Background: The multitude of the therapeutic usefulness of intra-articular injection of hyaluronate on oateoarthritis of the knee is still in question. The objective of this systemic review was to elucidate both the therapeutic efficacy and the safety of intra-articular administration of hyaluronic acid for degenerative osteoarthritis of the knee joints. Methods: I searched MEDLINE and Korea Medical Database (KMbase) from January 1990 to April 2007 using a combination search terms for knee osteoarthritis and hyaluronic acid and a filter for randomized controlled trials. I extracted data on pain at rest, and during or just after movement, on joint function, and on adverse events. Results: Ten trials that reported usable quantitative information on any of the predefined end points were identified and included in the systemic review. Intra-articular injection of hyaluronic acid can decrease symptoms of osteoarthritis of the knee. The study revealed significant improvements in pain and functional outcomes with few adverse effects. However, there was significant between-study heterogeneity in the estimates of the efficacy of hyaluronic acid. Sub-group analysis showed that lower methodological quality such as a single-blind or single-center design resulted in higher estimates hyaluronic acid efficacy, and that patients older than sixty years of age and those with the most advanced radiographic stage of osteoarthritis were less likely to benefit from intra-articular injection of hyaluronic acid. Conclusion: According to the currently available evidence, intra-articular hyaluronic acid has been proven clinically effective for the patients bearing the knee osteoarthritis with NSAID-induced GI troubles or inapplicable to any surgery, and may be associated with lower risk of adverse events.
견피브로인의 응용가능성을 높이기 위하여 대표적인 생체고분자인 히아론산을 이용하여 견피브로인/히아론산 브렌드 필름을 제조하였으며 구조 및 열 특성을 살펴보았다. 1. 적외선 분광분석 결과 견피브로인과 히아론산 두 고분자 사이에는 수소결합 등의 상호작용이 존재하지 않는 것으로 나타났다. 또한 브렌드 필름내에 존재하는 실크피브로인은 EDC 에탄올 용액처리에 의하여 $\beta$-sheet 구조로 전이되었다. 2. 견피브로인/히아론산 브렌드 필름에서 견피브로인과 히아론산 각각의 열분해 흡열피크와 발열피크 온도에는 큰 변화가 없었으나, 열분해 피크의 폭이 넓어지는 것으로 봐서 실크 피브로인과의 브렌드 또는 EDC 처리에 의하여 열분해 반응이 매우 복잡한 환경하에서 진행되고 있음을 알 수 있었다. 3 견피브로인/히아로산 브렌드 필름은 자기 중량 대비 평균 70배의 수분을 흡수하였다.
Harmaline을 포함한 ${\beta}-Carboline$ 알카로이드들은 마이크로조움의 효소성 또는 비효소성 지질 과산화를 억제한다고 제시되고 있으나, 이들의 항산화 작용기전은 분명하지 않다. 본 연구에서는 $Fe^{2+}$와 $H_2O_2$에 의한 hyaluronic acid, 지질과 콜라젠의 산화성 손상에 있어 harmaline과 harmalol의 항산화 능력을 관찰하였다. 또한 반응성 산소대사물에 대한 이들의 제거작용을 조사하였다. Harmaline, harmalol, superoxide dismutase, catalase와 DMSO는 $Fe^{2+}$와 $H_2O_2$에 의한 hyaluronic acid의 변성과 $Fe^{2+}$에 의한 지질 과산화를 억제하였다. 이들 반응에서 DABCO는 hyaluronic acid의 변성을 억제하였으나 지질 과산화에 영향을 나타내지 않았다. ${\beta}-Carboline$은 $Fe^{2+}$, $H_2O_2$와 ascorbic acid에 의한 cartilage collagen의 변성을 억제하였다. Superoxide dismutase에 의하여 억제되는 $Fe^{2+}$의 자가산화에 따른 ferricytochrome c의 환원은 harmaline과 harmalol의 영향을 받지 않았다. 또한 이들은 $H_2O_2$에 대하여 분해작용을 나타내지 않았다. $Fe^{2+}$와 $H_2O_2$의 존재하에서 OH 생성은 harmaline, harmalol과 DMSO에 의하여 억제되었다. Harmaline과 harmalol은 반응성 산소대사물인 OH 과 아마도 철이온-산소 복합체에 대한 제거작용으로써 $Fe^{2+}$와 $H_2O_2$에 의한 hyaluronic acid, 지질과 콜라젠의 산화성 손상을 억제하고, 항산화 능력을 나타낼 것으로 추정된다.
인체 보형물의 생체 재료로 사용할 목적으로 히아루론산 비드를 제조하였다. 히아루론산 수용액과 콩기름을 섞어서 얻어진 현탁상태에서 히아루론산을 1,3-butadiene diepoxide로 가교시켰다. 제조된 비드의 물성을 살펴보기 위해 직경, 표면적과 팽윤도를 측정하였고 전자현미경으로 표면상태를 관찰하였다. 가교제의 농도가 $5-12\;vol\%$ 범위에서 비드가 형성되었으며 제조된 비드는 단순분산성을 보였다. 히아루론산 농도 혹은 가교제 농도가 증가할수록, 가교 온도가 감소할수록, BET표면적과 팽윤도가 감소하였다. 혼합속도의 변화에 의해 비드의 물성은 거의 변하지 않았으나 비드의 크기는 효과적으로 조절되었다.

Purpose: The purpose of this study was not only to evaluate the relative mRNA expression of interleukin-$1{\beta}$(IL-$1{\beta}$), cyclooxygenase2 (COX-2) and prostaglandin E2 (PGE2) by RT-PCR analysis but to observe pattern of edema by light microscopic and electron microscope after topical apply of hyaluronic acid in inflammation-guided mouse. Material and methods: Mice of this study were devided into 4 groups: Control group (no inflammation guided), Positive control (inflammation guided + vaselin apply), Protopic group (inflammation guided + protopic apply), Hyaluronic group (inflammation guided + hyaluronic acid apply). Results: Hyaluronic group showed less expressions of IL-$1{\beta}$, COX-2, PGE2 than those of positive control & protopic group. Hyaluronic group revealed a decreased inflammation than positive control & protopic group in Light Microscope. Hyaluronic group appeared decreased edema of ear compare to positive control & protopic group in Elecron Microscope. Conclusion: It was considered that hyaluronic acid has an antiinflammatory effect for intercepting the gene expression of cytokines related to inflammation.
외인성 taurine이 HOCl, $NH_2Cl$과 그밖의 산화성 물질에 의한 hyaluronic acid의 변성에 미치는 영향을 관찰하였다. HOCl, $NH_2Cl$과 그밖의 산화성 물질에 대한 taurine의 제거 작용을 조사하였다. Taurine의 항 산화 작용을 또한 치올 화합물의 작용과 비교 관찰하였다. Hyaluronic acid의 점성도는 HOCl과 $NH_2Cl$에 의하여 뚜렷하게 용량에 따라 감소하였다. Hyaluronic acid에 대한 HOCl의 변성 효과는 $NH_2Cl$에 의한 것보다 현저하였다. Taurine은 HOCl과 $NH_2Cl$에 의한 hyaluronic acid의 변성을 효과적으로 용량에 따라 억제하였다. HOCl의 변성 효과는 DMSO에 의하여 뚜렷하게 억제되었다. $Fe^{2+}$과 $H_2O_2$에 의한 hyaluronic acid의 변성은 catalase와 DMSO에 의하여 억제되었으나 taurine의 영향은 받지 않았다. Xanthine과 xanthine oxidase의 변성 작용은 SOD와 catalase에 의하여 효과적으로 억제되었으나 taurine의 영향은 받지 않았다. HOCl은 taurine, DMSO, GSH와 MPG에 의하여 유의하게 분해되었다. 파장 250 nm에서의 HOCl의 흡광도와 파장 242 nm에서의 $NH_2Cl$의 흡광도는 taurine의 첨가로 유의하게 증가하였다. $NH_2Cl$과 GSH 또는 MPG의 상호 작용으로 초기에 최대의 흡광도가 관찰되었으나, 이러한 흡광도는 반응 시간에 따라 점차적으로 감소하였다. $Fe^{2+}$와 $H_2O_2$의 존재하에서 OH 의 생성은 catalase와 DMSO에 의하여 억제되었으나 taurine의 영향은 받지 않았다. DABCO와 DABA에 반응하는 자외선 조사에 따른 $^1O_2$의 생성은 taurine의 영향을 받지 않았다. GSH와 MPG는 HOCl의 변성 작용을 뚜렷하게 억제하였다. 이상의 결과로 부터 hyaluronic acid의 변성을 포함한 조직 구성성분의 산화성 손상에 있어서 taurine의 보호 작용은 산소 유리 라디칼에 대한 제거 작용과 관계없으며 HOCl과 $NH_2Cl$에 대한 제거 작용 그리고 taurine과 HOCl 또는 $NH_2Cl$의 복합체 형성에 기인할 것으로 시사된다. Taurine의 치올 기가 HOCl과 $NH_2Cl$에 의한 변성에 부분적으로 보호 작용을 나타 낼 것으로 추정된다.
회향은 미나리과에 속하는 다년생식물인 Foeniculum vulgare Mill.의 성숙한 과실로서 항염, 진통, 피부노화방지 등의 효과를 가지고 있어 다양한 질환에 우수한 치료효과를 나타내는 것으로 알려져 왔다. 따라서 본 연구에서는 기능성이 우수한 회향 열매 추출물을 이용하여 피부 질환 치료제 및 피부장벽 기능 개선 소재로서의 적용가능성을 확인하였다. 이를 위해 각질형성세포주인 HaCaT에 회향 열매 메탄올 추출물과 그 분획물(hexane, methyl chloride, ethyl acetate, butanol)을 처리한 후, involucrin, loricrin, filaggrin의 발현과 hyaluronic acid의 생성 및 β-defensin -1, -2, -3, LL-37의 mRNA 발현을 측정하였다. 그 결과butanol 분획물 50 μg/ml을 24시간 동안 처리시 involucrin과 filaggrin단백질 발현을 각각122.8%, 105%로 유의하게 증가시킴을 확인하였다. Elisa assay를 통해 분석한 결과, ethyl acetate와 butanol 분획물은 대조군에 비해 hyaluronic acid의 생성을 각각17%, 11% 증가시켰으며, 이는 hyaluronic acid synthesis-1의 mRNA 발현 증가에 의한 것임을 확인하였다. 그러나 회향 열매 메탄올 추출물과 분획물 모두에서 β-defensin -1, -2, -3, LL-37의 mRNA 발현은 증가되지 않았다. 이러한 결과는 회향 열매 butanol 분획물이 각질형성세포에서 물리적 장벽을 형성하고 보습인자 조절을 통해 피부장벽 기능을 강화하는데 효과적임을 의미한다.
Purpose: The purpose of this study was to evaluate the clinical efficacy of enhancing deficient interdental papilla with hyaluronic acid gel injection by assessing the radiographic anatomical factors affecting the reconstruction of the interdental papilla. Methods: Fifty-seven treated sites from 13 patients (6 males and 7 females) were included. Patients had papillary deficiency in the upper anterior area. Prior to treatment, photographic and periapical radiographic standardization devices were designed for each patient. A 30-gauge needle was used with an injection-assistance device to inject a hyaluronic acid gel to the involved papilla. This treatment was repeated up to 5 times every 3 weeks. Patients were followed up for 6 months after the initial gel application. Clinical photographic measurements of the black triangle area (BTA), height (BTH), and width (BTW) and periapical radiographic measurements of the contact point and the bone crest (CP-BC) and the interproximal distance between roots (IDR) were undertaken using computer software. The interdental papilla reconstruction rate (IPRR) was calculated to determine the percentage change of BTA between the initial and final examination and the association between radiographic factors and the reconstruction of the interdental papilla by means of injectable hyaluronic acid gel were evaluated. Results: All sites showed improvement between treatment examinations. Thirty-six sites had complete interdental papilla reconstruction and 21 sites showed improvement ranging from 19% to 96%. The CP-BC correlated with the IPRR. More specifically, when the CP-BC reached 6 mm, virtually complete interdental papilla reconstruction via injectable hyaluronic acid gel was achieved. Conclusions: These results suggest that the CP-BC is closely related to the efficacy of hyaluronic acid gel injection for interdental papilla reconstruction.
Purpose: This study sought to investigate the appropriate indications for intra-articular injection of hyaluronic acid in ankle osteoarthritis, its efficacy at each stage of osteoarthritis and to compare the efficacy related differences at each stage. Materials and Methods: A retrospective study was conducted from January 2016 to September 2019 on 43 patients (50 cases) diagnosed with ankle arthritis and given intra-articular injection of hyaluronic acid. Patients were classified according to the modified Takakura ankle osteoarthritis stages. Stage 2 comprised 24 cases; stage 3A, 15 cases; stage 3B, 8 cases; and stage 4, 3 cases. The clinical evaluation was carried out using the visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS) score, and the Foot and Ankle Outcome Score (FAOS). The patients' satisfaction with the injection was evaluated at 1 year after injection. A radiographic evaluation was performed to gauge the progression of osteoarthritis before and after the injection. Results: The VAS, AOFAS score, and FAOS at 3 months and 1 year after injection showed statistically significant improvements compared to those before the injection. There was a significant difference in the VAS for each stage over time; however, this was not the case with the AOFAS score and the FAOS. A significant improvement of the VAS after injection was seen only in stage 3A when compared with the other stages. The overall satisfaction rate with the injection was 90%. There were no ankle osteoarthritic stage changes in the serial follow-up radiograph. Conclusion: Intra-articular injection of hyaluronic acid resulted in a significant clinical improvement up to 1 year after the injection. Therefore, the intra-articular injection of hyaluronic acid could be a treatment option for ankle osteoarthritis.
Hyaluronic acid (HA) is a mucopolysaccharide, occurring naturally in living organisms. It is one of the most hydrophilic molecules, so it has been known as being related to skin hydration and skin aging. The purpose of this study was to examine the effects of Cimicifuga heracleifolia ethanol extract on the hyaluronic acid synthesis and the inhibition of hyaluronidase activity. To determine cytotoxicity, hyaluronic acid synthase 2 (HAS2) gene expression, HA production and, hyaluronidase inhibitory effects, 3-(4,5-dimethylthiazol-2-ly)-2,5-diphenyl tetrazolium bromide (MTT) assay, real time - polymerase chain reaction (RT-PCR), hyaluronic acid enzyme linked immunosorbent assay (HA-ELISA), and hyaluronidase assay were used, respectively. When the Cimicifuga heracleifolia extract was treated in the HaCaT cells up to 500 ㎍/mL concentration, cytotoxicity was confirmed by the Cimicifuga heracleifolia extract at concentrations above 200 ㎍/mL. Therefore, the optimum concentration of all experiments used in this study was determined to be 200 ㎍/mL. HAS2 gene expression increased by Cimicifuga heracleifolia extract in a concentration-dependent manner at all treatment concentrations. The production rate of HA was tended to decrease at the highest concentration of 200 ㎍/mL. The hyaluronidase activity inhibition effect of Cimicifuga heracleifolia extract was very high compared to the control group. Based on these results, Cimicifuga heracleifolia extract was expected to have a moisturizing effect on human skin and special attention should be paid to the determination of the concentration of Cimicifuga heracleifolia when developing cosmetic materials using it.
