의료서비스를 이용하는 고객의견 대한 구현은 효과적이고 효율적인 관리가 중요하다. 이에 본 연구는 고객의 불만족 사항에 대한 내용의 의료융합적 분석을 확인하여 전략을 수립하고자하는 후향적 조사연구이다. 2014년 3월부터 2015년 2월까지 일 상급종합병원의 민원 319건을 대상으로 분석하였다. 본 연구에서는 초진보다 재진이, 입원환자보다는 외래환자의 불만족이 높았으며 불만족 분류 시 행정서비스의 업무절차, 진료 및 간호영역의 의사소통 및 설명부분에서 불만족이 높았다. 질병 분류에서는 소화기계와 암 진단관련 대상자들의 불만족이 높았다(p<0.05). 본 연구를 바탕으로 의료서비스의 질을 향상시키고 불만족의 재발을 막는 고객 지향적 의료융합 경영전략을 수립하기 위해 각 환자의 질환별 불만족 요인의 내용을 융합분석하고 지속적인 융합연구를 통해 질환별 구체적인 의료서비스 전략에 대해 유형을 구조화 할 필요가 있다.
This study was to compare the functional status. complication and readmission rates. and client satisfaction with nursing care of home-based care and hospital-based care for clients with Coronary Artery Bypass Graft. Raw data were collected by interviewing and reviewing charts of 41 clients with Coronary Artery Bypass Graft between June 2001 and July 2002 at an university hospital located in Seoul. Korea. Out of 41 clients. 15 were in home care group and 26 were in hospitalization group. The baseline characteristics of the groups were almost identical. Mean age was 61.7 and 75.6% of clients being male. For home care group. the data collection was made at discharge and at termination of home care. and for hospitalization group. at discharge and at the first visit of outpatient department. Complication and readmission rates were investigated at one month after operation. Collected data were then analysed by conducting Chi-square test. Wilcoxon rank sum test. and Wilcoxon signed ranks test with SPSS program. The level of significance was .05. The results of the study are summarized as follows: 1. Postoperative length of stay of the home care group was shorter than that of hospitalization group by 1. 14 days(8.45 days vs. 9.59 days). On average. 1.8 home visits per client were observed. 2. The functional status (Barthel Index) at the termination of home care was significantly increased from that at discharge. For hospitalization group. a significant increase was observed between the functional status at the discharge and that at the first visit of outpatient department. The differencies in incremental of the scores. between the groups. were however not significant. 3. Complication and readmission rates; no statistically significant difference between the groups was observed. 4. The client satisfaction with nursing care (CSS) at termination of home care was significantly higher than that at hospital discharge. In conclusion. the outcomes of the analysis suggest that the home care benefits clients with Coronary Artery Bypass Graft. Client satisfaction with nursing care rises at termination of home care as compare to that measured at hospital discharge. Meanwhile. there was no significant differences in functional status. and complication and readmission rates. Further. home care reduced the length of stay in hospital.
Purpose: Most patients with acute low back pain visit emergency room (ER). They mostly need beds, and if their length of stay is longer, it can become difficult to accommodate new patients at the ER. We analyzed the treatment process of patients with back pain and tried to find method for shortening of the length of stay at the ER. Methods: We retrospectively analyzed the medical records of patients with back pain who visited at our ER for one year. Patients were divided into two groups according to their length of stay at ER and were compared the charateristcs of between two groups. Results: A total of 274 patients were included in the study. Eigthy-nine patients (32.5%) were in the group with less than 3 hours and 185 patients (67.5%) were in the other group. In the comparison of the two groups according to the medical departments, the number of patients who were in group with more than 3 hours were 25 (14.0%) in the emergency department, 94 (50.5%) in neurosurgery, 66 (35.5%) in orthopedic surgery. Length of stay was significantly increased in orthopedic surgery and neurosurgery (p=0.014). In addition, the length of stay was longer when computed tomography and magnetic resonance imaging examinations were performed (p=0.000). Regardless of the type of analgesic agent, the median time to the analgesic treatment was shorter in the group with less than 3 hours (p=0.034). Conclusions: In patients with back pain who visit the ER, the emergency medicine doctor will early control the pain and do not unnecessary image examination to reduce a length of stay at the ER.
Objectives : Patients who visit oriental medical hospital for growth treatment are increasing. So we aimed to classify the tendency of the patients. Methods : We studied 231 patients who visited Oriental medical hospital for growth treatment from January 2004 to August 2005. We classified sex ratio, height percentile, symptom form of the Oriental medicine, age ratio and developed complication of patients. We used X-ray, endoscope for nasal cavity, blood sample, the Standard Growth Table made by the Korean Association of Pediatrics, 1998. Results : The results which were classified as follows; boys were 50.2 percentile, girls were 49.8 percentile. The classification according to age stage resulted as follows; infant stage 1.3 percentile, preschool age 13.4%, school age 28.6%, rapid growth stage 22.9 percentile, puberty 33.8 percentile. The classification according to height percentile resulted as follows; 3 percentile or under 12.1%, 25 percentile or under 48.9%, 50 percentile or under 25.6%, 75 percentile or under 10.8%, 75 percentile excess 2.6%. The classification according to disease resulted as follows; paranasal sinusitis 12.1 percentile, allergic rhinitis 10 percentile, atopic dermatitis 5.6 percentile, nocturia 3 percentile, convulsion or tic disorder 2.2 percentile, precocious puberty 1.3 percentile, Tuner syndrome 0.9 percentile, developmental disability 0.4 percentile. Conclusions : Sex ratio of children patients who visited Orienatal medical hospital were nearly the same. 13.4% of hospital visit children patients were taller than average. Most of the male children patients were school ages while the female were in puberty. 35.5% of them have developed complication as follows; paranasal sinusitis, allergic rhinitis, atopic dermatitis, nocturia, convulsion or tic disorder, precocious puberty, Tuner syndrome and developmental disability.
This research is intended to inquire any discrepancy depending on the variables such as types, geography and size of hospitals of Korea in order to develop differentiated marketing strategy and to investigate how the aforementioned variables affect the management performance such as the increase in the number of foreign patients, their willingness to re-visit, the increase in profit and etc. The survey for this research was conducted for five weeks from July 10th 2014 to August 30th 2014 with 251 participants in charge of foreign patient attraction for 161 domestic hospitals. As the method of the research, a hypothesis was first established based on previous studies, followed by the incorporation of self-administered questionnaire to confirm the formulated hypothesis. Frequency analysis and ANOVA analysis were used to analyze the result of the survey. The outcome of the research and the implications are as follows. First, according to the demography of the persons-in-charge of foreign patient attraction, the proportion of female was superbly high by 76.5%. As for the age, those in their 30s were highest in proportion. Finally, for the education level, college graduates took up the largest portion by 46.5%. Second, in respect to the hypothesis assuming the difference in performance depending on the types of hospitals, national university-affiliated hospitals showed the highest level of the increase in foreign patients by the average of 3.25. Third, in respect to the hypothesis assuming the difference in performance based on geographical position, it was confirmed that hospitals in Ulsan City experienced the largest growth in the number in foreign patients. Fourth, in respect to the sizes of hospitals, those with 201 to 300 sickbeds showed the highest increase by the average of 3.45. The implication of the above research outcome indicates that while the number of foreign patients visiting Korea for medical purposes is on the rise, the number of professionals in place with necessary knowledge and capacity is insufficient and requires improvement.
Purpose: The purpose of this study was to identify parent's knowledge and cope with their child's fever. Confirmed data provide source and it based on parent's education program and nursing intervention. Method: The research design was a descriptive study by questionnaire. Data were collected from July, 16, 2007 to August, 6, 2007. A total 165 parents who visited the children's hospital in Gwangju. Data were analysed using the SPSS WIN 12.0 program. Results: The results of this study were as follows: 1. Fever was major cause which to visit hospital among children(50.3%). 2. Parents main concerned by child fever were convulsion(52.7%) and brain injury(46.7%). 3. When cause fever most parents checking interval are 30 minutes (38.8%), the most fever management was used antipyretics with tepid water massage(59.4%). 4. Antipyretics used interval was 4hours(56.4%). 5. When visit to hospital by fever, most treatment was antipyretics. 6. The method to get fever information, first by doctor(68.5%), second by family-relative(41.8%) and last by nurse(22.4%). Conclusion: It is necessary to provide information about management of fever. Development and distribution for effective education program for child fever at home are also necessary. Therefore we suggest the education program on the internet.
The objectives of this study were to identify factors associated with unexpected revisit to an emergency medical center (EMC) located in Seoul and to examine reasons for revisit. During March, June, September and December, 2002, a total of 168 patients had unexpected revisits to the EMC within 48 hours of a previous discharge. As a 1:1 matched control, we included 136 patients who: discharged from the EMC during the same time period: did not return to the EMC; had the same diagnosis and age(${\pm}5$) with the case. In this study, factors associated with unexpected revisits were defined as characteristics of a previous discharge, which were classified into three: sociodemographic, EMC visit-related, and discharge management factors. Reasons for revisit were categorized into disease, physician, patients, and system-related factors. Data were collected by medical chart review with assistance from clinicians of the EMC. Logistic regression results showed that patients who headed home after discharge without follow-up schedule had a 27.6 times higher risk of revisiting EMC than those who were hospitalized following EMC visit. Patients discharged on his own will had a 5.9 times higher risk of revisiting than those discharged following physician's advice. Patients requiring continual observation at the time of discharge were more likely to revisit by 8.7 times than those discharged with improved condition. About 69.13% of the revisits were due to disease-related factors, followed by 13.90% due to patient-related factors, 8.64% due to system-related factors, and 8.34% due to physician-related factors. It appears that the most significant factors influencing revisits are discharge management factors such as patient's condition at discharge, whether the discharge was accorded with physician's advice, and whether returning home without follow-up schedule. Therefore, appropriate discharge management is necessary to prevent EMC revisit.
Objectives: The aim of this study is to evaluate the association between shiftwork and skeletal muscle mass index in a single university health check-up. Methods: We used data from 98,227 workers who answered in a special interview on health check-up at a local university hospital from 2014 to 2020. Pearson correlation analysis was conducted for comparing the association between skeletal muscle mass index and demographic and hematological variables in shiftwork and non-shiftwork groups. Mixed linear model analysis after controlling demographic and hematological variables was used to analyze the difference of skeletal muscle mass index between groups at every visit for seven years. Results: In linear regression analysis, the variables most significantly correlated with skeletal muscle index in both groups were shiftwork(p=0.049), BMI(p<0.001), hypertension(p=0.024), platelet(p<0.001), total protein (p<0.001), AST(p=0.028), ALT(p=0.003), ALP(p<0.001), total cholesterol(p=0.002), triglyceride(p=0.019), BUN (p=0.001), creatinine(p<0.001), and uric acid(p=0.002). After the adjustment for demographic and hematologic variables, the skeletal muscle mass index at every visit was decreased both in the shiftwork group and non-shiftwork group. The slope of the shiftwork group was -0.240 and non-shiftwork group -0.149, showing a significant difference (p<0.001). Conclusions: In the shiftwork group, the skeletal muscle mass index showed a tendency to decrease markedly over time compared to the non-shiftwork group. It is presumed that shift workers' skeletal muscle health was adversely affected by changes in the biological clock due to changes in wake-up and sleep patterns, and changes in food intake.
Objectives The purpose of this study is to analyze the latest clinical trends in peripheral facial nerve palsy pediatric and adolescent patients who treated in Korean medicine hospital. Methods The study was conducted based on 185 cases of pediatric and adolescent patients with peripheral facial nerve paralysis who visited Korean medicine hospital from January 2017 to June 2022. Results The mean age of onset of facial nerve paralysis in children and adolescents was 11.7 years, and the incidence rate was higher in boys than in girls. The seasonal distribution was the most common in autumn, and 91.4% were diagnosed with Bell's palsy. The recurrence rate was 7.6%. The severity was evaluated on the House-Brackmann Grading System (HBGS) scale, and the most common was Grade III. It took an average of 6.5 days from the date of onset to visit the hospital, and 75.7% visited the hospital within a week from the date of onset. Hospitalized treatment was 69.2%. The higher the HBGS grade, the more hospitalized treatment was, and the total number of treatments and the duration of treatment tended to increase. The average treatment period from the first visit date was 119.6 days. 17.8% received only Korean medicine treatment, and 69.2% took Western medicine with herbal medicine. Conclusions In this study, there was a difference in the average treatment period from previous studies according to the study period setting. Long-term studies on the recovery rate and prognosis of pediatric facial paralysis are needed.

The purpose of this study is to research the actual conditions of hospital recruitment by using internet homepage of Korea College of Hospital Administrators(KCHA). Data were collected from 399 iob posting and job seeker's interest in Internet homepage of KCHA. This study analyzed the research model using data mining method. This study shows that the frequency of job posting and job seeker's interest depend upon a season. The result of this study indicates that there is necessity to present the various and specific employment conditions. As the recruitment becomes job seeker-centered in other industry labor market, it is also recommended in the health labor market. Therefore a job posting should provide a salary range and emphasize competitive benefits for job seekers in the health labor market.
