Purpose: Common complications of immediate breast reconstruction with implant are capsular contracture, malposition of implant, hematoma and seroma. Especially, the most severe complication is implant exposure caused by inflammation or necrosis of skin flap margin of mastectomy site. This article reports the experience of cases of salvage in such an exposure of implant. Methods: From July, 2002 to Feb., 2009, sixty-five patients who underwent immediate breast reconstruction with implant were retrospectively analyzed. Exposure of implant was happened in 5 of 65 patients and they were treated at out patient district. Two of five patients were reconstructed with saline implnt and all of them underwent the enveloping of the implant with AlloDerm$^{(R)}$ and Serratus muscle flap. Remaning three patients were reconstructed with silicone implant and all of them underwent the enveloping of the implant with AlloDerm$^{(R)}$. Results: In the group of patients who underwent reconstruction with saline implant, implant exposure was found in one patient due to partial necrosis of the margin of skin flap and debridement and primary repair were done. In the other one patient, dressing with antibiotic ointment were done. And debridement and primary repair were proceeded. In the group of patients who underwent reconstruction with silicone implant, implant exposure was found in one patient. After removal of the implant, tissue expansion was done and a new silicone implant was inserted. Implant exposure were found in the other two patients, antibiotics ointment application and primarily repaired. Conclusion: It was the common knowledge that the exposed implant should be removed. But salvage of the exposed implants may be possible with proper treatment. Four of five patients (80%) with exposed breast implant were salvaged with conservative management.
Though implant treatment is considered as a common treatment option for edentulous patients, there have been few studies on the temporomandibular disorder (TMD) related with implant treatment. The purposes of this study were to evaluate the relevance of TMD to the implant patients and to evaluate the risk factors of TMD in relation with implant treatment. For the evaluation of various risk factors of TMD in relation with implant therapy, clinical evaluation focused on patient factors and implant factors. From a group of 694 patients, 25 patients (3.6 %) were included in this study. The majority of the patients were included in the asymptomatic 'adaptive' group. Parafunction was detected in 11 patients, 8 patients were male. Four patients having parafunction showed complications such as implant failure or fracture of the suprastructure. From the results, it is possible that TMJ related symptoms are developed or aggravated after implant therapy, which requires relatively more chair time; so TMJ examination should be included in the pre-operative evaluation for dental implant patients. Also, it is important to treat patients after they sign an informed consent that includes a detailed explanation on the possibility of TMD during treatment.
Purpose: With the popularity of implant therapy, clinicians need to know about treating the dental implant in patients with gingival involvement of oral lichen planus (OLP). The aim of this study is to evaluate the survival and success rates of dental implant and propose of clinical guidelines for implant treatment in OLP patient with gingival involvement. Methods: A literature search was performed in PubMed/Medline, and Cochrane database. Papers in English language published between 1990 and 2019 were evaluated. The focused questions were following; 1) Dose gingival OLP affect the survival and success rates of dental implants? 2) The management of OLP patients with gingival involvement receiving dental implant. Results: There was no study about the evaluation of dental implant only in gingival OLP patient. Five studies evaluating dental implants in OLP patients were included in this review. Implant survival rate was 100.0% in well-controlled OLP patients in all included studies. The use of topical/systemic corticosteroid in OLP patients was performed before and/or after implant placement in all included studies. Conclusions: The implant survival and success rates in well-controlled OLP patients did not different from that of non-OLP healthy subjects. The gingival OLP is associated with higher rate of peri-implant mucositis. Adequate management of gingival OLP lesions before and after implant insertion is required to reduce inflammation and associated bone loss.
This study is to inquire into the correlation between patients' knowledge and attitude about the implant denture, as well as to provide exact information for patients. The results are expected to be basic data to make patients have positive attitudes to the implant denture. The study was conducted on patients who visited dental hospitals located in South Gyeongsang Province, South Korea, from 1 November to 31 December, 2009, the answer sheets from 198 patients analyzed. The results showed that 24.2% of respondents had implant dentures put in. Hereat, the t-test and the analysis of variance (ANOVA) were performed to analyze respondents' knowledge and attitude about the implant denture according to general characteristics. In relation to knowledge about the implant denture, respondents showed differences according to what levels of education they had and whether they had the implant dentures put in. The results of a correlation analysis showed that respondents' knowledge and attitude about the implant denture had a significant correlation with each other. In other words, respondents, who had a high degree of knowledge about the implant denture, had a more positive attitude to it. With the generalization of implant dentures, dental care providers have need to provide exact information about the implant denture for dental patients in order that they may comprehend it and have positive attitudes to it. In addition, there is a need to establish a system to provide the general public with exact information about the implant denture.

The dental implants in modern dentistry brought out a new era in everyday dental practice. Increasing of dental implants usage, various failures and complications has occurred. Failures and complications of dental implant treatments that can happen in implant surgery, in mechanical or prosthetic problem and in biological aspect. The aim of this study was to assess implant failures for 6 years as well as find out how to overcome implant failure. In Jeju National University hospital, 26 dental implants in 17 patients were removed by implant failure through 2013.1.1.-2018.12.31. Each implant failure case was analyzed in report form with various informations about failure retrospectively. The present study showed 26 failed dental implants of 17 patients were removed. 13 implants(50%) of 8 patients(47.06%) were failed before functional loading and 13 implants(50%) of 9 patients(52.94%) were failed after functional loading. 11 implants(42.31%) of 7 patients(41.18%) with medical disease were extracted. 6 implants(23.08%) of 5 patients(29.41%) with additive bone graft were failed. We discuss with 26 failed dental implants of 17 patients about their causes, solutions and prevention retrospectively for 6 past years. Precise diagnosis and treatment planning are needed. Medically compromised patients and patients with implants and additive bone grafts should be cautiously treated with high failing possibilities.

Objectives: This retrospective study aimed to analyze data on nerve damage in patients who complained of sensory changes after dental implant surgery, the clinical results according to proximity of the implant fixture to the inferior alveolar nerve (IAN) canal, and the factors affecting recovery of sensation. Materials and Methods: The electronic medical records of 64 patients who had experienced sensory change after implant surgery were reviewed. Patients were classified by sex, age, implant installation sites, recovery rate and the distance between the implant fixture and IAN canal on computed tomography (CT). The distance was classified into Group I (D>2 mm), Group II (2 mm≥D>0 mm), and Group III (D≤0 mm). Results: The 64 patients were included and the mean age was 57.3±7.3 years. Among the 36 patients who visited our clinic more than two times, 21 patients (58.3%) reported improvement in sensation, 13 patients (36.1%) had no change in sensation, and 2 patients (5.6%) reported worsening sensation. In Group II, symptom improvement was achieved in all patients regardless of the removal of the implant fixture. In Group III, 8 patients (40.0%) had reported symptom improvement with removal of the implant fixture, and 2 patients (33.3%) of recovered patients showed improvement without removal. Removal of the implant fixture in Group III did not result in any significant difference in recovery (P=0.337), although there was a higher possibility of improvement in sensation in removal cases. Conclusion: Clinicians first should consider removing the fixture when it directly invades the IAN canal. However, in cases of sensory change after dental implant surgery where the drill or implant fixture did not invade the IAN canal, other indirect factors such as flap elevation and damage due to anesthesia should be considered as causes of sensory change. Removal of the implant should be considered with caution in these situations.
Objectives : The purpose of this study is to investigate the correlation between quality of sleep and dental fear in implant surgery patients. Methods : A self-reported questionnaire was filled out by 153 implant patients from December 2012 to February 2013 in dental clinics and hospitals in Busan and Changwon. Data were analyzed by descriptive analysis, t-test, ANOVA and multiple regression analysis using SAS version 9.20. Results : The quality of sleep in the implant patients was $41.0{\pm}6.0$. The systemic diseases influenced on the quality of sleep and dental fear also affected the quality of sleep. Conclusions : The results of the analysis of a relationship between the fear perception of dental implant surgery patients and the quality of their sleep showed that the patients with systemic diseases, the patients with a fear by the physical stimulation relating to implant treatment showed the low quality of sleep.
Objectives : This study is to confirm the influencing factors on patients satisfaction and intention to revisit the implant service clinic by structural equation model. Methods : We surveyed 250 implant patients visiting 6 dental clnics(hospitals) in Busan, Changwon, Gimhae city and questionnaire was for four weeks in July 2012, a total 209 patients filled out the questionnaires within July and the complete data were analyzed. we were used structural equation model for analysis to confirm influencing factors on implant patients' satisfaction and intention to revisits. Results : This study finds that factors influencing on implant patients' satisfaction and intention to revisit are reasonable medical fee, medical skill treatment of dentist, dental hygienist and other support staff, administrative procedure, and there existed between influencing factors and the patient's treatment satisfaction as well as intention to revisit. In order to satisfy the implant patients, dentist and hygienists should try to provide high quality of treatment and dental services. Conclusions : The results of this study provide managerially important messages to the managers of dental clinics in order to develope management strategies for new customers.
Ideal results of augmentation mammaplasty consist of symmetry, natural shape, soft feeling and inconspicuous scar. In addition, patient's preferences about size and shape should be included. Static implants could not perfectly satisfy patients' desires for size and shape, but expandable implants enable to change the volume after the operation. From September 2001 to September 2004, 76 patients(150 breasts) underwent breast augmentation using permanent expandable implant. The procedure was unilateral in 2 women and bilateral in 74 women. Age ranged from 19 to 50 years(mean, 29 years). Fifty nine patients underwent simple augmentation mammaplasty, 7 patients were corrected of their severe asymmetry, 2 patients with the congenital breast deformity underwent mammaplasty using this, and 2 patients who had undergone unilateral mastectomy were reconstructed of their breasts using expandable implant. There were no definite complications such as capsular contracture, implant rupture, asymmetry. And there reported little dissatisfaction about the size. The permanent expandable implants might be good alternatives in cases of ordinary breast augmentation as well as tissue deficient patients, asymmetry, congenital anomaly, and breast reconstruction.
This study was conducted to improve patients satisfaction by comparing patients expectation prior to the implant therapy to patient satisfaction after the implant therapy. The data of 158 patients with dental implant in Seoul, Gyeonggi-Do and Cheonan city was obtained. The results are as follows : 1. 40.5% of patients placed one implant and 36.1 % of patients answered that the average cost for each implant is from 1,500,000 to 2,000,000won. 23.4% of them answered they decided to have implant because no tooth preparation is required. 2. Overall, the expectation level for implant therapy in general properties, women showed higher level of expectation than men. Especially, 'Implant has a longer life than other denture' showed significant difference between women(4.29) and men(3.87). Expectation by age, 'Implant has similar color and shape' was found the highest in age 20(4.38). Satisfaction by general properties, 'No tooth preparation required' was found the highest in age 20(4.57) and was statistically significant(p<.002). 3. Comparing patients' expectation prior to implant therapy to satisfaction afterward, 'Implant prevents jawbone resorption and keeps facial appearance' showed 3.7 of expectation but satisfaction was found increased to 3.87(p=.020). No tooth preparation required' was found 3.95 in expectation prior to implant therapy but after therapy it was found 4.23 in satisfaction (p<.001). 4. Overall satisfaction in implant therapy was found 4.25, recommendation to other patients was found 4.18 and repurchase was found 4.17.
