This study was performed to assess the analyzing methods developed to detect clinically and quantitatively longitudinal bone changes. Through preliminary experiment, accuracy of Cu-Eq value conversion to the mass of HA was examined. For main experiment, 15 intraoral radiograms taken at soon, 1st, 2nd, 4th, and 6th week after implantation of mixture in extracted sites of 3 cases were used. We took the radiograms with copper step wedge as test object and HA phantom X -ray taking was standardized by using Rinn XCP device customized directly to the individual dentition with resin bite block. The images inputted by Quick scanner into computer were digitized and analyzed by NIH image program. The stability of the copper equivalent transformation and the usefulness of two analyzing methods by ROI and Reslice were examined. Obtained results as follows: 1) On the Cu equivalent images, the coefficient of variation in the measurement of Cu-Eq. value of ROI ranged from 0.05 to 0.24 and showed high reproducibility. 2) All results obtained by resliced contiguous image were coincident with those obtained from the assessment by ROI and formation of plot profile. 3) On the stacked and resliced image at the line of interest, we could analyze directly and quantitatively the longitudinal changes at several portions by plot profile and qualitatively by surface plot. 4) Implant area showed marked resorption till 2 weeks after implantation and showed significant increase in Cu-Eq. value at 6th week(p<0.01) and periapical area showed increase in Cu-Eq. value at 6th week compared to after-operation's.
The purpose of this study is to evaluate histologically the resorption and tissue response of various resorbale membranes used for guided tissue regneration procedures, using a subcutaneous model on the dorsal surface of the rat. In this study, 12 Sprague-Dawley male rats(mean BW 150gm) were used and the commercially available materials included dense collagen membrane, freeze-dried bovine dura mater loos collagen membrane, PLA/PLGA membrane. Animals were sacrificed at 3, 6 and 8 weeks after implantation of various resorbable membranes. Specimens were prepared with Hematoxylin-Eosin stain for light microscopic evaluation. The results of this study were as follows: 1. Resorption : Loose collagen membrane group was resorbed most rapidly. Dense collagen membrane group and freeze-dried bovine dura mater group were rarely resorbed. 2. Inflammatory reactions : PLA/PLGA membrane group showed persistent and severe inflammatory reactions for 3 to 8 weeks. Moderate inflammatory reactions and the ectopic formation of calcified material were observed in dense collagen membrane group. Freeze-dried bovine dura mater group and loose collagen membrane group showed mild inflammatory reactions 3. In PLA/PLGA membrane group, multinucleated giant cells by foreign body reactions were observed. In conclusion, the resorption of freeze-dried bovine dura mater didn't happen for 3-6weeks, which showed the best bio-compatibility. Therefore, freeze-dried bovine dura mater was considered proper resorbable membrane for guided tissue regeneration.
Purpose: The aim of this study was to characterize the healing in the grafted calvarial defects of rats after adjunctive hyperbaric oxygen therapy. Methods: Twenty-eight male Sprague-Dawley rats (body weight, 250-300 g) were randomly divided into two treatment groups: with hyperbaric oxygen therapy (HBO; n=14) and without HBO (NHBO; n=14). Each group was further subdivided according to the bone substitute applied: biphasic calcium phosphate (BCP; n=7) and surface-modified BCP (mBCP; n=7). The mBCP comprised BCP coated with Escherichia-coli-derived recombinant human bone morphogenetic protein-2 (ErhBMP-2) and epigallocatechin-3-gallate (EGCG). Two symmetrical circular defects (6-mm diameter) were created in the right and left parietal bones of each animal. One defect was assigned as a control defect and received no bone substitute, while the other defect was filled with either BCP or mBCP. The animals were allowed to heal for 4 weeks, during which those in the HBO group underwent 5 sessions of HBO. At 4 weeks, the animals were sacrificed, and the defects were harvested for histologic and histomorphometric analysis. Results: Well-maintained space was found in the grafted groups. Woven bone connected to and away from the defect margin was formed. More angiogenesis was found with HBO and EGCG/BMP-2 (P<0.05). None of the defects achieved complete defect closure. Increased new bone formation with HBO or EGCG/BMP-2 was evident in histologic evaluation, but it did not reach statistical significance in histometric analysis. A synergic effect between HBO and EGCG/BMP-2 was not found. Conclusions: Within the limitations of this study, the present findings indicate that adjunctive HBO and EGCG/BMP-2 could be beneficial for new bone formation in rat calvarial defects.

Purpose: The aim of this study was to investigate the effects of synthetic fibronectin (FN) fragments, including fibrin binding sites from amino-terminal FN fragments containing type I repeats 1 to 5, on osteoblast-like cell activity. Methods: Oligopeptides ranging from 9 to 20 amino acids, designated FF1, FF3, and FF5, were synthesized by a solid-phase peptide synthesizing system, and we investigated the effects of these peptides on cell attachment and extent of mineralization using confocal microscopy, 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assays, and Alizarin red S staining. Results: FF3 and FF5 peptides increased the number of attached human osteoblastic cells, and FF3 administration led to prominent cell spreading. Mineralization was increased in FF3 and FF5 compared to FF1 and the untreated control. Conclusions: Taken together, it can be concluded that the fibrin-binding oligopeptides FF3 and FF5 enhanced cell attachment and mineralization on osteoblast-like cells. These results indicate that FF3 and FF5 have the potential to increase osteoblast-like cell activity. Performing an in vivo study may provide further possibilities for surface modification of biomimetic peptides to enhance osteogenesis, thus improving the regeneration of destroyed alveolar bone.
Purpose: Cytolethal distending toxin (CDT) is a family of heat-labile cytotoxins produced by several gram-negative mucosa-associated pathogens, including Aggregatibacter actinomycetemcomitans. CDT is well known to be capable of inducing growth arrest, morphological alterations, and eventually death in various cells. CDT belongs to a tripartite $AB_2$ toxin (CdtB: the enzymatic A subunit; CdtA and CdtC: the heterodimeric B subunit). Previous studies proposed that CdtA and CdtC together bind to a cell surface receptor and glycolipids act as a receptor for A. actinomycetemcomitans CDT (AaCDT). In this study, recombinant CdtA and CdtC proteins of AaCDT were co-expressed in a bacterial expression system and tested for their affinity for $GM_1$ ganglioside. Methods: The genes for CdtA and CdtC from A. actinomycetemcomitans Y4 were utilized to construct the expression vectors, pRSET-cdtA and pET28a-cdtC. Both CdtA and CdtC proteins were expressed in Escherichia coli BL21(DE3) and then purified using hexahistidine (His6) tag. The identity of purified protein was confirmed by anti-His6 antibody and monoclonal anti-CdtA antibody. Furthermore, the affinity of recombinant protein to $GM_1$ ganglioside was checked through ELISA. Results: Recombinant CdtA and CdtC proteins were expressed as soluble proteins and reacted to anti-His6 and monoclonal anti-CdtA antibodies. ELISA revealed that purified soluble CdtA-CdtC protein bound to $GM_1$ ganglioside, while CdtA alone did not. Conclusions: Co-expression of CdtA and CdtC proteins enhanced the solubility of the proteins in E. coli, leading to convenient preparation of active CdtA-CdtC, a critical material for the study of AaCDT pathogenesis.
Objective: To examine the effect of bite force on the displacement and stress distribution of orthodontic mini-implants (OMIs) in the molar region according to placement site, insertion angle, and loading direction. Methods: Five finite element models were created using micro-computed tomography (microCT) images of the maxilla and mandible. OMIs were placed at one maxillary and two mandibular positions: between the maxillary second premolar and first molar, between the mandibular second premolar and first molar, and between the mandibular first and second molars. The OMIs were inserted at angles of $45^{\circ}$ and $90^{\circ}$ to the buccal surface of the cortical bone. A bite force of 25 kg was applied to the 10 occlusal contact points of the second premolar, first molar, and second molar. The loading directions were $0^{\circ}$, $5^{\circ}$, and $10^{\circ}$ to the long axis of the tooth. Results: With regard to placement site, the displacement and stress were greatest for the OMI placed between the mandibular first molar and second molar, and smallest for the OMI placed between the maxillary second premolar and first molar. In the mandibular molar region, the angled OMI showed slightly less displacement than the OMI placed at $90^{\circ}$. The maximum Von Mises stress increased with the inclination of the loading direction. Conclusions: These results suggest that placement of OMIs between the second premolar and first molar at $45^{\circ}$ to the cortical bone reduces the effect of bite force on OMIs.
The treatment of skeletal Class III malocclusion in adolescents is challenging. Maxillary protraction, particularly that using bone anchorage, has been proven to be an effective method for the stimulation of maxillary growth. However, the conventional procedure, which involves the surgical implantation of mini-plates, is traumatic and associated with a high risk. Three-dimensional (3D) digital technology offers the possibility of individualized treatment. Customized mini-plates can be designed according to the shape of the maxillary surface and the positions of the roots on cone-beam computed tomography scans; this reduces both the surgical risk and patient trauma. Here we report a case involving a 12-year-old adolescent girl with skeletal Class III malocclusion and midface deficiency that was treated in two phases. In phase 1, rapid maxillary expansion and protraction were performed using 3D-printed mini-plates for anchorage. The mini-plates exhibited better adaptation to the bone contour, and titanium screw implantation was safer because of the customized design. The orthopedic force applied to each mini-plate was approximately 400-500 g, and the plates remained stable during the maxillary protraction process, which exhibited efficacious orthopedic effects and significantly improved the facial profile and esthetics. In phase 2, fixed appliances were used for alignment and leveling of the maxillary and mandibular dentitions. The complete two-phase treatment lasted for 24 months. After 48 months of retention, the treatment outcomes remained stable.
The selective migration, attachment and proliferation of periodontal ligament cells are the desired goal of periodontal regeneration therapy. Fibronectin is well known for an attachment protein for dentin surface. Also, Fibroblast growth factor (FGF) is well known to enhance the periodontal regeneration. The purpose of this study was to evaluation the effect of fibronection and FGF on the attachment rate and the cellular activity. Human gingival fibroblast and periodontal ligament cells were cultured from the teeth extracted for non-periodontal reson. Cultured human gingival fibroblast and periodontal ligament cells in vitro were treated with fibronectin and FGF a various dosage and culture times. Cellular activity was examined by MTT assay. The results of this study was demonstrated that cell attachment rate of experimental group was under the control value at 1st, 2nd, 3rd incubation day. But, at 3rd incubation day, attchment value tended to return to the control value. In case of fibronectin alone application, cellular activity was decreased than that of control at 1st, 2nd incubation day. But 3rd day, cellular activity was returned to the control value. The activity of gingival fibroblast in FGF alone application was decreased thatn that of control at each incubation day. But activity of periodontal cell group was increased cell activities at 2nd, 3rd day. Additionally cellular activity of fibronectin & FGF combined application on gingival fibroblast group was similar to control value at incubation day. But activity of periodontal ligament cell group was increased at 2nd, 3rd day compared with control group.This study demonstrated that combined application of fibronectin & FGF induced the selective chemotaxis for periodontal ligament cell in vitro.
The purpose of this study was to observe the effect of $TGF-{\beta}1$ on the regeneration of bone in guided bone regeneration. Four adult dogs aged 12 to 24 months were used in this study. Experimental bone defects were created surgically with surgical bur and chisel on the 3th. premolars. In experimental group, bone defect were grafted with DFDB and $TGF-{\beta}1$. In control groups, bone defects were grafted with only DFDB. At 1,2,3 and 4 weeks, dogs were serially sacrificed and specimens were prepared with Hematoxylin-Eosin stain and Goldner's stain for light microsopic evaluation. The results of this study were as follows: 1. The infiltration of inflammatory cells was prominent in control groups at 1, 2 and 3 weeks. 2. The lining of osteoblast was observed at 2 weeks in control group, but at 1 week in experimental group. 3. In both groups, osteoid was formed at 2 weeks. In control groups, osteoid was fromed on only bone surface. but in experimental groups, osteoid were formed on both bone & DFDB surfaces. 4. In only experimental groups, The fusion of new bone & DFDB was only observed at 3 weeks. and the fusion of new bone & DFDG was more prominent at 4 weeks. But in control groups, No fusion of new bone& DFDB was oberved at 3 and 4weeks. From the above result, the $TGF-{\beta}1$ was effective in bone formation and increased inductive effect of DFDB in guided bone regeneration technique. Inductive effect of DFDB was increased with $TGF-{\beta}1$.
연구 목적: 3개월 간의 잇솔질에 따른 칫솔의 마모의 정도와 양상을 관찰하고, 이를 이용한 잇솔질 시의 치태 제거능력의 변화를 평가하여 일반적으로 권장되고 있는 3개월 주기의 칫솔 교체 주기의 근거를 실제 임상적으로 확인해보고자 하였다. 연구방법: 치주적으로 건강한 치과 대학생 52명을 대상으로 설문지를 통해 잇솔질 습관을 조사하고, 치석제거술을 시행하고 실험기간 동안 동일한 칫솔과 치약을 사용하게 하였다. 1주일 후와 2달과 3달째에 구강 내를 erythrosine으로 염색한 후 6개의 Ramfjord 치아의 plaque score를 측정하고, 3달 동안 사용한 칫솔을 수거하여 brushing surface area의 면적으로 마모도를 평가하였다. 결과: 6명이 탈락하였고 전체 부위의 plaque score는 1주 때와 비교하여 2,3 개월의 값이 통계학적으로 유의한 차이가 없었으나, 치간 부위의 plaque score는 3개월의 plaque score와 상관관계는 약하였다. 교체 주기, 잇솔질 횟수, 시간 등의 잇솔질 습관과 마모도의 연관성도 없었지만, 잇솔질 시간과 3개월째의 plaque score의 상관관계는 유의성 있는 결과를 보였다. 결론: 3개월 동안 사용한 칫솔의 치간 부위 치태제거 능력은 감소하였으며, 이의 임상적 영향에 대한 장기간의 연구가 필요하겠다.
