Carcinoid-type tumorlets of the lung are nodular microscopic proliferation of round and spindle-shaped small cells which originated from bronchial or bronchiolar Kulchitsky-type neuroendocrine cells, which are usually encountered as an incidental finding during microscopic examination of the lungs at autopsy or surgically removed for bronchiectasis or other reasons. We report one case of carcinoid-type tumorlets in the lung which was surgically removed from a patient who had bronchiectasis, and the cells of tumorlets showed immunohistochemical reactivities for markers of epithelial and neuroendocrine differentiation.
Probability based design(PBD)method has some advantages against current design methods. First, it can provide the quantitative values for the structural safety or capacity through the reliability index, $^{\beta}$. That presented the certainty on the corresponding structure for the designer or user, also that permitted the broad consideration in the safety of structures. In addition, it can give the quantitative lifetime of the related structure in the calculation process of target reliability index. Also, incidental economical efficiency can be expected because decrease of required structural material can be obtained by using the practical material data. Unlikely current deterministic structural design methods, main advantage is the reflection of real condition in the structural design process by application of the data with not small clear specimen but structural size material. Advanced countries, namely America, Canada, Europe, Australia and New Zealand already converted from allowable stress design(ASD) method to PBD method and used as a standard wooden structures code in the late 1980s and 1990s. Other domestic constructions standards such as the steel or concrete constructions accepted and used the PBD methods already. Accordingly, wooden structural design method also should be converted from deterministic ASD to probabilistic LRFD(Load and resistance factor design) in order to keep pace with worldwide demands for PBD. Hence, to suggest the reason of introduction the PBD in domestic wooden structural design and analysis, a brief example was used to show the different reliability index by using the different design methods. Definition, merits and demerits of deterministic ASD and probabilistic LRFD were followed. Also the three examples were presented to show the similarity and differences between ASD and LRFD. Finally, connection problems that might cause a disputation in wooden structural design and analysis were broadly examined.

Emerging applications with high data rates will need to transport bulk data reliably in wireless sensor networks. ARQ (Automatic Repeat request) or Forward Error Correction (FEC) code schemes can be used to provide reliable transmission in a sensor network. However, the naive ARQ approach drops the whole frame, even though there is a bit error in the frame and the FEC at the bit level scheme may require a highly complex method to adjust the amount of FEC redundancy. We propose a bulk data transmission scheme based on erasure-resilient code in this paper to overcome these inefficiencies. The sender fragments bulk data into many small blocks, encodes the blocks with LT codes and packages several such blocks into a frame. The receiver only drops the corrupted blocks (compared to the entire frame) and the original data can be reconstructed if sufficient error-free blocks are received. An incidental benefit is that the frame error rate (FER) becomes irrelevant to frame size (error recovery). A frame can therefore be sufficiently large to provide high utilization of the wireless channel bandwidth without sacrificing the effectiveness of error recovery. The scheme has been implemented as a new data link layer in TinyOS, and evaluated through experiments in a testbed of Zigbex motes. Results show single hop transmission throughput can be improved by at least 20% under typical wireless channel conditions. It also reduces the transmission time of a reasonable range of size files by more than 30%, compared to a frame ARQ scheme. The total number of bytes sent by all nodes in the multi-hop communication is reduced by more than 60% compared to the frame ARQ scheme.
Calcifying fibrous tumors (CFTs) are unusual benign tumors of childhood, located primarily in soft tissues, pleura, and peritoneum. The cause and pathogenesis are unclear. We report a rare case of a CFT in a 2-year-old boy who presented with vomiting and abdominal distension. An abdominal X-ray showed an elliptical, calcific shadow in the LUQ area mimicking a foreign body. An internally protruding mass along the lesser curvature of the gastric body was an incidental finding during upper endoscopy, biopsies of which were negative. Abdominal CT showed a 4.5${\times}$3.2 cm soft tissue mass of the gastric wall with calcifications. A diagnosis of gastric submucosal mass was suspected and a wedge resection of the stomach was performed. On microscopic examination, the tumor was composed of whorls of dense hyalinized collagen bundles with a few fibroblasts. There were also amorphous dystrophic calcifications and nodular aggregates of mononuclear inflammatory cells. Immunohistochemically, spindle cells did not stain for anaplastic lymphoma kinase-1 (ALK-1), CK, smooth muscle actin (SMA), or desmin. Taken together, the mass was compatible with a CFT of the gastric wall. This is the first reported case of CFT in a Korean child.

In this paper flood discharge calculated by Clark watershed method and SCS synthetic unit hydrograph method about upside during each minute of among time distribution method of rainfall, Huff method choosing Bocheong Stream basin that is representative basin of International Hydrologic Project (IHP) about time distribution of rainfall that exert big effect at flood discharge estimate to research target basin because of and the result is as following. Relation between probability flood discharge that is calculated through frequency analysis about flood discharge data and rainfall - runoff that is calculated through outward flow model was assumed about $48.1{\sim}95.9%$ in the case of $55.8{\sim}104.0%$, SCS synthetic unit hydrograph method in case of Clark watershed method, and Clark watershed method has big value overly in case of than SCS synthetic unit hydrograph method in case of basin that see, but branch of except appeared little more similarly with frequency flood discharge that calculate using survey data. The purpose of current study was to investigate the effects of Ki-therapy which improves mood state and relieves pain on the elderly with chronic illness. The subjects were 42 elderly and convenience sampling(incidental sampling) was used to prevent contamination to the control group. Frequency, %, $X^2$, t-test, and repeated measure ANOVA by SPSS PC + program were used to analyze the data. The mean age of subjects was 72.95(Control group), 73.10(experimental group). Number of complains was 2.45(control group), 2.65 (experimental group). All of demographic characteristics of subjects. that is, age, weight. number of complains. educational background. family type. economic state, perceived health state were homogeneous. There are many difficulties in ADL & IADL. 14.2% in shopping, mode of transportation, ability to handle finances. 11.9% in ability to use telephone. continence, 7.1% in bathing, food preparation, transfer, and housekeeping. As the result of this study. slight improvement of mood state and pain relief were found. Although 3 improved items of 'clear-headed', 'lively', and 'nervous' were statistically significant. the pain relief was not statistically significant. It requires repeated treatment and measurement. And suggested case study or qualitative study in further studies because manufacturing of environment(i. e. rain) is very difficult in community people.
Objective : The present study evaluated overall surgical results for 3,000 patients with intracranial aneurysms, operated on in Busan Paik Hospital institution. Methods : Three thousand aneurysm cases, operated on in Busan Paik Hospital between January 1980 to June, 15th, 2005, were evaluated based on the following criteria; aneurysm form, aneurysm location, surgical results, postoperative complications, and seasonsonality of occuence. 957 cases were anterior communicating artery aneurysms, 776 were internal carotid artery[ICA] aneurysms, 755 were middle cerebral artery[MCA] aneurysms, 96 were anterior cerebral artery[ACA] aneurysms, 128 were vertebro-basilar artery[VBA] aneurysms and 288 were multiple aneurysms. The male to female ratio was 0.7 to 1 Surgical methods included 2.738 clippings, 219 coating and wrappings, 23 aneurysmoraphies, 20 proximal ligations. Results : Rebleeding occured in 5.1% of the early operation group and 16% of the late operation group respectively. Incidence of clinical vasospasm was 166% and angiographic vasospasm was 24.1%. The percentage of the multiple aneurysms was 9.5%, the percentage of the dissecting aneurysm was 6 cases [0.2%], 6 of the total [0.2%];De Novo" aneurysm, the percentage of lobectomies with clipping cases was 9 cases [03%] the percentage were incidental aneurysms; 164 [5.5%]. 88.1% had overall favorable surgical results with a 5.5 % mortality rate. Calcium-channel blocker and "Triple H" therapy did not improve mortality but did significantly improve morbidity. In the old age group, early operation reduced vasospasm, rebleeding and medical complications. The early surgery group exhibited a 86.2% favorable outcome with a 8.1% mortality rate. Intraoperative angiography reduced residual or remained aneurysms in large, giant aneurysm, especially in A.com artery aneurysm. Conclusion : The surgical results for the early surgery group according to surgical timming was better, but there were not statistically significant. ntraoperative angiography was especially useful on large aneurysms of the anterior communicating artery.
Objective : We investigated the incidence of the vascular abnormalities associated with spontaneous intracerebral hemorrhage [ICH] using three-dimensional computed tomographic angiography [3D-CTA]. Methods : We prospectively assessed consecutive 76 patients with spontaneous intracerebral hemorrhage [ICH] who underwent 3D-CTA between June 2003 and May 2005. The patients with a recent history of trauma or mainly subarachnoid hemorrhage were excluded. We investigated relationship between vascular abnormality and ICH location. The findings of 3D-CTA were classified as one of three patterns with ICH; type A [without evidence of vascular abnormality], type B [with no vascular abnormality as the source of hemorrhage, but with incidental vascular abnormality], and type C [presence of a vascular abnormality as the source of hemorrhage]. Results : Sites of ICH were lobar 26, basal ganglia 23, thalamus 17, posterior fossa 6 and dominant intraventricular hemorrhage [IVH] 4. Among 76 patients, sixteen [21.1%] vascular abnormalities were noted excluding 13 cases of stenoocclusive disease. Sixteen cases included 6 cases of cerebral aneurysms [7.9%], 5 moyamoya diseases [6.6%], 4 arteriovenous malformations [5.3%] and 1 dural sinus thrombosis [1.3%]. Lobar ICH [30.8%] had a higher vascular abnormalities than other types, and younger age [<40] group had a higher incidence of vascular abnormalities than old age group. The patterns of 3D-CTA include sixty cases [79.0%] of type A, 6 cases [7.8%] of type Band 10 cases [13.2%] of type C. The vascular abnormalities were found in 8 [13.5%] of 59 hypertensive patients and 8 [47.0%] of 17 non-hypertensive patients [p=0.006]. Conclusion : 3D-CT angiography is considered a useful screening tool for ICH patients with suspected cerebrovascular abnormalities and should be considered in such clinical settings, especially in lobar type and in non-hypertensive younger patients.
Objective : Persistent trigeminal artery variant (PTAV) is an anastomosis between the internal carotid artery (ICA) and the cerebellar artery without any interposing basilar artery segment. We discuss its probable embryological origin and emphasize clinical implications. Methods : Retrospectively 1250 conventional cerebral angiograms and 2947 cranial magnetic resonance angiographies (MRAs) were evaluated for the patients with PTAV. Results : Five patients (four men and one woman, 23 to 76 years of age, median age 65 years) had a PTAV. Three patients who underwent MRA had a PTAV (3/2947=0.1%). Four of the patients who underwent cerebral angiography had a PTAV (4/1250=0.32%). Two of 143 patients who underwent both conventional angiography and cranial MRA showed PTAV. The PTAV was an incidental finding in all five patients. The PTAV originated from the cavernous segment of the left ICA in four patients and from the cavernous segment of the right ICA in one patient. The terminal branch of the PTAV was the anterior inferior cerebellar artery (AICA) and superior cerebellar artery (SCA) in two patients and the AICA only in the other three patients. Conclusion : Neurosurgeons should be aware of possible presence of PTAV. Manipulation of this vessel during a surgical approach to the parasellar region and percutaneous gasserian ganglion procedure may result in hemorrhage or ischemia.
Background: Gastric cancer is the second most common gastrointestinal cancer and is largely attributed to Helicobacter pylori (H. pylori) infection. In addition, studies have also shown association with Epstein-Barr virus (EBV) in 10% of gastric cancers. This study assessed the characteristics of EBV associated gastric cancers (EBVaGC) in Brunei Darussalam. Materials and Methods: This study included gastric cancers diagnosed between 2008 and 2012, registered with the Department of Pathology RIPAS Hospital, Brunei Darussalam. Clinical case notes were systematically reviewed. Histology specimens were all stained for EBV and also assessed for intestinal metaplasia and H. pylori. Results: There were a total of 81 patients (54 male and 27 females) with a mean age of $65.8{\pm}14.8years$ included in the study. Intestinal metaplasia and active H. pylori infection were detected in 40.7% and 30.9% respectively. A majority of the tumors were proximally located (55.6%), most poorly differentiated (well differentiated 16%, moderately differentiated 30.9% and poorly differentiated 53.1%) and the stages at diagnosis were; stage I (44.4%), stage II (23.5%), stage III (8.6%) and stage IV (23.5%). EBV positivity (EBVaGC) was seen in 30.9%. Between EBVaGC and EBV negative gastric cancers, there were no significant differences (age, gender, ethnic group, presence of Intestinal metaplasia, tumor locations, stages of disease and degree of tumor differentiation). Conclusions: This study showed that a third of gastric cancers in Brunei Darussalam were positive for EBV, higher than what have been reported in the literature. However, there were no significant differences between EBVaGC and EBV negative gastric cancers. This suggests that the role of EBV in gastric cancer may be mostly incidental rather than any causal relation. However, further studies are required.
