영유아 건강증진을 위한 지역사회 간호센터 모형 구축 (Development of A Community-Based Nursing Center Model: Focused on Health Promotion of Infant & Toddler)

  • Objectives: The purpose of this study was to develop a community-based nursing center model that can provide health promotion for infants and toddlers as well as a research and practice site for nursing faculty and students. Methods: Review of current health care system and health promotion programs, and workshops with brain-storming were carried out to identify needy areas of services to be provided by the nursing center. Finally, the community nursing center model was developed through expert consultation and filed visits. Results: The services and programs were developed for infants and toddlers residing in Seoul area. These services and programs include growth and developmental screening, environment and health problem assessment, infant stimulation and parent education, and self-help group. Full-time and part-time staff nurses and nurse practitioners will provide those services and offer home visiting if needed. Database system, health information system, and trainers' training programs were developed as well. Conclusion: The nursing center model developed in this study will ensure clients direct access to nursing care and increase of autonomy and accountability of nursing practices. The nursing center model focused on disease prevention and health promotion will enhance the quality of life of the infants, toddlers and their families as well as to decrease national health care expenditure.

교사 평정용 영아발달 평가도구 (The Development of Infants and Toddlers: A Rating Scale for Teachers)

  • 본 연구의 목적은 영아프로그램을 수행하면서 실시할 수 있는 1, 2세 학급 영아를 위한 교사 평정용 발달평가도구를 개발하는 것이다. 이 평가도구는 영아프로그램의 일과를 중심으로 대소근육운동, 자조기술, 사회의사소통, 사회정서, 인지영역의 발달을 평가할 수 있도록 총 201문항으로 구성하였다. 전국적으로 어린이집에서 표집된 1245명의 1, 2세 학급 영아를 대상으로 평가문항의 양호도, 신뢰도 타당도를 검증하였다. 내용 타당도는 전문가 집단의 검토결과 타당하게 나타났으며, 신뢰도는 내적 일치도, 평정시간 일치도, 검사-재검사간 일치도가 높게 나타났다. Bayley와의 공인 타당도도 양호하였다. 구인 타당도 분석결과 영아의 발달은 영역별로 뚜렷이 구분되기보다는 통합적 특성이 있는 것으로 나타났다. 이 평가도구의 표준화를 위하여 전국 규준이 원점수와 표준점수로 제시되었다.

정신장애 영유아에 대한 K-CBCL 1.5-5 (Korean-Child Behavior Checklist 1.5-5) 조절곤란 프로파일의 요인구조와 정신장애 간 차이검증 (Factor Structure of the Korean-Child Behavior Checklist 1.5-5 Dysregulation Profile in Infants and Toddlers With Mental Disorders and the Difference Among Mental Disorders)

  • 목적 : 본 연구의 목적은 정신장애 영유아의 K-CBCL 1.5-5(Korean-Child Behavior Checklist 1.5-5) DP(Dysregulation Profile)의 요인구조를 확인하고, 정신장애 진단 집단 간 차이를 검증하였다. 연구방법 : 서울 소재의 종합병원 소아정신과, 발달장애 클리닉 등에서 2010년에서 2015년까지 수집된 자료를 활용하였다. DSM-IV에 근거하여 정신장애로 진단된 영유아 265명의 어머니에게 실시된 K-CBCL 1.5-5 자료를 최종분석에 사용하였다. 결과 : 정신장애 영유아의 K-CBCL 1.5-5 DP의 요인구조는 이중요인 모형(Bifactor)이 상대적으로 가장 적합하였다(χ2 = 893.722, TLI = .782, CFI = .822, RMSEA = .064). 또한 의사소통장애, 전반적 발달장애, 정서장애, 발달지체 등 영유아의 정신장애 진단 집단 간 차이가 유의하였다(F(3, 259) = 9.780, p < .001). 그 중 전반적 발달장애 집단과 정서장애 집단이 의사소통장애 집단보다 높은 조절곤란을 보이는 것으로 확인되었다. 결론 : 본 연구에서는 정신장애 영유아의 조절곤란 문제의 존재를 확인하였다. 이중요인 모형을 통해 K-CBCL 1.5-5 DP가 불안/우울, 공격행동, 주의집중문제의 특정요인과 특정요인 이상의 일반요인으로 구성된다는 것을 확인하였고, K-CBCL 1.5-5 DP의 다차원성을 규명하였다. 또한 정신장애 집단 간 유의한 차이가 있었고 조절곤란 문제는 영유아기부터 전반적 발달장애와 정서장애의 증상 발현과 심화에 기여하는 것으로 사료된다.

영아의 애착 안정성에 영향을 미치는 변인 : 어머니의 민감성과 영아의 기질 (Relations of Maternal Sensitivity and Temperament to linfants Attachment Security)

  • Maternal sensitivity and infant temperament were studied in relation to infant attachment security. Subjects were 47 one-year-old infants anti their mothers from intact upper-middle class families. Mothers sorted the Attachment Q-set (Waters, 1987, version 3.0) and filled out the Toddlers Temperament Questionnaire(Chu, 1992). Maternal sensitivity was observed in the home for two 3-hour sessions, and observers completed the Maternal Behavior Q-set(Pederson, Moran et al., 1990). Attachment security scores of the infants, using criterion scores developed with Korean infants, were 42, which is comparable to Previous studies. Infants who were securely attached to mothers showed more secure-based behaviors and compliance, enjoyed physical contacts, and showed less fussy/difficult behaviors with mothers. Mothers of girls were observed to be more sensitive to their infants' signals than mothers of boys. Maternal sensitivity scores were not related to infant security scores. However, maternal sensitivity was related to less compliance to mothers for boys.

도시와 농촌에 있어서의 영유아의 양육 및 보건에 관한 비교연구 -이유와 이유식을 중심으로- (A Comparative Study on Weaning Patterns and Methods of Health Care of Infants and Toddlers in the Farming Area and City, Bores)

  • The study was conducted during the period of July, 1972 to June, 1973 in Seoul city and Choong Chung Buk Do area. The purpose of this study was to find out tile Weaning Patterns and investigate the habits of health care of the babies. The conclusions obtained were as follows: 1. 65 percent of the infants were sufficiently fed by mothers'milk. But, the other 35 percent were not enough to be breasted. 2. Over 50 percent of the mothers started the weaning when tile infants became 9-12 months of age. The period of time needed for weaning was between 6-18 months of age. 3. Majority of the mothers preferred to give infants corns as the major diets. 4. It was noted that many mothers did not know the importance of weaning. And it was felt that public health nurses have to instruct these mothers who are bringing up the babies why weaning is necessary. 5. Only half of the babies who were investigated were found to be regularly vaccinated with all the necessary preventive inoculation at Public Health Centers both in farming and city areas.

Infant Crying Acoustic Characteristics Evoking Unpleasant Emotions in Mothers

  • This study examines the infant crying acoustic characteristics likely to arouse negative emotions and distress in mothers. We used samples of infant crying from three situations (hunger, pain, and the mother's absence) in six healthy infants over six months. We played the recordings of infants crying to 90 mothers in the care of infants and had them self-evaluate emotions and feelings. In addition, the sounds were analyzed acoustically through a CSL4400 to analyze frequency, energy, total expiratory time, and the number of the expirations. In this study, cries due to pain and the absence of the mother caused more unpleasant emotions and irritation in comparison to the infant sounds of hunger. In particular, crying from the absence of the mother caused the most distress. An analysis of these sounds showed that crying in the situations of pain and the absence of the mother were strong in frequency, high energy, and prolonged. These results suggest a relation between infant crying acoustical characteristics and the feelings of distress by the mother.

  • 본 연구는 유아교원양성과정에서 '영유아교수·학습방법'에 대한 예비보육교사들의 인식에 대한 연구이다. 문헌조사 및 예비유아교사의 '영유아교수·학습방법'에 대한 이해를 중심으로 주제에 대한 워드클라우드 분석 및 질적 사례분석을 활용한 혼합연구이다. '영유아교수·학습방법' 교과목에 대한 생각, 어려움, 배울점, 교사로서 잘할 수 있는 수업 역량 및 수업 전문성을 위한 경험 등을 통해 유아교사로서의 의미를 찾아보고자 하였다. 본 연구결과를 통해 향후 영유아들을 대상으로 수업을 진행할 때 교수학습방법에 대한 자신들의 효능감을 높힐 수 있는 방안을 찾고자하며, 유아교육의 질 개선을 위한 기초적인 자료를 제공할 수 있을 것으로 기대한다.

영유아 자녀를 둔 취업모와 전업모의 건강증진행위 비교 (Comparative Study on Health Promoting Behavior in Working and Non-working Mothers with Infants and Toddlers)

  • Purpose: The purpose of this study was to compare health promoting behavior between working and non-working mothers with infants and toddlers, and to investigate factors affecting the mothers'health promoting behavior. Methods: This descriptive study was conducted through conveniently sampled 403 women who visited the child health clinics at two public health centers. The questionnaire included the Health Promoting Life Style Profile (HPLP) and a visual analogue scale for subjective health status. ANCOVA, one-way ANOVA, correlation analysis, and stepwise multiple regression were conducted using SPSS ver. 21. Results: Working mothers' average HPLP score ($2.30{\pm}0.37$) was higher than non-working mother's ($2.15{\pm}0.37$). The score of the physical activity subscale was lowest among the subscales and there was a difference between the two groups. Subjective mental health status was the only predictor of working mothers'health promoting behavior, and it explained 23.2% of variance in health promoting behavior. Subjective mental health status, education, and age were the predictors of non-working mothers' health promoting behavior and they explained 27.2% of variance in health promoting behavior. Conclusion: According to the findings, both working and non-working mothers' health promoting behaviors were low. To promote mothers' health, it is necessary to develop diverse community health promotion programs to support mothers.

식품 보장 측정의 개발과 타당도 연구 (Development and Validation of Food Security Measure)

  • This study aims to develop an index of food security in order to assess food security status in Korea and to evaluate reliability and validity of the index. The index of food security was developed based on the US Household Food Security Survey Module (US-HFSS). After the US-HFSS was translated and back-translated, it was evaluated and modified by a focus interview for experts. The developed Korean Household Food Security Survey Module (K-HFSS) was tested by cognitive interviews and a pretest for general community population to assess its application for Korean population. For the reliability and validity of K-HFSS, the survey was conducted with 300 adults aged 40 years or more living in a rural community and 212 infants and toddlers living in an urban community. The reliability was assessed by Cronbach' alpha and the validity was evaluated by content validity, construct validity, and criterion-related validity. The questionnaire items of K-HFSS were partly modified, accounting for Korean social and cultural backgrounds and adapted by general community population. The reliability was relatively high, showing Cronbach' alpha coefficients ranged from 0.80 (for adults) to 0.87 (for infants and toddlers). The content and construct validities were all acceptable. The result of criterion-related validity showed that food security status was significantly related to the household income level. In conclusion, the K-HFSS would be used as a reliable and valid instrument to assess food security status in parts of the Korean population.

보육교사의 영유아 자폐스펙트럼장애 인식에 관한 연구: 포커스 그룹 인터뷰(FGI)를 중심으로 (A Study on Recognition of Autism Spectrum Disorder Among Infants and Toddlers by Childcare Teachers : With a Focus on Focus Group Interviews)

  • This study examines indicators and factors for identification of ASD(Autism Spectrum Disorder) characteristics of infants and toddlers and early screening of children with ASD by day care teachers. Focus group interviews were conducted with 11 teachers by a semi-structured questionnaire developed by the authors. The findings are: 1) key problems and indicators of behavioral characteristics of children with ASD included problems with social communication and interactions, restricted and unusual interests, outstanding ability in specific areas, insistence on sameness and ritualized patterns, problems with language development and communication, sensory abnormalities; 2) counseling with parents of children with ASD revealed a lack of recognition of parents, negative expressions of emotions and resistance, and difficulties in referral to and association with experts; 3) needs for ASD in the child care field included visiting by experts, provision of test scales, training of child care teachers and parents, and building cooperative systems; and 4) demands of development and execution of a Korean-style scale for early screening of children with ASD.
