The osseointegrated implants react biomechanically in a different pattern to occlusal force, due to lack of the periodontal ligament unlike the natural teeth. The implants show markedly less movement and limited tactile sensitivity compared with the natural teeth. The implant occlusion concept aims to avoid overloading on the implants and to direct occlusal loads along the longitudinal axis of the implants, in order to compensate for the different biomechanics of the implants. Although many guidelines and theories on implant occlusion have been proposed, few have provided strong supportive evidence. Moreover, the outcome of treatment often quite successful in spite of different concepts of occlusion and there is an increasing tendency to doubt about the strict theoretical implant-specific occlusion concept. The purpose of this article is to review the previous reports about the concept of implant occlusion and discuss clinical occlusal considerations in implant rehabilitations.
The appropriate occlusion is one of the most important factors for the long-term success of implant and its restorations. The purpose of this review is to investigate and define occlusal considerations to reduce failure of implant prostheses. The physiological movement of implants is markedly lower than that of natural teeth and they also lack in occlusal sensitivity. Proper occlusal pattern may be assigned to compensate for the biological disadvantages and occlusal contacts must be formed where the cantilever effect is minimized. Moreover, the long-term success of implants after osseointegration can be assured by reducing early occlusal loading to avoid implant overloading and selecting appropriate occlusion material. Occlusal overload was brought by the number and location of occlusal contacts, which are under the clinician's control. The concept of implant occlusion is based on the concepts derived from traditional prosthetics. Moreover, there are few evidence on the concept or design of implant occlusion. Several occlusal design was recommended for implant prosthesis. Mutually protected occlusion, group function occlusion and bilateral balance occlusion was recommended for the specific types of implant restorations. This article reviews proper design of occlusion for implant restoration and offers occlusion strategy clinically.
The term, 'crossed occlusion' implies clinical situation in which the residual teeth in one arch have no contact with those in the antagonistic arch, resulting in the collapse of occlusal vertical dimension. The treatment goal of this pathologic condition is restoration of the collapsed vertical dimension and stabilization of abnormal mandibular position. Previously, konus removable prostheses or tooth supported overdentures were suggested to solve crossed occlusion. Nowadays, dental implants have been used for definitive support to solve this problem. In this case report, a 65 years old female patient had a crossed occlusion, in which the maxillary posterior residual teeth and mandibular anterior residual teeth cross. Interim removable and fixed dental prostheses were used to confirm the proper vertical and horizontal jaw relation. After that, the mandibular posterior edentulous region was restored with implant-supported fixed dental prostheses. Computer tomography guided implant surgery was performed according to the concept of the restoration-driven implant placement. The maxillary anterior edentulous region was restored with Kennedy class IV removable prosthesis, considering the patient's economic status. The patient's jaw position and prostheses have been well maintained at the follow-up after 6 months of definitive restoration. The antero-posterior crossed occlusion problems appeared to be effectively solved with the combination of removable in one arch and implant-supported fixed prostheses in the other.
There is an increasing appreciation of the vital role that biomechanics play in the performance of oral implant. The aim of this article is to provide some basic principles that will allow a clinician to formulate a biomechanically valid treatment plan. However, at this point in the history of oral implantology, the clinician should realize that we do not know enough to provide absolute biomechanical rules that will guarantee success of all implants in all situations. To examine the biomechanical questions, one must begin with an analysis of the distribution of biting forcess to implants. Related topics, such as stress transfer to surrounding tissues and interrelationships between bone biology and mechanical loading are major subjects, deserving a separate discussion. Once rigid fixation, angulation, crestal bone level, contour, and gingival health are achieved, stress beyond physiologic limits is the primary cause of initial bone loss around implants. The restoring dentist has specific responsibilities to reduce overload to the bone-implant interface. These include proper diagnosis, leading to a treatment plan designed with adequate retention and form, and progressive loading to improve the amount and density of bone and further reduce the risk of stress beyond physiologic limits. The major remaining factor is the development of occlusal concept in harmony with the rest of the stomagnetic system.
최근 치과치료의 영역에서 디지털 임플란트 시스템은 그 영역을 넓혀 나가고 있다. 디지털 장비들의 기술적 발전에 힘입어 한계점들이 하나 둘씩 극복되었다. 초기 단일치아 수복 정도에나 사용되었던 디지털 임플란트 시스템은 임플란트 수술에서부터 보철물 제작까지 무치악의 영역에서도 사용되고 있다. 본 증례에서는 50세의 하악 무치악 환자가 임플란트를 이용한 교합 재건을 원하였다. 하악의 임플란트 수복을 위해서 "All-on-4" 개념의 디지털 임플란트 시스템 사용을 계획하였고, 무절개 수술을 위한 가이드 장치를 제작하여 수술을 시행하였다. 수술 후, 전악 보철을 디지털 장비를 이용하여 제작하였다. 전체 치료 과정에서 기존의 아날로그 방식(인상채득, 납형 제작, 주조 등)을 가능한 배제하고 디지털 시스템을 이용하여 기공과정 및 임상과정을 최소화하기로 계획하였고 만족할만한 결과를 얻었기에 보고하는 바이다.
CAD-CAM기법과 CT의 도움으로 무판막임플란트 치료술식이 가능하게 되었다. 본 연구의 목적은 1년간 전향적 연구를 통해 이러한 술식의 신뢰성을 검토하고자 하였다. 하악무치악상태의 연속적인 환자 12명이 포함되었다. 71개의 임플란트가 이공사이에 $NobelGuide^{TM}$법을 이용하여 식립되었고 환자는 정해진 주기에 따라 검사되었다. 한 개의 임플란트가 조기실패하였으며 나머지는 만족할 만한 기능을 보였다. 연구기간 중 평균적인 변연골흡수량은 중앙부($0.3{\pm}0.1$ mm), 견치부($0.5{\pm}0.1$ mm), 원심부($0.7{\pm}0.2$ mm)였으며 임플란트 안정성 수치의 변화량은 중앙부($-1.05{\pm}2.76$ mm), 견치부($-0.85{\pm}2.59$ mm), 원심부($-1.27{\pm}2.18$ mm)를 나타내었고 위치간 통계학적인 차이는 없었다(P>.05). 본 전향적인 기초조사연구에서 CAD-CAM기반의 무판막임플란트 치료술식은 하악무치악환자에게 신뢰성 있는 술식이 될 수 있을 것으로 사료되었다.
Many dentists and patients expect that implant function and esthetics will not change over time. However, even the most successful implant restorations with ideal position, vertical height, and occlusion can be aesthetically pleasing, but may hide a common problem. Many dentists noticed that there can be some circumferential bone loss around the neck of the implants. To circumvent this bone loss, a "platform switching" concept was introduced recently. The basic concept of platform switching is by moving the fixture-abutment interface further away from the crestal bone to minimize crestal bone loss. Since crestal bone loss is a multifactor problem, it is important to consider microgap formation and micromotion between the implant and abutment because platform switching does not solve the problem on its own. In this article, we reviewed studies concerning platform switching and discussed the clinical application and the problems that may occur with its use.
During the past several years, significant advances have occurred in the utilization of osseointegrated implants for the treatment of partially edentulous patients. One of the biggest purposes for treating of these patients is the high demand for improved esthetics, especially in the anterior region. For this esthetics, the new trend in dental implants is the immediate placement and immediate superstructure fabrication. The refined surgical technique, the skillful soft tissue management, and the proper prosthetic coordination are the main factors to achieve natural looking of implant supported prosthesis. The customized provisional restoration and the customized impression coping are recommended for the optimal peri-implant soft tissue contour. The basic concept of Frialit 2 system was the immediate replacement of a tooth with root-analog fixture after extraction. This system guarantees an ideal result in function and esthetics. The ceramic abutment system offers improved quality in the respect of esthetics, fitness, translucency, and biocompatibility. In this clinical report, the final restoration made with IPS Empress 2 crown on the CeraBase abutmen of Frialit 2 system allowed the reproduction of the natural vitality of tooth and adjacent gingiva.
목적: 본 연구는 platform matching, platform switching을 시행한 임플란트의 변연골 소실을 비교하여 platform switching 개념의 임상적 의의를 평가하고자 한다. 연구 재료 및 방법: 원광대학교 치과병원 임플란트 센터에서 치료를 진행한 환자를 대상으로 임플란트 식립 시기, 직경, 길이, 지대주 연결방법 및 보철물 연결고정 유무를 조사하였다. 변연골 소실량을 측정하기 위해서 Emago advanced v5.6 프로그램을 사용하여 임플란트 식립 시와 가장 최근 내원 시의 치근단 방사선 사진을 비교하였다. 결과: 환자 82명의 임플란트 150개를 대상으로 6 - 63개월 동안 관찰한 결과 platform matching 임플란트는 $1.16{\pm}0.54mm$, platform switching 임플란트는 $0.68{\pm}0.27mm$의 변연골 소실을 나타내었다. 결론: Platform switching을 시행함으로써 임플란트의 변연골 소실을 줄이는 효과가 있는 것으로 사료된다.
'Implant Supracrestal Complex (ISC)'의 개념은 임플란트 보철물의 디자인 형태가 단기 임상 결과와 임플란트 주변 경·연조직의 장기 예후에 미치는 영향을 확인하는 방법으로 도입되었다. 임플란트-지대주 디자인과 접합부(junction) 및 위치와 같은 임플란트-보철물-지대주 복합 디자인 형태는 임플란트 주변 조직의 장기적인 유지에 중요한 영향을 미칠 수 있다. 본 증례에서는 임플란트 보철물 수복 후 만성적인 연조직 염증이 발생한 환자에게 오목한 맞춤형 지대주와 출현각이 30도를 이루는 보철물을 수복하였다. 새로운 보철물은 충분한 경·연조직 공간을 확보하고 적절한 연조직 폐쇄를 이룸에 따라 합병증이 개선되고 임플란트의 장기적인 안정성을 유지하는 것을 확인하였다.
