한국형 자살위험 스크리닝 도구와 타당성 (Korean Suicide Risk Screening Tool and its Validity)

  • 자살 생각을 초기에 발견하는 것은 자살을 예방하는 데 중요하다. 본 연구는 의료 현장에서 사용될 수 있는 간단한 스크리닝 도구를 개발하는 것을 목적으로 하였다. 자살위험 스크리닝 도구는 의사들이 빠르고 쉽게 사용할 수 있도록 설계되었다. 미국 알래스카 Norton Sound region의 Screening for Depression and Thoughts of Suicide라는 도구로부터 2개의 항목으로 구성된 스크리닝 도구를 만들었다. 이 도구는 개인식별번호를 제외하는 등 한국의 실정에 맞게 양식이 수정되었으며 한국형 자살위험 스크리닝 도구로 이름붙였다. 이 도구의 신뢰성과 적용가능성은 7명의 의사들을 통해 검토되었다. 이 도구의 타당성은 4개의 의료기관에서 325명의 환자들을 조사하여 현재의 정신건강 상태를 외부 기준으로 하여 확인하였다. 이와 같은 2개 문항으로 구성된 자살위험 스크리닝 도구는 정신과 의사 및 기타 의사들에 의해 환자의 자살경향을 발견하는데 사용될 수 있을 것이다.

리조트 기업의 조직문화에 대한 기호학적 분석 : 그레마스 행위소 모형 적용 (Semiotic approach to Resort's Organizational Culture : Applying Greimas Actant Model)

  • 본 연구는 리조트 조직의 저변에 잠재해 있으며 리조트 직원들에 의해 공유되어야 하는 조직문화를 발견하는데 목적이 있다. 최근 힐링에 대한 욕구와 지역사회의 라이프스타일 체험을 강조하는 추세에 주목해 볼 때 이는 리조트가 원래 탄생 시기부터 가지고 있었던 고유의 역할이었다. 그럼에도 불구하고 최근 리조트가 호텔 산업과 경계가 모호해지고 테마파크를 닮은 복합리조트로 변화하는 등 정체성이 흔들리고 있음을 부인할 수 없다. 리조트의 조직문화를 찾아내기 위해 16명의 리조트 전문가들을 대상으로 그레마스 행위소 모형을 기초로 제작된 심층인터뷰를 진행하였는데 3박4일간의 리조트 체험을 통해서 응답내용의 충실성을 유도하였다. 모형이 가지고 있는 6개의 행위소를 리조트를 구성하는 제반 요소와 합치시키고 응답자로 하여금 자유롭게 은유적으로 표현하도록 하였다. 응답자들은 발신자, 수신자, 주체, 객체, 보조자, 반대자를 각각 리조트, 리조트 방문객, 리조트기업, 리조트상품서비스, 리조트지원집단, 리조트방해집단에 대응시킨 결과 순서대로 심신치유, 환자, 치유사, 치유약, 치유도우미, 돌팔이약장수로 은유하였다. 은유한 내용은 모형에 따라 신화(이야기)로 구성하였으며 더 나가 조직문화에 주는 시사점으로 연결하였다. 연구의 이론적 실무적 시사점 및 한계점은 논문 말미에 포함시켰다.

자궁근종 환자의 자궁적출경험 (A Study on Uterine Leiomyoma Patients' Experience of Total Hysterectomy)

  • This study is designed to describe and analyze the experiences of hysterectomy. It also attempts to develop the practical theory of their experiences and thus suggest a fundamental frame that can help nurses broaden their clinical understanding of hysterectomy patients and take better care. The data was collected through interviewing the selected subjects. The subjects were five hysterectomy patients. They were interviewed at first hand by this researcher and recorded under their permissions. Each interview took one hour or two hours per interviewer. The data was collected through the interview. The method of "the Ground Theory" by Strauss & Corbin (1990) was used for data analysis. A total of 129 concepts were extracted from the data analysis. which were classified into 27 subcategories through reanalysis. The subcategories were grouped under 12 higher categories. During the process of the data analysis the following three propositions repeatedly appeared. (1) The higher importance of sexual life and pregnancy, the greater the patients' despair. The support system was not satisfied. The closed defense intensified their despair. They felt that the operation result was out of control. (2) The lower the importance of sexual life and pregnancy, the lesser the patients' despair. The support system proved to be highly satisfactory. As a result more favorably. (3) The lower physical change, the lesser importance of pregnancy. the lesser the patients' despair, the patients' thought turned out to be more positive, which, in turn, left the patients the less despaired. As a results the patients accept the operation result more favorably.

Estimate of Vitamin $B_6$ Intake and Major Dietary Sources of Vitamin $B_6$ in Children Aged 7-12 Years in the Seoul Area

  • The dietary vitamin $B_6$ intake of 185 Korean children aged 7-12 years (y), who showed no health problems, in the Seoul area and its sources were estimated using a modified Korean vitamin $B_6$ database. The age and sex of each subject was classified according to the classifications in the Korean Recommended Dietary Allowance (RDA). Dietary vitamin $B_6$ intake and food sources were estimated using the three-day recall method with the help of a trained interviewer. Food portion sizes were estimated by using standard household measures and published average portion sizes. The average daily vitamin $B_6$ intake was 1.69$\pm$0.50 mg/d in children aged 7-9 y, 1.86$\pm$0.44 mg/d in male children aged 10-12 y and 1.77$\pm$0.62 mg/d in female children aged 10-12 y. Less than 5% of the subjects consumed less than the Korean RDA of vitamin $B_6$. The average ratio of vitamin $B_6$intake to daily protein intake was 0.028$\pm$0.006 mg/g in children aged 7-9 y, 0.028$\pm$0.004 mg/g in male children aged 10-12 y and 0.029$\pm$0.007 mg/g in female children aged 10-12 y. The intake of vitamin $B_6$ was significantly (p<.01) positively correlated to the intake of all other nutrients. Foods from animal and plant sources provided 37% and 73% respectively, of total vitamin $B_6$. Major dietary sources of vitamin $B_6$ in children in the Seoul area were rice, soybean sprouts, pork, beef, cereal, kimchi, milk, onions, and potatoes. As for major dietary sources of vitamin $B_6$, the top 20 foods provided nearly 73-75% of the total vitamin $B_6$ consumed by Korean children.

Quality of Breast Cancer Early Detection Services Conducted by Well Woman Clinics in the District of Gampaha, Sri Lanka

  • Background: Breast cancer is the most common cancer diagnosed in females in Sri Lanka and early detection can lead to reduction in morbidity and mortality. Aim: To evaluate selected aspects of breast cancer early detection services implemented through well woman clinics (WWCs) in the Gampaha District. Methods: The study consisted of two components. A retrospective descriptive arm assessed clinical breast examination (CBE) coverage of target age group women (TGW) of 35-59 years in all the WWCs in Gampaha district over 2003-2007. A cross sectional descriptive study additionally assessed quality of breast cancer early detection services. The Lot Quality Assurance Sampling (LQAS) technique was used to decide on the lot size and threshold values, which were computed as twenty and six clinics. Checklists were employed in assessing coverage, physical facilities and clinic activities. Client satisfaction on WWC services was assessed among 200 TGW attending 20 WWCs using an interviewer-administered questionnaire. Results: CBE coverage in the Gampaha district increased only from 1.1-2.2% over 2003-2007. With regard to physical facilities, the number of clinics that were rated substandard varied between 7-18 (35-90%). The items that were lacking included dust bins, notice boards, stationary, furniture and linen, and cleanliness of outside premises and toilets. With regard to clinic activities, punctuality of staff, late commencement of clinics, provision of health education, supervision, CBE and breast self-examination (BSE) were substandard in 7-20 clinics (35-100%). Client satisfaction for WWC services was 45.2% (IQR: 38.7-54.8%) and only 11% had a score of ${\geq}70%$, the cut off set for satisfaction. Conclusions: Breast cancer early detection service coverage in the Gampaha district remained low (2.2%) in 2007, 11 years after commencing WWCs. All 20 clinics were substandard for overall CBE and BSE.

일부(一部) 사업장(事業場)의 산업보건(産業保健) 사업(事業)에 있어서의 근로자(勤勞者) 참여(參與)에 관한 조사(調査) 연구(硏究) (Study on the Workers' Participation in Industries)

  • The concept of workers' participation in occupational health was introduced to Korea recently in relation to primary health care in occupational health. But there is confusion and debate about workers' participation concept. The purpose of this study was to review the concepts of workers' participation and to conduct evaluation the workers' participation status in occupational health and safety, 394 workers and 54 employers (5.6%) in north area of Kyunggi-Do, were selected and interviewed with a questionnaire by a trained interviewer from August to September 1990. In general, the concept of workers' participation is based on industrial domocratisation and Declaration of Human Rights which had been powerful ideologies in labor movement. Contrary to workers' participation, community participation is rooted in the Health Rights. So, it is necessary to consider concept of workers' participation to improve participation. The results of survey were as follows. 1. Most of companies (71.75) carried out occupational health education to workers in study area. 2. The Occupational Safety and Health Committee (OSHC) were set up in 24.1% among the study companies, and 72.7% of workers among respondents thought that OSHC was helpful to workers health. 3. The workers signed his name to personal health report in 43.1% and the rate of participation in occupational environment examination was 54.9%. 4. The workers prefer the OSHC (39.3%), owner (35.1%) and union (25.8%) as a occupation health organisation, but owners prefer OSHC (54.5%), manager (43.2%) and union (2.3%). 5. Among the factors of the general characteristics, the existence of labor union was a major determinant of workers' attitude and level of workers' participation. As we have seen, most levels of workers' participation are low in occupational health. The variable of existence of labor union among the factors was a ma determinant of workers attitude and level of workers' participation. Therefore, in order to promote workers health, it is necessary to ponder long deeply on occupational health care system under the viewpoint of workers' participation.

Associations of the Neighborhood Environment With Substance Use: A Cross-sectional Investigation Among Patients in Compulsory Drug Detention Centers in Thailand

  • Objectives: To identify the associations of characteristics of the neighborhood environment with substance abuse among clients receiving treatment for drug abuse in Thailand. Methods: A cross-sectional study was conducted of 1128 drug addicts from 28 neighborhoods who were receiving treatment at all 7 compulsory drug detention centers in Thailand. A trained interviewer conducted structured interviews with the subjects about substance use and the perceived neighborhood environment in their community. A multilevel logistic regression model was applied to estimate the effects of the neighborhood environment on substance use. Results: The majority of participants, 53.8% only used methamphetamine pills, 31.3% used other illicit drugs as well as methamphetamine pills, and 14.9% used an illicit drug other than methamphetamine. Three neighborhood characteristics were associated with substance use. A 1-unit increase in the perceived neighborhood cohesion score was associated with a 15% reduction in methamphetamine pill use and an 11% reduction of the use of both methamphetamine pills and another illicit drug. Conversely, a 1-unit increase in perceived neighborhood crime predicted 19 and 14% increases in the use of methamphetamine pills and the use of both methamphetamine pills and another illicit drug, respectively. In addition, a 1-unit increase in the scores for stigma surrounding addiction corresponded to a 25% increase of the use of methamphetamine pills and a 12% increase in the use of both methamphetamine pills and another illicit drug. Conclusions: Substance use among drug addicts was influenced by characteristics of the neighborhood environment. Therefore, prevention and intervention strategies should be designed based on a consideration of the impact of neighborhood context on substance use behaviors.

생산직 근로자의 수면시간과 수면의 질이 대사증후군 및 대사증후군 구성요소 유병률에 미치는 영향 (Effects of Sleep Duration and Quality on Prevalence of Metabolic Syndrome and Metabolic Syndrome Components in Korean Blue-collar Workers)

  • Purpose: The prevalence of metabolic syndrome (MetS), a cluster of metabolic abnormalities has rapidly increased in Korea. Sleep may play a role in determining its prevalence. However, the relationships between MetS and the duration and quality of sleep are not yet clear. This study aimed to examine the associations between the duration and quality of sleep and the prevalence of MetS. Methods: Study participants included 348 Korean blue-collar workers employed by six small-sized companies in Korea. The data were collected using an interviewer-administered questionnaire, and logistic regression analysis was conducted to estimate the effects of the factors related to MetS. Results: The multiple logistic regression analysis revealed that the independent factors that contributed to the prevalence of MetS were being male (adjusted odds ratio [aOR]=4.87, 95% confidence interval [CI]=1.58~15.0) and lower sleep quality (aOR=5.12, 95% CI=1.90~14.30). Sleep duration was related to the prevalence of some MetS components, but it was not associated with MetS prevalence. Conclusion: Sleep quality was negatively associated with MetS prevalence when covariates, such as sleep duration, were controlled. When developing a MetS risk-reduction program, focus should be given to sleep quality as well as sleep duration in an intervention for Korean blue-collar workers.

  • Purpose: This research was aimed to analyze the reports of qualitative researches published in Korean Journal of Women Health Nursing (KJWHN). Methods: Twenty qualitative researches using in-depth interviews or focus groups published in KJWHN during from 2002 to 2011 were selected for analysis. Selected reports were analyzed by consolidated criteria for reporting qualitative research (COREQ), which were a reporting guideline for qualitative researches, and consisted of thirty two items and three domains, i.e., (1) research team and reflexivity, (2) study design, and (3) data analysis and reporting. Results: Personal characteristics related to credentials, gender and interviewer's bias or assumption were less reported than other items in the 1st domain. In the 2nd domain, descriptions such as a presence of non-participants, pilot test for interview guide development, and transcript return to participants were less reported than other items. In the 3rd domain, items related to number of data coder, participants checking of the finding, and clarity of minor themes were less reported than other items. Conclusion: It could be concluded that COREQ is a useful guideline for reporting of qualitative studies. From these results, we suggest that full items of COREQ should be considered and described when researchers report qualitative research.

Evaluation of Risk Factors for Nasopharyngeal Carcinoma in a High-risk Area of India, the Northeastern Region

  • Northeastern India is a major nasopharyngeal carcinoma (NPC) high risk-area although the rest of the country has very low incidence. A case-control study of 105 NPC cases and 115 controls was conducted to identify the potential risk factors for NPC development in this region. Information was collected by interviewer about socio-demographic characteristics, cigarette smoking, alcohol consumption, dietary history, occupational history, and a family history of cancer. Epstein-Barr viral load was assayed from the blood DNA by real time PCR. Associations between GSTs genotypes, cytochrome P450 family including CYP1A1, CYP2E1 and CYP2A6 polymorphisms and susceptibility to relationship between the diseases were studied using PCR-RFLP assay. Results indicate that Epstein-Barr virus load was significantly higher in patients compared to controls (p<0.0001). Furthermore, concentration of blood EBV-DNA was significantly higher in advanced stage disease (Stage III and IV) than in early stage disease (Stage I and II) (p<0.05). Presence of CYP2A6 variants that reduced the enzyme activity was significantly less frequent in cases than controls. Smoked meat consumption, exposure to smoke, living in poorly ventilated house and alcohol consumption were associated with NPC development among the population of Northeastern India. Thus, overall our study revealed that EBV viral load and genetic polymorphism of CYP2A6 along with living practices which include smoked meat consumption, exposure to smoke, living in poorly ventilated houses and alcohol consumption are the potential risk factors of NPC in north eastern region of India. Understanding of the risk factors and their role in the etiology of NPC are helpful forpreventive measures and screening.
