Purpose: Surface modification is important techniques in modern dental and orthopedic implants. This study was performed to try embedding of bioactive materials in porous Ti implants. Methods: Porous Ti implant samples were fabricated by sintering of spherical Ti powders in a high vacuum furnace. It's diameter and height were 4mm and 20mm. Embedding process was used to suction and vacuum chamber. Loading properties of porous Ti implants were evaluated by scanning electron microscope(SEM), confocal laser scanning microscope(CLSM), and UV-Vis-NIR spectrophotometer. Results: Internal pore structure was formed fully open pore. Average pore size and porosity were $10.253{\mu}m$ and 17.506%. Conclusion: Porous Ti implant was fabricated successfully by sintering method. Particles are necking strongly each other and others portions were vacancy. This porous structure can be embedded to bioactive materials. Therefore bioactive materials will be able to embedding to porous Ti implants. Bioactive materials embedding in the porous Ti implant will induced new bone faster.
Purpose: An implant-supported prosthesis consists of an implant fixture, an abutment, an internal screw that connects the abutment to the implant fixture, and the upper prosthesis. Numerous studies have investigated the microorganisms present on the implant surface, surrounding tissues, and the subgingival microflora associated with peri-implantitis. However, there is limited information regarding the microbiome within the internal screw space. In this study, microbial samples were collected from the supragingival surfaces of natural teeth, the peri-implant sulcus, and the implant-abutment screw hole, in order to characterize the microbiome of the internal screw space in healthy subjects. Methods: Samples were obtained from the supragingival region of natural teeth, the peri-implant sulcus, and the implant screw hole in 20 healthy subjects. DNA was extracted, and the V3-V4 region of the 16S ribosomal RNA was sequenced for microbiome analysis. Alpha diversity, beta diversity, linear discriminant analysis effect size (LEfSe), and network analysis were employed to compare the characteristics of the microbiomes. Results: We observed significant differences in beta diversity among the samples. Upon analyzing the significant taxa using LEfSe, the microbial composition of the implant-abutment screw hole's microbiome was found to be similar to that of the other sampling sites' microbiomes. Moreover, the microbiome network analysis revealed a unique network complexity in samples obtained from the implant screw hole compared to those from the other sampling sites. Conclusions: The bacterial composition of the biofilm collected from the implant-abutment screw hole exhibited significant differences compared to the supra-structure of the implant. Therefore, long-term monitoring and management of not only the peri-implant tissue but also the implant screw are necessary.
마이크로 임플란트의 식립 혹은 제거 시의 토오크는 안정성과 기계적 파절 가능성에 대한 척도가 되므로 골조건 및 임플란트의 디자인과 연계한 생역학적 분석을 통해 이를 해석적으로 예측하는 수단을 갖는 것이 의미 있는 과제라 사료된다. 이에 본 연구에서는, 국산 Absoanchor (Dentos Inc. Daegu, Korea) 마이크로 임플란트 모델을 대상으로 하여 기본적 디자인 변수(design parameter)인 길이, 직경, 나사산 크기와 토오크 및 임플란트의 비틀림 파절강도와의 역학적 상관관계에 대해 해석하였으며, 골특성이 미치는 영향을 고려하기 위해서 임플란트/해면골 계면의 저항력$(S_{can})$을 $1.0{\sim}2.5MPa$ 범위로 설정하였다. 분석을 위해 임플란트 디자인 변수들만의 함수인 토오크 지수와 비틀림 파절 강도의 지표인 강도 지수를 정의하였다. 분석결과, 임플란트 직경이 증가할수록 토오크 지수가 증가하지만 강도 지수가 토오크 지수보다 증가율이 커서 비틀림 파절에 대한 안전영역이 증가하였으며, 나사산 높이가 커지면 토오크 지수가 증가하지만 강도 지수는 감소되는 것을 관찰하였다. Absoanchor 마이크로 임플란트는 해면골과 계면에서의 저항력$(S_{can})$이 1.0 MPa 이하에서는 전 모델이 토오크에 의한 비틀림 파절로부터 안전한 것으로 분석되었으나 $(S_{can})$이 증가하면 일부 모델에서는 파절 위험성이 있는 것으로 분석되었다. $S_{can}$이 1.5 MPa수준에서는 경부직경이 1.5 mm 이상이면 전체 길이$(5{\sim}12mm)$에서 안전하였다. 또한, $S_{can}$이 2.0 MPa 수준에서 식립깊이 8 mm를 가지려면 경부직경이 1.5 mm 이상인 모델을 선정하는 것이 안전한 것으로 분석되었다. 항불안을 측정하는 EPM 실험에서도 1회나 2주간 SCE 10mg/kg과 100mg/kg을 투여 받은 생쥐에서 생리 식염수만 투여 받은 생쥐보다 open arms에서 머무는 시간이 유의하게 증가되어 SCE가 단기 또는 장기간의 항불안에 효과가 있음을 나타냈다. 시험관내 실험 결과에서는, SCE와 SCE-40이 SERT, NET, 그리고 GABA ligand의 결합 억제능이 있음이 확인되었고, SCE와 SCE-40은 serotonin(5-hydroxytryptamine: 5-HT)과, norepinephrine의 uptake를 억제하는 것으로 나타났다. 그러므로 본 연구는 효모 추출물(SCE)과 그 분획(SCE-40)이 행동적, 신경학적으로 항우울, 항불안에 효과가 있음을 확인하였으며, 효모 추출물(SCE)과 그 분획(SCE-40)이 안전한 천연식품으로서 우울증, 불안증 등의 관련 질환의 예방, 치료용 의약품 개발과 기능성 식품에 효과적으로 이용될 수 있음을 시사한다.tall fescue 23%, Kentucky bluegrass 6%, perennial ryegrass 8%) 및 white clover 23%를 유지하였다. 이상의 결과를 종합할 때, 초종과 파종비율에 따른 혼파초지의 건물수량과 사료가치의 차이를 확인할 수 있었으며, 레드 클로버 + 혼파 초지가 건물수량과 사료가치를 높이는데 효과적이었다.\ell}$ 이었으며 , yeast extract 첨가(添加)하여 배양시(培養時)는 yeast extract 농도(濃度)가 증가(增加)함에 따라 단백질(蛋白質) 함량(含量)도 증가(增加)하였다. 7. 연구목적: 연조직 펀치를 이용한 무피판 임플란트 수술법은 임플란트를 식립할 부위에 환상형의 절개를 필요로 한다. 이에 연조직 펀치의 사용을 위한 지침을 만드는 데 도움을 주고자 성견의 하악에서 임플란트 주위 조직의 치유에 대한 연조직 펀치의 영향을 조사하는 실험이 수행되었다. 연구 재료 및 방법: 6 마리의 성견에서 하악의 양측에 무치악의 평편한 치조제를 만들었다. 3 개월 동안의 치유 기간 후, 3 mm, 4 mm, 5 mm 연조직 펀치를 사용하여 절개하고 양측에 각각 3개의 임플란트 (직경 4.0 mm)를 식립하였다. 그리고 즉시 치유지대주 (직경 4.5 mm)를 연결하였다. 그 후 치유 기간 동안 임플란트 주위 점막을 임상적, 방사선학적 그리고 조직학적 방법으로 평가하였다. 평가 요소는 치은 지수, 탐침시 출혈, 치주 탐침 깊이, 변연골 상실 그리고 임플란트 주위 조직의 수직적인 변화이다. 결과: 임플란트 식립 후 치유 기간 동안에 3 mm, 4 mm, 5 mm 연조직 펀치 군 사이에 접합 상피의 길이, 치주 탐침 깊이, 변연골 상실이 유의할 만한 차이가 관찰되었다 (P <.05). 3 mm 연조직 펀치를 사용한 경우가 4 mm 이상의 연조직 펀치를 사용한 것에 비해 상대적으로 접합 상피의 깊이는 짧았고, 치주 탐침 깊이는 얕았으며, 변연골 흡수량이 적었다. 결론: 무피판 임플란트 수술에서 연조직 펀치의 직경은 최적의 연조직 밀폐의 형성과 연조직의 치유에 있어 중요한 역할을 한다. 선택된 치유지대주보다 약 1 mm 작은 직경의 연조직 펀치를 사용하는 경우 임플란트 주변 점막과 긴밀한 접촉을 이룰 수 있고 창상 치유가 가장 양호 하다.
Statement of problem : Stress concentration on the neck bone affects the bone resorption, and finally the implant survival. Purpose: In order to examine the stress distribution on the neck bone and prosthesis abutment for implants, decreasing abutment sizes were used. Material and methods : Axisymmetric models were used to obtain the data required. These models were composed of 4mm implants with 3.4mm and 4mm abutments, 5mm implants with 3.4mm and 5mm abutments and 6mm implants with 3.4mm and 6mm abutments. All abutments were designed to received a 10mm high by 10mm diameter gold crown. Functional element analysis was used to obtain these results using data that consisted of 50 N vertical and 45 degree inclination forces. Results : 1. Changing the diameter of the abutment on the implant affects the effect of the inclination forces more than the effect of the vortical forces. 2. Changing the diameter of the abutment on the implant affect the effect of the inclination forces more than the effect of the vertical forces. 3. Experimentation showed that the larger diameter implants provided a decreased neck bone stress, whereas a larger diameter abutment provided a decrease marginal abutment stress. 4. Experimentation showed that the neck bone and abutment received more stress from inclination forces than vertical forces, Conclusions: By decreasing the size of the abutment on the implant we were able to diminishneck bone stress.

PURPOSE. This study tried to find the most significant factors predicting implant prognosis using machine learning methods. MATERIALS AND METHODS. The data used in this study was based on a systematic search of chart files at Seoul National University Bundang Hospital for one year. In this period, oral and maxillofacial surgeons inserted 667 implants in 198 patients after consultation with a prosthodontist. The traditional statistical methods were inappropriate in this study, which analyzed the data of a small sample size to find a factor affecting the prognosis. The machine learning methods were used in this study, since these methods have analyzing power for a small sample size and are able to find a new factor that has been unknown to have an effect on the result. A decision tree model and a support vector machine were used for the analysis. RESULTS. The results identified mesio-distal position of the inserted implant as the most significant factor determining its prognosis. Both of the machine learning methods, the decision tree model and support vector machine, yielded the similar results. CONCLUSION. Dental clinicians should be careful in locating implants in the patient's mouths, especially mesio-distally, to minimize the negative complications against implant survival.
Purpose: The aim of this study was to investigate the effect of (1) the size of the bony access window and (2) collagen membrane coverage over the window in sinus floor elevation in a rabbit sinus model. Methods: Small bony access windows (SW; ø 2.8 mm) were made in 6 rabbits and large windows (LW; ø 6 mm) in 6 other rabbits. Both sinuses in each rabbit were allocated to groups with or without coverage of a collagen membrane (CM) on the window, resulting in 4 groups: SW, LW, SW+CM, and LW+CM. After 4 weeks of healing, micro-computed tomographic, histologic, and histomorphometric analyses were performed. Results: Bony healing in the window area was incomplete in all groups, but most bone graft particles were well confined in the augmented cavity. Histologically, the pattern of new bone formation was similar in all groups. Histomorphometrically, the percentage of newly formed bone was greater in the groups with CM than in the groups without CM, and in the groups with SW than in the groups with LW (12.92%±6.40% in the SW+CM group, 4.21%±7.73% in the SW group, 10.45%±4.81% in the LW+CM group, 11.77%±3.83% in the LW group). The above differences were not statistically significant (P>0.05). Conclusions: The combination of a small bony access window and the use of a collagen membrane over the window favored new bone formation compared to other groups, but this result should be further investigated due to the limitations of the present animal model.
Loss of dentition can lead to not only compromised esthetics and functions of the patient, but also alveolar bone resorption. Bone grafting with prosthetic reconstruction of the gingiva can be selected for the treatment, and it provides many benefits as prosthetic gingival reconstruction does not require a complicated surgical process and is available within a short period of time, with stable clinical results. However, conventional porcelain fused to metal prosthesis has certain limits due to its size, and deformation after several firing procedures. In this clinical report, the author would like to introduce a patient with severe alveolar resorption who was treated with gingiva-shaped zirconia/titanium CAD/CAM implant fixed prosthesis for esthetic and functional rehabilitation. Clinical reports Clinical report 1, 2 : A case of loss of anterior dentition with atrophied alveolar bone. Implant retained zirconia bridge applied with Procera implant bridge system to simulate the gingiva. Upper structure was fabricated with zirconia all ceramic crown. Clinical report 3, 4 : A case of atrophied maxillary alveolus was reconstructed with fixed implant prosthesis, a CAD/CAM designed titanium structure covered wi th resin on its surface. Anterior dentition was reconstructed with zirconia crown. Conclusion and clinical uses. All patients were satisfied with the outcome, and maintained good oral hygiene. Zirconia/titanium implant fixed prosthesis fabricated by CAD/CAM system was highly accurate and showed adequate histological response. No critical failure was seen on the implant fixture and abutment overall. Sites of severe alveolar bone loss can be rehabilitated by implant fixed prosthesis with CAD/CAM system. This type of prosthesis can offer artificial gingival structure and can give more satisfying esthetics and functions, and as a result the patients were able to accept the outcome more fondly, which makes us less than hard to think that it can be a more convenient treatment for the practitioners.

STATEMENT OF PROBLEM. Many dental clinicians are concerned about immediate loading of inserted implants. However, there have been few clinical studies surveying the success rates of immediate loading, based on Korean implant systems. PURPOSE. The aim of this study was to evaluate the outcome of immediate functional loading of the implant ($SinusQuick^{TM}$EB, Neobiotech Co., Seoul, Korea) in partially edentulous maxilla or mandible. MATERIAL AND METHODS. Total 15 implants were placed. Within 2 weeks after implant insertion, provisional implant-supported fixed partial dentures were delivered to the patients. Quantitatively, marginal bone loss was measured at the time of immediate loading, after 3-months of continued loading and at the last follow-up. The mean follow-up period was 4.8 months. RESULTS. Mean marginal bone loss from implant surgery to early loading, 3-months follow-up and last follow-up was $0.03\pm0.07$ mm, $0.16\pm0.17$ mm and $0.29\pm0.19$ mm. No implant failed up to 6 months after insertion, resulting in a 100% survival rate. CONCLUSION. Immediate loading exhibited high success rate in partial edentulism for up to 6 months. Well-controlled long term clinical studies with large sample size are necessary to confirm this finding.

Background In 2012, a new anatomic breast implant of form-stable silicone gel was introduced onto the Korean market. The intended use of this implant is in the area of aesthetic breast surgery, and many reports are promising. Thus far, however, there have been no reports on the use of this implant for breast reconstruction in Korea. We used this breast implant in breast reconstruction surgery and report our early experience. Methods From November 2012 to April 2013, the Natrelle Style 410 form-stable anatomically shaped cohesive silicone gel-filled breast implant was used in 31 breasts of 30 patients for implant breast reconstruction with an acellular dermal matrix. Patients were treated with skin-sparing mastectomies followed by immediate breast reconstruction. Results The mean breast resection volume was 240 mL (range, 83-540 mL). The mean size of the breast implants was 217 mL (range, 125-395 mL). Breast shape outcomes were considered acceptable. Infection and skin thinning occurred in one patient each, and hematoma and seroma did not occur. Three cases of wound dehiscence occurred, one requiring surgical intervention, while the others healed with conservative treatment in one month. Rippling did not occur. So far, complications such as capsular contracture and malrotation of breast implant have not yet arisen. Conclusions By using anatomic breast implants in breast reconstruction, we achieved satisfactory results with aesthetics better than those obtained with round breast implants. Therefore, we concluded that the anatomical implant is suitable for breast reconstruction.

- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
