Chloral hydrate, hydroxyzine, 아산화질소($N_2O$), midazolam은 치과치료에 비협조적인 소아환자의 진정시 흔히 사용하는 약물이다. 이러한 진정약물을 두 가지 이상 병용투여 하게 되면 더 적은 용량으로도 진정이 되며 전신적인 부작용도 감소시킬 수 있다. 따라서 병용투여시 나타나는 진정효과를 비교해 본다면 행동조절이 어려운 아이들에게 사용할 수 있는 보다 나은 약물 진정방법을 얻을 수 있으리라 사료된다. 본 연구에서는 전남대학교 병원 소아치과에 내원한 환아중 첫 내원시에 간단한 구강검사에도 행동조절이 어려운 Frankl 1과 2에 해당되는 아동으로서 전신적으로 건강이 양호하고 위장장애가 없는 나이 $18{\sim}92$개월, 체중 $10{\sim}32kg$의 아동 64명(남자 42명, 여자 22명)을 대상으로 약물 병용에 따른 진정효과를 비교하여 다음과 같은 결과를 얻었다. 1. 수면효과는 chloral hydrate와 hydroxyzine의 복합투여군이 midazolam IM과 아산화질소를 병용한 군보다 모든 치료단계에서 매우 좋았으나(p<0.001), 3군과 4군간에는 유의한 차이가 없었다. 2. 울음, 움직임, 전반적 행동에 있어서 제 1군과 제 2군간에는 통계적으로 유의한 차이를 보였으나(p<0.05), 제 3군과 제 4군간에는 유의한 차이를 나타내지 않았다. 3. 전반적인 행동평가항목에 있어서 평균 score가 제 1군은 2.94, 제 2군은 2.07, 제 3군은 2.47, 제 4군은 2.24였다.
Recently combining regimen of minimal doses of chloral hydrate, hydroxyzine and midazolam is good in sedation of children. Midazoalm, water soluble benzdiazepine, has rapid onset and relatively short duration of action. And midazolam has prospective amnesic effect. With these advantages midazolam is popular in conscious sedation for children. This study was to reveal the dose-dependency of behavior and physiologic effects of sublingual midazolam. Sedation records were surveyed retrospectively, of which the patients admitted from April, 2005 to July, 2007. we assigned three groups according the dose of midazolam, 0.1 mg/kg, 0.2 mg/kg and 0.3 mg/kg, respectively and the behavioral evaluation was analyzed with Houpt scale statistically. Combined sublingual midazolam increased the success rate in sedation and the vital signs were stable during sedation.
Background: The aim of this study was to examine the difference of $SpO_2$, PR, $EtCO_2$, RR with submucosal injection of midazolam to oral chloral hydrate and hydroxyzine for pediatric patients Methods: Thirty two sedation cases were performed in this study. Patients were randomly classified into one group taking oral CH (60 mg/kg). hydroxyzine (1 mg/kg) and submucosal injection of midazolam (0.1 mg/kg) and the other group recieving oral CH (50 mg/kg), hydroxyzine (1 mg/kg) and submucosal injection of midazolam (0.2 mg/kg). For evaluating the depth of sedation. data including saturation percentage of oxygen ($SpO_2$), pulse rate (PR), end-tidal carbon dioxide ($EtCO_2$), respiratory rate (RR) and the behavior scale were checked every 2 minutes and were collected for only 40 minutes from the beginning of treatment and were analyzed using Two independent sample T-test. Results: Analysis showed no significant difference in the mean $SpO_2$, PR, $EtCO_2$, RR during sedation between two groups (P > 0.05). The values of $SpO_2$, PR, $EtCO_2$ and RR for both groups remained within the normal values. Conclusions: The results of this present study indicate that combination of oral CH, hydroxyzine, nitrous oxide gas inhalation and submucosal injection of midazolam improved the sedation quality without compromising safety.

배경: 소아 진정 치료 시 서로 다른 용량의 chloral hydrate와 hydroxyzine을 복용 후 midazolam을 구강 점막 하 주사했을 때 행동 반응을 비교하였다. 방법: 총 32회 진정법을 통해 치과치료를 받은 30명의 나이 24-72개월, 체중 20 kg 미만의 미국 마취과학회 신체등급 I의 건강하지만 겁이 많고 협조가 안 되는 소아 환자를 대상으로 2개 치아이상의 보존 치료 및 발치를 필요로 하는 환자를 대상으로 하였다. 호흡기 질환이 있는 아이들은 이 연구에서 제외되었다. 연구 계획은 이대 목동 병원의 임상 실험 심사 위원회에 제출되었다. 1군은 chloral hydrate 50 mg/kg와 hydroxyzine 1 mg/kg 복용 후 점막 하 midazolam 0.2 mg/kg을 추가 투여했고 2군은 chloral hydrate 60 mg/kg와 hydroxyzine 1 mg/kg 복용 후 점막 하 midazolam 0.1 mg/kg을 주사 받았다. 50% nitrous oxide는 치료 중 두 군 모두 유지되었다. 전날 수면 시간과 약물 복용 태도를 기록하였으며 모든 치료 과정은 비디오로 촬영되었다. 맥박 산소 계측기를 이용하여 경피적 산소 포화도와 맥박수를 기록하였고 행동 반응은 Houpt scale을 이용하여 매 2분마다 40분 동안 기록되었다. 전반적인 행동 반응은 Houpt scale를 이용하여 평가되었다. 모든 자료는 SPSS 통계 프로그램을 이용하여 two sample independent t-test를 사용하였다. P 값은 0.05 미만인 경우를 통계학적으로 유의하다고 보았다. 결과: 두 군 간의 경피적 산소 포화도와 맥박수는 모두 정상 범위이며 유의한 차이가 없었다. 행동 반응 비교에서는 치료 처음 10분 동안 2군이 1군에 비해 점수가 높게 나왔으며(P < 0.05), 그 외에는 유의한 차이가 없었다. 전날 총 수면 시간과 약물을 복용하는 태도는 수면 치료 중의 행동 반응에 영향을 주지 않았다. 결론: Chloral hydrate 50 mg/kg 복용과 점막 하 midazolam 0.2 mg/kg은 chloral hydrate 60 mg/kg 복용과 점막 하 midazolam 0.1 mg/kg과 비교할 때 두 약물의 조합은 모두 소아 환자 수면 치료시 안전하고 효과적인 용량이다. Overall behavior와 Q (quiet)의 분포를 비교해 볼 때 두 군 모두 성공적인 진정효과를 기대할 수 있다.
Background: The purpose of this study was to compare the clinical safety and effect with and without additional submucosal midazolam to oral chloral hydrate and hydroxyzine when used for pediatric conscious sedation in a clinical dental environment. Methods: Thirty one cases of pediatric conscious sedations were performed in this study. Selection criteria included good health (ASA I), under 6 years old, 20 kg of body weight, uncooperative behavior and the need for sedation to receive dental treatment including anesthesia and restorative or surgical procedure for at least two teeth. In each visit, patients were randomly assigned into one of two groups; CH group: chloral hydrate (60 mg/kg), hydroxyzine (1 mg/kg), CH-M group: chloral hydrate (60 mg/kg). hydroxyzine (1 mg/kg) and submucoal midazolam (0.1 mg/kg). 50% nitrous oxide-oxygen was maintained during the sedation period Sedations were monitored using a pulse oximeter for estimating pulse rate (PR) and percutaneous oxygen saturation ($SpO_2$). Behavior response rated using Houpt's scale and need of restraint was assessed every 2 minutes through 30 minutes of operative procedure reviewing the videotape recording. Evaluation of overall behavior success was performed using modified overall behavior rate of Houpt's scale. Data was analyzed using t-test. Results: PR and $SpO_2$ for both groups remained within the normal values. The mean scores for sleep and movement of CH-M group were higher than those of CH group (P < 0.05). There were no significant difference in mean score for crying between two groups. The mean scores of overall behavior of CH-M group was higher than those of CH group (P < 0.01). Reinstraint of CH-M group was less required than that of CH group (P < 0.05). Conclusions: Oral chloral hydrate (60 mg/kg) and hydroxyzine (1 mg/kg) combined with submucosal injection of midazolam was safer and showed more improved sedation effect than oral chloral hydrate (60 mg/kg) and hydroxyzine (1 mg/kg) without midazolam for sedation of pediatric dental patients.

Echinostoma hortense (E. hortense) is an intestinal trematode with the highest infection rate in South Korea. However, the immune response against E. hortense infection has not been explained well. In the present study, we investigated the effect of treatment with cyclosporin A (CsA) and histamine receptor antagonists on the cytokine expression and mucosal goblet cells in E. hortense-infected C3H/HeN mice. The alteration of cytokine mRNA expression ($TNF-{\alpha},\;IL-l{\beta},\;IL-4\;and\;IL-5$), intestinal worm recovery rate and goblet cell responses were measured weekly from 0 to 5 weeks post-infection (P.I.) in the control and the following three drug-treated groups: CsA, hydroxyzine and cimetidine. Compared with the control group, the expression of $TNF-{\alpha}$, IL-4 and IL-5 mRNAs decreased in the CsA- and hydroxyzine-treated groups, but only IL-4 mRNA expression did in the cimetidine-treated group. Worm recovery rate was significantly increased in the drug-treated groups. Mucosal goblet cells and their mucin response significantly decreased in the CsA-treated group (P<0.01), but significantly increased in the cimetidine- (P<0.05) and hydroxyzine- (P<0.01) treated groups. These data suggest that CsA treatment inhibits production of Th1- and Th2-type cytokines which are necessary for the worm expulsion. Histamine receptor increases goblet cells and their mucin activation, although it remains to be elucidated whether it directly affects the worm expulsion period of E. hortense in C3H/HeN mice.
이 후향적 연구는 chloral hydrate, hydroxyzine 및 N2O/O2 조합을 이용한 진정 치료를 시행한 149명 환아의 188회 진정 치료의 효과와 부작용에 대해 알아보고, 효과와 부작용에 영향을 미칠 수 있는 요소를 확인하고자 하였다. 진정 치료의 효과는 Houpt 분류법을 이용하여 측정하였고, excellent와 good의 경우 효과적으로 분류하였다. 진정 치료 시간을 15분 간격으로 나누어 효과와 부작용을 분석한 결과, 치료 시간이 증가할수록 효과는 감소하고 부작용은 증가하는 것으로 나타났다. 60분 동안의 진정 치료 효과는 57.4% 이었으며, 전체 부작용 발생률은 18.1%이었다. 진정 치료의 효과는 Body mass index(BMI)가 증가할수록 증가하였으며, 술식 시작 시 진정이 되어있는 경우 진정 치료 효과가 높은 것으로 나타났다. 부작용은 나이가 증가할수록 증가하였다. 진정 치료가 2회로 나누어 시행되었을 때, 진정 치료의 횟수는 효과와 부작용에 영향을 미치지 않았다. 진정 치료 시간을 무리하게 연장하는 것은 효과 감소와 부작용 증가를 불러일으킬 수 있으므로 진정 치료를 2회에 나누어 시행하는 것이 필요할 것으로 생각된다. 또한 BMI, 술식 시작 시 진정 여부 및 나이와 같은 진정 효과를 감소시키고 부작용을 증가시키는 요소에 대한 충분한 고려가 선행된 후 진정 치료를 시행하여야 할 것으로 생각된다.
Chloral hydrate (CH)와 Hydroxyzine의 경구 복용을 이용한 진정과 Midazolam을 점막하 주사로 추가 투여한 진정을 Bispectral(BIS) index를 이용하여 두 진정법의 진정 깊이의 차이를 비교 평가하고자 하였다. 모든 환아는 2세-6세 사이의 미국 마취과학회 신체평가 등급 I의 건강한 환아로 진정 요법이 요구되었으며 국소마취와 최소 2개 이상의 치아에 대한 수복 치료가 치료에 포함되었다. 무작위 방법으로 환아를 CH(60mg/kg)와 Hydroxyzine(1mg/kg)을 복용하는 군과 CH(60mg/kg)와 Hydroxyzine(1mg/kg)을 복용하고 점막하 주사로 Midazolam(0.1mg/kg)을 추가 투여하는 군으로 나누었다. 진정 요법 동안 Nitrous Oxide는 두 군 모두 50%로 유지하였다. 환아 행동 평가는 녹화된 비디오 테이프를 보며 매 2분마다 Behavior scale을 이용하여 quiet(Q), crying(C), movement(M), or struggling(S)으로 측정하였다. 두 군의 진정 깊이 비교 평가를 위해 치료가 시작된 시점부터 40분 동안 2분 간격으로 측정한 Pulse rate(PR), Saturation percentage of $O_2(SpO_2)$, behavior scale과 매 2분 단위로 계산된 평균 BIS index를 t-test와 Levene's test를 이용하여 분석하였다. 두 군의 평균 BIS index와 BIS index의 분포는 유의할 만한 차이를 보였다(P<0.001). 점막하 주사로 Midazolam을 추가 투여한 군에서 평균 BIS index가 더 낮은 값을 가지며 표준 편차가 작았다 PR와 $SpO_2$는 두 그룹 모두 정상범위내의 값을 보였다. 점막하 주사를 이용한 Midazolam의 추가 투여는 안전성을 크게 침해하지 않는 범위 내에서 진정 깊이를 깊게 해주며 안정적인 진정을 이루게 함으로써 보다 바람직한 진정을 유도해 줄 수 있는 것으로 사료된다.
The effects of anti-allergic drugs on intestinal mastocytosis and the expulsion of Neodiplostomum seoulense were observed in Sprague-Dawley rats, after oral infection with 500 metacercariae. The drugs used were hydroxyzine (a histamine receptor H$_1$ blocker), cimetidine (a H$_2$ blocker), cyclosporin-A (a helper T-cell suppressant), and prednisolone (a T- and B-cell suppressant). Infected, but untreated controls, and uninfected controls, were prepared. Worm recovery rate and intestinal mastocytosis were measured on weeks 1, 2, 3, 5, and 7 post-infection. Compared with the infected controls, worm expulsion was significantly (P < 0.05) delayed in hydroxyzine- and cimetidine-treated rats, despite mastocytosis being equally marked in the duodenum of all three groups. In the cyclosporin-A- and prednisolone-treated groups, mastocytosis was suppressed, but worm expulsion was only slightly delayed, without statistical significance. Our results suggest that binding of histamine to its receptors on intestinal smooth muscles is more important in terms of the expulsion of N. seoulense from rats than the levels of histamine alone, or mastocytosis.
