A CLASS I DISCREPANCY CASE TREATED BY SERIAL EXTRACTION (연속발치에 의한 치험일례 (제1보))

  • Serial extraction procedure, when cautiouly practiced in severe discrepancy case, can be a good clinical approach, but the treatment planning must always be based on accurate analysis of all conditions, especially patient's individual growth pattern and his family development. It is most difficult to determine the timing and selection of most effective sequence of deciduout extraction. It is the best candidate that patient is with class I malocclusion with harmony of the skeletal and muscular system and severe discrepancy of the tooth system. The authors have observed a female, who has complained of the malalignment of mandibular permanent incisor teeth. Serial extraction has been performed to relieve it through adequate various informations and resulted in a fairly good prognosis.

Anatomical study on the mandible of the Korean native goat (한국재래산양 하악골에 관한 해부학적 연구)

  • This study was carried out to identify metric and non-metric characteristics of the mandible of the Korean native goat. The results were summarized as follows : The length of the mandible was more correlated with the width of the base of the ramus mandibulae than the margo alveolaris(p<0.01). The height between incisura vasorum facialium and processus condylaris had very high correlation with the height between incisura vasorum facialium and incisura mandibulae(p<0.01). The former was also more correlated to the height of the mandible in front of the first molar tooth than the height of the mandible behind the last molar tooth(p<0.01). The symphysis was formed between both mandibles and it was not able to observe ossification in all case. The number of alveoli for incisor teeth were four each mandible, however three alveoli were observed in two cases. There was usually one or occasionally, two foramina mentalia on the lateral surface of the corpus mandibulae below the margo interalveolaris near the symphysis. The incisura vasorum facialium was poorly developed. A distinct groove was observed rostroventral to the foramen mandibulae. The processus condylaris was shown hammer shape, and the caput mandibulae was flat rectangular form. The processus coronoideus was formed a rectangle and curved laterally. The incisura mandibulae was well developed.

  • The endodontically treated tooth is generally restored with post and core, owing to the brittleness and the loss of large amount of tooth structure. Although there have been lots of studies about the endodontically treated teeth, the three-dimensional quantitative studies about the stress distribution of them are in rare cases. In this study, it was assumed that the coronal portion of the upper incisor had extensively damaged. After the root canal therapy it was post cored, and restored with PFG crown. The three-dimensional model, in which the root was supported with a normal alveolar bone, was constructed. Force was applied to the centric stop point with the angle of 135 degrees to the long axis of the tooth. Force was assumed to be 250N as functional maximum bite force of upper central incisors. The results analyzed with three-dimensional finite element method were as follows : 1. Stress was concentrated on the middle portion of the labial side dentin and the apical portion of the dentin. 2. Stress in the post was more concentrated on the post apex. 3. The displacement of the post at the post-cement interface was almost symmetrical la-bio-lingually. 4. It assumed that restoring extensively damaged tooth with a post-core and PFG crown is an adequate method of restoration.

Short-term evaluation of dental implants in a diabetic population: an in vivo study

  • PURPOSE. The study was conducted to evaluate the efficacy of implant supported tooth replacement in diabetic patients. MATERIALS AND METHODS. The study involved placement of implants (UNITI implants, Equinox Medical Technologies, Zeist, Holland, diameter of 3.7 mm and length 13 mm) in five diabetic patients (three females and two males) of age ranging from 35-65 years with acceptable metabolic control of plasma glucose. All patients included in the study were indicated for single tooth maxillary central incisor replacement, with the adjacent teeth intact. The survival of the restored implants was assessed for a period of three months by measurement of crestal bone heights, bleeding on probing and micro flora predominance. Paired t-test was done to find out the difference in the microbial colonization, bleeding on probing and crestal bone loss. P values of less than 0.05 were taken to indicate statistical significance. RESULTS. Results indicated that there was a significant reduction in bleeding on probing and colonization at the end of three months and the bone loss was not statistically significant. CONCLUSION. The study explores the hypothesis that patients with diabetes are appropriate candidates for implants and justifies the continued evaluation of the impact of diabetes on implant success and complications.

Esophageal Basaloid Squamous Carcinoma Treated by Endoscopic Resection and Radiation (내시경 절제술과 방사선 치료로 완치한 식도의 기저양 편평세포암)

  • Esophageal basaloid squamous carcinoma (BSC) is a rare, aggressive variant of squamous cell carcinoma. BSC is usually diagnosed in advanced stage and its prognosis is relatively poor. A 59-year-old male with subepithelial lesion of the esophagus that was incidentally discovered during health promotion examination was referred to our hospital. Esophagogastroduodenoscopy showed a 10-mm bulging mucosa with an intact surface at 34 cm from incisor teeth. Endoscopic ultrasonography revealed a smooth margined homogenous hypoechoic lesion, measuring $11.3{\times}3.9mm$ with a submucosal layer of origin. The patient underwent endoscopic mucosal resection of the subepithelial lesion. Pathologic examination of the resected specimen revealed BSC with involvement of vertical margin by tumor. The patient then underwent radiotherapy, and is doing well without recurrence for 35 months. We report a case of esophageal BSC confined to submucosal layer successfully treated with endoscopic resection followed by radiation.

Effectiveness of Invisalign® aligners in the treatment of severe gingival recession: A case report

  • In this report, we demonstrate the effectiveness of the Invisalign® system in the treatment of severe gingival recession and bone dehiscence through torque, translation, and intrusion movements in a young woman. Cone-beam computed tomography was used to assess bone parameters and check the teeth during treatment. The root of the mandibular right central incisor, which was buccally positioned and exhibited bone dehiscence of 9.4 mm, was moved toward the center of the alveolar process by using the Invisalign® system and SmartForce® features. The patient was monitored by a periodontist throughout the orthodontic treatment period. Her gingival recession reduced, while the bone dehiscence reduced from 9.40 mm to 3.14 mm. Thus, movement of the root into the alveolus promoted bone neoformation and treated the gingival recession. The findings from this case suggest that orthodontic treatment using the Invisalign® system, along with periodontal monitoring, can aid in the treatment of gingival recession and alveolar defects.

Factors influencing the axes of anterior teeth during SWA on masse sliding retraction with orthodontic mini-implant anchorage: a finite element study (교정용 미니 임플랜트 고정원과 SWA on masse sliding retraction 시 전치부 치축 조절 요인에 관한 유한요소해석)

  • Objective: With development of the skeletal anchorage system, orthodontic mini-implant (OMI) assisted on masse sliding retraction has become part of general orthodontic treatment. But compared to the emphasis on successful anchorage preparation, the control of anterior teeth axis has not been emphasized enough. Methods: A 3-D finite element Base model of maxillary dental arch and a Lingual tipping model with lingually inclined anterior teeth were constructed. To evaluate factors influencing the axis of anterior teeth when OMI was used as anchorage, models were simulated with 2 mm or 5 mm retraction hooks and/or by the addition of 4 mm of compensating curve (CC) on the main archwire. The stress distribution on the roots and a 25000 times enlarged axis graph were evaluated. Results: Intrusive component of retraction force directed postero-superiorly from the 2 mm height hook did not reduce the lingual tipping of anterior teeth. When hook height was increased to 5 mm, lateral incisor showed crown-labial and root-lingual torque and uncontrolled tipping of the canine was increased.4 mm of CC added to the main archwire also induced crown-labial and root-lingual torque of the lateral incisor but uncontrolled tipping of the canine was decreased. Lingual tipping model showed very similar results compared with the Base model. Conclusion: The results of this study showed that height of the hook and compensating curve on the main archwire can influence the axis of anterior teeth. These data can be used as guidelines for clinical application.

SPONTANEOUS ERUPTION OF ECTOPIC IMPACTED TOOTH BY INTENTIONAL EXTRACTION OF DECIDUOUS TOOTH (의도적 유치발치술에 의한 이소매복 영구치의 맹출유도)

  • When many factors involved in the eruption of the teeth act as negative effects, they can cause eruption disturbance. Periodic observation, space acquirement, surgical exposure, orthodontic traction, orthodontic traction accompanied with surgical exposure, and surgical repositioning are considered as the treatment options of an impacted tooth, which is a form of eruption disturbance. In the first case, a male patient, age 9, visited Yonsei University Dental Hospital (YUDH) with a chief complaint of ectopic impaction of the upper left lateral incisor. We extracted the upper left primary lateral incisor and primary canine, and 5 months later, a window opening procedure was executed. Eight months later, the upper left lateral incisor partially erupted, and 18 months after the extraction, the axis of the tooth improved and the tooth erupted spontaneously. In the second case, a male patient, age 10, visited YUDH with a chief complaint of ectopic impaction of the upper right first premolar. We extracted the upper right first primary molar. Ten months later, the upper right first premolar erupted partially, and 19 months after the extraction, the upper right first premolar erupted spontaneously. We reported two cases in which improvement of eruption path and spontaneous eruption of an ectopic impacted tooth was achieved by extracting the deciduous tooth which interfered with the proper eruption of it.

Reproducibility and accuracy of tooth size measurements obtained by the use of computer (컴퓨터를 이용한 치아크기 계측시 재현도와 정확도에 관한 연구)

  • The purpose of this study was to evaluate the availability of computer system for the measurement of tooth size in the model analysis through the comparison of two measurements: One was to use a computer; and the other was to use vernier calipers. Twenty sets of casts were used, which showed a moderate degree of crowding and full eruption of all teeth. The mesio-distal width of 12 teeth from the left central incisor to the left first molar at each set of the casts were measured twice with vernier calipers and a computer respectively. This measurement was repeated two weeks later. First, for the reproducibility analysis, the two computer measurements were compared then the vernier calipers measurements were compared. Second, all the teeth were sepapated into the region of mesiodistal contact points and its width was measured by a micrometer to obtain standard measurements. For the accuracy analysis, these standard measurements were compared with the measurements from the dental casts using two methods. The difference between them was defined as the measurement error. To investigate the cause of measurement error, an examination was made for the presence and degree of contact point deviation on each tooth from the upper and lower occlusograms, and the mesio-distal angulation of each tooth was measured with TARG. Following results were obtained through statistical analysis. 1. In the analysis for reproducibility; the measurements with vernier calipers showed significant differences in three out of twelve teeth while the computer measurements showed significant differences in one out of twelve teeth. 2. In the analysis for accuracy; compared with the standard measurements, the measurements with vernier calipers showed significant differences in three out of twelve teeth while the computer measurements showed significant differences in two out of twelve teeth. 3. Compared with the standard measurements, the measurements with vernier calipers were apt to be larger at the upper first molar, and smaller at the lower first molar The computer measurements, however, were apt to be larger at both upper and lower first molars. 4. The measurements with vernier calipers showed the largest error at the lower first molar and the degree of error was variable according to the tooth while the difference of error was small in the computer measurements. 5. In the analysis for the correlation of the degree of measurement errors with the contact point deviation index and the mesio-distal crown angulation of each tooth, the measurements with vernier calipers did not show significant correlation while the measurements with computer showed slight Positive correlations. The results of this study indicate that a computer system may be useful for the measurement of tooth size in the model analysis.

THE COMPARATIVE STUDY ON THE COLOR OF THE DECIDUOUS TEETH AND RESTORATIVE MATERIALS (유치의 치아색과 수복재의 색조선택에 관한 비교연구)

  • The purpose of this study was to analyse the color of natural deciduous teeth in Korean children and to compare with that of composite resin specimens. The subjects were 148 children (80 boys and 68 girls) with good general condition and normal teeth color, aged between 3 and 6 years. The color of middle third of maxillary central incisor in deciduous teeth was examined with shade guide and then measured by means of the colorimeter CV300 which can be measured by CIELAB system. The data were analyzed statistically by SPSS program. The results were summerized as follows; 1. Over 90% of the color for the deciduous anterior teeth was in A1, A2, B1, B2 and P shade. 2. The means of deciduous teeth color were $L^*=58.72,\;a^*=-1.18,\;b^*=-0.63$ by colorimeter CV300. 3. $L^*,\;a^*\;and\;b^*$ prices for A1, A2, B1, B2, P were $L^*=52.52,\;a^*=-1.90,\;b^*=1.18$ in A1 specimen, $L^*=54.90,\;a^*=-1.87,\;b^*=1.60$ in A2 specimen, $L^*=59.80,\;a^*=-2.70,\;b^*=-0.63$ in B1 specimen, $L^*=56.90,\;a^*=-1.70,\;b^*=1.63$ in B2 specimen, $L^*=52.93,\;a^*=-2.33,\;b^*=1.10$ in P specimen. The means of B1 color specimen were most similar to those of deciduous teeth color. The A1 color values were similar to the P color values. 4. The standard deviation of $L^*,\;a^*$ was small among colors, but that of $b^*$, in the yellowish color, was large.
