일부지역 치과의 기관, 감염관리자, 환자의 감염관리에 관한 연구 (The Study on Organization, Infection Controller, Patient Infection Control of Dental Clinic in Certain Areas)

  • 치과의료기관의 감염관리 수행 조사를 위해 2014년 6~7월 기간 동안 일부 지역사회 치과위생사 158명을 대상으로 기관, 감염관리자, 환자의 감염관리 수행을 자기기입식 설문조사하였다. 치과병원은 감염관리지침서 구비, 감염관리 담당자 지정, 정기감염관리 교육지원, 1인 멸균핸드피스 사용 비율은 57~74%였으나 치과의원은 13~20% 수준이었다(p<0.05). 감염관리자의 감염관리지침 인지는 치과병원 17.4%, 치과의원 5.9%였고 정기예방접종 시행은 치과병원 60.9%, 치과의원 34.1%였다(p<0.05). 환자의 손 위생 수행, 진료 전 칫솔질, 교차 감염 교육경험은 20% 미만이었고, 병력조사 협조는 90% 이상이었다(p<0.05). 감염관리 실천 상관분석 결과 기관 지원은 치과위생사 감염관리자실천, 환자감염관리협조, 개인방호실천, 진료실감염관리실천, 폐기물관리실천과 양의 상관관계를 나타내었으며 통계적으로 유의하게 분석되었다(p<0.05). 감염관리 실천을 위해 의료진, 환자, 기관의 지원과 협조가 요구되며 감염관리 관리자는 지속적인 교육을 통해 감염관리지침을 인지하고 실천할 것이 요구된다.

집중치료실 및 수술장 개선이 개심술후 흉골감염에 미치는 영향 (The Effect of Improved Operating Room and Intensive Care Unit on the Sternal Infection After Open Heart Surgery)

  • 배경: 개심술후 발생하는 흉골감염은 심각한 합병증일 뿐만 아니라 술후 사망률을 높이는 한 요인이 된다. 저자들은 1999년 7월 집중치료실 및 수술장 이전을 계기로 이를 전후로한 환자들의 흉골감염 발생현황을 비교 분석함으로써 집중치료실 및 수술장 개선이 흉골감염에 미치는 영향을 알고자 하였다. 대상 및 방법: 집중치료실 및 수술장 이전 시점을 전후로 하여 1997년 1월 1일부터 1998년 12월 31일까지 개심술을 시행받은 환자 237명을 Group I으로, 2000년 1월 1일부터 2001년 7월 31일까지 개심술을 시행받은 환자 216명을 Group II로 분류하여 총 453명을 대상으로 하였다. 만 15세 이상의 성인 환자에서 정중흉골절개술 및 심폐바이패스를 시행한 경우만을 포함하였고 흉골감염 이외의 원인으로 사망한 예는 포함하지 않았다. 결과: 흉골감염은 GroupI에서 18례(8.0%), Group II에서 1례(0.49%) 발생하였다. 흉골감염에 영향을 미치는 것으로 알려진 위험인자 중에서 응급수술여부, 심폐바이패스시간, 수술시간, 수혈량, 기관절개술유무, 재수술여부 등이 유의한 연관성이 있었으며, 이들을 대상으로 시행한 로지스틱 회귀분석에서 집중치료실과 수술장의 이전이 흉골감염에 유의한 영향을 미치는 것으로 나타났다.(p=0.029, 95% 신뢰구간 0.011∼0.788). 결론: 개심술후 흉골감염에 영향을 주는 여러 인자들이 알려져 있지만 집중치료실 및 수술장의 개선이 흉골감염의 발생을 줄일 수 있는 하나의 방법으로 생각된다.

구강 내 Helicobacter pylori (Helicobacter pylori in the Oral Cavity)

  • 위염 및 위암 등의 발생과 관련되어 있는 Helicobacter pylori(H. pylori)는 구강의 치태와 타액에서 주로 발견이 된다. 유년시절 동안 주로 감염되는 것으로 알려져 있지만 감염경로는 불분명하다. 구강이 H. pylori의 두 번째 서식지로서 전염경로 및 위장내 H. pylori의 제균 후 재감염에 중요한 영향을 끼칠 수 있는지는 논쟁이 되고 있다. 따라서 본 저자는 문헌고찰을 통하여 구강 내에 존재하는 H. pylori에 관하여 알아보고자 하였다. 위장에 존재하는 H. pylori는 위인두반사나 구토에 의해 구강 내 발현될 수도 있으나, 구강과 위의 감염은 서로 관련성이 없는 것으로 보고되고 있다. 진단방법으로는 혈청학적검사, 요소호기검사, 중합효소연쇄반응(polymerase chain reaction: PCR)방법, Urease검사, 조직검사 등이 있으나, 타액과 치태에서는 nested PCR 방법이 주로 추천되어 진다. 구강 내 감염율은 다양하게 나타나며, 치과질환과의 연관성은 없는 것으로 사료된다. 그러나 치주질환 환자의 구강 내에서 발현율은 높게 나타나므로, 주의가 요구되며 향균 구강세척제의 사용이 권유된다. 결과적으로 구강 내 H. pylori는 정상세균총으로 사료되며, 향후 구강내 H. pylori에 관한 추가적인 많은 연구가 필요하리라 사료된다.

간호학생의 병원감염관리에 대한 지식, 태도 및 자신감 (Knowledge, Attitude and Self-Confidence of Student Nurses Regarding Nosocomial Infection Control)

  • Purpose: This study was conducted to identify the knowledge, attitude and self-confidence of student nurses regarding control of nosocomial infections and to provide data for preparation of an efficient policy and education program regarding nosocomial infections. Method: A descriptive survey design was utilized, and, using a questionnaire, which included 15 items for knowledge, 81 items for attitude, 81 items for self-confidence, data were collected from 520 student nurses. Descriptive statistic, t-test, one-way ANOVA and Pearson's correlation coefficients, and Stepwise multiple regression analysis were used with SPSS win 12.0 to analyze the data. Results: The mean score for knowledge was 0.63, for attitude, 4.38 and for self-confidence, 3.50. There was a significant positive correlation between knowledge, attitude and self-confidence in control of nosocomial infections. Attitude, practical attitude, and education were significant factors affecting self-confidence regarding nosocomial infection control. Conclusion: These research findings should be useful in promoting continuous and repeated educational programs on nosocomial infection for student nurses.

Biological control of Flavobacterium psychrophilum infection in ayu (Plecoglossus altivelis altivelis) using a bacteriophage PFpW-3

  • The efficacy of using a bacteriophage (phage) to control Flavobacterium psychrophilum (F. psychrophilum) infection of ayu (Plecoglossus altivelis altivelis) was evaluated in this study. Intramuscular challenge failed to induce sufficient infection levels; therefore, a newly designed net-scratch challenge method was also used to induce bacterial infection. Administration of phage PFpW-3 in F. psychrophilum-infected ayu showed notable protective effects, increased survival rates and mean times to death. Additionally, the fate of inoculated bacteria and phage in ayu were investigated. Our results suggest that the phage PFpW-3 could be considered an alternative biocontrol agent against F. psychrophilum infections in ayu culture.

Corona-Cov-2 (COVID-19) and ginseng: Comparison of possible use in COVID-19 and influenza

  • In the 1918 influenza pandemic, more than 95% of mortalities were ascribed to bacterial pneumonia. After the primary influenza infection, the innate immune system is attenuated, and the susceptibility to bacteria is increased. Subsequent bacterial pneumonia exacerbates morbidity and increases the mortality rate. Similarly, COVID-19 infection attenuates innate immunity and results in pneumonia. In addition, the current pneumococcal conjugate vaccine may have limited defense against secondary pneumococcal infection after influenza infection. Therefore, until a fully protective vaccine is available, a method of increasing immunity may be helpful. Ginseng has been shown to increase the defense against influenza in clinical trials and animal experiments, as well as the defense against pneumococcal pneumonia in animal experiments. Based on these findings, ginseng is suspected to be helpful for providing immunity against COVID-19.

경정맥(經靜脈) 내적(內的) Pacemaker 이식후(移植後) 발생(發生)한 복잡한 감염합병증(感染合?症)의 1치험례(治驗例) (Complicated Wound Infection Following Transvenous Endocardial Pacemaker)

  • Implantation of a permanent pacemaker is a widely accepted procedure for the patient with complete heart block.As a result of these device, the prognosis for patients with Adams-Stokes syndrome caused by complete A-V block and other cardiac arrhythmia have become much more optimistic. Permanent pacemaker implantation by means of a transvenous approach has made the operative risk much less and the procedure simpler. However, a number of complications have been reported in the literature regarding transvenous endocardial pacemaker implantation during the last a decade. The patient presented in this paper is a 26-year old girl who was implanted with a permanent pacemaker at 14 years of age because of a congenital A-V block. Following first exchange of pulse generator, the electrode (lead) was fractured, so that by the pulse generator, a change to the transvenous technique of implantation was made, After this, there were episodes of recurrent wound infection on three occasions, even though the site of pulse generator implantation was exchanged to the contralateral side of chest wall, massive doses of antibiotics were administered and sensitivity tests for coagulase positive staphylococcal infection were performed. Though there was no definite evidence of blood stream infection by blood culture, we decided not to use the transvenous technique and not to implant the pulse generator in the chest wall because the venous system and the entire anterior chest wall appeared to be diseased or contaminated by virulent pyogenic organisms. Finally this intractable systemic and local wound infection was successfully controlled by myocardial lead implantation via a subxiphoid approach and implantation of the pulse generator far down in the abdominal wall. The causes and routes of recurrent wound infection and possible blood born infection in this particular patient are still obscure. We strongly believe that myocardial pacemaker implantation is much safer than transvenous endocardial pacemaker implantation & myocardial pacemaker implantation is a definite method for controlling such an intractable wound infection. following transvenous pacemaker implantation.

치과에서의 감염방지를 위한 개인보호용구착용에 관한 연구 (A study on personal protection equipment for Infection control at dental offices)

  • Objectives : The purpose of this study was to minimize cross infection that can take place within dental office, to make dental workers recognize importance of infection management, and to improve dental-care environment that is exposed to infection. Methods : It conducted the self-reported questionnaire survey on the practice of wearing PPE(personal protective equipment) from April 1, 2008 to May 30 targeting 180 dental workers who are working for dental offices where are located in Jeonnam region. Results : 1. As for research subjects' general characteristics, age was the largest in under 29 years old with 75.0%. It was surveyed to be large in over 5 years(56.1%) for working career, in a city(83.3%) for working region, and in having licence of dental hygienist(75.0%). 2. As a result of surveying the practice of wearing PPE, wearing mask given dental care was surveyed to have the highest practice level with 75.6%. Wearing protective glasses showed the lowest practice ratio with 23.3%. 3. As a result of surveying the difference in the practice of wearing PPE according to general characteristics, the difference according to the working region was surveyed to be the greatest. Conclusions : As the above result, the education of infection management for dental workers needs to be performed continuously even after curriculum of school. Even the in-service education on infection management needs to be vitalized. Also, the necessity was examined for always recognizing and practicing importance of infection by using a method of publishing newsletter via post to dental clinics where are located in rural area, with considering regional deviation.

경남 남부지역 사육견에 대한 심장사상충 감염률 조사 (Prevalence of heartworm infection among dogs on breeding farms in southern Gyeongnam area)

  • This study was attempted to survey the prevalence of heartworm infection among 253 dogs (male 167, female 86) on the 16 breeding farms in southern Gyeongnam area in May to December 2007. The results obtained from this experiment were summarized as follows; Fifty seven (22.5%) of the 253 examined dogs were heartworms-antigen positive, while twenty five dogs were Microfilaria positive by the modified Knott's method. The regional infection rates were of 25% in Tongyeong and 21% in Goseong, and on the breeding environment aspects, infection rates were lower in rural area than urban area farms. The infection rates were higher in female (26/86, 30.2%) than male dog (31/167, 18.6%). The infection rates by breeding size of farms were 31% (11/35) at <20 head, 26%(35/135) at 20~50 head and 13% (11/82) at >50 head. The infection rates of heartworm in examined dogs at the age of <2, 2~4 and >5 were 10.7% (8/75), 26% (40/154) and 37.5% (9/24), respectively. In the microfillaria-infected 25 dogs, identified Dirofilaria immitis of 21 (84%) using PCR. The dogs infected with heartworm showed the increased number of eosinophil.

조직인공소화법과 ELISA를 이용한 국내 출하돈의 선모충(Trichinella spiralis) 감염실태 조사 (The survey of Trichinella spiralis infection in finishing pigs using the pepsin-digestion method and ELISA in Korea)

  • Trichinella spiralis is one of the important zoonotic parasites with a wide variety of vertebrates hosts in nature. The purpose of this study were to analyze ESP(Excretory-Secretory Protein) antigen, to evaluate ELISA for the serological diagnosis of Trichinosis, and to survey T. spiralis infection in finishing pigs using the pepsin digestion method and ELISA in Korea. In the analysis of ESP antigen by SDS-PAGE and Western blot, 4 major bands (70, 55, 52.6, and 49 kDa) were revealed from the ESP antigen. Predilection sites of T. spiralis were the diaphragm, the tongue, masseter muscles, intercostal muscle, and hindlimb in orders in the experimentally infected rats. Sera from 581 swine were tested by ELISA with ESP antigen. The 54 (9.3%) sera were suspected as positive reactors, however, these 54 sera were determined as false positives by the use of Western blotting. This study demonstrated that the ELISA was not suitable for the examination of T. spiralis in pork. The diaphragm muscle samples of 251 finishing pigs were tested by the method of pepsin-digestion for the presence of Trichinella larvae, however, T. spiralis was not detected from the samples. We could not find out T. spiralis infection in pig in Korea pork.
