Incidental traumatic right diaphragmatic rupture: a missed case after trauma

  • Traumatic diaphragmatic hernia is among the most uncommon conditions after severe trauma, and it is associated with high morbidity and mortality. The diagnosis is difficult and might be missed, but a multimodal investigation might help in terms of diagnostic yield. In this case report, we present a missed right diaphragmatic rupture 14 years after the trauma.

Prevalence of incidental maxillary sinus findings in Italian orthodontic patients: a retrospective cone-beam computed tomography study

  • Objectives: To determine the prevalence of incidental maxillary sinus findings in a large sample of orthodontic patients by cone-beam computed tomography (CBCT) with a wide field of view and assess the relationships of such abnormalities with age and gender. Methods: Five hundred thirteen CBCT scans obtained for orthodontic diagnosis and treatment planning in a Northern Italian population (N = 513; 292 female and 221 male subjects; 1,026 maxillary sinuses) were studied. The frequencies of pseudocysts and mucosal thickening of the maxillary sinus were recorded. Logistic regression analysis was used to determine the influence of age and gender on these abnormalities. Results: Pseudocysts were detected in 52 patients (10.1%) and 59 sinuses (5.75%). Mucosal thickening was observed in 206 patients (40.1%) and 258 sinuses (25.1%). Gender and age were significantly associated with pseudocysts (p = 0.027) and mucosal thickening (p < 0.001), respectively. Conclusions: Half of the orthodontic patients had incidental maxillary sinus findings. Men were more likely to show pseudocysts, and older patients (aged 41 - 60 years) were more likely to show mucosal thickening.

Incidental findings in a consecutive series of digital panoramic radiographs

  • Purpose: The aim of this study was to determine the prevalence of incidental findings(IFs) on digital dental panoramic radiographs(DPRs) of asymptomatic patients attending a general dental practice. Materials and Methods: This was a retrospective study of 6,252 consecutive digital (photostimulatable phosphor) DPRs of patients who visited a Canadian general dental practice for a complete new patient examination. The IFs were grouped into dental-related anomalies, radiopacities and radiopacities in the jaws, changes in the shape of the condyles, and other findings in the jaws, such as tonsilloliths and mucosal antral pseudocysts. Their prevalence was determined. Results: Thirty-two percent of the DPRs showed at least 1 IF. The highest prevalence was found for dental-related anomalies(29% of all DPRs), of which impacted teeth were the most prevalent finding (24% of all DPRs), followed by idiopathic osteosclerosis(6% of all DPRs). A lower prevalence was noted for tonsilloliths(3%), and the prevalence of root tips, mucosal antral pseudocysts, and anomalies in condylar shape was approximately 1% each. Conclusion: The observed prevalence of 32.1% for IFs of any type underscores the need for a dental practitioner to review the entire DPR when a patient presents for an initial dental examination (or check-up) or for dental hygiene. Only a single IF (a central giant cell granuloma) provoked alarm, as it was initially considered malignant. Similarly, impacted teeth and suspected cysts need careful evaluation upon discovery to determine how they may be optimally managed.

  • Nasopalatine duct cysts (NPDCs), the most common non-odontogenic cysts of maxilla, are often incidental findings on diagnostic imaging. When symptomatic, they usually present as a painless swelling with possible fistula. Conventional radiography shows a round-to-ovoid or heart-shaped radiolucency between the roots of central maxillary incisors. While the radiographic features of NPDCs in X-ray-based modalities have been well described, their magnetic resonance imaging (MRI) features have rarely been reported. Developments in dental MRI in recent years and the introduction of various dental MRI protocols now allow a wide range of applications in dental medicine. MRI is becoming an important tool for the detection and diagnosis of incidental or non-incidental dentomaxillofacial cysts. This report presented and discussed the characteristics of 2 NPDC cases visualized on MRI using both conventional and newly implemented specific dental MRI protocols with a novel 15-channel mandibular coil, demonstrating the use of these protocols for radiation-free maxillofacial diagnoses.

Accuracy of FDG-PET/CT for Detection of Incidental Pre-Malignant and Malignant Colonic Lesions - Correlation with Colonoscopic and Histopathologic Findings

  • Purpose: We evaluated all PET/CTs acquired for patients without a primary diagnosis of colorectal cancer, and compared results for those who had subsequent colonoscopy within 6 months, to assess the accuracy of FDG PET/CT for detection of incidental pre-malignant polyps and malignant colon cancers. Materials and Methods: Medical records of 9,545 patients who underwent F-18 FDG PET/CT studies over 3.5 years were retrospectively reviewed. Due to pre-existing diagnosis of colorectal cancer, 818 patients were excluded. Of the remainder, 157 patients had colonoscopy within 6 months (79 males; mean age 61). We divided the colon into 4 regions and compared PET/CT results for each region with colonoscopy and histopathologic findings. True positive lesions included colorectal cancer, villous adenoma, tubulovillous adenoma, tubular adenoma and serrated hyperplastic polyp/hyperplastic polyposis. Results: Of 157 patients, 44 had incidental colonic uptake on PET/CT (28%). Of those, 25 had true positive (TP) uptake, yielding a 48% positive predictive value (PPV); 9% (4/44) were adenocarcinoma. There were 23 false positive (FP) lesions of which 4 were hyperplastic polyp, one was juvenile polyp and 7 were explained by diverticulitis. Fifty eight patients had false negative PET scans but colonoscopy revealed true pre-malignant and malignant pathology, yielding 23% sensitivity. The specificity, negiative predictive value (NPV) and accuracy were 96%, 90% and 87%, respectively. The average SUVmax values of TP, FP and FN lesions were 7.25, 6.11 and 2.76, respectively. There were no significant difference between SUVmax of TP lesions and FP lesions (p>0.95) but significantly higher than in FN lesions (p<0.001). The average size (by histopathology and colonoscopy) of TP lesions was 18.1 mm, statistically different from that of FN lesions which was 5.9 mm (p<0.001). Fifty-one percent of FN lesions were smaller than 5 mm (29/57) and 88% smaller than 10 mm (50/57). Conclusions: The high positive predictive value of incidental focal colonic FDG uptake of 48% for colonic neoplasia suggests that colonoscopy follow-up is warranted with this finding. We observed a low sensitivity of standardly acquired FDG-PET/CT for detecting small polyps, especially those less than 5 mm. Clinician and radiologists should be aware of the high PPV of focal colonic uptake reflecting pre-malignant and malignant lesions, and the need for appropriate follow up.

  • The purpose of this study is to understand the special character and the tendency of the movement change that 12 meridians possess. We studied about the incidental and fundamental rule of the six atmospheric influence that was presented by Pyobonjoong(the incidental, fundamental, intermediate) theory, and the Hwang won-uh's the Six Channel activity of the Qi theory that presented movement form of the meridian through the six atmospheric influence. Then we found the following substances. The Pyobonjoong(the incidental, fundamental, intermediate) theory is applicable to understand the special character and the tendency of the movement change that Meridians possess. But, because the Pyobonjoong(the incidental, fundamental, intermediate) theory can't classify the characteristic difference of the hand and foot meridians, then the principle that can devide the meridians of the hand and foot, must be supplied. The Jungwha(right changing)-Daewha(opposite changing) theory is able to concretely classify the special character of the hand and foot six meridians. And Hwang's Shawha(superintending change)-Jongwha(following change) theory that is base on Jungwha(right changing)-Daewha(opposite changing) theory, is able to classify the special character of the hand and foot six meridians, too. If the concept of the Meridians is understood by the Shawha(superintending change)-Jongwha(following change) theory, the special character of the hand and foot six meridians could be concretely classified and then the meridian of the Bowels and the six atmospheric influence corresponded to the meridians could be expansively explained as the point that take charge and control these special Qi in the human body The Bon-Qi(fundamental Qi) act on the special character of Shawha(superintending change) six meridians and the tendency of the movement change is cause by the insufficiency and excessiveness of the Bon-Qi(fundamental Qi). The Qi of the Shawha(superintending change) meridians act on the special character of Jongwha(following change) six meridians and the tendency of the movement change is cause by the exuberance of Yang and the deficiency of Yang.

신임 소방대원의 직무관련 출동 스트레스와 대처 (Duty-related incidental stress and the coping method in new firefighters)

  • 연구 목적: 본 연구는 목적은 신임소방대원 직무 관련 출동 스트레스, 대처방법과 직무관련 출동 스트레스의 영향요인을 알아보고자 하는데 있다. 연구 방법: 직무관련 출동 스트레스원은 Beaton 등(1998)이 사용한 33개 문항의 VAS(Visual Analogue Scale)을 사용하여 측정하였고 대처 방식 척도는 이장호와 김정희(1988)의 62문항으로 구성된 질문지를 사용하였다. 수집된 자료는 SPSS WIN 14.0 program을 이용하여 t -test와 ANOVA, Pearson correlation coefficient과 다중 회귀분석을 실시하였다. 연구 결과: 잘못된 신고로 인한 출동이 가장 빈번하게 노출되는 스트레스원(95건, 71.4%)이고 이때 가장 많은 스트레스(3.84)를 받는 것으로 나타났다. 직무관련 출동 스트레스는 적극적 대처(r = .420, p < .001)와 소극적 대처(r = .450, p < .001)에서, 적극적 대처는 소극적 대처(r = .890, p < .001)에서 통계적으로 유의한 강한 상관관계를 나타났다. 직무관련 출동 스트레스 영향요인은 수동적 대처(t = 2.12, p < .05), 동료가 부상이나 죽음위험을 경험을 한 경우(t = 3.30, p < .001), 근무기간이 6개월 이내(t = -2.30, p < .05), 본인이 부상이나 죽음경험을 한 경우(t = 2.05, p < .05)로 나타났다. 결 론: 외상 후 스트레스 장애로 진행되지 않도록 적극적 대처에 대한 교육과 중재의 제도개선이 요구되고 사회적 지지와 지속적인 관심이 필요하다.

How Derivational Prefix Instruction Impacts Incidental Vocabulary Acquisition and Reading Comprehension

  • The study examined the effects of explicit derivational morphology instruction (henceforth DMI) on the incidental vocabulary acquisition and reading comprehension of 132 Korean 1st-year high school students who responded to a battery of tests (two vocabulary tests and a reading comprehension test). Multiple statistical tools were used to analyze the data: Analysis of Covariance (ANCOVA), Analysis of Variance (ANOVA), Simple Regression Analysis, Tests of Simple Main Effects, and effect size computation using Cohen's d. The results indicated that (a) DMI enhanced students' ability to infer word meanings in context, (b) DMI promoted high proficiency students' reading comprehension, whereas it impeded intermediate proficiency students' reading comprehension, (c) vocabulary knowledge has a strong positive predictive value for reading comprehension, and (d) the gaps of vocabulary knowledge across proficiency levels were still substantial, despite the observation that DMI promoted students' vocabulary acquisition. These results have a bearing on English as Foreign Language (EFL) reading pedagogy.
