Background Methods for identifying local lymph node metastasis in malignant melanoma include sentinel lymph node biopsy (SLNB) and lymph node dissection (LND). In particular, SLNB has been widely used in recent years. This study aimed to retrospectively confirm the adequacy of the current indication criteria for SLNB by applying those criteria to a mixed group of patients who previously received SLNB and LND. Methods This study included 77 patients with malignant melanoma of the extremities and trunk who were monitored for >24 months. The patients were classified according to whether the current indication criteria for SLNB were applicable. The sentinel lymph nodes were evaluated for each group. Patients for whom the indication criteria for SLNB and LND were applicable were analyzed according to whether SLNB or LND was performed. Finally, the outpatient records of these patients were reviewed to evaluate recurrence, metastasis, and prognosis. Results Of the 77 patients, SLNB was indicated according to the current criteria in 60 cases. Among the 60 patients for whom SLNB was indicated, 35 survived the follow-up period disease-free, 21 died during the follow-up period, and four experienced metastasis. The 17 patients for whom SLNB was not indicated had no recurrence or metastasis. Conclusions Patients for whom SLNB was not indicated had no recurrence or metastasis. In cases where SLNB is indicated, the possibility of metastasis and recurrence may be high even if SLNB is negative or LND is performed, so more aggressive treatment and careful follow-up are crucial.

Although Kimchi is a Korean traditional food, domestic consumption has been decreasing steadily and the trade inversion phenomenon has reached a serious level due to the surge of Chinese Kimchi imports. Moreover, cases where foreign Kimchi is transformed illegally into Korean Kimchi are frequent, which impedes the expansion of Korean Kimchi exports. To sustain the Korean Kimchi industry in a situation where the domestic and overseas conditions are deteriorating, it is necessary to positively review the introduction of Kimchi into a geographical indication (GI) system. This study examined the intention of foreign consumers to purchase Korean Kimchi with GI and analyzed the impact on the trade balance. Approximately 42.8% of 500 Japanese consumers answered that they would purchase Korean Kimchi with GI and they were willing to pay 7.8% more than the present price. Approximately 78.7% of 300 Taiwanese consumers replied that they purchase it and would pay 25.1% more. In addition, Japanese and Taiwanese consumers reported that they expected to increase their Korean Kimchi purchases by 21.9 and 22.4%, respectively. Based on the survey results, the effects of the trade balance were measured using the methodology of a preliminary impact assessment using the KREI-KASMO model. The trade balance of Kimchi is expected to improve slightly at an annual average of 11.718.6 million US$ to as much as 27.7~35.8 million US$.

Background: Knowledge of the base rate of signal changes consistent with distal biceps tendinopathy on magnetic resonance imaging (MRI) has the potential to influence strategies for diagnosis and treatment of people that present with elbow pain. The aim of this study is to measure the prevalence of distal biceps tendon signal changes on MRIs of the elbow by indication for imaging. Methods: MRI data for 1,306 elbows were retrospectively reviewed for mention of signal change in distal biceps tendon. The reports were sorted by indication. Results: Signal changes consistent with distal biceps tendinopathy were noted in 197 of 1,306 (15%) patients, including 34% of patients with biceps pain, 14% of patients with unspecified pain, and 8% of patients with a specific non-biceps indication. Distal biceps tendon changes noted on radiology reports were associated with older age, male sex, and radiologists with musculoskeletal fellowship training. Conclusions: The finding that distal biceps MRI signal changes consistent with tendinopathy are common even in asymptomatic elbows reduces the probability that symptoms correlate with pathology on imaging. The accumulation of signal changes with age, also independent of symptoms, suggests that tendon pathology persists after symptoms resolve, that some degree of distal biceps tendinopathy is common in a human lifetime, and that tendinopathy may often be accommodated without seeking care. 본 연구에서는 한 EU FTA 협상에서 중요이슈가 되고 있는 지적 재산권으로서의 지리적 표시제에 대한 의의 및 보호체계를 검토 연구했다. 지리적 표시는 상품의 품질 등이 지리적 특성과 연계된 지리적 명칭을 지적재산권으로 보호해주는 제도이다. 19C 후반부터 이에 대한 보호노력이 여러형태로 계속되어 오다가 드디어 WTO TRIPS 협정 상에 지적 재산권 보호대상으로 규정되었고, 이에 따라 지리적표시는 양자 규범 복수규범 내지 임의 규범에서 다자 규범 의무 규범으로 발전 된 것이다. 지리적 표시는 이같이 다자규범으로 자리 잡은 후에도 특히 EU를 주도로 하여 DDA 협상에서 계속 확대 개선을 위한 논의가 활발히 이루어지고 있다. 지리적표시보호에 대한 각국의 체계 및 형태는 유사하나 크게는 EU형과 미국형이 있다. 특히 EU는 지리적 표시가 품질정책 및 공동농업정책의 근간이 되어있고 현재 남부 공업국가의 품목들이 대부분을 점하고 있는 5,000여개의 지리적 표시가 주류 및 가공식품을 중심으로 등록되어 있으며 이에 따라 지리적표시의 국제적 보호규범체계의 정립 및 확대에서 항상 주도적 입장을 유지하고 있다. 우리나라는 1997년 7월부터 농수산 품질관리법에 의한 등록제를 마련했고 2004 7월에는 상표법에 의한 지리적표시 단체장 등록제도를 도입 운영하고 있으며 이에 기초하여 40여개의 품목들이 지리적 표시품목으로 등록되어있다. 한국과 EU 간의 지리적 표시에 관련한 협상에서는 EU 측의 적극적 상호인정 요구가 추진이 예상되고 이에 따라 우리의 대응이나 전략여하가 문제된다. 물론 EU는 그 보호가 발전되어 있고 보호등록이 많으며 그것이 농업정책의 개혁과 고품질 농산물의 화대 정책의 근간을 이루고 있다는 점에서 적극적 상호인정이 쉽지 않다. 그러나 여기서 우리는 장기적 구도로 본다면 지리적 표시 보호의 상호 인정을 회피나 무조건적 거부보다는 긍정적 단계적 측면에서 조심스럽게 접근하되 국제적 규범과 조화하면서도 후속적 관리와 대응에 철저한 이른바 전략적 수용과 대비를 고려할 수 있을 것이다. 특히 우리의 지리적표시제는 우리 농산물만의 특성에 따른 품질 향상과 경쟁력 강화 및 고 부가치 전략 구축에 결정적이고도 중요한 요인이 될 수 있다는 강점도 있다는 점이다. 정부와 농업인의 지혜를 모아 지리적 표시로 등록된 40여개 품목에 대한 지속적 품질개선은 물론 타 농산물 및 식품의 지속적 표시확대 그리고 특성화에로의 개발전략 등으로 EU시장에 과감히 진출할 경우 유럽시장에의 우리의 접근성 창출성이 크게 증폭 될 것이다.

목적: 회전근 개 파열의 봉합 가능성은 수술적 치료의 적응증을 정립하는데 매우 중요하다. 저자는 회전근 개의 봉합 가능성 및 치유에 영향을 주는 요소들과 회전근 개 파열의 수술적 치료의 적응증에 대해서 고찰하였다. 대상 및 방법: 회전근 개 파열의 봉합 가능성은 수술적 봉합 가능성과 술 후 회전근 개 파열의 치유 가능성의 측면에서 고려되어야 한다. 회전근 개 파열의 자연 경과에 대한 이해 역시 수술적 치료를 할 것인지 보존적 치료를 할 것인지를 결정하는데 큰 도움을 준다. 결과: 회전근 개의 치유 가능성을 예측하는 세가지 일반적인 범주가 있는데 첫째로 수술적 요소, 생물학적 요소, 환경적 요소이다. 수술의는 수술적 기법을 스스로 선택함으로써 수술적 요소를 제어할 수 있으며, 생물학적 요소는 나이와 급성 외상의 유무, 만성화 정도, 파열의 정도 등을 고려하여 주의 깊게 수술 적응 대상을 선택함으로써, 환경적 요소는 흡연 및 비스테로이드성 소염진통제(nonsteroidal anti-inflammatory drug, NSAID) 사용을 피함으로써 더 좋은 임상 결과를 기대할 수 있다. 결론: 회전근 개 파열의 자연 경과와 봉합 가능성에 대한 정확한 이해는 회전근 개 파열의 수술적 치료의 적응증을 정립하고 만족스러운 임상결과를 얻는데 큰 도움을 줄 수 있다.

When a telecommunications line is encountered with neighboring electric lines, induced currents are produced in the circuit formed by those lines and earth. The induced currents sometimes impose intolerable noise problems on the telecommunications lines. In this study, screening factor indication the level of screening effect is defined and calculated several cases together with numerical results.

A dimensionality assessment procedure DETECT uses the property of being near zero of conditional covariances as an indication of unidimensionality .This study provides the convergent properties to zero of conditional covariances when the dta is unidimensional, with which DETECT extends its theoretical grounds.
