  • Pulmonary atresia with intact ventricular septum is morphologically heterogeneous, and the surgical outcome remains suboptimal compared to other complex congenital heart defects. We evaluated the long-term results for repair of pulmonary atresia with intact ventricular septum, Material and Method: Between January 1992 and June 2004, 38 patients underwent repair of pulmonary atresia with intact ventricular septum. The average age was 18 days $(2\~382\;days)$. The average Z-value of the tricuspid annulus diameter was -3.1$(-5.6\~0.8)$. Thirteen $(36\%)$ patients had right ventricle-to-coronary artery fistulas, and $4(11\%)$ patients had right ventricle-dependent coronary circulation. Average follow-up was 55 months $(3\;months\~2.2\;years)$, Result: Twenty-four patients under-went initial right ventricle (RV) decompression and 14 patients underwent systemic-to-pulmonary arterial shunt only. The average size of the tricuspid annulus of the patients who underwent RV decompression was significantly larger than that of the patients who underwent systemic-to-pulmonary arierial shunt only (Z-value -2.2 vs. -4.8, p=0.000). There were $5(13\%)$ early and 1 late deaths. Early deaths occurred in 3 patients who had undergone RV decompression, and in 2 patients who had undergone systemic-to-pulmonary arterial shunt only (p=1.0). Biventricular repair was achieved in $12(32\%)$ patients, single ventricular repair in $8(21\%)$, and one and a half ventricular repair in $4(l1\%)$ patients. Nine $(24\%)$ patients are waiting for the definitive repair. Kaplan-Meier survival at 5 and 8 years was $83.2\%$, respectively. Conclusion: Most of the deaths occurred after the initial palliation. Overall long-term survival was satisfactory. Early mortality should be reduced with careful preoperative evaluation and proper surgical strategy.

  • Near infrared reflectance spectroscopy (NIRS) was used to develop a rapid and nondestructive method for the determination of fatty acid composition in sesame (Sesamum indicum L.) seed oil. A total of ninety-three samples of intact seeds were scanned in the reflectance mode of a scanning monochromator, and reference values for fatty acid composition were measured by gas-liquid chromatography. Calibration equations were developed using modified partial least square regression with internal cross validation (n=63). The equations obtained had low standard errors of cross-validation and moderate $R^2$ (coefficient of determination in calibration). Prediction of an external validation set (n=30) showed significant correlation between reference values and NIRS estimated values based on the SEP (standard error of prediction), $r^2$ (coefficient of determination in prediction) and the ratio of standard deviation (SD) of reference data to SEP. The models developed in this study had relatively higher values (more than 2.0) of SD/SEP(C) for oleic and linoleic acid, having good correlation between reference and NIRS estimate. The results indicated that NIRS, a nondestructive screening method could be used to rapidly determine fatty acid composition in sesame seeds in the breeding programs for high quality sesame oil.

  • In this paper, a structural damage identification method (SDIM) is developed to identify the line crack-like directional damages generated within a cylindrical shell. First, the equations of motion for a damaged cylindrical shell are derived. Based on a theory of continuum damage mechanics, a small material volume containing a directional damage is represented by the effective orthotropic elastic stiffness, which is dependent of the size and the orientation of the damage with respect to the global coordinates. The present SDIM is then derived from the frequency response function (FRF) directly solved from the equations of motion of a damaged shell. In contrast with most existing SDIMs which require the modal parameters measured in both intact and damaged states, the present SDIM may require only the FRF-data measured at damaged state. By virtue of utilizing FRF-data, one may choose as many sets of excitation frequency and FRF measurement point as needed to acquire a sufficient number of equations for damage identification analysis. The numerically simulated damage identification tests are conducted to study the feasibility of the present SDIM.

  • The object of this study is to offer the standarized data for the design and calculating engineering cost of the rock excavation an the construction of the 3rd and 4th Seoul Subway lines From Jnauary to March in 1983, this study was carried out by the both methods of the field and laboratary studies. In the field, the geological survey in the entire area of Seoul City and sites on the subway lines were carried out and also a site measure of uniaxial compressional strength of rock masses by using Schmidt hammer was done. The labartory studies were carsied out by a study of preuions surveyes, microscopic studies of the mineral composition and degree of weathering of rocks, and measure of uniaxial compressional strengths Finally an engineering classification of each rock masses of South Africa council for Scientific and Industrial Research, CSIR, after Bieniawski, 1974. was done. In this method of classification 6 parameters such as strength of intact rock material, rock quality designation, spacing of fractures, condition of fractures, groundwater conditions, and the effect of fracture strike and dip orientation in tunnelling were used to evaluate rating of each rock mass.

  • Chondrosarcoma is a very uncommon malignant primary bone tumor, especially, it occurs extremely rare in the spine. A 52-year-old man was admitted to the emergency room with sudden paraplegia. Twelve hours prior to a paraplegic event, he visited an outpatient clinic with discomfort and tenderness around the medial border of the right scapular, and his neurologic status was absolutely intact. Magnetic resonance imaging showed a lobulated soft tissue mass from T3 to T5, which extended to the epidural space. Computed tomography scans showed soft tissue mass on the spinal posterior arch and osteolytic change of the adjacent bony structures. Emergent surgery was performed and the lesion was removed. Dark reddish blood and gel-like material were encountered around the dura and posterior arch during the operation. Multiple pulmonary nodules were found on a chest CT scan and a biopsy of one of them had been proven to be a metastasis of chondrosarcoma. The histologic examination showed dedifferentiated chondrosarcoma. The patient's neurologic deficit was improved slowly from ASIA A to ASIA D. Chondrosarcoma in the spine is extremely rare, even more with acute hemorrhage and sudden expansion into the epidural space. We named it chondrosarcoma apoplexy. We should consider the possibility of a hemorrhagic event when the patient's neurologic deficit worsens suddenly with spinal bone tumor.

  • An 1-year-old, intact female Yorkshire Terrier weighing 1.83 kg was referred for vomiting and anorexia. Survey abdominal radiographs revealed marked gastric distention. Ultrasonography showed a hyperechoic material with acoustic shadowing near the pylorus. A foreign body and ulceration were identified during gastroscopic examination. After endoscopic retrieval of gastric foreign body, unexpectedly abdominal distention was developed. Abdominal radiographs revealed pneumoperitoneum, but no leakage of contrast media was observed in gastrointestinal contrast study. Three days after hospitalization with supportive care, no abnormal findings were detected, and then the dog was discharged. This case describes the development of peumoperitoneum during endoscopic retrieval of gastric foreign body.

  • Waste fills resulting from coal mining should consist of large, free-draining sedimentary rocks fragments. The successful performance of these fills is related to the strength and durability of the individual rock fragments. When fills are made of shale fragments, some fragments will be durable and some will degrade into soil particles resulting from slaking and inter-particle point loads. The degraded material fills the voids between the intact fragments, and results in settlement. A laboratory program with point load and slake durability tests as well as thin section examination of sixty-eight shale samples from the Appalachian region of the United States revealed that pore micro-geometry has a major influence on degradation. Under saturated and unsaturated conditions, the shales absorb water, and the air in their pores is compressed, breaking the shales. This breakage was more pronounced in shales with smooth pore boundaries and having a diameter equal to or smaller than 0.060 mm. If the pore walls were rough, the air-pressure breaking mechanism was not effective. However, pore roughness (measured by the fractal dimension) had a detrimental effect on point load resistance. This study indicated that the optimum shales to resist both slaking as well as point loads are those that have pores with a fractal dimension equal to 1.425 and a diameter equal to or smaller than 0.06 mm.

  • Retroperitoneal teratoma was described in a 20-month-old intact female ferret (Mustela putorius furo). Retroperitoneal mass about $8{\times}5$ cm in size was surgically removed and histopathologic examination was performed. Grossly, on cross section of the mass, the consistency was soft to firm and contained several cystic structures which are filled with dried keratinous material. Histologically, the retroperitoneal mass consisted of embryologically heterogeneous tissues that include skin, bone and cartilage, adipose tissue, respiratory epithelium, and exocrine pancreatic tissue. Based on the characteristic histologic features of the mass, a diagnosis of retroperitoneal teratoma was made. Adrenal gland or ovary was suspected as the origin of the tumor.

  • APAL (Aramid Patched ALuminum alloy) was manufactured, which was a material that was consisting of a A12024-T3 aluminum alloy plate bonded to single-side of it with aramid/epoxy laminates. The aramid/epoxy laminates were bonded to it in condition of 1, 2 ply and fiber orientation of .+-.45, 0.deg./90.deg. Fatigue crack propagation tests were performed at stress ratio R-0.2, 0.5 with Al 2024-T3, APAL 45-1P, APAL 0/90-1P, APAL 45-2P, APAL 0/90-2P specimens to examine behavior of retardation in fatigue crack propagation. All the APAL specimens showed superior fatigue crack resistance. Number of cycle spended for crack to propagate from $a_{M}$=37 to $a_{M}$=65 mm in case of APAL 0/90-2P specimen was half that of Al 2024-T3 specimen. Fatigue crack propagation rate of APAL 0/90 specimens were retarded more compared to APAL 45 specimens and the amounts of retardation at R=0.5 were larger than that at R=0.2. It was found that the retardation in fatigue crack propagation was caused by intact fibers in the wake of crack.ack.

  • We present reconstruction of a complicated scalp-dura defect using acellular human dermis and latissimus dorsi myocutaneous free flap. A 62-year-old female had previously undergone decompressive craniectomy for intracranial hemorrhage. The cranial bone flap was cryopreserved and restored to the original location subsequently, but necessitated removal for a methicillin-resistant Staphylococcal infection. However, the infectious nidus remained in a dermal substitute that was left over the cerebrum. Upon re-exploration, this material was removed, and frank pus was observed in the deep space just over the arachnoid layer. This was carefully irrigated, and the dural defect was closed with acellular dermal matrix in a watertight manner. The remaining scalp defect was covered using a free latissimus dorsi flap with anastomosis between the thoracodorsal and deep temporal arteries. The wound healed well without complications, and the scalp remained intact without any evidence of cerebrospinal fluid leak or continued infection.
