  • Chronic sclerosing mediastinitis is a rare disease of unknown etiology, pathologically characterized by chronic inflammation and fibrosis of mediastinal soft tissue. The process is often progressive and can occur either focally or diffusely throughout the mediastinum. This can result in compression of adjacent mediastinal structures, most commonly the low-pressure superior vena cava but also the pulmonary artery and vein, trachca and bronchi, esophagus and can result in a variety of functional and roentgenographic manifestation and occasionally death. We experienced a case of chronic sclerosing mediastinitis of unknown cause, which was confirmed by biopsy with thoracotomy, so reported it with a review of literature.

  • Recurrent respiratory papillomatosis(RRP) is a chronic disease that is caused by the human papillomavirus(HPV) type 6 and 11. The most common site of the lesions is the larynx, but papillomas can occur throughout the respiratory and upper gastrointestinal tracts. Frequent recurrence of disease, can result in airway compromise and even death when papillomas either obstruct the airway or spread to the lung parenchyma. We encountered a case of a recurrent respiratory papillomatosis in a 23-year-old patient, who improved after treatment with interferon-${\alpha}$ The patiented with hoarseness, exertional dyspnea, and a productive cough. We report this case with a brief review of the relevant literature.

Traditional Korean Medicine Treatment of a Traumatic Brain Injury Patient with Aggressive Behavior (외상성 뇌손상 환자의 공격성에 대한 치료 1례)

  • Objective: To describe the effectiveness of traditional Korean medicine (TKM) on a patient showing aggressive behavior after a traumatic brain injury. Methods: We used complex TKM treatments. The acupoints used were as follows: Soyangin Taegeuk acupuncture (HT3, KI3, and SP3) and acupoints frequently used in hemiparesis (GV20, GV24, ST4, LI11, LI4, ST36, GB34, SP9, and LR3). Moxibustion was also applied to GV20, and herbal medicine (Samulanshin-tang-gami, 四物安神湯 加味方) was administered. The efficacy was evaluated at baseline, week 8, and week 16 using the Modified Overt Aggression Scale (MOAS). Results: After 8 weeks, the total MOAS score was decreased from 13 to 5, and this score was maintained until week 16. The average sleep time was increased and waking up during the night was decreased. Conclusions: We suggest that TKM could be effective for reducing aggressive behavior and alleviating sleep disturbances. Further studies are needed.

  • Aortobronchial fistula may cause a massive fatal hemoptysis. Recently prosthetic aortic graft insertion or endovascular stent graft is a cause of aortobronchial fistula. We report a rare case of hemoptysis from a fistula between an aortic arch aneurysm and the left main bronchus in a patient who had undergone an endovascular stent graft in pseudoaneurysm of descending thoracic aorta one year before.

  • Standard antituberculous therapy, including isoniazid (INH), rifampin, ethambutol, and pyrazinamide (PZA), is widely used to treat active tuberculosis. The most important side effect is hepatotoxicity. In a standard four-drug regimen, PZA was the most common cause of drug-induced hepatitis and was dose-related. The incidence of drug-induced hepatitis is high at doses of 40~70 mg/kg per day but has fallen significantly since the recommended dose was reduced. Liver toxicity induced by PZA is rare at doses of 25 mg/kg per day or less. PZA-induced fulminant hepatic failure is also rare but fatal. We report a case of fulminant hepatic failure caused by a re-challenge of PZA.

A Review of Domestic Research on Traditional Korean Medicine for Chemotherapy-Induced Peripheral Neuropathy (항암화학요법 유발 말초신경병증의 한의학적 치료에 대한 국내 연구동향 고찰)

  • Objectives: This study was performed to research Korean medical treatment methods for chemotherapy-induced peripheral neuropathy by summarizing domestic studies. Methods: All domestic research papers on the Korean medical treatment of chemotherapy-induced peripheral neuropathy were selected from the literature published until April 2024 in four domestic databases. In total, 26 domestic papers were selected. Results: Eighteen clinical studies and eight experimental studies were published by April 2024. Most of the studied patients were in their fifties, and 93.4% of the subjects were women. Uchashinki-hwan (牛車腎氣丸) was the most frequently prescribed herbal formula, and Rehmanniae Radix Preparata was the most commonly used herb. All studies showed significant effects of Korean medical treatment, and any adverse events were not reported. Conclusion: This study included all domestic research papers on chemotherapy-induced peripheral neuropathy to identify research trends in the Korean medicine community. Based on this, it is meaningful to confirm areas that need to be supplemented in future research plans. However, large-scale studies are needed.

A Case Report of Secondary Dementia Patient Caused by Diffuse Axonal Injury (미만성 축삭 손상으로 유발된 속발성 치매 환자 1례에 대한 증례 보고)

  • The purpose of this study is to present a case of secondary dementia caused by diffuse axonal injury. We diagnosed this patient with diffuse axonal injury by using brain computerized tomography(CT) and magnetic resonance imaging(MRI), and also diagnosed secondary dementia based on DSM-IV. To evaluate prognosis of the patient, we used K-DRS(Korean-Dementia Rating Scale) and gave him a written test. As a result of treating this patient with oriental medicine, the K-DRS score increased and the overall clinical symptoms improved. In oriental medicine, case studies of diffuse axonal injury are yet insufficient, hence more clinical studies and researches will be needed.

  • Variant angina causes chest pain due to abnormal coronary vasospasms. A 60-year-old male who was diagnosed with variant angina was admitted to the Oriental Medicine Hospital and treated with herbal medicines, including Pyungjinsujeom-san and Simjeok-hwan. After treatment, the frequency of chest pain and use of nitroglycerin decreased. Subjective symptoms of chest pain measured using a numerical rating scale (NRS) also decreased. These improvements persisted throughout the hospitalization period. In conclusion, Korean medicines, including Pyungjinsujeom-san and Simjeok-hwan, can be effective for patients with chest pain due to variant angina.

  • This case report is to show the effects of Traditional Korean medicine (TKM) on the patient with delayed encephalopathy after carbon monoxide poisoning. A patient with delayed encephalopathy after carbon monoxide poisoning occurs rarely and only a few cases have been reported. We treated a patient with cognitive deficit, gait disturbance was treated with herbal medicine(Boshiniknoe-tang-gami), acupuncture, moxibustion. We evaluated the improvements of symptoms by Korean Version of Mini Mental Status Examination score(K-MMSE), Modified Barthel Index(MBI) and Glasgow Coma Scale(GCS). After 3 weeks, total score of K-MMSE was increased from 12 to 17 and it was maintained to week 6. After 6 weeks, total score of MBI was increased from 30 to 86. And GCS was increased from 11 to 14. This report proved effect of Traditional Korean medicine on the improvement of delayed encephalopathy after carbon monoxide poisoning
