herbal medicine, acupuncture, psychotherapy, and other treatments were performed. Clinical effect was evaluated using Heart Rate Variability (HRV), BDI (Beck Depression Inventory)-II, BAI (Beck Anxiety Inventory), Global assessment (G/A), and Numerical rating scale (NRS). Results: After 36 days of treatment, BDI-II score was reduced from 17 to 5. BAI score was reduced from 23 to 8. Overall somatic symptoms and pain showed decreases. Conclusions: Korean medicine treatment might be effective for somatic symptom disorder that is worsened after a traffic accident.

One Clinical Case Report of Lung Cancer Patient with Depression and Anxiety Disorder Improved by Korean Traditional Medical Treatment and Breathing Meditation (한방치료와 호흡명상으로 호전된 폐암 환자의 불안 및 우울 증상 치험 1례)

  • This study was case report to show the good effects of korean traditional medical treatment and breathing meditation. The methods were applied to lung cancer patients. We treated one patient who scored 27 point at Beck's depression inventory(BDI) and 28 point at Hamilton rating scale for depression(HRSD) and 22 point at Beck's anxiety inventory(BAI) and 20 point at Anxiety status inventory(ASI) by korean traditional medical treatment and breathing meditation. After treatment, the clinical symptoms were improved and the score of BDI, HRSD, BAI, ASI were decreased. According to this study korean traditional medical treatment is effective for the cure of depression and anxiety disorder and breathing meditation forifies the korean traditional medical treatment.

A Case Report of Tinnitus with Panic Disorder treated by Daehamhyung-tang from the Disease Pattern Identification Diagnostic System based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 대함흉탕(大陷胸湯) 투여로 호전된 공황 장애를 동반한 이명 1례)

  • Objective : The purpose of this paper is to report the effect of Daehamhyung-tang(DHHT) on tinnitus with panic disorder from the disease pattern identification diagnostic system based on Shanghanlun provisions. Methods : According to a disease pattern identification diagnostic system based on Shanghanlun provisions, the patient was diagnosed with 'Taeyang-byung Gyeolhyung'. He took DHHT for 82 days. This paper evaluated the results of treatment on tinnitus by Tinnitus Handicap Inventory(THI) and Verbal Numercial Rating Scale(VNRS). To assess the mental state of anxiety, this paper used Hamilton Anxiety Rating Scale(HAM-A) Results : The THI score was declined from 58 to 6 And the VNRS score was declined from 6 to 1. HAM-A score was declined from 28 to 3. Conclusions : This case study suggests that DHHT has effectiveness on a patient suffers from tinnitus with panic disorder and diagnosed with 'Taeyang-byung Gyeolhyung'.

Maternal Separation Anxiety on Quality of Day Care and Mother's Self-Esteem (탁아의 질과 어머니 자아존중감에 따른 어머니 격리불안)

  • The purpose of this study was to examine the maternal separation anxiety depending on the quality of day care and mother's self-esteem. subjects consisted of 187 mothers who had children from 60 t 36 months of age. the instruments used were the questionnaires which consisted of the Maternal separation anxiety scale(Cho and park, 1992), Self-Esteem Inventory(Coopersmith, 1967), and the checklist which consisted of the early childhood environment Rating scale(Harms & Clifford, 1980) and the Family Day Care Rating scale(Harms & Clifford, 1989). The datas were analyzed by the statistical method based o frequency, percentile, t-test, and one-way ANOVA. The main results were as follows : First, there was a significant difference in maternal separation anxiety by mothers' age and not by their educational levels. Second, there were no significant differences in maternal separation anxiety by children's age, sex, and girth order. Third, there was no significant difference in maternal separation anxiety by quality of day 2 care, size of group, and adult-child ratio. Forth, there was a significant differences in maternal separation anxiety by mother's self-esteem.

An Overview of Pain Measurements (통증평가도구에 관한 고찰)

  • Objectives : The aim of this study is to introduce pain measurement tools that are considered suitable for clinical practice and research for Korean Medicine Doctors. Methods : We analysed some widely used and also useful pain measurement tools in terms of their methods and dimensions. Results : Diagrams, scales and questions are usually used to measure pain intensity, temporal pattern, treatment including exacerbating and/or relieving factors, pain location, pain interference, pain quality, pain affect, pain duration, pain beliefs and pain history. Specific pain measurements are also available for specific conditions such as Western Ontario and McMaster Universities Osteoarthritis Index, Oswestry Disability Index and Neck Disability Index. Conclusions : Faces Pain Rating Scale, numeric rating scale, visual analogue scale, McGill Pain Questionnaire and Brief Pain Inventory and commonly used pain measurements. Specific measurements should be considered depending on research topics.

The Relationship between Children's Social Intelligence and Demographic Variables (아동의 사회지능과 인구통계학적 변인의 관계 연구)

  • The present study investigated the relationship between demographic variables(gender, grade in school, etc.) and children's social intelligence. Elementary and middle school students responded to the self-reported inventory on social intelligence, and their teachers used the social intelligence rating scale for children. The results show gender, grade, and regional differences in children's abilities to deal with various social situation. Furthermore, parents' education and residence is also related to their children's social intelligence. Based on regression analyses, it was found that grade in school and parent education were the most influential variables in explaining variance in children's social intelligence.

  • Objects:This study was carried out to develop the Korean form of Moral Development Inventory for Adolescents designed to assess the principled morality in adolescents. And the auther also intended to find the demographic variables that influence the moral development. Methods:The Korean form inventory was applied to 507 middle and high school adolescents, and retest was done in 42 second year middle school adolescents with 3 weeks interval. Together this inventory, short form of Rest moral development which had been proved its reliability and validity, were administered to the all subjects for concurrent validity. And then the author examined whether this inventory discriminates between conduct disorder patients, psychiatric controls and normal controls in the level of moral development. Results:Test-retest reliability and internal consistency were very satisfactory. Concurrent validity with this inventory was also high level. And conduct disorder patients were scored significant lower than psychiatric and normal controls in the level of principled morality. The mean PM scores did not show significant difference according to the variables such as sex, birth order and religion. But there was significant difference according to the educational level of mother and places. Conclusions:These results indicate that the Korean form of Moral Development Inventory is a reliable and valid rating inventory to assess the moral development level in Korean adolescents.

Effect of a Qigong-Based Stress Management Program on Emotion of Pregnant Women (기공에 기초한 스트레스 관리 프로그램이 산모의 정서에 미치는 영향)

  • Objectives: The aim of this study was to examine the effectiveness of a qigong-based stress management program in pregnant women. Method: A 8 week controlled clinical trial compared 52 pregnant women assigned qigong-based stress management program to 40 pregnant women assigned pregnancy health education program. Edinburgh postnatal depression scale(EPDS), Beck depression inventory(BDI), Spielberger trait state inventory-1(STAI-1), Hamilton depression rating scale(HAM-D), Hamilton anxiety scale(HAM-A) were used to measure anxiety and depression level. Result : Qigong-based stress management group showed significant improvement compared to the education control group on four measures above. Qigong-based stress management group also showed significant improvement at 8 weeks termination on all measures of anxiety and depression compared to pretreatment level. Conclusion : This study showed that qigong-based stress management program can be an effective method for pregnant women.

Synergistic Effect of Forest Environment and Therapeutic Program for the Treatment of Depression (산림활동이 우울증 환자들의 호전에 미치는 영향: 산림치유 프로그램 집단과 병원 프로그램 집단, 산림욕 집단, 대조군 비교 연구)

  • This study was conducted to evaluate the effect of forest environment and therapeutic program to the patients with major depressive disorder in antidepressant medication. The first group participated the four sessions of therapeutic program in the forest environment ("forest therapy"), the second group did in the program in a hospital environment, the third group did merely walking in the forest ("forest bath"), and the last group was controls. All the participants was diagnosed with major depressive disorder and had been taking antidepressant medication longer than 3 months. Depressive symptoms were measured by the Hamilton Rating Scales for Depression(HRSD), Montgomery-Asberg Depressin Rating Scales(MADRS), Beck Depression Inventory(BDI), and general health perception was measured by Short Form Health Survey Questionnaire(SF-36). Heart rate variability(HRV) were also examined to observe the physiological parameters before and after the program. In the results, HRSD score of forest program group was significantly lower than controls after the program. MADRS score of forest therapy group and hospital program group was also significantly lower than controls after 4 sessions of the program. The remission rate defined as below 7 points in HRSD was higher in the forest therapy group, hospital program group, forest bath group, and controls in order. These results reveals that the therapeutic program performed in forest environment may improve unremitted depressive symptoms of patients with major depressive disorder. Although not significant, the forest therapy program showed better outcome than hosptial program and forest bath.

Study of Efficacy and Safety of Tianwangbuxin-dan for Poststroke Depression: A Randomized, Double Blinded, Non-inferiority Trial (뇌졸중 후 우울증에 대한 천왕보심단의 유효성 및 안전성 연구 : 무작위 배정, 이중맹검, 비열등성 연구)

  • Objectives : The aim of this study was to compare Tianwangbuxin-dan to Fluoxetine for antidepressive efficacy and safety in poststroke depression (PSD). Methods : A randomized, double blinded, non-inferiority trial was conducted. 113 PSD patients were recruited from a stroke center. Except for the 25 excluded patients, 88 PSD patients were randomly given either Tianwangbuxin-dan 1100 mg or Fluoxetine 20 mg per a day for 16 weeks. PSD was evaluated using Beck's depression inventory (BDI) and Hamilton depression rating scale (HDRS) and followed every fourth week. Repeated measure analysis of variance (ANOVA) was used to compare and contrast the depression scores of the two groups and to compare them among the evaluation times, at the beginning, $4^{th}$, $8^{th}$, $12^{th}$ and $16^{th}$ weeks. In addition, independent t-tests were used to find the difference between two groups at every evaluation time. Results : Finally, 88 PSD patients were included in the study, 63 PSD patients completed the procedure and 25 PSD patients were dropped out by the incompliance or withdrawal of consent. Tianwangbuxin-dan (or Fluoxetine) improved the depression of stroke patients and the efficacy of Tianwangbuxin-dan was not inferior to that of Fluoxetine. In addition, there was no significant side effect in two groups. Conclusions : This study showed that antidepressive efficacy and safety of Tianwangbuxin-dan in PSD patients.
