남아에서 발생한 색소실조증 1례 (A Case of Incontinentia Pigmenti Developed in a Male Newborn Infant)

  • 저자들은 출생시부터 군집상의 홍반성 수포와 부분적으로 사마귀양 병변이 섞여 있는 피부병변을 보인 남아에서 생후 10주부터 갈색 및 검은 색의 색소 침착이 나타나 Bloch-Sulzberger 형의 색소실조증(incontinentia pigmenti)이 의심되어 시행한 피부 생검소견으로 호산성질을 함유한 다수의 수포성 병변이 과각화증과 함께 관찰되는 수포기(bullous stage)와 우상기(verrucous stage)에 속하는 색소실소증으로 확진되고 동반된 증상으로는 경련과 좌안 망막의 삼출성 병변이 관찰된 증례를 경험하였다. 이에 저자들은 지금까지 우리 나라에서는 2례 밖에 소개된 바 없는 남아에서 발생한 색소실조증을 문헌고찰과 함께 보고하였다.

Tyrosinase Inhibitory and Antioxidant Activities of Korean Mistletoe (Viscum album var. coloratum) Extract and Its Fractions

  • The copper-containing enzyme, tyrosinase, catalyzes the oxidation of tyrosine into dihydroxy phenylalanine (DOPA) and subsequently DOPAquinone. It is responsible, not only for the pigment melanin biosynthesis in human skin, but also for browning in foods. In the present study, tyrosinase inhibitory and antioxidant activities of Korean mistletoe extract and its fractions were investigated. As a result, both water and methanol (MeOn) extracts inhibited the tyrosinase activity. Among the fractions, the fraction eluted with $95\%$ MeOn significantly inhibited the tyrosinase activity. The fraction was further purified, and the purified fraction C strongly inhibited the enzyme activity up to $92\%$. In addition, water and methanol extracts exerted radical scavenging effects. The fractions eluted with $70\%\;MeOn\;and\;95\%$ Me on showed high radical scavenging activities. In conclusion, these results suggest that Korean mistletoe extract and its fractions might be useful for the treatment of various dermatological disorders such as epidermal hyperpigmentation and for improving food quality.

사상자 미백성분 Torilin의 추출조건 최적화 (Optimization of Extraction Conditions of Torilin, a Melanogenesis Inhibitor from Torilis japonica Fruits)

  • Torilin is a major sesquiterpene of Torilis japonica (Umbelliferae) fruits and known to be a melanogenesis inhibitor. Extraction conditions are important factor for the efficient preparation to save cost and time in economic aspects. For this reason, this study was conducted to optimize the extraction condition for maximal yield of torilin. For optimization, extraction factors such as extraction solvent, extraction temperature and sample/solvent ratio were tested and optimized for maximum yield of torilin using response surface methodology with Box-Behnken design (BBD). The optimal condition was obtained as a EtOAc concentration in MeOH of 31.8%, an extraction temperature at $30.3^{\circ}C$ and a sample/solvent ratio, 1000 mg/2 ml. The torilin yield under optimal conditions was found to be 9.9 mg/g dried samples, which were well-matched with the predicted value of 10.4 mg/g dried samples. These results will provide useful information about optimized extraction conditions for the development of torilin as cosmetic therapeutics to reduce skin hyperpigmentation.

다발성 안면 반흔을 가진 환자의 반흔 절제술에서 얻은 진피지방을 이용한 함몰 반흔의 동시교정 (Simultaneous Correction of Depressed Scar Using Dermofat Graft Harvested during Scar Revision)

  • While no scar can be completely erased, most linear facial scars can result in a relatively good scar after revision. However, in case of round shaped depressed scar, the scar is often lengthened following an incisional technique. This study focuses on the technique of dermofat graft harvested from the adjacent scars for correction of depressed scars. 18 patients having multiple facial scars with a depressed scar among 375 patients who had undergone scar revision were treated from June 2003 to May 2004. Dermofat was harvested from the adjacent linear scar, then it was deepithelialized, reshaped, and grafted to the depressed scar through a small incision. Cosmetic results were generally good. Complications were overcorrection in 4 patients; hyperpigmentation occured in 1 patient. However, 13 patients were satisfied with the results. The advantages of our technique are as follows: it does not require additional operation; dermofat graft has low absorbable rate; it can maintain the volume with an adequate texture.

  • Cutaneous larva migrans (CLM) is a rare serpiginous cutaneous eruption caused by accidental penetration and migration in the skin with infective larvae of nematode that normally do not have the human as their host. Although CLM has a worldwide distribution, the infection is most frequent in warmer climates. More recently, they have been increasingly imported from the tropics or subtropics by travelers. We experienced two patients who had prutitic serpiginous linear eruption in their skin for a few weeks after traveling to the endemic areas (Brazil and Thailand, respectively) . After the treatment with albendazole, the skin lesions resolved with post-inflammatory hyperpigmentation. We report herein two cases of cutaneous larva migrans successfully treated with albendazole.

Adult-onset Demodicosis Caused by Demodex canis and Demodex cornei in a Yorkshire Terrier Dog

  • An 11-year-old female Yorkshire terrier presented with alopecia, hyperpigmentation, and pruritus. Dermatological examination revealed a large number of Demodex mites including D. cornei and D. canis. Mean length of total body length of D. cornei was one third shorter than that of D. canis. In addition, sequential analysis of DNA extracted from the skin lesions showed a 97% and 99% identity with the two sequences of D. canis and D. cornei. This is the first report of a combined infestation of D. cornei and D. canis in Korea, along with its morphological and molecular characterization.

A comparison of different gingival depigmentation techniques: ablation by erbium:yttrium-aluminum-garnet laser and abrasion by rotary instruments

  • Purpose: The aim of this study is to compare two different gingival depigmentation techniques using an erbium:yttrium-aluminum-garnet (Er:YAG) laser and rotary instruments. Methods: Two patients with melanin pigmentation of gingiva were treated with different gingival depigmentation techniques. Ablation of the gingiva by Er:YAG laser was performed on the right side, and abrasion with a rotary round bur on the opposite side. Results: The patients were satisfied with the esthetically significant improvement with each method. However, some pigment still remained on the marginal gingival and papilla. The visual analog scale did not yield much difference between the two methods, with slightly more pain on the Er:YAG laser treated site. Conclusions: The results of these cases suggest that ablation of the gingiva by an Er:YAG laser and abrasion with a rotary round bur is good enough to achieve esthetic satisfaction and fair wound healing without infection or severe pain. Prudent care about the gingival condition, such as the gingival thickness and degree of pigmentation along with appropriate assessment is needed in ablation by the Er:YAG laser procedure.

보리·현미·율무의 미백효과에 관한 연구 (Depigmentation Activity of Barley, Unpolished Rice, Job's-tear)

  • Objectives: The aim of this study was to investigate the depigmentation effects of Barley, Unpolished rice, Job's-tear. Metbods: We investigated that the extracts of Barley, Unpolished rice, Job's-tear inhibit activity of tyrosinase, the enzyme which convert ts 3-(3,4-dihydroxyphenyl) alanine to dopachrom in the biosynthetic process of melanin, the UV absorbance of the extracts in the UV-A region and UV-B region was measured by UV scanning, the effect of extracts on cell viability and melanin production in cultured B16 mouse melanoma cells were measured, and cytoprotective effects of extracts on PC12 cells injured by hydrogen peroxide measured by MTT assay. Results: The extracts of Barley, Unpolished rice, Job's-tear inhibited activity of tyrosinase in low density. The Barley, Job's-tear extracts not only showed inhibitory effects on melanin production in cultured B16 mouse melanoma cells, but also exhibited cytoprotective effects on PC12 cells injured by hydrogen peroxide in low density. Unpolished rice extract showed inhibitory effect on melanin production in cultured B16 mouse melanoma cells, but did not showed cytoprotective effect Barley, Unpolished rice, Job's-tear extracts did not showed an absorbance effect in the UV-A region and UV-8 region. Conclusions: These results suggest that Barley, Unpolished rice, Job's-tear inhibit melanin biosynthesis which is involved in hyperpigmentation and could be used as a whitening agent.

B16 흑색종세포에서 아피제닌에 의한 멜라닌 합성에 미치는 NADPH 산화효소-유래 활성산소종의 역할 (Role of NADPH Oxidase-mediated Generation of Reactive Oxygen Species in the Apigenin-induced Melanogenesis in B16 Melanoma Cells)

  • Previously, we have reported that apigenin, a natural flavonoid found in a variety of vegetables and fruits, stimulated melanogenesis through the activation of $K^+-Cl^-$-cotransport (KCC) in B16 melanoma cells. In this study we investigated the possible involvement of reactive oxygen species (ROS) in the mechanism of apigenin-induced melanogenesis in B16 cells. Apigenin elevated intracellular ROS level in a dose-dependent manner. Treatment with various inhibitors of NADPH oxidase, diphenylene iodonium (DPI), apocynin (Apo) and neopterine (NP) significantly inhibited both the generation of ROS and melanogenesis induced by apigenin. In addition these inhibitors profoundly inhibited apigenin-induced $Cl^-$-dependent $K^+$ efflux, a hallmark of KCC activity. However, the apigenin-induced ROS generation was not significantly affected by treatment with a specific KCC inhibitor R-(+)-[(2-n-butyl-6,7-dichloro-2-cyclopentyl-2,3-dihydro-1-oxo-1H-inden-5-yl)oxy]acetic acid (DIOA). These results indicate that the ROS production may be a upstream regulator of the apigenin-induced KCC stimulation, and in turn, melanogenesis in the B16 cells. Taken together, these results suggest that the NADPH oxidase-mediated ROS production may play an important role in the apigenin-induced melanogenesis in B16 cells. These results further suggest that NADPH oxidase may be a good target for the management of hyperpigmentation disorders.
