1. Many design features including suture type, anchor size and geometry, and anchor material, play a role in the overall strength of the anchor. In addition, technical considerations such as implant orientation, pattern, and location may affect the ultimate success of the repair. 2. Multiple fixation points provide a biomechanically sounder construct in Bankart repair. The size of the glenoid and its rim make anchor size a critical consideration in implant selection and implementation.

Migration of dental implants into the maxillary sinus is uncommon. However, poor bone quality and quantity in the posterior maxilla can increase the potential for this complication to arise during implant placement procedures. The aim of this report is to present a dental implant that migrated into the maxillary sinus and disappeared. A 53-year-old male patient was referred to us by his dentist after a dental implant migrated into his maxillary sinus. The displaced implant was discovered on a panoramic radiograph taken five days before his referral. Using computed tomography, we determined that the displaced dental implant was not in the antrum. There was also no sign of oroantral fistula. Because of the small size of the displaced implant, we think that the implant may have left the maxillary sinus via the ostium.
The purpose of this article is to present the clinical and laboratory procedures for single tooth restoration using 'Combination Implant Crown'. It is cemented on implant abutment and that abutment is screw-retained over implant body. This type of implant restorations has the advantages of cement-retained restoration while being antirotational and retrievable. And, more esthetic and functional result can be achieved by minimizing the size of access hole. The results were as follows : 1. Preparation of abutment below the cuff line should be avoided 2. Axial reduction of implant abutment should not be excessive because it may weaken the abutment 3. More esthetical and functional occlusal surface was achieved with a minimal access hole which is slightly larger than the diameter of hex driver to enable future total retrievability. 4. Combination Implant Crown has the advantages of both the cement-retained and screw-retained type implant restoration. 5. Background The number of cosmetic and reconstructive surgeries that use breast implants is increasing in Korea. Recently, it has been reported that breast implant-associated anaplastic large-cell lymphoma is related to textured breast implants, and interest in classification according to the texture of breast implants is increasing. However, there is currently no clear and unified classification. In particular, the definition of "microtextured" is highly varied. In this study, we retrospectively investigated and analyzed the clinical outcomes of smooth and microtextured breast implants. Methods A retrospective chart review of all patients who underwent breast augmentation surgery with smooth and microtextured silicone gel implants between January 2016 and July 2020 was performed. We retrospectively analyzed implant manufacturer, age, body mass index (BMI), smoking status, incision location, implant size, follow-up period, complications, and reoperation rate. Results A total of 266 patients underwent breast augmentation surgery, of which 181 used smooth silicone gel implants and 85 used microtextured silicone gel implants. Age, BMI, smoking status, implant size, and follow-up period were not significantly different between the two groups. Similarly, complications and reoperation rates were not significantly different between the two groups. Conclusion It is important to provide information regarding the clinical risks and benefits of breast implants to surgeons and patients through a clear and unified classification according to the texture of the breast implant.
Statement of problem: Main consideration was given to the stresses at the site of implant entry into the cortical bone at the alveolar crest. As a suspectible factor affecting the occurrence of stress concentrations, the contact angle between the implant and the alveolar crest bone was addressed. Purpose: The purpose of this study is to evaluate angles between the alveolar crest bone and the implant effect on the implant crestal area induced stresses using a finite element method. Material and methods: Cylindrically shaped, standard size ITI implants entering into alveolar crest with four different contact angles of 0, 15, 30, and 45 deg. with the long axis of the implant were axisymmetrically modelled. Alterations of stresses around the implants were computed and compared at the cervical cortical bone. Results and conclusion: The results demonstrated that regardless of the difference of the implant/alveolar crest bone contact angles, stress concentration occurred at the cervical bone and the angle differences led to insignificant variations in stress level.
In this study, the mold technology for manufacturing of porous implant was investigated. Firstly, we considered the concept of insert molding technology with 3D printing of porous inert part. The part on implant was designed in the end region of the implant. And then main implant bodies were manufactured using conventional machining method. The other porous parts were designed and optimized with molding simulation. As the feature size of porous implant was so small that perfect feature of it using 3D printing technology could not be obtained. So, we proposed another scheme for manufacturing of the porous implant in the replace of the former approach. Polymer mold cores with 3D printing technology were considered. The effects of addictive manufacturing process parameters on the properties of mechanical and dimensional accuracy were investigated. Direct 3D printed polymer mold cores were designed and manufactured under the simulation of thermal and molding analysis. It was shown that 3D printed mold core with polymer could be adapted to the injection molding for porous implant.
Homogenization technique is employed to investigate the series of stress analyses of mandible for three different types of dental implants. This technique helps to make proper material model of bone and analyze such a non homogeneous structure at the level of individual microstructural unit. The stress analyses with homogenization technique show much higher stress level in the sponge bone, compared to those of conventional FEM. It also manifested that even a minor lateral force results in crucial stresses in the dental implant system and that the macroscale model should take the shape and size after real mandible to produce reasonable solution in the analyses of dental implant systems. The shapes of dental implants simulated in this study are rectangular-cross-sectioned type, hemi-sphere rooted type, and wedge type implant. The stress states of mandible with hemisphere rooted type implant and wedge type implant show similar levels, while those with sectioned rectangular implant results in higher stresses. It is suggested that the distance between the implant tip and cortical bone be kept far enough to prevent stress concentrations in the mandible.

Background The aim of unilateral breast reconstruction after mastectomy is to craft a natural-looking breast with symmetry. The latissimus dorsi (LD) flap with implant is an established technique for this purpose. However, it is challenging to obtain adequate volume and satisfactory aesthetic results using a one-stage operation when considering factors such as muscle atrophy, wound dehiscence and excessive scarring. The two-stage reconstruction addresses these difficulties by using a tissue expander to gradually enlarge the skin pocket which eventually holds an appropriately sized implant. Methods We analyzed nine patients who underwent unilateral two-stage LD reconstruction. In the first stage, an expander was placed along with the LD flap to reconstruct the mastectomy defect, followed by gradual tissue expansion to achieve overexpansion of the skin pocket. The final implant volume was determined by measuring the residual expander volume after aspirating the excess saline. Finally, the expander was replaced with the chosen implant. Results The average volume of tissue expansion was 460 mL. The resultant expansion allowed an implant ranging in volume from 255 to 420 mL to be placed alongside the LD muscle. Seven patients scored less than six on the relative breast retraction assessment formula for breast symmetry, indicating excellent breast symmetry. The remaining two patients scored between six and eight, indicating good symmetry. Conclusions This approach allows the size of the eventual implant to be estimated after the skin pocket has healed completely and the LD muscle has undergone natural atrophy. Optimal reconstruction results were achieved using this approach.
상실한 하악 4전치에서의 치료 계획은 어느 다른 부위보다 수술의 성공률이나 보철치료시 유리한 환경을 지니고 있다고 흔히 생각되는 부위이다. 하지만 하악 전치들의 M-D 사이즈나 crowding으로 인한 공간적 제한, 임시치아 문제, 치조골 흡수로 인해 생기는 심미적인 문제 등 의외로 까다로운 경우가 종종 발생하는 부위기도 한다. 본문에서는 하악 4전치 상실 시의 치료 계확에 대해서 케이스를 통해 알아 보겠다. 하악 4전치 상실시 가능한 옵션들: 하나 혹은 4전치중 일부 상실의 경우나 잔존 치조골의 흡수가 거의 없어 임플란트 위치 관계가 심미적으로 중요하다고 판단되는 경우라면 직경이 작은 임플란트들을 원래 발치와 중앙에 식립(Narrow type 이나 One body mini implant type) 최종 보철물 형태가 Pink porcelain을 포함하는 이미 광범위한 치조골의 흡수가 일어난 경우 즉 임플란트 식립 위치가 덜 중요한 상황이라면 regular size의 임플란트들을 측절치와 중절치의 사이 Interseptal bone위치에 식립 4전치중 하나 혹은 일부가 상실됐고 남아 있는 전치들의 예후가 불량하다고 의심되나 환자가 당장 나머지 전치들의 발치에는 동의하지 않을 때 남아 있는 전치들이 발치 될 때까지의 Tentative restoration로서 Resin Bonded Bridge(Resin Retained Bridge/Resin Bonded Fixed Partial Denture)의 적용.

Background: The purposes of the present study were to compare implant stabilities of mandibular block bone graft and bovine bone graft and to evaluate influencing factors for implant stability in mandibular block bone (MBB) graft. Methods: This retrospective study investigated 1224 cases and 389 patients treated by one surgeon in the Department of Oral and Maxillofacial Surgery of Pusan National University Dental Hospital (Yangsan, Korea) between January 2010 and December 2014. Proportions that MBB graft cases constitute in all implant restoration cases and in all bone graft cases were measured. Implant stability quotient (ISQ) values were achieved by the same surgeon before loading. The average ISQ values of the experimental groups were compared. In addition, ISQ values of influencing factors, such as age, sex, implant size, and implant placement site, were compared within the MBB group using $Osstell^{TM}$ Mentor ($Osstell^{(R)}$, $G{\ddot{o}}teborg$, Sweden). Paired t test and ANOVA were conducted for statistical analysis with a significance level of 0.05. Results: Fifty-five percent of all implant restoration cases performed bone graft while MBB cases constituted 34% of all implant restoration cases and 61% of all bone graft cases. Comparing ISQ values according to bone graft materials, the MBB group manifested sufficient implant stability by presenting comparable ISQ value to that of the experimental group without bone graft. Among the reviewed factors, females, mandibular molar regions, and implants in larger diameter displayed greater implant stabilities. Conclusions: Satisfactory implant stability was accomplished upon administration of MBB graft. Within the limitation of this study, gender, implant site, and implant diameter were speculated to influence on implant stability in MBB graft.
