Study on the origin and theoretical foundation of I Dong-won(李東垣)'s pulse diagnosis distinguishing internal and external injuries(內外傷辨脈法) (이동원(李東垣) 내외상변맥법(內外傷辨脈法)의 유래와 이론적 근거에 대한 고찰)

  • Pulse diagnosis that distinguishes internal injury from external injury by comparing the left and right of the chon pulse was formed in the process of Naegyoung's pulsation theory of ST9 and LU9 being assimilated into diagnostic method by taking chon pulse. The founder of school of internal injury, I Dong-won, expanded the horizon for this method to be widely used in clinical practice by especially explaining the specific application and theoretical background. According to him, pulse at ST9 which means chon pulse at the left hand, is bigger than the chon pulse at the right hand, it reflects external injury. Bigger "entrance pulse", a chon pulse at the right hand means internal injury. The reason is the left side of the body is a path for Yanggi so it controls the exterior part and the right side of the body is a path for Eumgi to descend so it controls the interior part. Internal injury develops as the spleen and stomach get injured. If the spleen and stomach is damaged essence derived from food cannot ascend to the stomach and will flow back to the lower part. As a result, fire of Eum type formed at the lower part will shoot up to the upper part and manifests external injury-like exterior syndromes. In this case, evidence distinguishing between internal and external injury is the fact that right hand pulse is bigger than the left hand. The important reason for distinguishing between internal and external injury is because when treating external injury caused by excess syndrome, pathogenic Gi should be dispelled. However, treating internal injury cased by deficiency syndrome, requires promoting the primordial Gi.

A Case Report of Korean Medicine Treatment of Diffuse Axonal Injury in a Patient with Traumatic Brain Injury (외상성 뇌손상(Traumatic Brain Injury)에서 미만성 축삭손상(Diffuse Axonal Injury)으로 진단된 환자 한방 치험 1례)

  • The purpose of this study was to evaluate the effect of Korean medicine in a patient with cognitive impairment, emotional disturbance, and sleep disturbance due to a diffuse axonal injury associated with a traumatic brain injury. The patient was treated with herbal medicine and acupuncture. The treatment effects were evaluated using the Korean version of the Mini-Mental State Examination (MMSE-K) and the Global Detraction Scale (GDS), and by observing clinical symptoms. Improvements in the total scores of MMSE-K and GDS were observed after the Korean medicine treatments; the MMSE-K score increased from 13 to 23 and the GDS score decreased from 5 to 4. The emotional and sleep disturbances were also reduced. These case report findings suggest that Korean medicine may be effective for treating symptoms of diffuse axonal injury in patients with traumatic brain injury.

Asymptomatic Isolate Tricuspid Regurgitation with Chordae Tendineae Rupture Caused by Blunt Chest Injury

  • The incidence and importance of tricuspid valve regurgitation after a blunt chest injury has risen with the increase in the number of automobile accidents and steering wheel traumas. This kind of injury has been reported more frequently in the last decade because of the better diagnostic procedures and understanding of the pathology. However, tricuspid valve regurgitation following a blunt chest injury can still be easily missed because most patients do not show symptoms at the time of the trauma. A 55-year-old male patient presented himself at our facility after suffering a chest injury from an automobile accident. His transthoracic echocardiography (TTE) revealed severe tricuspid valve regurgitation due to the prolapse of his anterior valve leaflet. We report a case of asymptomatic tricuspid regurgitation that developed after a blunt chest injury.

Comparison of Herbs in Prescription Composition of Consumptive Disease and Internal Injury in Donguibogam Through Network Analysis (네트워크 분석을 통한 동의보감(東醫寶鑑) 내상(內傷)문과 허로(虛勞)문의 처방 구성 본초 비교)

  • Objective: Internal injuries and consumptive disease have different causes, yet they can affect each other. The relationship and combination of prescription drugs in the clinical practice of internal injuries and consumptive disease were analyzed for various diseases of "Donguibogam" through network analysis. Methods: The prescriptions used in consumptive disease and internal injury were established by conducting a full survey on the papers extracted from Donguibogam. The R version 4.0.3 (2020-10-10) and the igraph and arules package were used to perform network analysis and association rule relationship mining analysis in the first and second prescription compositions. Results: The herb frequently used for internal injury was Glycyrrhizae Radix, while the herb combination frequently used was Citri Pericarpium-Glycyrrhizae Radix. For centrality, the main factor was generally Glycyrrhizae Radix. In the case of consumptive disease, the herb most frequently used was Angelicae Gigantis Radix, and the combination most frequently used was Rehmanniae Radix Preparata-Angelicae Gigantis Radix. In terms of centrality, it was Angelicae Gigantis Radix. As a result of the network analysis of herbal prescription frequency, each group was divided into three. Conclusion: The interrelationship between internal injury and consumptive disease prescription drugs may reveal the differences and similarities between internal injury and consumptive disease and may serve as a basis for the development of new drugs or materials that can enhance mutual effectiveness in the treatment of internal injury and consumptive diseases.

Korean Medicine Treatment for Pressure Injury in Terminal Stage Cancer Patients with Debridement and Local Flap: A Case Report (변연절제 및 국소 피판술을 시행한 말기 암환자의 욕창에 대한 한의학적 치료 1례)

  • Objectives: Pressure injury is a common symptom of end-stage cancer, which impact quality of life. This case study reports on use of traditional Korean medicine in an end-stage cancer patient with pressure injury after debridement and local flap. Methods: A pressure injury with debridement and a local flap was treated using herbal medicine, a carbon arc, acupuncture, dressing, and cooperation in plastic surgery. Pressure injury was followed up with photographs. Results: On the 22nd day of treatment (26 days after the debridement and local flap), redness, swelling, and the condition of pressure injury were all improved. Moreover, the condition of the pressure injury was good without dressing. Conclusions: These results show that traditional Korean medicine may have a positive effect on a pressure injury and improve the quality of life of cancer patients. However, further study is needed to confirm these findings.

  • Lee Dong-won based on the machanism of internal disease founded on Umhwa and Wonki in discriminating internal injury from external. His general idea of discrimination of internal injury from external could be thought as an reinterpretation of the concept from "Neijing", to Triple heater Wonki and ascending and descending of stomach qi. He distinguished between Yin disease and Yang disease and classified into internal and external injury. "Insufficiency of yang brings about cold syndrome." and "An excess of yang brings about heat syndrome." are considered as external infection, and "Insufficiency of yin brings about heat syndrome." and "An excess of yin brings about cold syndrome." as internal injury.

  • The purpose of this study is to present a case of secondary dementia caused by diffuse axonal injury. We diagnosed this patient with diffuse axonal injury by using brain computerized tomography(CT) and magnetic resonance imaging(MRI), and also diagnosed secondary dementia based on DSM-IV. To evaluate prognosis of the patient, we used K-DRS(Korean-Dementia Rating Scale) and gave him a written test. As a result of treating this patient with oriental medicine, the K-DRS score increased and the overall clinical symptoms improved. In oriental medicine, case studies of diffuse axonal injury are yet insufficient, hence more clinical studies and researches will be needed.

A Case Report of Scald Burns Treated with Pharmacoacupuncture (약침과 한약의 복합 치료를 이용한 탕화상 환자 치험 1례)

  • We report the case of a 65-year-old female with burn injury from scalds. She was observed with a burn injury of the right arm and leg that occurred 6 days before. She was treated with pharmacoacupuncture Soyeom, and herb medicine daily for 3 weeks. After Twenty days, burn wound size was reduced and tissue regeneration was accelerated. In conclusion, it is our belief that pharmacoacupuncture and herb medicine is effective to treat burn injury, and future studies will be required to ascertain this method on burn injury.

Effects of Astragali Radix (AR) on Chronic Renal Injury in Rats (황기가 흰쥐의 만성 신손상에 미치는 영향)

  • Objective : This study was designed to investigate the possibility of AR for chronic renal injury. Methods : The author first investigated the expression levels of DNA by inducing of chronic renal injury. Then, the author investigated the effects of AR on chronic renal injury induced by combination treatment with Adriamycin and cisplatin in terms of changes in body weights and renal tissues, urine volume, BUN and creatinine levels, creatinine clearance and histopathological changes in renal tissues. Total expression levels of 546 genes were elevated or lowered by induction of chronic renal injury. Genes of which whose expression levels were elevated by induction of chronic renal injury were related to the PPAR signaling pathway and fatty acid mechanism, etc. Genes of which expression levels were lowered by induction of chronic renal injury were related to the neuroactive ligand-receptor signaling pathway. Results : Oral administration of AR restored renal mass which was reduced by induction of chronic renal injury. AR also restored creatinine clearance and lowered serum BUN level. In histopathological observation, the AR group has a tendency to prevent tissue damages as shown in the chronic renal injury group. Conclusions : AR can be used to treat patients with chronic renal injury although further study will be needed to elucidate the exact mechanisms in the efficacy of AR on chronic renal injury.

The Effect of the Water Extract of Angelica Sinens on Gliosis Repression of Astrocyte after Hypoxic injury (당귀가 저산소로 손상된 성상세포의 gliosis 억제에 미치는 영향)

  • Object : Gliosis becomes a physical and mechanical barrier to axonal regeneration. Reactive gliosis induced by hypoxic brain injury is involved with up-regulation of CD81 and GFAP. The current study was to examine the effect of the Angelica Sinens on CD81 and GFAP regulation after hypoxic brain injury in the astrocyte. Methods : MTT assay was performed to examine cell viability, and cell based ELISA, western blot and PCR were used to detect the expression of CD81 and GFAP. Results : The following results were obtained: 1. Using ELISA, western blot and PCR from the astrocyte after hypoxic injury, CD81 and GFAP expression was seen to have increased. 2. After the administration of Angelica Sinens extract to astrocyte following hypoxic injury, CD81 and GFAP expression was down regulated significantly. The water extract of Angelica Sinens prevented cell destruction by hypoxic induced with $CoCl_2$. Conclusion : These results indicate that Angelica Sinens could suppress reactive gliosis, which disturbs astrocyte regeneration after hypoxic brain injury by controlling the expression of CD81 and GFAP.
