형상기억합금 호선의 역학적 해석 (MECHANICAL ANALYSIS ON THE SHAPE-MEMORY ARCH WIRE)

  • This study was designed to investigate the displacements and reaction forces of teeth caused by the application of the rectangular shape-memory arch wires with curve of Spee. Computer-aided three dimensional finite element method was adopted. This finite element model consists of brick element for teeth, beam element for the wire, and contact element for the periodontal ligament. And the application of the MEAW(Multiloop Edgewise Arch Wire) was also studied so that the results of the two methods can be compared each other. Total number of the nodes and elements were found to be 5925 and 4031, repectively. In addition, several types of elastics and corresponding displacements and reaction forces were examined. The findings of this study were as follows: 1. When the rectangular shape-memory arch wire with curve of Sun was used alone, the intrusion and labioversion was noticeable on the upper incisors, while the upper molars showed less intrusion. With MEAW, the intrusion and labioversion of the upper incisors were slightly larger than those when the shape-memory arch wire was used, but on the upper molars the opposite result was obtained with respect to the intrusion. 2. The shape-memory arch wire with the vertical elastics caused the larger downward displacement on the upper canine than that when the MEAW was used with the vertical elastics. However, the downward displacement of the upper incisors was larger in MEAW. The uprighting and buccoversion of the molars were observed in both cases. 3. The use of the Class II or III elastics showed the extrusion and changes in torque of the corresponding teeth. The downward displacement of the upper canine was increased when the Class II and vertical elastics were applied simultaneously, but it was decreased when both of the Class III and vertical elastics were used.

Intrusion arch wire activation시 상악 견치에 가해진 초기응력의 유한요소법을 통한 고찰 (A FEM ANALYSIS FOR INITIAL STRESS ON THE UPPER GAMINE BY ORTHODONTIC FORCE OF INTRUSION ARCH WIRE ACTIVATION)

  • 본 연구는 유한요소법을 사용하여 상악견치에 intrusion arch wire를 강제로 engage시켰을 때 상악 견치와 치근막에서 발생되는 압축력과 인장력의 분포의 양을 알아보고 어느 부분에 집중되는가를 분석하였다. 또한 각 부분에서의 압축력과 인장력의 비율을 비교하기 위해서 시행하여 다음과 같은 결과를 얻었다. 1. 상악 견치의 FA point와 백악법랑경계부분에서는 압축력과 인장력의 비율이 거의 비슷하게 나타났다. 2. 치근첨 부분에서는 압축력이 인장력의 거의 4배정도 크게 나타났다. Intrusion force적용시 치근흡수 정도가 치근첨에서 발생되는 것을 정량화시켜서 보여준 결과이다. 3. 백악법랑경계부분은 즉 FA point를 제외하고는 압축력과 인장력이 가장 큰 값을 보이고 있다. 즉 치아에 교정력이 직접 적용된 부분을 제외하고는 치아와 치주인대가 처음 접촉되는 백악법랑경계부에서 응력이 집중되는 것을 유추할 수 있다.

Multiloop Edgewise Arch Wire가 야기하는 응력분포에 관한 광탄성학적 연구 (A PHOTOELASTIC STUDY OF THE STRESS DISTRIBUTION BY MULTILOOP EDGEWISE ARCH WIRE)

  • The purpose of this study was to investigate the force mechanism of Multiloop Edgewise Arch Wire and the intensity and distribution of stresses with vertical and intermaxillary elastics. The obtained results were as follows. 1. When plain wires were inserted and vertical and intermaxillary elastics were used in the upper and lower arch, the stresses of the anterior and posterior ends of wires were observed greatly but the stresses of the premolar were very small. 2. When MEAW were inserted in upper and lower arch, the upper 1st and 2nd premolar and the lower 1st premolar were extruded greatly. 3. In the area of the upper 1st molar and the lower 2nd premolar and the lower 1st molar, any stresses were not observed. 4. The vertical elastic counteracted the intrusion force of the MEAW in the anterior teeth but could not affect on posterior teeth. Using with the Class II elastics, the distal tipping force and extrusion force were exerted in the upper anterior teeth and the intrusion forces of the lower anterior teeth were relieved. Using with the Class III elastics, the extrusion force were exerted in the upper and lower anterior teeth, the distal tipping force were increased in the lower posterior teeth. 5. The Class II elastic counteracted the anterior intrusion force of the MEAW and extruded and tipped mesially the lower 2nd molar. The intrusion force of the MEAW also could not overcome the extrusion force of the class II elastics. 6. When the Class III elastics were engaged, the upper 2nd molar was extruded in spite of the intrusion forces of the MEAW and the extrusion forces of the lower anterior teeth and distal tipping forces in the posterior teeth were observed.

Three-dimensional analysis of tooth movement in Class II malocclusion treatment using arch wire with continuous tip-back bends and intermaxillary elastics

  • Objective: The aim of this study was to analyze three-dimensional (3D) changes in maxillary dentition in Class II malocclusion treatment using arch wire with continuous tip-back bends or compensating curve, together with intermaxillary elastics by superimposing 3D virtual models. Methods: The subjects were 20 patients (2 men and 18 women; mean age 20 years 7 months ${\pm}$ 3 years 9 months) with Class II malocclusion treated using $0.016{\times}0.022-inch$ multiloop edgewise arch wire with continuous tip-back bends or titanium molybdenum alloy ideal arch wire with compensating curve, together with intermaxillary elastics. Linear and angular measurements were performed to investigate maxillary teeth displacement by superimposing pre- and post-treatment 3D virtual models using Rapidform 2006 and analyzing the results using paired t-tests. Results: There were posterior displacement of maxillary teeth (p < 0.01) with distal crown tipping of canine, second premolar and first molar (p < 0.05), expansion of maxillary arch (p < 0.05) with buccoversion of second premolar and first molar (p < 0.01), and distal-in rotation of first molar (p < 0.01). Reduced angular difference between anterior and posterior occlusal planes (p < 0.001), with extrusion of anterior teeth (p < 0.05) and intrusion of second premolar and first molar (p < 0.001) was observed. Conclusions: Class II treatment using an arch wire with continuous tip-back bends or a compensating curve, together with intermaxillary elastics, could retract and expand maxillary dentition, and reduce occlusal curvature. These results will help clinicians in understanding the mechanism of this Class II treatment.

Evaluation of strategic uprighting of the mandibular molars using an orthodontic miniplate and a nickel-titanium reverse curve arch wire: Preliminary cephalometric study

  • Objective: To evaluate the overall treatment effects in terms of the amount of uprighting with changes in the sagittal and vertical positions of mandibular molars after applying an orthodontic miniplate with a nickel-titanium (NiTi) reverse curve arch wire (biocreative reverse curve [BRC] system). Methods: A total of 30 female patients (mean age, 25.99 ± 8.96 years) were treated with the BRC system (mean BRC time, 10.3 ± 4.07 months). An I-shaped C-tube miniplate (Jin Biomed) was placed at the labial aspect for the alveolar bone of the mandibular incisors. A 0.017 × 0.025-inch NiTi reverse curve arch wire was engaged at the C-tube mini-plate anteriorly and the first and second premolars and molars posteriorly in the mandibular arch. Pre- and post-BRC lateral cephalograms were analyzed. A paired t-test was used to analyze the treatment effects of BRC. Results: The mandibular second molars were intrusively uprighted successfully by the BRC system. Distal uprighting with a controlled vertical dimension was noted on the first molars when they remained engaged in the BRC and the distal ends of the arch wire were laid on the second molars. The mandibular first and second premolars showed a slight extrusion. The changes in the mandibular incisors were unremarkable, while the mandibular molar angulation improved significantly. The lower occlusal plane rotated counterclockwise (MP-LOP: 1.13° ± 2.60°). Conclusions: The BRC system can provide very effective molar uprighting without compromising the position of the mandibular anterior teeth.

치근 부착 스트레인 게이지를 이용한 응력 분포 분석 (An Analytical Study on Strain Distribution Using Strain Gauge Attached On Root Surface)

  • 바람직한 교정치료는 최적의 교정력을 치아에 가하여 정확하게 치아이동을 조절하고 예측함에 따라 가능해진다. 치관에 힘과 모멘트가 동시에 적용될 때 그 비율(M/F ratio)에 따라 치주인대에 나타나는 응력 분포가 변화하게 되고 이에 따른 회전중심의 위치가 변하게 되는데 이런 양상을 파악함으로써 정확한 치아이동을 조절하고 예측할 수 있다. 본 연구에서는 교정력에 따른 치주인대 및 치조골 부위에서의 응력 분포를 알아보기 위하여 상악 중절치 치근 8부위에 스트레인 게이지를 부착한 인공 치아를 광탄성 레진에 매식한 4mm 정중이개 증례의 상악 전치부 실험모형을 제작하여 응력 분포를 조사하였다. 폐쇄 방법으로 다음과 같은 3가지 방법을 적용하였다. 1. 금속 호선의 삽입 없이 100g의 힘을 발휘하는 elastomeric chain만 사용한 경우, 2. .016" 금속호선을 삽입하고 elastomeric chain을 사용한 경우, 3. .016“x.022” 금속호선을 삽입하고 elastomeric chain을 사용한 경우. 각각의 경우에서 치근의 근심면, 원심면 및 순면, 설면에 나타나는 응력을 계측하여 비교 분석한 결과 다음과 같은 결론을 얻었다. 1. 치근 부착 스트레인 게이지를 이용한 응력 분포 조사 방법으로 치아이동시 치근의 근 원심면 및 순 설면에서의 응력 분포를 파악할 수 있었으며 이에 따른 모멘트 팔의 형성을 추정함으로써 치아의 회전양상을 알 수 있었다. 2. 호선없이 교정력을 적용한 경우 정출 및 경사이동이 컸으며 호선 적용에 따라 압하 및 치체이동 양상을 보이는 것을 응력 분포 파악을 통해 확인할 수 있었다.

전방부분절 골절단술에 의한 상하악 전돌증의 악교정 1 예 (Combined Surgical and Orthodontic Treatment of Bimaxillary Dento-Alveolar Protrusion: A Report of Case)

  • 영남대학교 의과대학 부속병원 치과 구강외과에 상 하악 전돌을 가진 환자가 안모 추형으로 인한 심미적 장애와 상악 전치부 치간이개 및 하악전치부 치열부정을 주소로 내원하였다. 안면치조기형에 대한 육안적평가, 방사선학적검사, 두부계측사진분석 및 진단 모형분석 등 다각적인 분석 결과 상 하악 치조 전돌증으로 진단을 내렸으며, 내원 당시 본 환자는 전반적으로 치주병학적 문제점을 가지고 있었고 개인적으로 군입대라는 특수상황하에 있었기 때문에 치주조직에 대한 손상을 최소로 하면서 치료기간을 최대한 단축시킬 수 있는 치료계획을 세웠다. 상하악 천치부에 대한 술전교정 치료 후 상악골은 전방부분절 골절단술로 후방으로 이동시키고 하악골은 전방부분 치근단 하방 골절단술로 후하방으로 이동시키므로서 상하악 전돌증에 대한 악교정 외과적 처치를 하여 좋은 결과를 얻었기에 이에 보고하는 바이다.

소구치의 intrusion시 치근 흡수 및 회복에 대한 주사전자 현미경적 연구 (A SCANNING ELECTRON MICROSCOPIC STUDY OF THE ROOT RESORPTION AND REPAIR IN BICUSPID INTRUSION)

  • The purpose of this study was to investigate the root resorption and repair pattern of human teeth under intrusive orthodontic loadings of varying magnitude and duration. 50 gms, 100 gms, 200 gms of force applied to the 96 experimental teeth with duration of 2, 5, 7, 10 weeks, after removing the arch wire and retaining during 2 and 6 weeks, experimental teeth were extracted. Results were as follows: 1. The initial resorption site was visible in the apex within 2 weeks with 50 gms, 100 gms samples. 2. The sites of resorption increased markedly with duration of the force. 3. The 50 grams samples of the all duration reveal the shallow resorption and the 100 gms samples of the all duration except 2 weeks reveal the shallow and deep resorption sites. 4. After 7 weeks with 50 gms and 100 gms, cellular cementum repair accompanied the continuing resorption. 5. 2 and 6 weeks after removing the force with 200 gms, the same cellular cementum repair accompanied with 50 gms, 100 gms samples.

  • 병적 치아 이동된 상악 전치를 가진 중등도 치주염 환자에서는 치태 관리를 통한 치주 염증의 해소뿐만 아니라 치아를 재위치 시키기 위한 교정치료가 함께 필요하다. 이 때의 교정치료는 더 적은 힘의 적용과 치아의 저항중심, 최적의 힘 등을 세심하게 고려해야 하는데, 이동하고자 하는 치아에만 적용되는 분절호선은 이동의 예측성을 높여주고, 힘을 조절할 수 있어 보다 효율적이다. 또한 교정치료 시, 고정원으로 사용되는 치아의 원치 않는 움직임을 차단하여 구치 교합 상태를 유지할 수 있는 장치 설계가 필요하며, 이를 위해 다양한 종류의 골성 고정원이 사용될 수 있다. 본 증례에서는 정출되고 변위된 상악 전치를 가진 환자에서, 철저한 치주 치료와 적절한 힘과 역학이 설계된 교정 장치로, 양호한 치료 결과를 얻을 수 있었으므로, 소개하고자 한다.

Finite element analysis of maxillary incisor displacement during en-masse retraction according to orthodontic mini-implant position

  • Objective: Orthodontic mini-implants (OMI) generate various horizontal and vertical force vectors and moments according to their insertion positions. This study aimed to help select ideal biomechanics during maxillary incisor retraction by varying the length in the anterior retraction hook (ARH) and OMI position. Methods: Two extraction models were constructed to analyze the three-dimentional finite element: a first premolar extraction model (Model 1, M1) and a residual 1-mm space post-extraction model (Model 2, M2). The OMI position was set at a height of 8 mm from the arch wire between the second maxillary premolar and the first molar (low OMI traction) or at a 12-mm height in the mesial second maxillary premolar (high OMI traction). Retraction force vectors of 200 g from the ARH (-1, +1, +3, and +6 mm) at low or high OMI traction were resolved into X-, Y-, and Z-axis components. Results: In M1 (low and high OMI traction) and M2 (low OMI traction), the maxillary incisor tip was extruded, but the apex was intruded, and the occlusal plane was rotated clockwise. Significant intrusion and counter-clockwise rotation in the occlusal plane were observed under high OMI traction and -1 mm ARH in M2. Conclusions: This study observed orthodontic tooth movement according to the OMI position and ARH height, and M2 under high OMI traction with short ARH showed retraction with maxillary incisor intrusion.
