소방공무원의 근골격계 질환: 2011년~2013년 공상신청 분석 (Muscuoloskeletal Disorders of Korean Fire Fighters: Applicants for Public Worker's Compensation from 2011 to 2013)

  • 본 연구의 목적은 전국 소방서에 근무하는 소방공무원 중 2011년부터 2013년까지 근골격계 질환으로 공상을 신청 한 510명을 대상으로 자료를 분석하여 그 특성에 대해 알아보고, 공상 예방을 위한 대책을 제시하는 것이다. 2011년 1월부터 2013년 12월까지 공상을 신청한 1596명의 공상신청자의 정보를 국민안전처로부터 제공받아 주병명을 기준으로 근골격계 질환을 판별하여 이 중 510명을 최종 연구대상으로 하였다. 공상신청자는 남성이 92%로 여성보다 월등히 많았고, 공상발생 연령은 40대 초반이 가장 많았다. 공상이 발생한 시기는 5월과 6월이 가장 높았다. 지역별로 공상신청이 가장 많은 지역은 서울이었고 공상 발생만인율이 가장 높은 곳은 전북이었다. 공상신청이 가장 잦은 부위는 허리가 제일 많았고 무릎이 그다음 이었다. 신체구조는 인대가 가장 많고 추간판과 뼈 순이었다. 공상발생 당시 직무는 화재진압 38.3%, 구급 37.7%, 구조 12.4%, 기타 순이었다. 앞으로 지역 간 공상신청 빈도차이를 좀 더 세밀히 조사할 필요가 있고, 소방 직부별 부상기전과 예방방안에 대한 연구가 필요하다.

어린이(0-12세) 손상환자의 재원일수에 미치는 요인분석: 퇴원손상심층자료를 중심으로 (Analysis of Factors Affecting the Length of Stay in Children(Aged 0 to 12) with Injuries: Centering Around the Data from the Korea National Hospital Discharge In-Depth Injury Surveys)

  • 이 연구는 어린이(0-12세)손상 환자의 특성에 따른 재원일수의 관계를 규명하여, 어린이손상 환자 재원일수에 미치는 요인을 분석하고자 시행하였다. 조사대상은 퇴원손상심층조사 자료에서 2016년 1월1일부터 2020년 12월31일 까지 퇴원한 환자 중 0-12세 이하이며, 주진단이 손상 및 사고의 외인(S00-T98)인 환자 7,804명이었다. 분석은 빈도 분석 및 독립표본 t-검정, ANOVA 검정을 실시하였다. 재원일수에 영향을 미치는 요인을 알아보고자 회귀분석을 시행하였다. 연구대상자의 평균재원일수는 5.5일 이었다. 학령기(7-12세)기 아동이 학령전기(0-6세)보다 재원일수가 길었으며, 진료비 지불방법이 의료급여 및 기타보험 환자가 재원일수가 길었다. 의료기관 소재지가 수도권보다 비수도권(전라도, 경상도)일 때, 병상규모가 100-299병상인 경우 재원일수가 길었다. 내원경로는 외래인 경우 재원일수가 짧았고, 손상기전이 운수사고, 화상인 경우 재원일수가 길었다. 부진단이 있고, 주수술을 시행한 경우 재원일수가 길었으며, 퇴원후 방향이 사망인 경우 재원일수가 길게 나타났다. 이 연구는 전국 단위 자료인 퇴원손상심층조사 자료를 이용하여 우리나라 어린이 손상환자의 재원일수의 특성 및 결정요인을 파악하였다는 점에서 의의를 갖는다. 또한, 안전교육 및 예방 활동만으로도 발생을 줄일 수 있는 어린이 손상의 관련 요인 대한 적극적인 치료와 다양한 예방정책 마련을 위한 기초 자료를 제공했다는 점에서 의의가 있다.

소규모 사업장 산업보건인력의 업무수행 분석 (Facilitating Factors of and Barriers to Performance Improvement of Small Scale Enterprise Occupational Health Personnel in Korea)

  • In Korea, based on the Revised Law of Occupational Safety and Health a new entity of institution was set up in 1990 to provide occupational health services to SSE in which three sorts of personnel as a team have to be involved. These institutions, in charge of scores to hundreds of workplace area-wide, have been providing occupational health services without payment from employers or employees, and government reimburses through the Occupational Injury Prevention Insurance since 1993. As a service provider, a team is composed of doctors, nurses and industrial hygienists. Undergraduate and postgraduate educations for the SSE occupational health are not specified and the question on the performance of the personnel has been raised. This study was designed to analyze the facilitating factors of and barriers to the performance and its improvement of these personnel. In 1997, the survey was conducted with all 58 institutions. Structured questionnaires were mailed to 200 personnel who were providing the occupational health service for SSE. The response rate was 51.7% for doctors, 58.6% for nurses, and 60.3% for industrial hygienist, respectively. Results are as follows : 1) There is a guideline for occupational service mandated by the government. Under the guideline, the minimum frequency of visiting workplace is assured with six times of doctors, 17 times for nurse and industrial hygienist in a year. There are one doctor for every 200 factories, one nurse and one hygienist for every 100 factories. 2) All respondents have basic qualification for occupational health service. About 16.7%. of doctors are certified in industrial medicine or preventive medicine, and 64.7% of industrial hygienists had first grade certification. Totally 66.7% of personnel have been involved in occupational health for more than one year. 3) As a support system for the performance improvement, 66.3% respond that they have been provided with educational materials, advice related to industrial environment and guidance of MSDS from Korea Industrial Safety Corporation. Most respondents indicate the lack of concern of employers and employees as a main barrier to the improvement of the service. Also they are in the need of the training opportunity more focused on SSE. The Governments policy for SSE is a principal facilitating factor. Training program focused on SSE situation, manpower, technical support, etc. are areas to be improved to have a better occupational health service for SSE in Korea.

구급대원으로 활동하는 응급구조사의 표준작전절차에 대한 융복합적인 인지도와 수행도 (Convergent Awareness and Performance of the Standard Operating Procedure in Emergency Medical Technicians Serving as Paramedics)

  • 본 연구는 구급대원으로 활동하는 응급구조사의 표준작전절차에 대한 융복합적인 인지도와 수행도를 알고자 하는데 그 목적이 있다. 이러한 목적을 달성하기 위하여 본 연구에서는 구급대원으로 활동하는 응급구조사 400명을 연구대상으로 표집하여 SPSS 20.0 Version을 사용하여 분석하였다. 연구 결과는 다음과 같다. 첫째, 연구대상자의 융복합적인 인지도는 호흡기 감염예방 부분이 가장 높았고, 손위생이 가장 낮았으며, 수행도는 치료장비 및 직원의 안전 부분이 가장 높았고, 손위생이 가장 낮았다. 인지도가 전체적으로 수행도 보다 높았다. 둘째, 일반적 특성에 따라 인지도는 연령, 성별, 상처 경험에 따라 통계적으로 유의한 차이가 있었으며, 수행도는 근무경력만이 유의한 차이를 보였다. 응급구조사의 융복합적인 인지도와 수행도 간에 정적 상관관계를 보이는 것으로 나타났다. 이 같은 결과로 구급대원으로 활동하는 응급구조사의 표준작전절차에 관한 인지도와 수행도를 복합적으로 높이기 위해 인지도를 선행으로 한 프로그램을 지속적으로 반복 교육 시 수행도를 증가시킬 수 있을 것으로 사료된다.

주거공간의 부엌 환경 특성에 대한 뇌졸중 환자의 선호 (Preferences of Stroke Patients for Kitchen Environment in Residential Space)

  • 목적 : 뇌졸중 환자를 대상으로 부엌 환경에 대한 개선 요구도, 부엌 디자인 선호도, 부엌에 필요한 물품 선호를 알아보고자 하였다. 연구방법 : 부산 지역의 병원에서 재활치료를 받고 있는 뇌졸중 환자 97명을 대상으로 2014년 2월 24일부터 3월 13일까지 설문조사를 실시하여 빈도분석을 하였다. 결론 : 부엌환경에 대한 개선 요구에서 작업할 수 있는 의자 사용, 미끄럼방지 타일의 설치에 대한 요구가 높았다. 부엌 디자인 선호 조사에서는 미끄럼방지 매트, 난방설비의 보일러, 서랍식 수납, L자형 안전손잡이, 높낮이 조절이 되는 싱크대, 가스누출 자동차단기, 터치식 수도꼭지, ㄷ자형 주방, 환기시설은 천장형 후드를 선호하는 것으로 나타났다. 부엌에 필요한 물품 선호에서는 내려오는 싱크대 선반, 채 썰기 가위, 손 다침 방지 칼을 선호하였다. 결론 : 뇌졸중 환자의 부엌환경에 대한 선호를 알 수 있었으며 이는 향후 뇌졸중 환자의 부엌 사용의 자립도, 낙상 예방 및 주방용품의 안전성과 편리성이 고려된 부엌 설계에 도움이 될 것으로 보인다.

초등학생의 보건관리 만족도와 태도에 관한 연구 - 양호교사 유무를 중심으로 - (A Study on the Health Care Satisfaction and Attitude of Elementary School Students - by the presence or absence of nurse teacher -)

  • The purpose of this study was to serve as a basis for school health care of better quality, by making a comparative analysis of the health care satisfaction and attitude of elementary school students in consideration of their general characteristics and the presence or absence of nurse teacher. The subjects in this study were 919 selected six graders in 16 elementary schools in the city of Tongduchun, Koyang and Euijungbu, Yangju-kun and Yeunchun-kun. A survey was conducted with questionnaire designed for measurement of health care satisfaction and attitude. As a result of analyzing the data collected from June 1 through 15, 2000, the conclusions were as follows. 1) As for the general characteristics of the students investigated, the subjects included 513 boys(55.8%) and 406 girls(44.2%). The schools where 390(42.4%) students attended were located in municipal area, and the schools where 529(57.6%) students attended were located in kun area. 608(66.2%) students had a nurse teacher at their schools, while 311(33.8%) students had no nurse teacher. 498(54.2%) had an experience to use the health room this year, but 421(45.8%) had no such an experience. Their mean school life satisfaction was scored $3.42{\pm}.71$, above the average. And their health condition was rated $3.81{\pm}.87$, which implied they tended to be in good health. 2) The mean satisfaction at the health room operation was scored $3.33{\pm}.71$, above the medium level. What they were most satisfied with($4.02{\pm}1.08$) was, among the health room facilities, that there were beds. But they expressed the least satisfaction($2.83{\pm}1.17$) at the location of health room. The presence or absence of nurse teacher made a significant difference to their satisfactionat health room operation, because the students in schools with nurse teacher showed greater satisfaction($3.42{\pm}.72$) than the others in schools with no nurse teacher did($3.15{\pm}.66$). 3) Concerning their attitude to use the health room in case of disease or accident occurrence, a lot of students in schools with a nurse teacher, who had ever suffered from indigestion, headache or traumatic injury, used the health room. In schools with no nurse teacher, there was a tendency to talk to their class teachers(p<.001). The recognition of the necessity for health counseling was generally on a medium level. The counselor whom they wanted to discuss health problem with was family or friend in the largest cases. Few students discussed with class teachers in case there was a nurse teacher in school. Instead, some of them discussed with friend, family or nurse teacher, and there was a significant difference between them(p<.001). 4) The mean satisfaction at health, sanitation and environmental management was rated $3.20{\pm}.90$, above the average. The classroom lighting gave them the best satisfaction with $3.67{\pm}1.07$, but the satisfaction at toilet cleanness and disinfection was not good with $2.83{\pm}1.19$. By the presence or absence of nurse teacher, those who had a nurse teacher expressed better satisfaction at water supply facilities including hot water than the others who had no nurse teacher did(p<.001). But no significant difference was observed in the other items. 5) The health education satisfaction was rated $3.19{\pm}.99$, which was on a medium level. By item, the mean satisfaction level was $3.36{\pm}1.19$ at nurse teacher's explanation about treatment, $3.13{\pm}1.15$ at the frequency of health education, and $3.08{\pm}1.16$ at the explanation on the cause of disease. By the presence or absence of nurse teacher, the students with nurse teacher showed significantly better satisfaction at every factor0(p<.001). 6) Regarding health education attitude, their recognition of the need for school health education was scored $3.89{\pm}.96$. Those who had a nurse teacher felt it more necessary($3.96{\pm}.92$), yet the others who had no nurse teacher felt its necessity a little less($3.74{\pm}1.01$). The most preferred thing for them to learn in health education was first aid, followed by sex education, obesity prevention, safety accident prevention in school and outdoors, smoking-related health, good use of leisure time, and environmental pollution cause in the order named. According to the presence or absence of nurse teacher, there was a significant difference in sex education(p<.01), but no significant disparities were found in the other factors. The most preferred person who would offer health education was a lecturer from the outside(45.8%) and nurse teacher(45.4%). Their preference for class teacher as a person in charge of health education was just 8.8%. But the presence or absence of nurse teacher didn't produce any differences to their preference for a person in charge of health education.
