재제조에서 제품 해체 및 재조립 공정의 상해 위험성평가 (Injury Risk Analysis for Product Disassembly and Reassembly Process in Remanufacturing)

  • In this paper, we focused on the safety of workers in a remanufacturing process where a risk analysis is not carried out and suggested a criteria for evaluating injury risk. We analyzed a disassembly and a reassembly, which are important for the remanufacturing process. The disassembly includes the disassembly of product and the disassembly process of part and the reassembly includes only reassembly of part. First of all, we analyzed the remanufacturing process and a type of injury. Then, we reviewed the standards and determined the criteria for a severity and an occurrence. We set a bigger weight for the severity to allow the greater impact. And the injury risk score was defined as a sum of the weighted severity and the weighted occurrence. We conducted a qualitative analysis of the experience of field workers based on the criteria we set up. Questionnaires for the evaluation were formulated through interviews with experts.

교차로, 횡단보도, 터널 구간에서 사고유형에 따른 상대적 위험도 분석 (Analysis of Relative Risk by Accident Types at Intersections, Crosswalk and Tunnel Sections)

  • 본 연구는 교차로, 횡단보도, 터널 구간에서 교통사고 유형에 따른 위험 순위를 비교 분석한 것이다. 서울, 경기도에서 발생한 2014년부터 2017년까지의 교통사고자료 중 교통량 및 속도 자료와 결합 가능한 58,868건의 자료를 구축하고, 순서형 로짓모형을 활용하여 사고심각도 추정모형을 구축하였다. 추정된 모형을 기반으로 사고심각도에 영향을 미치는 요인을 규명하고, 영향을 미치는 정도를 살펴보았다. 또한 사고발생위치(교차로, 횡단보도, 터널)별, 사고유형별, 사고발생 시간이나 상황별로 상대적 위험도 분석을 통해 위험순위를 제시하였다. 분석 결과 사고발생 위치와 시간에 따라 사고심각도에 현격한 차이를 보이는 것으로 나타났으며, 분석 자료로부터 산출된 사고 발생 빈도와 사고심각도와 비교해봤을 때, 발생 빈도가 적더라도 사고심각도 측면에서는 위중하거나, 발생 빈도가 높고, 사고심각도 또한 위중한 경우가 존재하는 것으로 분석되었다. 이러한 교통사고유형의 위험 평가를 통해 도로별로 상대적으로 위험한 사고유형에 대한 이해가 가능하고 도로별, 사고유형의 교통사고 위험도 지수 개발에 기초자료 활용할 수 있을 것이다.

영유아 탑승자의 차량사고에서 보호장구에 따른 손상 분석 (Injury Analysis of Child Passenger According to the Types of Safety Restraint Systems in Motor Vehicle Crashes)

  • Purpose: To compare injury sustained and severity of child occupant according to the types of safety restraint systems in motor vehicle crashes. Methods: This was a retrospective observational study. The study subjects were child occupants under the age of 8 years who visited a local emergency center following a motor vehicle crash from 2010 to 2014. According to safety restraint: child restraint systems (CRS), belted, and unbelted, we compared injuries sustained and injury severity using the maximal Abbreviated Injury Scale (MAIS) and Injury Severity Score (ISS), and analyzed the characteristics of severe injuries (AIS2+). Results: Among 241 subjects, 9.1% were restrained in CRS, 14.5% were only belted, and 76.3% was unbelted at the time of the crashes. Fourteen had severe injuries (AIS2+), all of whom didn't be restrained by CRS. Injuries in face and neck were the highest in unbelted group, and MAIS and ISS were the lowest in CRS group. Conclusion: Among safety restraint systems for child occupant in motor vehicle crashes, the CRS have the preventive effect of face and neck injuries, and are the most effective safety restraint systems.

봉독약침과 오공약침이 좌골신경 손상 흰쥐의 기능 회복과 염증 억제에 미치는 영향 (Effect of Bee Venom Pharmacopuncture and Scolopendra Subspinipes Pharmacopuncture on Functional Recovery and Anti-inflammation after Sciatic Crushed Nerve Injury in Rats)

  • Objectives : Peripheral nerve injuries are commonly encountered clinical problems and often result in serve functional deficit. Bee venom and scolopendra subspinipes have been traditionally used in oriental medicine to treat several inflammatory diseases and chronic pain conditions. Methods : In the present study, the effects of bee venom pharmacopuncture and scolopendra subspinipes pharmacopuncture on functional recovery, severity of pain, and expressions of neurofilament, cycloxygenease-2(COX-2), and tumor necrosis factor-${\alpha}$(TNF-${\alpha}$) following sciatic crushed nerve injury in rats were investigated. For this study, walking tract analysis, plantar test, western blot for COX-2 and TNF-${\alpha}$, and immunohistochemistry for neurofilament were performed. Results : In the present results, sciatic functional index(SFI) in walking tract analysis was significantly decreased following sciatic crushed nerve injury, and pain severity in plantar test was significantly increased. COX-2 and TNF-${\alpha}$ expressions were increased whereas neurofilament expression was decreased by sciatic crushed nerve injury. On the other hand, bee venom pharmacopuncture and scolopendra subspinipes pharmacopuncture improved SFI in walking tract analysis and suppressed the pain severity in sciatic crushed nerve injury. Bee venom pharmacopuncture and scolopendra subspinipes pharmacopuncture suppressed COX-2 and TNF-${\alpha}$ expression and enhanced the neurofilament expression in sciatic crushed nerve injury. Conclusions : In the present study, we have shown that treatment with bee venom or scolopendra subspinipes is the effective therapeutic modality to ameliorate the symptoms of sciatic crushed nerve injury. The efficacies of bee venom and scolopendra subspinipes were similar.

융·복합 시대 스키 리조트의 안전사고와 상해유형의 차이 (Analysis of the Difference between Accident Type and Injury Type on Ski Resort)

  • 본 연구의 목적은 융 복합 시대 스키 리조트 안전사고(스키, 스노보더) 상해유형의 차이를 검증하여 보다 개선된 안전관리 방안마련에 기초자료로 제공하는데 있다. 이러한 연구목적을 달성하기 위하여 2016-2017 겨울시즌 경기도에 위치한 2개 스키장의 안전사고일지에 기록된 총522명을 유 목적 표집하여 연구대상으로 선정하였다. 수집된 자료는 SPSS 21.0을 이용하여 빈도분석, 교차분석을 실시하였으며, 그 결과는 다음과 같다. 첫째, 인구통계학적특성변인(강습유무)에 따른 부상부위에는 차이가 있는 것으로 나타났다. 둘째, 인구통계학적특성변인(종목, 강습유무, 종목${\times}$연령, 강습유무${\times}$성별)에 따른 부상형태에는 차이가 있는 것으로 나타났다. 셋째, 스키리조트 슬로프 수준에 따른 부상부위 및 부상형태에는 차이가 있는 것으로 나타났다.

정면충돌에서 노인운전자의 중증도에 영향을 주는 요인 분석 (An Analysis of Factors Affecting Severity of Elderly Driver in Frontal Collision)

  • 노인인구의 증가로 노인운전자의 손상과 사망자도 증가하였다. 하지만 노인운전자의 손상과 중증도에 대한 연구는 활발히 이루어지지 않아 영향 요인을 파악하지 못하고 있다. 본 연구에서는 정면충돌에서의 노인운전자에 손상과 중증도에 영향을 미치는 요인을 찾아 중증도 분류에 추가적으로 활용하고자 하였다. Collision Deformation Classification Code를 통해 차량 파손 정도를 확인하였으며 간편손상척도(Abbreviated Injury Scale, AIS)로 손상부위와 정도를, 손상중증도점수(Injury Severity Score, ISS)로 환자의 중증도를 확인하였다. 중증외상환자의 발생률은 5이상의 차량 파손 정도를 가진 대상자에서 Odds ratio가 7.381로 나타났으며 선형회귀분석을 통한 중증도 요인 분석에서도 차량 파손 정도의 ${\beta}$값이 0.453으로 나타났다. 따라서 5이상의 차량 파손 정도는 노인운전자에서 중증도 분류에 추가적으로 활용될 수 있는 기준으로 제안될 수 있다.

국내 남녀 중고 골프 선수들의 운동 상해 빈도 조사 (The Frequency of Sport Injury for the Junior Elite Golf Players in Korea)

  • The purpose of this study was to investigate the frequency of sports injury among Korean junior elite golf players. The survey was made through the questionnaire which consist of eight categories and 280 valid answers were used to perform frequency analysis. The results indicated that 197 out of 280 participants experienced golf injuries (30%: during a game, 70%: during a practice) and 70% of them also experienced injuries within a year. The frequency analysis for the site of injury has shown that back was the most vulnerable area for all players(33%), followed by the right wrist, left shoulder and elbow for male players. Among female players, left wrist was the second weak site of injury, followed by the right wrist, right thoracic spine, and left shoulder. A few of them also have shown abnormal symptoms of ruptured lumbar disk, scoliosis, and lordosis and these injuries might be caused by the lack of flexibility and the fatigue due to over-practice. High frequency of wrist injury also demonstrated that excessive chopping or punching shot and poor environmental course conditions may cause this injury to be common. Overall, lack of warm up time, lack of strength and flexibility, and excessive amount of practice were the major reasons of injury for the junior elite golfers in Korea. The study also proved that these young Korean golfers actually had more chances to be faced with the injury than average. Thus, it is necessary to develop systematic and scientific methods of training, and prevention, treatment of these injuries with medical professionals.

Management of High-grade Blunt Renal Trauma

  • Purpose: Blunt injury accounts for 80-95% of renal injury trauma in the United States. The majority of blunt renal injuries are low grade and 80-85% of these injuries can be managed conservatively. However, there is a debate on the management of patients with high-grade renal injury. We reviewed our experience of renal trauma at our trauma center to assess management strategy for high-grade blunt renal injury. Methods: We reviewed blunt renal injury cases admitted at a single trauma center between August 2007 and December 2015. Computed tomography (CT) scan was used to diagnose renal injuries and high-grade (according to the American Association for the Surgery of Trauma [AAST] organ injury scale III-V) renal injury patients were included in the analysis. Results: During the eight-year study period, there were 62 AAST grade III-V patients. 5 cases underwent nephrectomy and 57 underwent non-operative management (NOM). There was no difference in outcome between the operative group and the NOM group. In the NOM group, 24 cases underwent angioembolization with a 91% success rate. The Incidence of urological complications correlated with increasing grade. Conclusions: Conservative management of high-grade blunt renal injury was considered preferable to operative management, with an increased renal salvage rate. However, high-grade injuries have higher complication rates, and therefore, close observation is recommended after conservative management.

동승석 에어백 핵심 성능 인자 및 상해위험도 예측 기법 개발에 대한 연구 (A Study on the Key Performance Factors of Passenger Airbag and Injury Risk Prediction Technique Development)

  • Until now, passenger airbag design is based on the referred car design and many repetitive crash tests have been done to meet the crash performance. In this paper, it was suggested a new design process of passenger airbag. First, key performance factors were determined by analyzing the injury risk effectiveness of each performance factor. And it was made a relationship between injury risk and performance factor by using the response surface model. By using this one, it can be predicted the injury risk of head and neck. Predicted injury risk of optimal design was obtained through this injury risk prediction model and it was verified by FE analysis result within 18% error of head and 9% error of neck. It was shown that a target crash performance can be met by controlling the key performance factors only.

Effect of suprascapular nerve injury on muscle and regenerated enthesis in a rat rotator cuff tear model

  • Background: Massive rotator cuff tears (RCTs) are complicated by muscle atrophy, fibrosis, and intramuscular fatty degeneration, which are associated with postoperative tendon-to-bone healing failure and poor clinical outcomes. We evaluated muscle and enthesis changes in large tears with or without suprascapular nerve (SN) injury in a rat model. Methods: Sixty-two adult Sprague-Dawley rats were divided into SN injury (+) and SN injury (-) groups (n=31 each), comprising tendon (supraspinatus [SSP]/infraspinatus [ISP]) and nerve resection and tendon resection only cases, respectively. Muscle weight measurement, histological evaluation, and biomechanical testing were performed 4, 8, and 12 weeks postoperatively. Ultrastructural analysis with block face imaging was performed 8 weeks postoperatively. Results: SSP/ISP muscles in the SN injury (+) group appeared atrophic, with increased fatty tissue and decreased muscle weight, compared to those in the control and SN injury (-) groups. Immunoreactivity was only positive in the SN injury (+) group. Myofibril arrangement irregularity and mitochondrial swelling severity, along with number of fatty cells, were higher in the SN injury (+) group than in the SN injury (-) group. The bone-tendon junction enthesis was firm in the SN injury (-) group; this was atrophic and thinner in the SN injury (+) group, with decreased cell density and immature fibrocartilage. Mechanically, the tendon-bone insertion was significantly weaker in the SN injury (+) group than in the control and SN injury (+) groups. Conclusions: In clinical settings, SN injury may cause severe fatty changes and inhibition of postoperative tendon healing in large RCTs.
