A Case Report of Korean Medicine Treatment for Sudden Hearing Loss Accompanied by Tinnitus (한의 치료로 호전된 이명 동반 돌발성 난청 환자 1례)

  • Objectives : This study reports a 61-year-old man with sudden hearing loss and tinnitus whose symptoms improved remarkably after treatment with Korean medicine. Methods : The patient was treated with herbal medicine(gamisoyo-san, samul-tang gagambang, gongjindan, and yukmijihwang-hwan), acupuncture and moxibustion. Puretone audiometry(PTA), speech audiometry(SA), Korean tinnitus handicap inventory(K-THI), patient self-report, numeric rating scale(NRS) 11, and review of system(ROS) were performed. Results : After a month of treatment, PTA was improved from profound to mild and audiometric findings were improved in speech reception threshold and word recognition score. After 2 months of treatment, K-THI decreased from 92 to 28 and NRS 11 score decreased more than 6 in every item. Conclusions : These results demonstrate that Korean medicine could be effective sudden hearing loss and tinnitus.

A Case Report on Papillary Thyroid Cancer for the Recurrence of Regional Cervical Lymph Nodes improved by Korean medicine (갑상선 유두암으로 수술 후 좌우 림프절 전이 환자의 장기간 복합 한의약 치료 증례보고)

  • Objective : This study assessed the effect of a combination of Korean medicine on a thyroid papillary cancer patient who was diagnosed with local lymph node metastasis after thyroidectomy and lymph node dissection but did not want surgery. Methods : Gami-Palmultang administration and moxibustion(large Bmoxa cautery) were performed for six years. Treatment outcomes were evaluated with Brief Fatigue Inventory (BFI), Numerical Rating Scale (NRS), Insomnia Severity Index (ISI), Functional Assessment of Cancer Therapy-General (FACT-G), blood test/ CT imaging results, and patient's statements. Results : After the treatment, all symptoms have been alleviated, the quality of life has increased, and it has been maintained without further metastasis of tumors for six years. Conclusion : Korean medicine treatment along with active observation can be an alternative to patients who do not want surgical treatment after recurrence of local lymph nodes in thyroid papillary cancer, and can have positive results in improving the quality of life.

The Clinical Adequacy of Internalized Shame: correlation with Self-control, Aggression, and Addiction Potential (내면화된 수치심의 임상적 타당성: 자기통제력, 공격성 및 중독가능성과의 상관을 중심으로)

  • The purpose of this study was to search for empirical evidence between internalized shame with self-control, aggression, and addiction potential. Also examine the differences between clinical group and normal group. The data was collected from 100 AA participants and 380 Non-AA participants. For measuring each variable, internalizes shame scale(ISS), self-control rating scale(SCRS), Buss-Durkee Hostility inventory(BDH), addiction potential scale(APS) were used. The survey result was analyzed with Pearson Correlation, Partial correlation and t-test. The results shows: Frist, the internalized shame and the aggression, the addiction potential shows a positive correlation but the self-control shows negative correlation. Second, after control self-control, internalized shame positively correlate to addiction potential and aggression. Third, clinical group recorded a higher level in internalized shame, aggression and addiction potential than normal group; a lower level in self-control. Based on the finding, the implications of understandings, the interventions, the limitations of this study, and the suggestions were discussed.

A Case Report: Korean Medicine Treatment for a Systemic Lupus Erythematosus Patient Persistently Complaining of Fatigue and Chest Tightness (피로와 가슴답답함(胸悶)을 주증으로 하는 전신성 홍반성 루푸스 환자에 대한 한의치료 1례)

  • Objective: This study reported the improved symptoms of integrative Korean medicine-based treatment in a patient diagnosed with Systemic Lupus Erythematosus (SLE) who persistently complained of fatigue and chest tightness. Methods: A 51-year-old female SLE patient persistently complaining of fatigue and chest tightness underwent an 18-day treatment with Kracie Shihogayonggolmoryo-tang, Cheongpajeon-H, Gwanjeol-go, acupuncture, pharmacopuncture, cupping therapy, deep tissue meridian hot pack therapy, manual therapy, Interferential Current Therapy, and Extracorporeal Shock Wave Therapy. The Brief Fatigue Inventory (BFI) and Numeric Rating Scale (NRS) for each symptom were measured on admission, at one week, two weeks of hospitalization, and upon discharge. Clinical outcomes were assessed using these two figures. Results: After treatment, there was a reduction in the BFI score (7 to 3). The NRS score of chest tightness with hot flashes decreased from 8 to 5, neck pain lessened from 7 to 5, pain in both ankles diminished from 6 to 4, the cold hypersensitivity of both feet fell from 6 to 4, and heartburn decreased from 5 to 2. In order, the biggest clinical improvement was to heartburn, fatigue, and chest tightness with hot flashes. Conclusion: This case study shows that integrative Korean medicine-based treatment can improve symptoms of a Systemic Lupus Erythematosus patient persistently complaining of fatigue and chest tightness with hot flashes and suggests that Shihogayonggolmoryo-tang may be an effective option for managing and treating SLE patients.

A Case Report of Central Post-stroke Pain Improved by Gami SSanghwa-tang (가미쌍화탕으로 호전된 뇌졸중 후 중추성 통증 환자 치험 1례)

  • ■ Objectives The purpose of this case study is to report the effect of Gami SSanghwa-tang on a patient with central post-stroke pain. ■ Methods The patient was treated with herbal medicine Gami SSanghwa-tang, acupuncture, pharmaco-acupuncture, and moxibustion. The treatment effect was evaluated by Numerical Rating Scale(NRS), Neuropathic Pain Symptom Inventory(NPSI), and 36-item Short-form Health Survey(SF-36). ■ Results After the treatment, the NRS score of pain intensity was reduced from moderate to mild degree. The total NPSI score and subscores also decreased, as the various features of the pain were relieved. The SF-36 score increased, as the patient's quality of life improved. ■ Conclusion This case study suggests that Gami SSanghwa-tang, could be effective in reducing pain and improving quality of life of patients suffering from central post-stroke pain.

A Case Study of A Patient Complaining of Disequilibrium Due to Proprioceptive Disorder (고유수용감각이상으로 인한 균형장애를 호소하는 환자 치험 1례)

  • ■Objectives This work reports the case of a patient complaining of disequilibrium due to proprioceptive disorder whose condition improved following Korean medicine treatment. ■Methods The patient was hospitalized for 15 days and treated with herbal medicine, mainly doinseunggi-tang-gami, and acupuncture, electroacupuncture, and moxibustion. Disequilibrium was clinically estimated using Korean dizziness handicap inventory(K-DHI) and the patient's subjective discomfort in this regard was also checked by numeral rating scale. The degree of improvement in blood stasis syndrome was evaluated using modified Blood Stasis Questionnaire II(modified BSQ-II) to assess the clinical effects of the treatment. ■Results After treatment, the K-DHI classification improved from severe to mild, and the patient's subjective discomfort was greatly improved from NRS 6 to NRS1. In addition, the degree of improvement in blood stasis syndrome improved from blood stasis group to blood stasis risk group compared to time of admission. ■Conclusion These results suggest that Korean medicine is effective in treating disequilibrium caused by proprioceptive disorder.

  • Objective : Fatigue is the most common symptom in post-COVID-19 syndrome. We report the case of a patient with post-COVID-19 syndrome with fatigue treated using herbal medicine (Saengkangsasim-tang). Methods : A 64-year-old man had severe fatigue for 4 months after a severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) infection. Saengkangsasim-tang was administered to this patient based on the disease pattern identification diagnostic system by Shanghanlun provisions (DPIDS). We used the Numeric Rating Scale (NRS) and Beck Depression Inventory (BDI) to evaluate the effectiveness of Saengkangsasim-tang for fatigue in the patient with post-COVID-19 syndrome. Results : According to DPIDS, the patient was diagnosed with the provision 157. After administration of Saengkangsasim-tang for 30days, the fatigue based on NRS and depression based on BDI were improved. Conclusions : The administration of Saengkangsasim-tang to patients with post-COVID-19 syndrome with fatigue, diagnosed according to the Shanghanlun provision 157, may be effective.

  • Objective: The purpose of this study is to report the efficacy of using Korean medicine treatment with cervicogenic dizziness and headache. Methods: We used Korean medicine treatments (herbal medicines, acupuncture, pharmaco-acupuncture, and Chuna) to treat a hospitalized patient with cervicogenic dizziness and headache. The treatments were assessed using the Korean Dizziness Handicap Inventory (K-DHI) to evaluate the degree of dizziness. The Numerical Rating Scale (NRS) was used to evaluate headaches and post-traumatic neck pain. General health status was evaluated using the European Quality of Life Five Dimensions (EQ-5d) scale. Results: The patient's NRS and EQ-5d scores improved after treatment. The change in the K-DHI scores showed that the patient's cervicogenic dizziness symptoms were relieved. Conclusions: The results of this study suggest that Korean medicine therapies may be effective for treating cervicogenic dizziness and headaches.

The Relationship Between Specific Adult Attention-Deficit/Hyperactivity Disorder Symptoms and Anxiety Using Self-Report Diagnostic Scales

  • Objectives: This study aimed to investigate the relationship between adult attention-deficit/hyperactivity disorder (ADHD) and anxiety symptoms. Methods: A total of 52 patients diagnosed with adult ADHD completed self-report assessment scales. The clinical features of ADHD were assessed using the Adult ADHD Self-Report Scale (ASRS) and Korean Adult ADHD Rating Scale (K-AARS). The State-Trait Anxiety Inventory (STAI) was used to assess anxiety-related symptoms. Correlation and linear regression analyses were conducted to examine the relationships between the diagnostic scales of adult ADHD and anxiety. Results: Higher scores on the ASRS were related to higher scores on the STAI-S (r=0.482) and STAI-T (r=0.573), which assess state anxiety and trait anxiety, respectively. Subscales of inattention (STAI-S, r=0.485; STAI-T, r=0.648), impulsivity (STAI-S, r=0.404; STAI-T, r=0.491), emotional dysregulation (STAI-S, r=0.498; STAI-T, r=0.639), disorganization (STAI-S, r=0.427; STAI-T, r=0.545) on the KAARS, and the subtotal of K-AARS clinical subscales (STAI-S, r=0.517; STAI-T, r=0.540) had significant correlations with both STAI-S and STAI-T scores. After adjusting for demographic characteristics, there were significant associations between ASRS total scores (β=0.299) and STAI-S scores, and between inattention (β=0.297), impulsivity (β=0.560), emotional dysregulation (β=0.393), disorganization (β=0.817), subtotal scores on the K-AARS subscales (β=0.114), and STAI-S scores. The multivariate regression analysis also showed significant associations between ASRS total scores (β=0.409) and STAI-T scores. Associations between K-AARS subscales of inattention (β=0.468), impulsivity (β=0.817), emotional dysregulation (β=0.598), disorganization (β=1.120), subtotal scores on the K-AARS subscales (β=0.134), and STAI-T scores remained significant after adjusting demographic variables. Conclusion: Comprehensive assessment of ADHD symptoms related to anxiety would be important for the evaluation and treatment of anxiety in adult ADHD patients.

Characteristics of Physiological Variables(EDR, EMG) in Biofeedback Treatment (바이오피이드백 치료에서 나타나는 신체변수(EDR, EMG)의 특성)

  • Objectives: We explored the characteristics of physiological variables such as electrodermal response(EDR) and electromyography(EMG) in patients with insomnia, panic disorder, and other anxiety disorders. we aimed to decide the minimum sessions in biofeedback treatment to make the treatment effective and examine the effects of long-term biofeedback treatment by measuring the physiological variables. Methods: Thirty seven outpatients who received biofeedback treatment were divided into 3 groups according to the number of biofeedback sessions(patients who received 4-5 sessions, who received 6-9 sessions, and who received more than 10 sessions). We measured mean and delta values of EDR and EMG levels, and the Hamilton Anxiety Rating Scale(HARS), and Slef-Relaxation Inventory(SRI) in all patients. Data were analyzed by t-test and repeated measures analysis of variance. Results: The mean and delta values of EDR and EMG levels were not different among the 3 groups during the first 4 biofeedback sessions. However, patients who received more than 10 biofeedback sessions had higher baseline mean EDR value(F=2.233, p=0.036) in the first session, compared with other patients. In patients who received more than 10 biofeedback sessions, mean EDR was significantly reduced after $5^{th}$ session(F=10.41, p<0.01). They showed significant improvement in SRI scores at 12th biofeedback session(t=2.726, p<0.05) and in HARS scores at $6^{th}$(t=3.10, p<0.05) and $12^{th}$ biofeedback session(t=10.93, p<0.001). Conclusions: Wesuggest that patients should receive more than 5 biofeedback sessions to experience internal cues and get a good clinical response to biofeedback treatment.
