Modeling Adherence to Therapeutic Regimens in Patients with Hypertension

  • Purpose. This study was done to identify and test a model of the psychosocial variables that influence adherence to therapeutic regimens in patients with hypertension. Method. A convenience sample of 219 patients with hypertension who were enrolled in an outpatient clinic of a cardiovascular center in Korea participated in the study. They completed self-administered questionnaires anonymously. The questionnaire was based on the Social Action Theory model and a literature review. The explanatory model was constructed and tested using structural equation modeling in order to examine the effects within the model. Results. The results of this study showed that perceived self-efficacy was the strongest factor influencing patient adherence in this sample. Adherence to therapeutic regimens in patients with hypertension was influenced by self-efficacy, patient-provider relationship, social support, and depression. Conclusions. Adherence to therapeutic regimens in patients with hypertension was most strongly influenced by self-efficacy. These findings suggest that nursing interventions to promote patient adherence should focus on the promotion of self-efficacy including improvement in patient-provider relationship and social support, and reduction in depression.

  • Objective : This case report presents the effect of Galgunhwangryunhwanggum-tang and Galgunhwangryunhwanggum-tang gagambang on hypertension. The patient was diagnosed with Taeyang-byung based on Shanghanlun provisions. Methods : According to Shanghanlun seven meridian patterns diagnostic system, we diagnosed the patient as Taeyang-byung and he has been suffering from hypertension. The patient was treated with Galgunhwangryunhwanggum-tang & Galgunhwangryunhwanggum-tang gagambang and it was provided for 40days Results : After the treatment, the degree of hypertension reduced from BP 138-90 to 130-80 Conclusions : This case study show an effectiveness of using Galgunhwangryunhwanggumtang and Galgunhwangryunhwanggum-tang gagambang according to Diagnostic system based on Shanghanlun provisions on hypertension.

Factors Affecting Patients' Compliance with Antihypertensive Medication in a Rural Area (고혈압환자의 치료순응도에 영향을 미치는 요인)

  • Noncompliance with treatment is a serious problem in the management of hypertension. We explored self-reported medication taking compliance behavior of 194 high blood pressure patients using modified health belief model hypothesizing interaction between model components. Data were collected from patients resistered hwachon community hypertension control program during February, 1993. Bivariate analysis showed perceived severity of complication, present symptom experience(p<0.05), perceived severity of hypertension and education leve(p<0.01) were significantly related to treatment experience. Logit analysis revealed that perceived severity of hypertension, perceived benefits of treatment, perceived barriers to treatment and interaction term between perceived severity of hypertension and perceived benefits of treatment contributed treatment experience. Health education from mass media was siglificantly related to continuity of treatment. We also concluded that the inclusion of interaction effects between health belief model components and the use of patient group as analysis unit lead to better study results.

  • In this article, we have improved the prediction of hypertension detection using the feature selection method for the Korean national health data named by the KNHANES database. The study identified a variety of risk factors associated with chronic hypertension. The paper is divided into two modules. The first of these is a data pre-processing step that uses a factor analysis (FA) based feature selection method from the dataset. The next module applies a predictive analysis step to detect and predict hypertension risk prediction. In this study, we compare the mean standard error (MSE), F1-score, and area under the ROC curve (AUC) for each classification model. The test results show that the proposed FIFA-OE-NB algorithm has an MSE, F1-score, and AUC outcomes 0.259, 0.460, and 64.70%, respectively. These results demonstrate that the proposed FIFA-OE method outperforms other models for hypertension risk predictions.

Development of Nutrition Education Program for Hypertension Based on Health Belief Model, Applying Focus Group Interview (건강신념 모델을 적용한 고혈압 영양교육 프로그램 개발 -포커스그룹 인터뷰에 기초하여-)

  • Health Belief Model is a socio-psychological theory of decision making to individual health-related behaviors. This study was aimed to develop an effective education program for hypertension based on health belief model. The main factors of health belief model were investigated by focus group interview (FGI) with 23 hypertensive or prehypertensive subjects aged over fifty years. 'Perceived susceptibility' to hypertension was family history, neglect of health care, preference for salty food, broth of soup and stew. Lifelong medication, complications, and medical costs were reported as 'perceived severity' of hypertension. 'Perceived benefits' of hypertension management were decrease of medicinal dose, reduction of medical costs, and healthy eating habits of the family, while 'perceived barriers' were lack of palatability of low salt diet, convenience-oriented dietary habits, and limited choice of foods when eating out. Subjects mentioned TV health programs, public health center programs, and advice from doctors and family as 'cues to action' of hypertension management. These qualitative information provided basis for developing a nutrition education program for hypertension which could be implemented in the public health center. Eight week program was composed of understanding hypertension, risk factor management (eating habits, weight), low salt diet (principles, cooking), advanced management for healthy diet in 2 sessions, and summary. Each session was designed to alert the susceptibility and severity, to emphasize the benefits, and to reduce the barriers by providing dietary monitoring, practical advice, and action tips.

Factors Associated with Hypertension Control and Antihypertensive Medication among Hypertensive Patients in a Community (지역사회 고혈압 환자에서 고혈압 조절 및 항고혈압제 복용에 관련된 요인)

  • Objectives : The main objectives of this study are to identify the factors associated with hypertension control and to determine the influencing factors associated with antihypertensive therapy. Methods : The study was conducted on 107 subjects who participated in the 1999 and 2002 Gwacheon Study and who had had uncontrolled hypertension (systolic$\geq$140 or diastolic$\geq$ 90mmHg) in 1999, We compared the characteristics of the controlled and uncontrolled hypertensive group and investigated the characteristics of those taking antihypertensive medication. Univariate associations between hypertension control and its characteristics and the association between antihypertension treatment and its characteristics were tested with $x^2$-test. We also peformed logistic regression analysis. Results : The participants who had their blood pressure checked within 6 months before their first measurement and those who had taken the antihypertensive medication showed significantly better hypertension control during the follow-up. The multivariate analysis showed that baseline antihypertensive therapy was the most important determinant factor of hypertension control during the follow-up. Socioeconomic level and life style did not affect hypertension control when controlled by the treatment variable in this study. The factors associated with antihypertensive therapy at follow-up were previous antihypertensive therapy, old age, and high educational level. Conclusions : Those who received antihypertensive therapy and those who had their blood pressure re-checked within 6 months both showed well controlled hypertension. The subjects with high educational level complied well with the antihypertensive regimen, but those in their forties did not.

The control of the hypertension in rural primary health care settings in Korea (농촌지역 일차건강관리 기관의 고혈압환자 관리실태 -전남지역을 중심으로-)

  • The control of the chronic degenerative diseases becomes a challenge in Korea as the aging of the population progresses. Accordingly, the importance of the control of the hypertension, one of the major chronic degenerative diseases, in the primary health care settings increases. However, such control activities are still minimal. This study gives brief description of how the rural residents who are currently registered for the control of the hypertension feel about the activities of the health centers, subcenters and the CHP posts, in relation to the control of the hypertension. We also describe the knowledge and practice of the registered hypertensive. In general, the knowledge, the acceptance and the satisfaction of the respondents about the hypertension control activities were not very high. Respondents being managed by the health center showed the highest knowledge, acceptance and satisfaction, and those being managed by the subcenters ranked lowest. The knowledge about the hypertension was not satisfactory. There were great variations among the items in terms of the proportion answered correctly. As many as 60% of the respondents take medication regularly but only 4.5% were practicing weight control regularly. Despite some limitations of the study, the results can be very useful for those planning hypertension control programs. The educational materials and the dietary protocols need to be developed in accordance with the regional social and food pattern, so that they can be utilized with minimal modification for each beneficiary. We also suggest to minimize the area a health personnel covers. In other word, running several small health posts at village level would be more effective than running a subcenter at the township level, in terms of the hypertension control.

A Comparison of Controlled and Uncontrolled Hypertension Groups Regarding Comprehensive Diagnosis of Qi Blood Water and Quality of Life (고혈압 환자에서 혈압 조절 여부에 따른 기혈수(氣血水) 변증(辨證)과 삶의 질 비교)

  • This study is about a comparison of controlled and uncontrolled hypertension groups regarding comprehensive diagnosis of Qi blood water and quality of life. We surveyed "controlled and uncontrolled hypertension patients" using questionnaires for comprehensive diagnosis of Qi blood water, SF-36 and HTN QoL (Measurement Scale for the quality of life in hypertensive patients). There was no difference in comprehensive diagnosis of Qi blood water between the controlled and uncontrolled hypertension groups. Within the controlled hypertension group, the patients diagnosed with a Qi deficiency, Qi stagnation, Qi counterflow, blood deficiency, and water retention received lower total scores in SF-36 and HTN QoL than in undiagnosed patients. Within the uncontrolled hypertension group, the patients diagnosed with Qi deficiency, blood deficiency, and water retention got lower total scores in SF-36 and HTN QoL than in undiagnosed patients. These results were statistically significant. These results are insufficient that we and use comprehensive diagnosis of Qi blood water for a diagnosis tool of hypertension. But if we have better studies that make up for weak points, these results will help to make a diagnosis tool for hypertension.

The Comparison of Health Status and Health Behavior among Hypertension Group, DM Group, and Hypertension DM Group for the Aged Provided with Customized Home Care Service by Visiting Nurses (맞춤형 방문건강관리사업 노인대상자의 고혈압군, 당뇨군, 고혈압.당뇨군별 건강상태 및 건강행태 비교)

  • Purpose: This study aims to compare health status and health behavior among the hypertension group, the DM group, and the hypertension-DM group for aged clients of customized home visiting health care services. Methods: This study was conducted as cross-sectional research. The subjects of this study were 2,235 aged people over 65 living in J City. Data were collected using structured questionnaires and measurements. The collected data were analyzed using the SPSS/WIN 12.0 program, and descriptive statistics, $x^2$ test, t-test and ANOVA were used for the analyses. Results: BP and glucose control showed a significant difference among the groups. BMI and waist circumference were significantly higher in the hypertension-DM group than in the other groups. Stroke, arthritis, and perceived health status were significantly different among the three groups. Depression was high, but not significantly different among the groups. Smoking and drinking were not significantly different among the groups. Physical activity was very low, especially in the hypertension group. Medication was high, especially in the hypertension-DM group. Conclusion: It is necessary to consider care plans for the hypertension-DM group, and educate the group for care management. Also, depression and physical activity programs are needed for the age.

Changes of Plasma Tumor Necrosis Factor α and C-Reactive Protein Levels in Patients with Hypertension Accompanied by Impaired Glucose Tolerance and their Clinical Significance

  • Background: Chronic inflammation could affect the occurrence and development of malignant tumors. To explore the levels of tumor necrosis factor ${\alpha}$ (TNF-${\alpha}$) and C-reactive protein (CRP) in patients accompanied by impaired glucose tolerance (IGT) and their clinical significance. Materials and Methods: A total of 210 patients hospitalized in Affiliated Hospital of Taishan Medical University from Jun., 2013 to Dec., 2014 were selected, in which 92 cases were accompanied by IGT. Meanwhile, 80 randomly-selected healthy people by physical examination were as the control. The levels of routine biochemical indexes, plasma TNF-${\alpha}$ and CRP in all subjects were measured. Results: Both systolic and diastolic pressures in hypertension group and hypertension plus IGT group were significantly higher than in control group (p<0.01), but there was no statistical significance between these two groups (p>0.05). The levels of fasting plasma glucose (FPG) and blood glucose 2 h after taking glucose in hypertension plus IGT group were markedly higher than other groups (p<0.01). Homeostasis model assessment-insulin resistance (HOMA-IR), TNF-${\alpha}$ and CRP contents were on the progressive increase in control, hypertension and hypertension plus IGT groups, but significant differences were presented among each group (P<0.01). Hypertension accompanied by IGT had a significantly-positive association with CRP, TNF-${\alpha}$, FPG and blood glucose 2h after taking glucose. Conclusions: The levels of plasma TNF-${\alpha}$ and CPR in patients with hypertension accompanied by IGT increase significantly, indicating that inflammatory reaction in these patient increases, thus suggesting that these patients should be focused regarding cancer prevention.
