  • The authors present a case of giant serpentine aneurysm (a partially thrombosed aneurysm containing tortuous vascular channels with a separate entrance and outflow pathway). Giant serpentine aneurysms form a subgroup of giant intracranial aneurysms, distinct from saccular and fusiform varieties, and in this case, too, the clinical presentation and radiographic features of CT, MR imaging and angiography were distinct.

  • Recent topics on quality assurance (QA) of X-ray diagnosis in Japan were reported in this presentation. These were related to mass screening mammography (MMG), lung screening CT (LSCT), skin injury caused by interventional radiology (IVR) and traceable system of dosimeters for x-ray diagnosis. In these successful stories, the author would like to stress the cooperation of all the medical am: clinical staff including medical doctors, radiological technologists, medical physicists, manufacturers of medical devices and others.

Interventional radiography in management of high-flow arteriovenous malformation of maxilla: report of a case

  • Arteriovenous malformations are extremely rare conditions in that can result from abnormalities in the structure of blood vessels, which may be potentially fatal. A 30-year-old female patient visited our hospital with a complaint of swelling on the right maxillary posterior gingiva along with the large port-wine stain on right side of face. On clinical examination, the swelling was compressible and pulsatile. Radiographic examination revealed a lytic lesion of maxilla. Diagnostic angiography revealed a high-flow arteriovenous malformation of maxilla which was treated by selective transarterial embolization of maxillary artery using polyvinyl alcohol particles.

  • Dynamic X-ray (DXR) is a functional imaging technique that uses sequential images obtained by a flat-panel detector (FPD). This article aims to describe the mechanism of DXR and the analysis methods used as well as review the clinical evidence for its use. DXR analyzes dynamic changes on the basis of X-ray translucency and can be used for analysis of diaphragmatic kinetics, ventilation, and lung perfusion. It offers many advantages such as a high temporal resolution and flexibility in body positioning. Many clinical studies have reported the feasibility of DXR and its characteristic findings in pulmonary diseases. DXR may serve as an alternative to pulmonary function tests in patients requiring contact inhibition, including patients with suspected or confirmed coronavirus disease 2019 or other infectious diseases. Thus, DXR has a great potential to play an important role in the clinical setting. Further investigations are needed to utilize DXR more effectively and to establish it as a valuable diagnostic tool.

  • Transarterial chemoembolization (TACE) was introduced in 1977 with the administration of chemotherapeutic agent to gelatin sponge particles through the hepatic artery in patients with hepatocellular carcinoma (HCC) and was established as conventional TACE using Lipiodol in the 1980s. In the 2000s, drug-eluting beads were developed and applied clinically. Currently, TACE is a commonly used non-surgical treatment modality for patients with HCC who are unsuitable for curative treatment. Considering the vital role of TACE in the management of HCC, it is crucial to organize current knowledge and expert opinions regarding patient preparation, procedural techniques, and post-treatment care in TACE, which can enhance therapeutic efficacy and safety. A group of 12 experts in the fields of interventional radiology and hepatology, convened by the Research Committee of the Korean Liver Cancer Association (KLCA), has developed expert consensus-based practical recommendations in TACE. These recommendations have been endorsed by the Korean Society of Interventional Radiology and provide useful information and direction in performing TACE procedure as well as pre- and post- procedural patient care.

심장내과의 중재적 시술시 시술조건에 따른 방사선사의 방사선 노출 특성 (The Characteristic of Radiation Exposure for Radiologist with Applying Condition in Interventional Radiology in Cardiology)

  • 최근 방사선 의료에서 중재적 시술의 확대로 인하여 시술 건수가 증가하고 있으며, 시술자에 따라 방사선 노출이 다르게 나타 날 수 있으며 시술자와 환자가 받는 방사선 피폭의 증가를 가져온다. 본 연구는 2011년 11월 01일부터 2012년 01월 31일 까지 경북지역 S대학 병원에 내원한 303명의 심장내과 환자를 대상으로 중재적 방사선시술을 시행한 시술자 5명에 따른 방사선 노출특성을 비교분석하였다. 투시시간의 경우 5명의 시술의의 평균 투시시간은 697.95초, cumulative DAP(exp)의 경우 평균 환자 면적선량은 $52,730mGycm^2$, total DAP의 경우 평균 환자면적선량은 $104,875.14mGycm^2$, acquired image의 경우 평균 영상은 855.52frame, exposure image의 평균 영상은 802.2frame로 나타나 통계적으로 유의한 차이를 보였다(p<0.05). 장비의 X선 노출 특성 즉, 투시시간, cumulative DAP(fluro), cumulative DAP(exp), total DAP, acquired image, exposure image는 서로 높은 상관관계를 보이나 cumulative DAP(exp)와 acquired runs는 상관관계를 보이지 않았다. 시술자와 가장 큰 비중을 차지하는 것은 투시시간으로 나타나 투시시간이 길어질수록 방사선 피폭이 증가함을 의미한다. 시술시 피폭선량은 시술자에 따라 시술능력과 경험, 시술의 난이도 및 정밀 시술여부와 관련 있으며, 혈관 조영의 횟수와 투시시간이 정해진 것이 아니어서 인위적인 조절이 어려운 부분이라 할 수 있다. 따라서, 중재방사선 절차를 수행하는 의료진의 실질 피폭을 합리적으로 평가하는 시스템이 필요하며, 불필요한 피폭을 줄이기 위해 시술자를 비롯한 방사선 작업종사자에 대한 자체교육과 훈련이 필요할 것으로 사료된다.

중재적방사선검사에서 환자 피폭선량에 관한 연구 (A Study on the Evaluation of Patient Dose in Interventional Radiology)

  • 본 연구의 목적은 주요 중재적 시술을 시행 받는 환자를 대상으로 각 시술 당 환자의 피폭선량을 측정 및 평가하여, 우리나라 환자들이 중재적 시술을 시행 받을 때에 받게 되는 방사선 피폭의 위험도를 평가하고, 중재적 방사선 시술 시 환자 피폭선량에 대한 기준 선량 권고안을 마련하며, 환자의 피폭선량을 감소시킬 수 있는 방안을 모색하는데 있다. 각국의 중재적 방사선 분야에서의 환자 피폭선량을 조사하였고, 주요 병원에서 대표적인 중재적 시술을 대상으로 각 시술 당 환자가 받게 되는 피폭선량을 주요 신체 부위에 부착한 열형광선량계(TLD) chip을 이용하여 측정하였고, 혈관조영장비에서 얻어지는 면적선량(dose area product ; DAP)값을 이용하여 유효선량(effective dose; ED)을 측정하였다. 중재적 방사선분야에서의 환자 피폭선량과 관련된 연구와 자료는 진단 방사선 영역과는 달리 상당히 적은편이었다. 이번 연구에서 TACE의 평균 ED는 25.19 mSv로 나타났다. 다른 나라에 비해 높은 편은 아니지만, 몇 달 간격으로 반복적으로 시행 받아야 하는 시술의 특성상 누적선량(cumulative dose)에 주의를 기울일 필요가 있다. TACE의 평균 표면입사선량(ESD)은 511.75 mGy로 비교적 안전한 편이나, 최대 ESD는 4,346 mGy까지 측정되어 시간이 오래 걸리는 일부 TACE 시술에서는 결정적 효과에 대해서도 시술자의 주의가 필요하다. AVF 시술과 PTBD의 평균 ED는 각각 0.28 mSv와 4.8 mSv로 비교적 낮은 수준의 환자 피폭선량을 보였다. TFCA의 경우 평균 ED는 22.6 mSv로 다른 나라에 비해 상대적으로 높은 환자 피폭선량을 보였다. GDC embolization의 경우 대부분의 병원에서 DAP값이 지원되지 않는 구형장비를 사용하는 관계로, 이번 연구에서는 ED값을 구하지 못하였다. 하지만 평균 51.1분의 투시시간과 평균 2,264 mGy의 높은 ESD를 보이고 있어, ED 역시 상당히 높으리라 예상되며, 이에 대한 추가연구가 필요하다. 또한 TFCA와 GDC embolization을 병행하여 시행하는 경우 약 3 Gy의 선량이 피폭되므로 방사선으로 인한 장해를 방지하기 위해서 조사야를 변경하여 시술하는 것을 권고한다.

Clinical Study on Safety and Efficacy of Microwave Ablation for Primary Hyperparathyroidism

  • Objective: To evaluate the safety, feasibility, and efficacy of microwave ablation (MWA) for the treatment of primary hyperparathyroidism (PHPT). Materials and Methods: This study enrolled 67 PHPT patients (22 men, 45 women; mean age, 56.0 ± 16.3 years; range, 18-83 years) from January 2015 to December 2018. The laboratory data, including the serum intact parathyroid hormone (iPTH), calcium, phosphorus, and alkaline phosphatase (ALP) levels, were evaluated before MWA and again 2 hours, 1 day, 7 days, 1 month, 3 months, 6 months, 12 months, 18 months, and 24 months after. Results: Complete ablation was achieved with all 72 hyperplastic parathyroid glands found on the 67 patients enrolled, 64 of whom were treated in one session and 3 were treated over two sessions. The technical success rate was 100%. The median follow-up time was 13.6 months (range, 10.0-31.1 months). The clinical success rate was 89.4%. The volume reduction rate was 79.4% at 6 months. Compared to pre-MWA, the serum iPTH, calcium, phosphorus, and ALP levels had significantly improved 6 months post-MWA (iPTH, 157.3 pg/mL vs. 39.2 pg/mL; calcium, 2.75 ± 0.25 mmol/L vs. 2.34 ± 0.15 mmol/L; phosphorus, 0.86 ± 0.20 mmol/L vs. 1.12 ± 0.22 mmol/L; ALP, 79 U/L vs. 54 U/L, respectively; all, p < 0.01). Hoarseness was a major complication in 4 patients (6.0%), but it improved spontaneously within 2-3 months. Conclusion: MWA is safe, feasible, and effective for the treatment of PHPT.

Interventional Therapy for Renal Artery Pseudoaneurysms

  • The aim of this study was to explore the angiographic diagnosis and embolization therapy for renal artery pseudoaneurysms due to acute urinary tract hemorrhage after conservative medical management failed. Seven out of ten cases had fever symptoms after the kidney surgery. The pseudoaneurysms were treated with gelatin sponge and (or) spring coil and the majority demonstrated rapid blockage of hemorrhage. Angiography diagnosis and trans catheter embolization are rapid, safe and effective methods for diagnosis and treatment of renal artery pseudoaneurysms.
