  • The purpose of this study is to design strategic hospital service based on each hospital's features. For this study, an assessment was conducted by 398 in-patents of one university hospital located in Seoul. The self-questionnaires, which were investigated from Oct. 15th to 29th in 2008, compared central reception desk with ward reception desk in satisfaction and re-use rate of patients. The major results of this study are as follow. First, according to each reception desk user, they have different satisfaction of it. As for the staff kindness, admission procedure, discharge procedure and manner of staff, those made patients be gratified as well. Second, when it comes to the intention of re-use, there were no significant features between them. Only convenience in admission and discharge procedure, however, was an attractive factor for the recommendation. Third, this study found out the reasons for higher re-use rate of central reception desk users. Regarding service, they were contented with the time for test and treatment. As for the hospital service, they would like to re-use this hospital because of convenient steps of paying interim fee and getting certificates. Forth, this study found out the reasons for higher re-use rate of ward reception desk users. As a point of hospital service view, they responded that respected privacy, hospital facility and general service were good for staying. As for the manner of staff, they mentioned nurses and staff in charge and whole staff members were kind. When it comes to the procedures of patient management, steps of discharge and paying interim fee were convenience. In conclusion, the results of this study suggest that providing a ward reception desk service can boost the satisfaction and re-use rate of in-patients. Furthermore, this strategic management method would be good for not only cutting the moving line but also efficient in-patient care system. These results can be used for the strategic hospital marketing field, as well. Even though this study has a limitation of the targeted populations which were only in a ward reception desk running hospital, it can say that having competitiveness in satisfaction of hospital service is good for promoting and differencing each hospital. Consequently, whole general management system would be adjusted first for differencing each hospital; however, this sort of additional factor should be concerned as well. I expect that this study would give meaningful data for designing strategic and differencing marketing method to lots of hospitals.

  • Viroids are about 250-400 base pair of short single strand RNA fragments have been associated with economically important plant diseases. Due to the lack of protein expression capacity associated with replication, it is very difficult to diagnosis viroid diseases in serological methods. For detecting viroid at plants, molecular-based techniques such as agarose gel electrophoresis, polyacrylamide gel electrophoresis (PAGE), DNA-hybridization, blotting analysis and conventional RT-PCR are reliable. Real-time RT-PCR methods that grafted on RT-PCR methods with improved confirmation methods have been also utilized. However, they are still labor-intensive, time-consuming, and require personnel with expertise. Loop-mediated Isothermal Amplification (LAMP) method is a nucleic acid amplification method under the isothermal condition. The LAMP methodology has been reported to be simple, rapid, sensitive and field applicable in detecting a variety of pathogens. The results of LAMP method can be colorized by adding a visible material such as SYBR green I, Evagreen, Calcein, Berberine and Hydroxy naphthol blue (HNB) with simple equipment or naked eyes. The combination of LAMP method and nucleic pathogens, viroids, can be used to realize simple diagnosis platform for the genetic point-of care testing system. The aim at this review is to summary viroid-caused diseases and the simple visible approach for diagnosing viroids using Loop-mediated Isothermal Amplification (LAMP) method.

  • This research was conducted to improve the image of female emergency workers in a fire service organization by studying the perception trends of the increasing number of female paramedics. This human resource can be utilized more effectively by using the study results as baseline data. We conducted a survey from May 1 to June 30, 2013. The survey was analyzed using Q methodology and the program QUANL. For the Q sample, 31 emergency rescue paramedic students who had experienced ambulance field education were selected. As a result, four types of female paramedics were found; "superwoman," "hero" (who pushes against limits of stamina), "delicate guardian angel," and "skillful desk worker." The explanation powers were as follows: type I: 42.2%; type II: 4.8%; type III: 4%; and type IV: 3.6%. The sum of the figures explained 54.7% of the total variables. Generally, the students recognized female paramedics as professionals, but physical strength, baby care, and caring were considered as burdens. This requires changes in the organization's perception of female paramedics and an institutional strategy.

  • Patent ductus arteiosus (PDA) is the persistent opening of ductus arterious located between pulmonary artery and descending aorta after birth. Although PDA can be occluded by surgical ligation, interventional closure of PDA using various devices is being rapidly replaced to surgical ligation, because of its non-invasive nature of treatment and minimal post-care after treatment. Main problem encountering in interventional treatment in toy breed dogs is vascular inaccessibility due to small size of artery. Although transvenous approach using jugular veins has been developed, this technique requires experienced skill to locate occlude devices into PDA and has high risk of accidental dislodgement of occluding devices (i.e. embolization coils or vascular plugs). Therefore, in this study, we developed a modified technique for coil embolization using mini-angiocatheter with trans-femoral approach in toy breed dogs (< 3.0 kg of body weight). With this modified method, we were successfully treated two toy breed (Maltese) dogs with the left to right shunted PDA.

  • Health care services in a residential area would accelerate the aging in place. In addition, these changes would play a key role in terms of reducing healthcare costs and leading a healthy lifestyle. The purpose of this study draws the design methodology of community facilities and each dwelling with healthcare services in apartment complex, which the elderly people will enjoy a better health status. This paper presents the result focused on the welfare building in Gayang 7th apartment complex. The intention to receive healthcare services was investigated by occupants. Moreover, design requirements were drawn through in-depth interviews and the state observation to use the service. In the complex, stronger intention to serve the u-Healthcare services was shown to the pre elderly group than the elderly. Both of them had a problem to use and keep the health equipment due to the fact there is not enough space in the unit. Reporting the observation results, the upright-posture furniture attaching the healthcare equipment and the equipment storage should be prepared in the unit. In the public space, the program for these healthcare services can be divided into three parts, i.e. the health status measurement, the healthcare, and the service connected to the surrounding facilities. The health status measurement can be the basic to the health services and its function should be gradually extended. In the complex, the hybrid type with various functions could be applied owing to a new building for welfare; moreover, semi-independent user should be able to receive the home healthcare service.

  • We report a case of heart-lung transplantation in a 32 year-old female with Eisenmenger syndrome secondary to patent ductus arteriosus. She has been suffered from congestive heart failure since June 1996 and repeatedly treated at Intensive Care Unit with intravenous inotropic support since July 1997. Preoperative echocardiography showed a patent ductus arteriosus with right to left shunt, severe regurgitation of tricuspid valve and estimated right ventricular systolic pressure of 100mmHg. The brain-dead donor was an 18 year-old male with head trauma from traffic accident 3 days ago. Heart-lung block procurement was performed at another general hospital and was transported to the Seoul National University Hospital by ambulance. Total ischemic time of the transplanted heart and lung were 249 minutes and 270 minutes, respectively. The immunosuppressive therapy was commenced preoperatively with cyclosporine and azathioprine. Corticosteroid was not used until postoperative 3 weeks in order to avoid infection and delayed healing at the tracheal anastomotic site. The patient was discharged at 31st postoperative day, and has been regularly followed up at outpatient clinic without specific complication. The follow-up bronchoscopy, performed 2 weeks and 4 months after surgery, revealed no evidence of cellular rejection.

  • Myasthenia gravis is a neuromuscular transmission function disorder characterized by fatigue and weakness of voluntary muscles. This muscular weakness is intensified by activity and stress, and improved by the use of anticholinesterase compounds. It was initially described by Erb in 1879 and later named myasthenia gravis by Jolly in 1895. Although the pathogenesis is Known to be an autoimmune related reduction in the number of available acetylcholine receptors at neuromuscular junctions, the role of thymus in myasthenia gravis is still unclear and under investigation. Thymectomy in the management of myasthenia gravis has become increasingly important since Dr. Blalock observed in 1939 that some patients with thymic tumors and myasthenia gravis improved following thymectomy. A clinical study of 102 cases of myasthenia gravis was performed at Yonsei University College of Medicine. Seoul, Korea from Jan. 1976 to Jun. 1986. In order to determine which factors are of prognostic significance, attention is focused upon pre-operative patient evaluation, problems in operative and post-operative care, and long-term follow-up observations. The results were as follows: 1. The sex distribution was 67 females and 35 males, the mean age of onset was 28.95*1.69 years, and the maximal incidence occurred between 21 and 40 years of age [56 cases: 54.9%]. 2. Clinical manifestations of ocular symptoms were seen to 70 patients [68.6%] extremities weakness in 33 [32.3%], bulbar weakness in 29 [28.4%], and dyspnea in 13 [12.7%]. 3. Study cases more than two thirds were classified as mild types [MG 1 and MG 11A] and 6 cases as grave [MG 1V] based on the modified Osserman`s classification system, 4. Thymectomy was performed in 19 cases which presented in severe myasthenia symptoms and showed no improvement with cholinergic drugs. Histologic examination of the excised thymus glands revealed no abnormalities in 4 cases, thymic hyperplasia in 5, benign thymoma in 5, and malignant thymoma in 5. 5. Immediate post-operative complications included 2 cases of pneumothorax which were treated by tube thoracostomies, there was no operative mortality. 6. The response to cholinergic drugs in 36 cases younger than 20 years old and in 27 cases older than 40 years was relatively poor, while that in 35 cases between the ages of 21 and 40 years old was good. 7. Thirty of 39 cases in groups IIB, III & IV improved markedly with medical or surgical management while only 16 of 59 cases in the mild groups [I and IIA] improved, almost all surgical cases improved in all categories. 8. There were 5 deaths. occurring between 7 months and 3 years 3 months of treatment of myasthenia gravis. The causes of death were myasthenic crisis in 2 cases, respiratory failure due to candidiasis & radiation pneumonitis in one case, cerebral hemorrhage due to high blood pressure in two case.

  • Perilla (frutescens) seed oil, which is widely used as a source of vegetable oil in Korea, contains a strikingly large amount (58.4% of total fatty acids) of polyunsaturated linolenic acid (18 : 3) which is one of the essential fatty acids. Our hypothesis was that vitamin E contained in this oil would not be enough to prevent peroxidation of this polyunsaturated oil. A comparative study was carried out using rats and chicks devided into seven groups with various diet combinations emphasizing fat sources for the period of four weeks. The level of fat in each diet was 15% and animals were fed ad libitum. Various diet combinations were as follows; perilla seed oil and sesame seed oil with and without vitamin E supplementation, tallow as a saturated fat source and perilla seed hull group (10% at the expense of carbohydrate). The fat constituents of control group were consisted of 50% vegetable oil and 50% animal fat. A few important findings are as follows: 1. Rats fed perilla seed oil lost their hair focally around the neck and suffered from a bad skin lesion at the same place. In chicks, yellow pigmentation both of feather and of skin was clearly observed only in groups fed perilla seed oil with or without vitamin E supplementation. The basis of biochemical mechanisms of this phenomena remains as an important research interest. 2. The mean value for hematocrit was significantly lower for the chicks fed perilla seed oil than for those fed control diet. This result seems to be attributable to the effect on the red cell membrane known as peroxidation-hemolysis of vitamin E deficiency. 3. The serum cholesterol level was higher for the rats fed perilla seed oil than for those fed control diet, whereas in chicks the group fed perilla seed oil showed lower value than the control group indicating that different animal species could vary in their responses to the same diet. 4. In pathological examinations, the sign of hepatic fibrosis was seen in the perilla seed hull group and it was noticeable that the level of hepatic RNA was significantly increased in the rat recovering from vitamin E deficiency. It is hoped that more detailed studies on perilla seed oil and hulls will soon be carried out in many aspects especially i) at various levels of fat in the diet, ii) in relation to dietary selenium level and iii) to find an optimum level of dietary essential fatty acids in terms of P/S ratio using various animal species. In the mean time, the public should be informed to preserve this particular oil with care to minimize fatty acid oxidation and should be discouraged from overconsuming this oil. • TMA(Tissue Mineral Analysis) is very popular method in hair mineral analysis for health care professionals in over 48 countries medical center. Assesment of nutritional minerals and toxic elements in the hair is very important not only for determining adequacy, deficiencies and unbalance, but also for assessing their relative relationships in a body. In Korea, there are some problems in TMA method. Because of not haying a medical information database which is suitable for korean to do analyze, the requested TMA has to send to TEI-USA. However, as the TMA results from TEI-USA is composed of English documents and graphic files prohibited to open, its usability is very low and a lot of dollars has to be payed. Also, it can make some problems in the reliability of the TMA results, since the TMA results are based on the database of western health and mineral standards, To solve these problems, I developed the first Medical Information System of TMA in Korea here. The system can analyze the complex tissue mineral data with multiple stage decision tree classifier. It is also constructed with multiple fuzzy database and hence analyze the TMA data by fuzzy inference methods. The effectiveness test of this systems can be shown the increased business efficiency and satisfaction rate 86% and 92% respectively.
