Assessment of interhospital transport care for pediatric patients

  • Background: Many critically ill patients require transfer to a higher-level hospital for complex medical care. Despite the publication of the American Academy of Pediatrics guidelines for pediatric interhospital transportation services and the establishment of many pediatric transport programs, adverse events during pediatric transport still occur. Purpose: To determine the incidence of adverse events occurring during pediatric transport and explore their complications and risk factors. Methods: This prospective observational study explored the adverse events that occurred during the interhospital transport of all pediatric patients referred to the pediatric intensive care unit of Ramathibodi Hospital between March 2016 and June 2017. Results: There were 122 pediatric transports to the unit. Adverse events occurred in 25 cases (22%). Physiologic deterioration occurred in 15 patients (60%). Most issues (11 events) involved circulatory problems causing patient hypotension and poor tissue perfusion requiring fluid resuscitation or inotropic administration on arrival at the unit. Respiratory complications were the second most common cause (4 events). Equipment-related adverse events occurred in 5 patients (20%). The common causes were accidental extubation and endotracheal tube displacement. Five patients had both physiologic deterioration and equipment-related adverse events. Regarding transport personnel, the group without complications more often had a physician escort than the group with complications (92% vs. 76%; relative risk, 2.4; P=0.028). Conclusion: The incidence of adverse events occurring during the transport of critically ill pediatric patients was 22%. Most events involved physiological deterioration. Escort personnel maybe the key to preventing and appropriately monitoring complications occurring during transport.

A Study Left Ventricular Volume Change Measurement using Nuclear Stethoscope (핵청진기를 이용한 좌심실 부피변화 측정에 관한 연구)

  • In the present study, we developed a nuclear stethoscope interfaced with a personal computer (APPLE- II) and evaluated it's performances as compared with the gamma camera. This single-probe, ECG-gated system has been used for displaying the left ventricular time-activity curves, the peak ejection time and its rate, the peak filling time and its rate, and the R-R interval distribution. The radioactivity was measured at every 10msec around the ventricular region, where the activity curves shows the maximal peak to peak variations. The background activity was measured around the lung area showing its counts approximately 50% of the end-diastolic count with minimal variations. The average time-activity curves of 100 beats were used for analysis in the equilibrium study after intravenous injection of 15-20mCi of Tc. The ejection fractions measured by the nuclear stethoscope(Y) were compared with those measured by gamma camera(X) in 47 patients with various heart diseases. The correlation coefficient between two measurements was 0.766 with a relation of Y=1.04 x-8.48. Also, the high reproducibility was obtained for the same patient. Also, the high reproducibility was obtained for the same patient. From this study, we conclude that this device is useful for continuous monitoring in the intensive care unit, as it is portable, compact, and inexpensive.

The Choice of Management in Patients with Splenic Blunt Trauma : A Single Center Study (외상성 비장 손상 환자의 치료방법의 선택: 단일 기관 연구)

  • Purpose: Nowadays, non-operative management increases in patients with blunt splenic injury due to development of diagnostic and interventional technique. The purpose of this study is to evaluate the management in patients with blunt splenic injury and effect of clinical state such as shock on the choice of management. Methods: From April 2007 to July 2013, we retrospectively reviewed the medical charts of fifty patients who had splenic injury after blunt trauma. The demographic characteristics, American Association for the Surgery of Trauma (AAST) grade of splenic injury, management method (emergency operation, angiographic embolization or observation) and clinical outcome were analyzed. Results: The mean age was $41.5{\pm}21.4$ years and male was 44(88%). Twenty patients(40%) were in shock condition initially and five patients(10%) underwent emergency operation due to hemodynamic instability. Emergency angiographic embolization was performed in 20 patients(40%) and 25 patients were managed conservatively. When patients were divided into shock group (SG) and non-shock group (NSG), Patients in SG had significantly higher serum lactate level and base deficit than NSG (lactate; $4.5{\pm}3.4$ mmol/L, base deficit; $5.8{\pm}4.4$ mmol/L vs $1.9{\pm}1.4$ mmol/L, $2.8{\pm}2.5$ mmol/L, p=0.007, p=0.013). There was no significant difference of AAST grade and contrast blush rate in abdomen CT between two groups. Among 45 patients with non-operative management, four patients(8.9%) got delayed angiographic embolization and 3 patient died from companied organ injury. Conclusion: Non-operative management can be acceptable management option in patients with splenic blunt trauma under intensive hemodynamic monitoring.

Estimation of Nitrate Leaching Rates for a Small Rural Watershed Using a Distributed Watershed Model (분포형 유역모델을 이용한 농촌지역 소유역의 질산성 질소 지하침출량 평가)

  • A distributed watershed model CAMEL (Chemicals, Agricultural Management and Erosion Losses) was applied to a small rural watershed where intensive livestock farming sites are located to estimate nitrate leaching rates from soil to groundwater. The model was calibrated against the stream flows, and T-N and $NO_3-N$ concentrations were observed at the watershed outlet for three rainfall events in 2014. The simulation results showed good agreement with the observed stream flows ($R^2=0.67{\sim}0.93$), T-N concentrations ($R^2=0.40{\sim}0.58$) and $NO_3-N$ concentrations ($R^2=0.43{\sim}0.65$). The estimated annual nitrate leaching rate of the watershed was 33.0 kg N/ha/yr. The contributing proportions of individual activities to the total nitrate leaching rate of the watershed were estimated for livestock farming, applications of chemical fertilizer, and manure. The simulation results showed that the highest contributor to the nitrate leaching rate of the watershed was chemical fertilizer applications. The simulation period was for one year only, however, and results may vary depending on different conditions. Gathering input data over a longer period of time and monitoring data for calibration is needed. When this has been accomplished, it is expected that this model can be applied to small rural watersheds for evaluating temporal and spatial variations of nitrogen transformations and transport processes.

Material and Deterioration Characteristic Analysis for Stone Sculptures in Gyeongbokgung Royal Palace, Seoul (경복궁 석조조형물의 재질 및 손상특성 분석)

  • The stone sculptures in Gyeongbokgung Royal Palace are mainly composed of granite (884 sculptures, 96.7%) and marble (25 sculptures, 2.7%) that originated in several different quarries. Main deterioration forms are crack (24%), break-out (21%), exfoliation (36%), granular disintegration (9%) and blackening (20%). Crack and break-out are dominant contributors to high physical deterioration degree, and blackening is major weathering form of intensive discoloration. The Gyeonghoeru, Geunjeongjeon and Pumgyeseok areas require urgent and high conservation maintenance with short-term periodic monitoring since proportions of 3-grade deterioration were calculated higher than others as 55% for Gyeonghoeru, 29% for Geunjeongjeon, 11% for Pumgyeseok area. The Pumgyeseok (officials' rank stone), especially, needs intervention for protective facility due to its material vulnerability to weathering in outdoor environment.

  • This paper presents a cal center which is a subsystem of a web-based real time monitoring system of intensive care units. Based on Computer-Telephony Integration (CTI) technology, the call center attempts to efficiently and automatically send messages to patients\` families, doctors, and other staffs in hospital via communication media suitable to the occasion. The problem of determining appropriate media can be very complicated by the urgency of a message, calling time, and communication media available to the target person. We use the Dempster-Shafer theory, one of the uncertainty handling methods, to determine the most suitable communication media that will transmit a message rapidly and safely. In addition, we use agent technology to perform the calling process without requiring the intervention of the user of the call center. this call center enables message transfer through various communication media in an integrated environment, and relieves medical staff from the calling process, which in turn will make a contribution toward enhancing medical service.

Physico-Chemical Characteristics of Visibility Impairment by Airborne Pollen (공중화분에 의한 시정장애 현상의 물리적 및 화학적 특성 규명)

  • Intensive visibility monitoring was conducted to investigate physical and chemical characteristics of visibility impairment by airborne pollen. Light attenuation coefficients were optically measured by a transmissometer, a nephelometer, and an aethalometer. Elemental, ionic, and carbonaceous species were chemically analyzed on the filters collected by $PM_{2.5}$ and $PM_{10}$ samplers. Aerosol size distribution was analyzed using a cascade impactor during airborne pollen period. Airborne pollen count was calculated using a scanning electron microscope. Airborne pollen was emitted into the atmosphere in springtime and funker degraded visibility through its scattering and absorbing the light. Average light extinction coefficient was measured to be $211{\pm}36Mm^{-1}$ when airborne pollen was not observed. But it increased to $459{\pm}267Mm^{-1}$ during the airborne pollen period due to increase of average $PM_{2.5}$ and $PM_{10}$ mass concentration and relative humidity and airborne pollen count concentration for $PM_{10}$, which were measured to be $46.5{\pm}29.1{\mu}g\;m^{-3},\;97.0{\pm}41.7{\mu}g\;m^{-3},\;54.1{\pm}11.6%$, and $68.2{\pm}89.7m^{-3}$, respectively. Average light extinction efficiencies for $PM_{2.5}$ and $PM_{10}$ were calculated to be $5.9{\pm}0.9$ and $4.5{\pm}0.8m^2 g^{-1}$ during the airborne pollen period. Light extinction efficiency for $PM_{10}$ increased further than that for $PM_{2.5}$. The average light extinction budget by airborne pollen was estimated to be about 24% out of the average measured light extinction coefficient during the airborne pollen period.

Classification of the Algal Monitoring Points by Histogram Analysis of Chlorophyll-a (Chlorophyll-a의 히스토그램 분석을 통한 녹조발생 우심지역 분류)

  • In this study, we analyzed the value of Chl-a by histogram to classify the points where algal management is required. The degree of algal bloom by point was analyzed using the ogive curve, and the algal control points were classified into three stages according to the shape of the frequency distribution table. Of the four major rivers, low concentration of Chl-a appeared most frequently in the Han River, while the high concentration of Chl-a was frequently found at the points of the Geum and the Yeongsan Rivers. In the case of the Han River, no apprehensive areas were found thatrequire intensive management, while most points on the Geum and the Yeongsan Rivers required algal management. Finally, the Nakdong River basin was identified as points requiring algal management from the mid to downstream. The results of this study have confirmation of the possibility that the frequency distribution could be used as a supplementary indicator to express the algal bloom.

Estimation of Mean Residence Time using Soil Moisture at a Hillslope on the Forested Catchment (산림 사면에서 토양수분을 이용한 물 평균체류시간 추정)

  • The mean residence time is the time scale for intermediate status between infiltration and runoff and one of the critical factors for understanding runoff response, erosion, and eco-hydrological processes. This research explored a direct method to estimate the mean residence time over existing indirect, isotope tracer method. Spatial and temporal distributions of soil moisture have been monitored for a year with 2-hours monitoring interval. Mean residence time for soil moisture showed apparent increasing tendency to deeper depth and decreasing trend during summer periods, which had intensive rainfall events. The mean residence times obtained from this research showed similar trend to those obtained from other isotope methods, which means the direct method can be an efficient approach to obtain the mean residence time.

Characteristics of Washed-off Pollutants from Railway Station During Storms (강우시 철로 역사에서 발생하는 비점오염물질의 유출 경향 및 특성)

  • The ministry of Environment, Korea, are designing the TMDL(Total Maximum daily Load) program far 4 major large rivers to improve water quality from possible pollutants. It can be successfully performed as controling of nonpoint pollutants from watershed area. Railway stations are stormwater intensive land use because of high imperviousness and high pollutant mass emissions from various activities. Especially the metal pollutants from the railway station were recognized as an important pollutants because of its toxicities. In order to characterize the washed-off pollutants. the monitoring were performed on a railway station during storms. Pollutant concentrations are exponentially decreasing during the storm duration. The 95% confidence interval of pollutant concentrations in an hour storm duration ranges from 61.6 to 115.4mg/L for TSS(mean=88.50mg/L),103.8-244.1mg/L for COD(mean=174 mg/L) and 7.68-17.32mg/L for Oil & Grease(mean=12.5mg/L). The ranges of metals were 39.2-84.0 $\mu\textrm{g}$/L for total Cu(mean=61.6$\mu\textrm{g}$/L), 14.0-25.8$\mu\textrm{g}$/L for total Pb(mean=19.9$\mu\textrm{g}$/L) and 182.2-376.l $\mu\textrm{g}$/L for total Zn(mean=279.2$\mu\textrm{g}$/L). The first flush criteria for best management practices can be suggested to 50% pollutant mass emissions during 30% of the total flow.
