Objective : For relief of back pain related to spondylolisthesis, fusion with or without internal fixation may be necessary. Currently, bilateral wide posterior decompression and fusion with or without internal fixation are used for treatment of spondylolisthesis. In case of severe osteoporosis, discectomy may be done without fusion to decompress the nerve roots. However, the procedure may aggravate radicular symptom because slippage may increase and thus foramen may become narrower. In these settings, inferomedial partial pediculectomy has been done in our institution, The object of this study was to evaltuate such cases with regard to per- and postoperative clinical courses. Patients and Methods : Fifteen patients complaining radicular pain with mild or no back pain underwent inferomedial partial pediculectomy. It was done in lytic spondylolisthesis after extension of isthmic defect. In degenerative spondylolisthesis of L4-5 interspace, intertransverse pediculectomy was done via paraspinal approach not to injure the posterior segment with intact isthmus. But, in degenerative spondylolisthesis of L5-S1 interspace however, partial pediculectomy was done after making artificial isthmic defect via midline approach. Then, reconstructive laminoplasty of L5 was performed to avoid iatrogenic instability resulted from artificial isthmic defect. They were followed up for average 14.4 months(3-31 months). Results : Radiating pain was relieved in all cases(100%). Postoperative result was excellent in 7 cases(46.7%), good in 7(46.7%) and fair in 1(6.6%). Only 1 case(6.7%) showed increase in slippage during follow-up period. Conclusion : Inferomedial partial pediculectomy is considered as a useful procedure for severely osteoporotic patients with spondylolisthesis with regards to keeping stability and decompressing the nerve roots.

Background: The organ preservation approach of choice for the treatment of locally advanced head and neck cancers is concurrent chemoradiation with three weekly high doses of cisplatin. Although this is an efficacious treatment policy, it has high acute systemic and mucosal toxicities, which lead to frequent treatment breaks and increased overall treatment time. Hence, the current study was undertaken to evaluate the efficacy of concurrent chemoradiation using 40 mg/m2 weekly cisplatin. Materials and Methods: This is a single institutional retrospective study including the data of 266 locally advanced head and neck cancer patients who were treated with concurrent chemoradiation using 40 mg/m2 weekly cisplatin from January 2012 to January 2014. A p-value of < 0.05 was taken to be significant statistically for all purposes in the study. Results: The mean age of the study patients was 48.8 years. Some 36.1% of the patients had oral cavity primary tumors. The mean overall treatment time was 57.2 days. With a mean follow up of 15.2 months for all study patients and 17.5 months for survivors, 3 year local control, locoregional control and disease free survival were seen in 62.8%, 42.8% and 42.1% of the study patients. Primary tumor site, nodal stage of disease, AJCC stage of the disease and number of cycles of weekly cisplatin demonstrated statistically significant correlations with 3 year local control, locoregional control and disease free survival. Conclusions: Concurrent chemoradiotherapy with moderate dose weekly cisplatin is an efficacious treatment regime for locally advanced head and neck cancers with tolerable toxicity which can be used in developing countries with limited resources.
Objectives: Screening for second primary cancer (SPC) is one of the key components of cancer survivorship care. The aim of the present study was to explore oncologists' experience with promoting second primary cancer screening. Methods: Two focus group interviews were conducted with 12 oncologists of diverse backgrounds. Recurrent issues were identified and placed into thematic categories. Results: Most of the oncologists did not consider SPC screening promotion as their responsibility and did not cover it in routine care. All of the study participants had experience with unexpected SPC cases, and they were under emotional tress. There was no systematic manner of providing SPC screening. Oncologists usually prescribe SPC screening in response to patients' requests, and there was no active promotion of SPC screening. Short consultation time, limited knowledge about cancer screening, no established guideline for SPC screening, and disagreement with patients about oncologists' roles were major barriers to its promotion. An institution-based shared care model was suggested as a potential solution for promoting SPC screening given current oncology practices in Korea. Conclusion: Oncologists could not effectively deal with the occurrence of SPC, and they were not actively promoting SPC screening. Lack of knowledge, limited health care resources, and no established guidelines were major barriers for promoting SPC screening to cancer survivors. More active involvement of oncologists and a systematic approach such as shared-care models would be necessary for promoting SPC screening considering increasing number of cancer survivors who are vulnerable.

Purpose: Early hypopharyngeal squamous cell carcinoma (HPSCC) is a rarely diagnosed disease, for which the optimal treatment has not been defined yet. We assessed patterns of failure and outcomes in early HPSCC treated with various therapeutic approaches to identify its optimal treatment. Materials and Methods: Thirty-six patients with stage I (n = 10) and II (n = 26) treated between January 1992 and March 2014 were reviewed. Patients received definitive radiotherapy (RT) (R group, n = 10), surgery only (S group, n = 19), or postoperative RT (PORT group, n = 7). All patients in both the R and PORT groups received elective bilateral neck irradiation. In the S group, 7 patients had ipsilateral and 8 had bilateral dissection, while 4 patients had no elective dissection. Results: At a median follow-up of 48 months, the 5-year locoregional control (LRC) rate was 65%. Six patients had local failure, 1 regional failure (RF), 3 combined locoregional failures, and 2 distant failures. There was no difference in 5-year LRC among the R, S, and PORT groups (p = 0.17). The presence with a pyriform sinus apex extension was a prognosticator related to LRC (p = 0.01) in the multivariate analysis. Patients with a bilaterally treated neck showed a trend toward a lower RF rate (p = 0.08). Conclusion: This study shows that patients with early stage HPSCC involving the pyriform sinus apex might need a tailored approach to improve LRC. Additionally, our study confirms elective neck treatment might have an efficacious role in regional control.
Objective : Aneurysms arising from the pericallosal artery (PA) are uncommon and challenging to treat. The aim of this study was to report our experiences of the endovascular treatment of ruptured PA aneurysms. Methods : From September 2003 to December 2013, 30 ruptured PA aneurysms in 30 patients were treated at our institution via an endovascular approach. Procedural data, clinical and angiographic results were retrospectively reviewed. Results : Regarding immediate angiographic control, complete occlusion was achieved in 21 (70.0%) patients and near-complete occlusion in 9 (30.0%). Eight procedure-related complications occurred, including intraprocedural rupture and early rebleeding in three each, and thromboembolic event in two. At last follow-up, 18 patients were independent with a modified Rankin Scale (mRS) score of 0-2, and the other 12 were either dependent or had expired (mRS score, 3-6). Adjacent hematoma was found to be associated with an increased risk of poor clinical outcome. Seventeen of 23 surviving patients underwent follow-up conventional angiography (mean, 16.5 months). Results showed stable occlusion in 14 (82.4%), minor recanalization in two (11.8%), and major recanalization, which required recoiling, in one (5.9%). Conclusion : Our experiences demonstrate that endovascular treatment for a ruptured PA aneurysms is both feasible and effective. However, periprocedural rebleedings were found to occur far more often (20.0%) than is generally suspected and to be associated with preoperative contrast retention. Analysis showed existing adjacent hematoma is predictive of a poor clinical outcome.

Objective : The authors reviewed their experiences of combined surgery (open door laminoplasty with unilateral screw fixation) for unstable multi-level cervical stenosis, to clarify the situation regarding the surgical approach most appropriate for the treatment of diffuse unstable multi-level cervical stenosis. Methods : From January 2011 to January 2012, combined surgery was performed for unstable multi-level cervical stenosis by one surgeon at our institution. The subjects of this study were 6 men of mean age 53.7 years (range, 48-71) with a mean follow-up of 9.3 (range, 3-14) months. All imaging studies showed severe multi-level cervical stenosis with spinal cord signal change, and instability or kyphotic deformity. A retrospective review of clinical, radiological, and surgical data was conducted. Results : Average laminoplasty level was 4.8 and the average screw fixation level was 5.0. Japanese Orthopedic Association score improved from an average of 5.2 to 11.2 points. According to Nurick's grades and Odom's criteria, symptom improvement was statistically significant. On the other hand, Cobb's angle changes were not significant. Average operation time was 5.86 hours with an average blood loss of 460 mL. No significant surgical complication was encountered. Conclusion : Despite the small cohort and the short follow-up duration, the present study demonstrates that laminoplasty with unilateral screw fixation is a safe and effective treatment for unstable multi-level cervical stenosis.

Objective : Transradial angiography has become popular among many cardiologists as a diagnostic and therapeutic tool. However, transradial cerebral angiography is not utilized to the same extent. The purpose of this study is to present our experience regarding the usefulness of transradial cerebral angiography, especially in elderly patients. Methods : Between May 2011 and February 2012, a total of 126 cerebral angiographies were performed via a transradial approach in a single center. Of them, only 47 patients were over 60 years old. In our institution, we shifted the initial access from the right femoral artery to the right radial artery in all patients requiring cerebral angiography in 2011. We did not attempt radial access in 40 cases for variable reasons. Results : The procedural success rate was 92.2%. We have four failures of transradial angiography; two because of loop formations of the radial and brachial artery and two due to multiple puncture failures. All supra-aortic vessels were successfully catheterized. However, the selective catheterization rates of the left side distal vessels were lower, as success rates were 89.7% for the right internal carotid artery and 75% for the left internal carotid artery. Procedure-related vascular complications, such as puncture site hematoma, hand ischemia, pseudoaneurysm, arteriovenous fistula and arterial dissection were not observed in our series. However, intraprocedural thrombosis developed in one patient, which was resolved completely by intraarterial thrombolytic agents. Conclusion : With advancing patient's age, we believe that transradial cerebral angiography is a useful tool to decrease patient's discomfort and more effectively manage the vessel tortuosity.

Background The prenatal ultrasound detection of cleft lip with or without cleft palate (CL/P) and its continuous management in the prenatal, perinatal, and postnatal periods using a multidisciplinary team approach can be beneficial for parents and their infants. In this report, we share our experiences with the prenatal detection of CL/P and the multidisciplinary management of this malformation in our institution's Congenital Disease Center. Methods The multidisciplinary team of the Congenital Disease Center for mothers of children with CL/P is composed of obstetricians, plastic and reconstructive surgeons, pediatricians, and psychiatrists. A total of 11 fetuses were diagnosed with CL/P from March 2009 to December 2013, and their mothers were referred to the Congenital Disease Center of our hospital. When CL/P is suspected in the prenatal ultrasound screening examination, the pregnant woman is referred to our center for further evaluation. Results The abortion rate was 28% (3/11). The concordance rate of the sonographic and final diagnoses was 100%. Ten women (91%) reported that they were satisfied with the multidisciplinary management in our center. Conclusions Although a child with a birth defect is unlikely to be received well, the women whose fetuses were diagnosed with CL/P on prenatal ultrasound screening and who underwent multidisciplinary team management were more likely to decide to continue their pregnancy.

Japan has been tried to integrate Western planning theories into its parks and green systems in the past. However this effect has been regarded unsuccessful. One of reasons for this is that the green systems have been treated as an urban facility, instead as systems. Another reason has to deal with greenbelt's stationary and reservation characteristics that make difficulty to respond timely to the rapid urbanization. The parks and green systems in Korea was introduced in 1990 and began to be promoted fully with the 'Parte and Green Master Plan' established in 2005. However, due to its short history, incomplete aspects exist in its philosophy and methods for promoting the system. Based on these experiences, the current green zone plans in Japan considers the green structure as an independent institution and amenity plan, not a tool of urban planning, that can be a medium for realization of urban visions. And main characteristics of this approach are 1) classifying the green structure into four categories in order to respond to urban forms and purposes, 2) pursuing confirmity with urban planning, and 3) executing the landscape plan based upon core green areas. From reviews upon experiences of Japan as well as considerations on demands and conditions of green systems in Korea, we proposed the following five new strategies for the construction of the urban parte and green system in Korea. They are 1) to develop urban parks and green systems into a genre of urban development improving the quality and local competitiveness of cities rather than a means of urban planning; 2) to form the integrated structure of urban green zones; 3) to transform the urban parte and green systems into practical systems as the centers of localities; 4) to raise criteria for planning and execution plans for effective implementations of the urban parte and green systems, and 5) to provide specific guidelines for the construction of urban parks and green systems through organizing research groups or specialists.
The rental housing has played a key role to supply a decent living space for the low-income households. The owner of the rental housing should maintain and manage the building physical condition. On the other hand, rents should use the housing without any damage and pay a certain fare for the rent. A rent is classified into two types : private and public. The public rent is mainly to supply a living space for non-available home owner with a low-rent fare. Many of public rent are built and supplied by the public institution or local government. The supplier would take a responsibility to maintain the building and components, reflected by the maintenance plan and repair scope. In this paper, it aimed at providing the repair time in building components of the public rental housing such as lighting, electrical cable, paintings and etc.. The repair time is analyzed with three calculation methods which are solved by the probability and empirical approach. Results are as follows : First, the repair time of the electrical facilities are maintained with 11yr, 10yr and 7 in permanent, public and redevelopment rent respectively. The roof proof has a repair time with 14yr, 11yr and 8 in permanent, public and redevelopment rent housing respectively. Second, Most of the components has a prior length of the repair time in permanent, public and redevelopment rent sequently. There is a difference in repair time according to the rental types. Therefore, it would continue to research the difference in aspect of the living style, building physics, living consciousness and etc.
