Automatic Identification of the Lumen Border in Intravascular Ultrasound Images (혈관 내 초음파 영상에서 내강 경계면 자동 분할)

  • Accurately segmenting lumen border in intravascular ultrasound images (IVUS) is very important to study vascular wall architecture for diagnosis of the cardiovascular diseases. After each of IVUS image is transformed to a polar coordinated image, initial points are detected using wavelet transform. Then, lumen border is initialized as the set of important points using non parametric probability density function and smoothing function by removing outlier initial points occurred by noises and artifacts. Finally, polynomial curve fitting is applied to obtain real lumen border using filtered important points. The evaluation of proposed method was performed with related method and the proposed method produced accurate lumen contour detection when compared to another method in most types of IVUS images.

Efficacy of Isoproterenol as a Marker of Epidural Test Dose in Patients Anesthetized with Enflurane (Enflurane 전신마취중 경막외 시험용량의 표식자로서 Isoproterenol의 효율성)

  • Background: Epidural test doses containing epinephrine are an incomplete marker for the detection of inadvertent intravascular injection. Therefore, many investigators have attempted to find a more reliable marker as an alternative to epinephrine in adult patients anesthetized with enflurane. The present study was designed to test whether two different simulated intravenous test doses of isoproterenol could be used as a reliable marker for the detection of inadvertent intravascular injection in adult patients anesthetized with $O_2-N_2O$-enflurane. Methods: Forty healthy adult patients were anesthetized with 1% end-tidal enflurane and nitrous oxide after endotracheal intubation and were randomized to one of two groups according to the dose of isoproterenol. Group 1 and 2 (n = 20 each) received 3 ml of 1.5% lidocaine with 3 and 5 g isoproterenol intravenously, respectively, to simulate an intravascularly administered test dose. Heart rate (HR) and systolic blood pressure (SBP) were measured at 20-second intervals for 4 min after injection. Results: Mean maximal HR increases were $24{\pm}17$, $35{\pm}11$ bpm (P < 0.05), mean maximal SBP increases were $14{\pm}8$, $13{\pm}9$ mmHg and mean maximal SBP decreases $20{\pm}11$, $22{\pm}9$ mmHg following the IV injection of 3, $5{\mu}g$ isoproterenol, respectively. The incidence of hypotension was similar in both groups. Isoproterenol 3 and $5{\mu}g$ produced 75%, 100% sensitivity in the HR criteria ($\geq$ 20 bpm increase) and 60%, 70% sensitivity in the SBP criteria ($\geq$ 15 mmHg), respectively. Conclusions: These results indicate that based on the HR response, the epidural test dose containing $5{\mu}g$ isoproterenol to simulate an intravascular administration is a more reliable marker than $3{\mu}g$ isoproterenol in adult healthy patients during enflurane anesthesia.

Enhancement of Gas Transfer Efficiency in an Intravascular Lung Assist Device using Blood Substitutes (혈관내의 폐보조장치에서 혈액대용물질을 사용한 기체전달 효율향상)

  • Intravascular oxygenation represents an attractive. alternative support modality for therapy originated with acute respiratory distress syndrome(ARDS). However. the clinical study concluded that more gas exchange was needed for intravascular oxygenation to be clinically effective in ARDS treatment. In this study, we tried to enhance gas exchange on the VIVLAD using microencapsulation of hemoglobin and perfluorocarbon emulsion(PFC emulsion). Blood gas measurements were performed by collecting blood samples from the arterial and venous sides of the circuit, and processing them in a blood/gas analyzer. The function of hemosome. blood/hemosome mixed solution. and blood/PFC emulsion mixed solution were tested by an oxygen dissociation curve using a blood/gas analyzer. As a result, it was shown that the oxygen transfer of hemosome and blood/hemosome mixed solution were higher than that of whole blood. Also. it showed that the carbon dioxide transfer of whole blood/PFC emulsion mixed solution was higher than that of others. Therefore, we determined that hemosome and PFC emulsion could increase oxygen transfer and carbon dioxide transfer. respectively.

Intravascular Hemolysis after Prosthetic Valve Replacement (인공판막 치환수술후의 용혈)

  • Forty two consecutive patients who had had valve replacement with St. Jude Medical prosthesis were studied on a view point of intravascular hemolysis. Patients were consisted of 14 mitral valve replacement, and 7 aortic valve replacement, and 21 double, mitral and aortic, valve replacement. Serum LDH, indirect bilirubin, GOT, hemoglobin levels and ret-iculocyte count were pursued in postopeative 1st day, 3rd day, 7th day, 14th day and 21th day. Postoperatively, all patients were not detected paravalvular leakage on the ech-ocardiographical study. The patients with double valve replacement revealed higher levels of LDH on postopeative 14th day[P<0 05] than those with single valve replacement. Among the patients with single valve replacement, the patients with aortic valve replacement revealed slightly higher levels of entire postopeative data, but considered insignificant. There was correlation between the severity of hemolysis and the size of replaced aortic valve. In the postoperative LDH levels, the patients with small sized-aortic valve[less than 21mm in diameter] replacement revealed higher levels of postoperative 3rd day, 7th day and 14th day than those with large size[more than 23mm in diameter]. The patients with high level LDH of greater than 800 WU /L on postoperative 7th day were 61.9%[26 of 42]. The high LDH frequency of DVR was 71.4%[15 of 21], MVR 50.0%[7 of 14] and AVR, 57.1%[4 of 7]. The level of LDH declined gradualiy thereafter through postoperative 3 weeks. In conclusion, intravascular hemolysis after prosthetic valve replacement was dependent on position of valve replacement and size of valve. And this study supports the conventional valve selection and usage in our hospital. The patients with subclinical hemolysis after valve replacement should be placed on a close observation.

  • Paroxysmal nocturnal hemoglobinuria(PNH) is an acquired hematologic disorder characterized by intravascular hemolysis, nocturnal hemoglobinuria, thrombotic events and bone marrow failure. It rarely occurs in children and can be complicated by acute renal failure(ARF). Here, we present two cases of ARF complicating PNH which has not been reported yet in Korean children. We suggest that PNH should be considered in differential diagnosis of ARF in children associated with intravascular hemolysis.

  • In this study, we try to formularize simultaneous equations to make a prediction about pressure drop for designing intravascular artificial lung assist device. Designing parameters to predict the effect of pressure drop and designed modules under various conditions were studied through an experimental modeling before inserting the artificial lung assist device into as venous. We measured pressure drop in various number of hollow fiber membranes, when the inside diameter of shell is fixed in 3 cm, and tried to develope the prediction equations by curve fitting based on the correlation between the experimental pressure drop and the device frontal area or packing density. The results showed that pressure drop increased with 2nd order functional formula as the liquid flow rate, the frontal area, and the packing density increased. Also, we can estimate the pressure drop as a function of the frontal area or packing density. The pressure drop obtained from the experiment was similar to that from the equation, confirming the usefulness of the equation.

Effects of Kamigyemyoungsan and Each Compositive Herbs on the Intravascular Coagulation and Subcutaneous Hematoma in Rats (加味鷄鳴散 및 그 構成藥物이 白鼠의 瘀血病態模型에 미치는 影響)

  • This study was performed to investigate the effects of Kamigyemyoungsan and each compositive herbs on the intravascular coagulation and subcutaneous hematoma. The intravascular coagulation was induced by the injection of endotoxin into the caudal vein of the rats. These rats were treated with endotoxin after administering orally extracts of Kamigyemyoungsan and each compositive herbs. Then the number of platelet, prothrombin time, the concentration of fibrinogen and FDP(fibrinogen degration product), hematocrit and RBC and WBC were measured. The subcutaneous hematoma was induced by the subcutaneous injection of the autologous whole blood to produce clotting in situ in rats. Then the extracts of Kamigyemyoungsan and each compositive herbs were administered orally. The lesions were then dissected and observed. The results were obtained as follows ; 1. The number of platelets of the trial groups was increased as compared with the control group, and revealed significance in sample Ⅰ. 2. The concentration of fibrinogen of the trial groups was increased as compared with the control group, and revealed significance in sample Ⅰ, sampleⅡ and sample Ⅴ. 3. The prothrombin time was shortened significantly in the trial groups, except sampleⅡ and sampleⅣ as compared with the control group. 4. The concentration of FDP decreased in the trial groups, and revealed significance in sample Ⅰ and sample Ⅵ as compared with the control group. 5. The hematocrit significantly increased in sample Ⅰ, sampleⅢ and sample Ⅵ as compared with the control group. 6. The number of RBC significantly increased in sample Ⅵ only as compared with the control group. 7. The number of WBC significantly increased in the trial groups, except sampleⅣ and sample Ⅵ as compared with the control group. 8. Histologically, the lesions in the trial groups showed significantly thinner fibroblastic neomembrane than the control group, except sampleⅢ. According to the above results, it is considered that Kamigyemyoungsan and each compositive herbs have inhibitive effects on the thrombosis and the fibroblastic neomembrane development. Therefore, it seems to be applicable to the diseases related to thrombosis and hematoma.

Entelon150® (Vitis vinifera Seed Extract) Attenuates Degenerative Changes in Intravascular Valve Prostheses in Rabbits

  • Background and Objectives: The therapeutic strategy for inflammation and degenerative calcification is of utmost importance for bioprosthetic heart valve (BHV) implanted patients. The purpose of this study was to compare the anti-inflammatory and anti-calcification effects of Entelon150® (grape seed extract), losartan, and rosuvastatin, in a rabbit model of intravascular BHV leaflet implantation in bovine pericardium. Methods: A total of 28 rabbits were implanted with BHV leaflet in the external jugular veins. The Entelon150® group was administered 7.7 mg/kg Entelon150® twice daily for 6 weeks after surgery. The losartan and rosuvastatin groups received 5.14 mg/kg and 1 mg/kg, respectively, once per day. The control group received 1 ml of saline once daily. And then, calcium concentration was measured in the implanted BHV, and histological and molecular analyses were performed on the surrounding tissues. Results: The calcium content of the implanted tissue in the Entelon150® group (0.013±0.004 mg/g) was lower than that in the control group (0.066±0.039 mg/g) (p=0.008). The losartan (0.024±0.016 mg/g, p=0.032) and rosuvastatin (0.022±0.011 mg/g, p=0.032) groups had lower calcium content than the control group, and higher tendency than the Entelon150® group. Immunohistochemistry revealed that the expressions of bone morphogenic protein 2 (BMP2), S-100, and angiotensin II type 1 receptor in the Entelon150® group showed lower tendency than those in the control group. The protein expression levels of BMP2 were reduced in the Entelon150® group compared with those in the control group. Conclusions: Entelon150® exhibited a significant effect, similar to other drugs, in reducing calcification and inflammation in the intravascular bovine pericardium.
