식품섭취에 따른 유방암의 발생 위험 : 환자- 대조군 연구 (Food Intake and Breast Cncer Risk: A Case-Control Study)

  • We performed case-control study to evaluate relationship between food intake and risk of breast cancer. We interviewed breast cancer cases(n=108) who were newly histologically identified and selected from Hanyang and Soonchunhyang University Hospital in Seoul. We used hospital-based control subjects(n=121), who were selected from the patients in the department of plastic surgery, general surgery and opthalmology of the same hospital by frequency matching. Matching variables were age($\pm$4 age) and menopausal status. We collected information on general characteristics of subjects, history of disease, family history of breast cancer, vitamin supplementation, alcohol intake, and food intake through individual interview. Dietary information was ascertained via a food frequency questionnaire method(total item=98). All analyses was conducted according to menopausal status. Especially, pepper and grape intake in premenopausal women, showed protective effect in breast cancer. Otherwise consumption of meat, pork, and fish was not associated with breast cancer risk in this study. At milk and dairy products, milk intake was associated with lower risk of breast cancer in postmenopausal women and overall intake of soy products was associated with breast cancer risk, but not significant. These findings suggested that consumption of some fruit and vegetable intake was a protective factor on breast cancer and further study with more number of subjects should be need to evaluate the breast cancer risk. 정신과 여자 입원 환자의 체중 증가 (Weight Gain of the Psychiatric Female Hospitalized Patients)

  • Objectives : The purposes of present study were to identify weight increment in female psychiatric patients during hospitalization and to evaluate the relationship between weight gain and daily calorie intake, daily activity and other variables of disease itself. Methods : 20 patients were studied. Body weight were measured once a week, and daily activity(total amounts of walking/day) and total amount of daily calorie intake were measured twice a week. We examined psychiatric and medical illness history, eating disorders' history and family history of eating disorders and obesity. Results: Mean body weight and Body Mass Index(BMI) at admission are 49.40kg and 19.59kg/$m^2$. Mean weight increment during hospital stays of mean 65 days is 4.90kg. Mean amounts of daily calorie intake and daily activity(daily walks) were increased during hospital stays, but not statistically significant. The degree of weight increment is higher in longer hospital stay group, but not statistically significant. Mean body weight at admission of mood disorder group is higher than that of schizophrenia group, but not statistically significant. Conclusion : This results suggested that weight increment in female psychiatric hospitalized patients is present. However, it is not resulted by amount of calorie intake and daily activity level.

광주지역 성인의 나트륨 배설량과 비만의 관계 (Association between Sodium Excretion and Obesity of Adults in Gwangju)

  • Purpose: The aim of this study was to analyze the association between sodium excretion and obesity for healthy adults in the Gwangju area. Methods: The participants included 80 healthy adults aged 19 to 69 years in Gwangju. The dietary intake and sodium excretion were obtained using the 24-hour recall method and 24 hour urine collection. The participants were classified into two groups according to the amount of urinary sodium excretion: (${\leq}141.75mmol/dL$, > 141.75 mmol/dL). Results: After adjusting for sex, age, smoking history, and income, the high excretion of sodium group was significantly higher for weight, body mass index, body fat mass, percent body fat, visceral fat area (VFA), waist circumference, hip circumference, and WHR. The energy and nutrients intake were significant after adjusting for sex, age, smoking history, and income. The LSE group had a significantly higher fat intake and Na/K intake ratio. The HSE group had significantly higher fiber intake, and K intake. As the amount of urinary sodium excretion increased, the risk of obesity before correction was 3.57 (95% CI: 1.13-11.25) times greater, and the risk of obesity of T3 increased significantly by 3.33 times (95% CI: 1.05-10.59). After correcting for sex and age, the obesity risk of T2 increased significantly by 4.23 times (95% CI: 1.11-16.06), and after correcting for sex, age, smoking history, and income, the obesity risk of T2 increased significantly by 6.81 times (95% CI: 1.44-32.19) the risk of obesity. Conclusions: An association exists between sodium excretion and obesity in Korean adults. In this study, the high excretion of sodium group was obese and the risk of obesity was higher than the low excretion of sodium group.

농촌지역 성인들의 혈청 총콜레스테롤치 및 관련요인 (Serum total Cholesterol Levels and Related Factors in a Rural Adult Population)

  • Objectives : To investigate the fetal cholesterol levels and related factors in a rural adult population. Methods : 3,207 adults(1,272 men and 1,935 women) were examined in 1997 Their heights, weights, and fasting serum total cholesterol levels were measured, and their body mass indices were calculated. A questionnaire interviewing method was used to collect risk factor data. Results : The mean value of total cholesterol were 190.5 mg/dl, and 198.8 mg/dl, and the age-adjusted prevalence rates for hypercholesterolemia (above 240 mg/dl) were 13.7% and 10.2%, in men and women, respectively. From simple analyses, age, educational level, coffee intake, amount of meat and food intake, waist circumference, waist-hip circumference ratio(WHR), and body mass index(BMI) were significant risk factors in men (p<0.05) relating to serum total cholesterol levels. In women, age, educational level, hypertension history, diabetes history, herbal drug history, amount of feed intake, alcohol consumption, waist circumference, WHR, BMI, and menopausal status were significant risk factors (p<0.05). In multiple linear regression analyses, waist circumference (p<0.01), BMI (p<0.01), and coffee intake (p<0.05) proved to be significant risk factors in men. Whereas, menopausal status (p<0.01), BMI (p<0.01), herbal drug history (p<0.01), amount of food intake (p<0.05), waist circumference (p<0.05), and alcohol consumption (p<0.05) were significant risk factors in women. Conclusions : The significant risk factors, for both men and women, relating to the serum total cholesterol were waist circumference and BMI. Thus, for the reduction of serum total cholesterol level, it is recommended that health education for the control of obesity should be implemented.

  • In the present study, effective hydrodynamic pressure modeling methods for three-dimensional earthquake safety analysis of a dam intake tower structure are investigated. Time history analysis results using the Westergaard added mass and Chopra added mass methods are compared with the one by the CASI (Coupled Acoustic Structural Interaction) method, which is accepted as giving almost exact solutions, to evaluate the difference in displacement response, stress and dynamic eccentricity. The 3D time history analysis of a realistic intake tower, which has the standard geometry widely used in Korea, shows that the Chopra added mass method gives similar results in displacement and stress and less conservative results in dynamic eccentricity to CASI ones, while the Westergaard added mass yields much more conservative results in all measures. This study suggests to use the CASI method directly for three-dimensional earthquake safety analysis of a dam intake tower, if computationally possible.

인삼의 섭취가 흡연성인의 인체임파구 SCE 빈도수에 미치는 영향 (The Effect of Ginseng Intake on the Frequency of Sister Chromatid Exchanges of Human Lymphocyte of Adult Smokers)

  • This study was intended to investigate the anticarcinogenic effect of ginseng previously elucidated by other researches in animal studies. The sister chromatid exchange(SCE) method of human lymphocytes was used as a biomarker. Based on the literature search and the results of our laboratory, smoking was used as a parameter elevating the SCE frequency of general human population. To evaluate the smoking and ginseng effect on SCE frequency, 98 male healthy factory workers aged 23 to 58 years were divided into 4 groups : smoker with ginseng (SG), smoker control(SC), non-smoker with ginseng(NSG), and non-smoker control(NSC) groups, according to their smoking habits and ginseng intake. The mean sponteneous SCE per cell for the SG(10.8$\pm$0.3) and SC(10.4$\pm$0.3) groups were significantly higher than the NSG(9.1$\pm$0.2) and NSC(9.3$\pm$0.3) groups(p<0.05). High frequency cells (HFCs, cells with 15 SCEs) in SG and SC groups were also greater than those in NSG and NSC groups. However, the SCE levels of the SG and SC groups were not associated with the personal smoking history and the number of cigaretts smoked per day. Ginseng intake did not show any effect on the increased SCE caused by smoking. There were no correlations of the elevated SCE among smoking and ginseng types, history of ginseng intake, and consumption frequencies of ginseng intake. These results does not support the findings of other researchers that ginseng could be a protective agent to DNA damage.

한국 중년여성의 대두식품을 통한 이소플라본 섭취 수준 조사 (Estimated Isoflavone Intake from Soy Products in Korean Middle-aged Women)

  • There is growing evidence that soy isoflavone play beneficial roles in the prevention of chronic diseases such as hormone dependent cancers, cardiovascular diseases, osteoporosis and also reduced incidence of menopausal symptoms. However current data are not sufficient to determine the effective doses for beneficial as well as harmful effects and to support dietary recommendation for isoflavones. Since soy products containing isoflavone are one of the common food items that Koreans consume daily basis, assessing consumption of soy isoflavone by Koreans will give a valuable information on making dietary recommendations for isoflavones. the present study was designed to assess dietary intake of isoflavone of Korean middle-aged women who might receive the most beneficial effects from isoflavone consumption thereby prevent post menopausal related symptoms and diseases. In this study isoflavone contents of soy products were analyzed and soy products and isoflavone consumption of these subjects were assessed by a self-reported dietary questionnaire and isoflavone intake data base. Subjects were consisted of 178 Korean women (35~60 y) who live in urban and rural area. All subjects provided detailed information on demographics, anthropometry, health history, menopausal symptoms, health history, menopausal symptoms, and dietary intake using food frequency questionnaire (FFQ) which was developed for Koreans. The average age of the subjects was 46.6 years. The soy products which the subjects frequently consumed were soybean paste soup, braised fried soybean, tofu residue stew, tofu, soy milk, natto stew, bean sprouts, and soybean broth which contained 10.68 mg, 3.34 mg, 2.44 mg, 2.42 mg, 2.42 mg, 1.12 mg, 1.02 mg, 0.33 mg of isoflavone per 100 g, respectively. The estimated daily intake (EDI) of isoflavones ranged from 0 to 144.3 mg, and the mean daily isoflavone (sum of daidzein and genisten) intake of the subjects was 24.41 mg.

유아에서 영양섭취와 알레르기성 비염과의 관련성 - 2013년~2016년 국민건강영양조사 자료이용 - (Relation of Nutritional Intake and Allergic Rhinitis in Infants - Using the Korea National Health and Nutrition Examination Survey (KNHANES) 2013~2016 -)

  • Objectives: This study examined the relationship between the presence of allergic rhinitis and the nutritional intake levels of Korean infants. Methods: The study involved a total of 1,214 infant subjects aged 1~5 months from the 2013~2016 KNHNES (Korea National Health and Nutrition Examination Survey). The Subjects were classified into two groups based on the presence of allergic rhinitis: Non-allergic rhinitis infants (NARI, n=1,088) and allergic rhinitis infants (ARI, n=126). The general characteristics and family history of allergies, nutrient intake status, nutrient supplement intake, and breast milk and baby food start period data of the two groups were compared. All statistical analyses accounted for the complex sampling design effect and sampling weights. Results: The mean age was 0.5 years old in the ARI group compared to the NARI group. In the residence, the rate of urban was higher in ARI. The family history revealed a significant difference between the two groups, particularly those of mothers rather than fathers. The nutrient intake levels were high in energy, phosphorus, sodium, potassium, iron, riboflavin, niacin, and polyunsaturated fatty acids. Breastfeeding was significantly higher in the ARI group than in the NARI group. The baby food start period was 0.3 months earlier in NARI group than in ARI group. The height, body weight, and birth weight were higher in ARI group than NARI group. The result of Odds ratio analysis showed that excess energy, protein, calcium, phosphorus, iron, riboflavin, and niacin intake increases the risk of allergic rhinitis. Conclusions: These results can be used as data to develop nutrition guidelines for allergic rhinitis infants.

우리나라 성인 남성 당뇨병의 발생양상과 위험요인에 관한 전향적 코호트 연구 (Incidence and Risk Factors for Diabetes Mellitus in Korean Middle-aged Men : Seoul Cohort DM Follow-up Study)

  • Objectives : It is known that the prevalence of diabetes mellitus(DM) appears to be rapidly increasing in recent times in Korea, presumably due to a westernized diet and change of life style followed by rapid economic growth. Based on the Seoul male cohort which was constructed in 1993, this study was conducted to estimate the annual incidence rates of DM through 4 years' follow up and to determine which factors are associated with DM risk in Korean middle-aged men. Methods : Among 14,533 men recruited at baseline, 559 were excluded because they reported a history of diabetes or were found to be diabetes at 1992 routine health examination. During 4 years' follow-up, 237 incident DM cases were ascertained through chart reviews and telephone contacts for those who have ever visited hospitals or clinics under suspicion of DM during 1993-1996 and the biennial routine health examinations in 1994 and 1996. Results : In this study the annual incidence of DM among the study population was estimated to be 0.5 per 100. This study showed that fasting glucose level at initial baseline examination was a powerful predictor of risk for diabetes several years later(fasting blood glucose of $\geq$ 110 mg/dl compared with $\leq$ 80 mg/dl, Hazard Ratio[HR]:15.6, 95% Confidence interval[CI]=9.1-26.6) after considering potential covariates such as age, family history, smoking and alcohol history, body mass index, physical activity, total energy intake, and total fiber intake. Adjusted hazard ratios of family history of diabetes was 1.95(95% CI=1.38-2.75); of obesity as measured by BMI(BMI $\geq$ 25.3 compared with $\leq$ 21.3) was 7.19(95% CI=3,75-13.8); of weight change during middle life(>10kg compared with $\leq$ 5) was 1.77(95% CI=1.16-2.69); of smoking(current vs none) was 1.93(95% CI=1.06-3.51); and fat intake(upper fertile compared with lower fertile) was 1.88(95% CI=1.01-3.49), while fiber intake was associated with the reduced risk(HR=0.36, 95% CI=0.19-0.67). Conclusion : The factors identified in this study indicate that the greatest reduction in risk of diabetes might be achieved through population-based efforts that promote fiber intake and reduce obesity, smoking, and fat intake.

임부의 염분 기호도와 식이섭취, 소디움 섭취 실태 (Salt Preference and Sodium Intake among Pregnant Women)

  • Purpose: This study was to estimate salt preference and sodium intake of pregnant women, and identify the relationship between salt preference and sodium intake. Methods: Research design was a cross sectional correlational survey with 197 pregnant women who visited outpatient clinics for antenatal care. The sodium intake levels were estimated by the amounts of sodium intake using the 24-hour recall method and sodium concentration in spot urine. The data were analyzed using descriptive statistics, t-test, ANOVA and Pearson's correlation. Results: Sodium intake using 24-hour recall method was $3,504{\pm}1,359mg$. Sodium intake levels had statistically significant differences depending on income. The average amount of sodium in spot urine was $2,882{\pm}878mg/day$. Sodium excretion levels had statistically significant differences depending on whether participants had preexisting hypertension in their family history and Body Mass Index (BMI) pre-conception. Salt preference was $62.61{\pm}20.96$ out of 180 points. Salt preference had significant differences depending on income, parity, gestational age, BMI pre-conception and showed negative correlation with sodium quantity in spot urine. Conclusion: Sodium intake in pregnant women recommended by World Health Organization recommended is 175%. Salt preference was not significantly different between sodium intake levels, however it was negatively correlated with sodium quantity in spot urine among pregnant women.
