A Case of Successful Recovery from High Dose Intravenous Nicorandil Infusion in Refractory Coronary Vasospasm with Hemodynamic Collapse

  • A 70-year-old male came to the emergency room of the authors' hospital because of sudden cardiac arrest due to inferior wall ST elevation myocardial infarction. His coronary angiography revealed multiple severe coronary spasms in his very long left anterior descending artery. After an injection of intracoronary nitroglycerine, his stenosis improved. The cardiac arrest relapsed, however, accompanied by ST elevation of the inferior leads, while the patient was on diltiazem and nitrate medication to prevent coronary spasm. Recovery was not achieved even with cardiac massage, intravenous injection of epinephrine and atropine, and intravenous infusion of nitroglycerine. The patient eventually recovered through high-dose nicorandil intravenous infusion without ST elevation of his inferior leads. Therefore, intravenous infusion of a high dose of nicorandil must be considered a treatment option for cardiac arrest caused by refractory coronary vasospasm.

The Effects of Management and Technical Capabilities in the Performance of Plug Seedling Production (육묘기술수준별 경영성과 분석과 경영지도 방향)

  • This study was carried out to identify the major characteristics of plug seedling farm's management. Thirty glasshouse farms were surveyed out of 42 farms that adopted plug seedling production technology in Korea. The major objectives of this survey were to determine the technical capability of the glasshouse farmers, and to analyze the significant differences in terms of production performance and management capabilities. The major results of the survey were as follows : 1) The production quantity of plug seedlings of the superior group was 43% higher than the inferior group. 2) The plug seedling loss rate of the superior roup was lower by 4% than the inferior group. 3) The income of superior group was 4.2 million Won per $1,000m^2$, while the income of the inferior group was only 0.45 million Won. 4) The cause of low production of grafted plug seedlings was primarily due to the lack of technical knowledge and skills. 5) The results indicated that the technical knowledge level and production skills of grafting plug seedling should be improved.

Anatomical Considerations of the Recurrent Laryngeal Nerve During Thyroidectomy (갑상선 수술중 반회 후두 신경의 해부학적 고찰)

  • This study reports a prospective analysis of anatomical variations of recurrent laryngeal nerves during 300 thyroidectomies. During thyroidectomies for variable thyroid diseases. the course of recurrent laryngeal nerve was completely isolated from root of neck to the inferior comus of thyroid cartilage. In left side, nerve(53.7%) predominantly ran posterior to the inferior thyroidal artery(p<0.05) but in right side there was no predominant pattern. There were three nonrecurrent laryngeal nerves in the right side. About half of the cases in both sides(51.2% in right, 50.5% in left side) had one or more branches before terminating at cricothyroidal muscles. The average length of branches from inferior comus of thyroid cartilage to the origination of individual branch were l2.0mm in right side and 13.3mm in left side. In right side, majority(50.7%) of nerves ran though paratracheal space but difference did not reach the statistical but in left side, majority(88.3%) ran through tracheoesophageal groove and it was the dominant pattern(p<0.01), the overall status of passages of the nerve were relatively straight in left side(straight 87.8%, oblique 52.1%).

  • Autologous breast reconstruction after mastectomy in breast cancer patient is now increasing. The deep inferior epigastric artery (DIEA) free flap is well known as an ideal donor site for the microsurgical breast reconstruction. The branching pattern of the DIEA was well described in the literature. In that study, DIEA has three branching patterns near the arcuate line. We describe a case in which branching variation of the DIEA before entering the posterior surface of the rectus abdominis muscle. In three cases, DIEA originated from the external iliac artery ascended as a double trunk at 1cm, 2cm, and 4cm above the originating point, respectively. In one case, DIEA ascended as a single trunk along the linea alba toward to the umbilicus until it supply overlying subcutaneous tissue. Preoperative 3D computed tomographic angiography to identify the anomaly is recommended and meticulous dissection to the originating point of DIEA is needed.

Periareolar Dual Plane Augmentation Mammaplasty (유륜절개 이중평면 유방확대술)

  • Although several reports have been introduced about dual plane augmentation mammaplasty, the description of periareolar approach dual plane augmentation mammaplasty was few. This article describes specific characteristics, and different classification and techniques for the periareolar dual plane breast augmentation while postoperative scars resulted from inframammary crease approach caused complaints. A total of 124 patients(248 breasts) had periareolar dual plane augmentation surgery from 1998 to 2004. Anatomic implants were used in 43 cases. Most of the patients were satisfied with the outcomes of periareolar dual plane augmentation. Periareolar dual plane augmentation mammaplasty adjusts implant and tissue relationships to ensure adequate soft-tissue coverage while optimizing implant-breast parenchymal dynamics to offer increased benefits and fewer faults compared to a single pocket location in a wide range of breast types with minimal scars. Two types of dual plane classifications are discussed in this study for the periareolar approach exclusively. The boundaries of retroglandular dissection remain constant, as the costal origin of pectoralis major are divided. Type A dual plane implies that the inferior edge of pectoralis muscle lies below the inferior areolar border, and type B dual plane implies that the inferior edge lies above the superior areolar border.

  • Nosocomial infection and community-acquired infection with Staphylococcus aureus, especially methicillin-resistant S. aureus (MRSA), has become a strong concern in human body sites and related effects. The aim of this study is investigate the isolation rate of MRSA from nasal cavity inferior regions and cellular phones to assess the risk factor of nosocomial infection and community-acquired infection. 34.7% and 37.2% isolates were MRSA from the nasal cavity inferior regions and cellular phones according to a Mannitol salt agar (added oxacillin $6{\mu}g/mL$) culture and PCR according to S. aureus specific 16S rRNA and mecA primers. Thus, the distribution of S. aureus and the isolation rate of MRSA represent a very high risk factor regards nosocomial infection and community-acquired infection.

  • A 34-year-old man was admitted to the hospital because of ascites, abdominal fullness. computed tomography and cavography revealed inferior vena cavil occlusion just above the hepatic vein and diagnosed as Budd-Chiari syndrome. conservative medical therapy failed to control the symptoms produced from both portal hypertension and versa caval stasis. Therefore, under extracorporeal circulation with moderate hypothermia and normal cardiac contraction, membranoto y and inferior vena casa venoplasty with Gore-tex (10mm) was performed. Postoperatively, physical examination revealed oral ulceration, subcutaneous thrombophlebitis, folliculitic lesions. uveitis And increased reactivity of the skin to needle punctures. 10 month later, superior vena ciiva obstruction symptom was found. Hehcet's disease was diagnosed.

Altered Fine Structure of Amylopectin Is Induced by Exogenous Gibberellin During Rice Grain Ripening

  • When $GA_{4}$ was applied to heading stage, it was examined to understand the change of plant hormones and starch during grain filling and ripening. Exogenous gibberellin caused a dramatic decrease in endogenous ABA content. Endogenous $GA_{4}$ content in both superior and inferior part was more promoted in $GA_{4}-treated$ rice grain than in the control. $GA_{1}$ content of an inferior part was not detected in the control and $GA_{4}-treated$ rice otherwise $GA_{4}$ was detected in all grain parts. Ripened grain rate in $GA_{4}-treated$ rice grain was lower than that of the control plant. Amylopectin from $GA_{4}-treated$ grain contained more very short chains with degree of polymerization (DP) between 4 and 8 than amylopectin from the control plant. It suggests strongly that fine structure of rice endosperm may be changed by exogenously applied $GA_{4}$ in rice plants.

Perforator Reconstruction to Salvage the Jeopardized Flaps

  • During flap elevation, most perforators are cut except one or more perforators that are essential to flap survival. However these cutout perforators can cause deterioration of the blood circulation of the flap. To salvage the jeopardized flaps, rebuilding the perforator system is essential for flap survival. In the first case, after flap elevation, the upper abdominal flap margin was severely ischemic. To supply blood to the upper abdominal flaps, we found and used a major perforator underneath the upper abdominal flap which was cut earlier during the elevation, and we performed reanastomosis with ipsilateral deep inferior epigastric artery. Upper abdominal flap ischemic area was limited to a narrow suture area. In the second case, we performed free superficial inferior epigastric artery (SIEA) flap reconstruction. After successful anastomosis of the SIEA and superficial inferior epigastric vein (SIEV) with internal mammary artery and vein, serious venous congestion occurred immediately because of SIEV malfunction. We found the largest perforator vein under the flap, as an alternate way to drain, then connected it with the thoracoacromial vein with a vein graft harvested in the contralateral SIEV. Circulation has improved. In conclusion, perforator system reconstruction is essential in a jeopardized flap salvage.

A PERIPHERAL AND CENTRAL NEURILEMMOMA OF THE LOWER JAW (하악에 발생한 주변성 및 중심성 신경초종의 치험례)

  • Intraosseous neurilemmoma(Schwannoma) is an extremely rare benign neoplasm. The site most commonly involved is the mandible. This occurrence is understandable because of the length of the inferior alveolar canal through the mandible. No other bone contains a canal that transmits a neurovascular bundle of such size and length. We report on a peripheral and central neurilemmoma along pathway of inferior alveolar nerve of the lower lip and mandible in a 28-year old man. A panoramic radiograph of the mandible showed a well-defined bilocular lesion with a thin uniform sclerotic margin located in the ramus and body of the mandible. The CT scan confirmed a well-defined lesion with thinning of the cortex of the body of the left side of the mandible. Histologically, the lesion was a cellular neoplasm with distinct palisading and numerous Verocay bodies. Complete excision was achieved by removing the tumor with the inferior alveolar nerve.
