만성폐쇄성폐질환 환자 사망 원인 - 한 3차 병원 연구 (Cause of Death in COPD Patients of a Referral Hospital)

  • 연구 배경 : 만성폐쇄성폐질환(COPD)은 45세 이상의 성인에서 국내 유병률 17.2%로 주요 질환이다. 하지만, 국내 COPD 환자의 사망원인에 대한 연구는 불충분한 상황이다. 이에 국내 COPD 환자 사망원인에 대해서 알아보고자 서울아산병원 의무기록을 후향적으로 조사하였다. 방 법 : 2003년 1년간 서울아산병원에서 COPD로 진료한 1,078명의 사망여부를 통계청에 의뢰하여 총 사망자 88명을 얻었고 이중 폐결핵 후유증, 기관지확장증, 폐암 등 암 환자를 제외한 후 남은 28명의 COPD 환자 대상으로 사망원인을 분석하였다. 결 과 : COPD 환자의 사망원인은 폐렴 등 호흡기 원인이 16명 (57%), 심장 원인 5명 (18%), 급사 3명 (11%), 기타 4명 (14%) 등이었다. 서울아산병원 내에서 사망한 환자와 외에서 사망한 환자의 호흡기 관련 사망이 각각 83%(10명/12명)과 38%(6명/16명)이었다 (P=0.05) $FEV_1$이 50%예측치보다 큰 환자와 작은 환자의 호흡기 관련 사망은 각각 43%과 55%이었다 (P=0.89). 결 론 : 국내 3차 병원에서 진료하는 COPD 환자의 사망 원인은 폐렴 등 호흡기 원인 다수를 차지한다.

A Case of Endobronchial Aspergilloma Associated with Foreign Body in Immunocompetent Patient without Underlying Lung Disease

  • Aspergillus causes a variety of clinical syndromes in the lung including tracheobronchial aspergillosis, invasive aspergillosis, chronic necrotizing pulmonary aspergillosis, allergic bronchopulmonary aspergillosis, and aspergilloma. Aspergilloma usually results from ingrowths of colonized Aspergillus in damaged bronchial tree, pulmonary cyst or cavities of patients with underlying lung diseases. There are a few reports on endobronchial aspergilloma without underlying pulmonary lesion. We have experienced a case of endobronchial aspergilloma associated with foreign body developed in an immunocompetent patient without underlying lung diseases. A 59-year-old man is being hospitalized with recurring hemoptysis for 5 months. X-ray and computed tomography scans of chest showed a nodular opacity in superior segment of left lower lobe. Fiberoptic bronchoscopy revealed an irregular, mass-like, brownish material which totally obstructed the sub-segmental bronchus and a foreign body in superior segmental bronchus of the lower left lobe. Histopathologic examinations of biopsy specimen revealed fungal hyphae, characteristic of Aspergillus species.

『동의보감(東醫寶鑑)』 중 연제(烟劑)·훈제(熏劑)와 함금제(含噙劑)의 활용(活用)에 관한 연구(硏究) (Study on a Usage of Smoking Preparations, Fumigant and Buccal Preparations in 『Dongeuibogam』)

  • Objectives : The purpose of this study is to provide grounds for the development of preparation of Korean medicine through investigating the usage of smoking preparation, fumigant and buccal preparations in "Dongeuibogam". Methods : We have researched the usage of related preparations as following procedure : 1. Choosing a related words such as combustion(燒, 燎, 燃) smoking(烟), fumigation(熏, 熏烟, 熏洗), inhalation(聞, 嗅, 齅), holding in mouth(含, 噙). 2. Searching sentences : We have searched sentences containing the related words in "Dongeuibogam", analyzed the selected sentences and made tables of the usage including indication, formula, material. Conclusions : 1. Smoking preparations and Fumigant can be classified into two different groups : 1) internal usage for the treatment of internal diseases such as dizziness, congestion, stroke, cough. 2) external usage for the treatment of wounds, sores, genital diseases, anal disease and for the prevention of diseases. 2. Buccal preparation is mainly used for the treatment of mouth, tongue, throat and lung diseases. 3. These preparations have been used not only for external diseases but internal diseases. 4. Each preparation has chief herbs for its peculiar properties of treatment.

  • A 31-year-old male has a $11.5{\times}10\;cm$ sized relatively well marginated round mass containing peripheral calcification in left upper lung field on chest roentgenograms and computed tomogram. There are no presenting symptoms including weight loss, and occupational exposures. Definitive diagnosis must differentiate from the variety of diseases whether malignancy or benign, extrapulmonary or intrapulmonary. This report described one case of malignant localized fibrous mesothelioma histologically.
