아동병원 간호사의 감염관리조직문화, 감염예방환경이 감염관리 수행에 미치는 영향 (Factors Influencing Infection Control Performance by Children's Hospital Nurses Infection Control Organizational Culture and Infection Prevention Environment)

  • The purpose of the study is to determine the relationship between the infection control organizational culture, infection prevention environment, and infection control performance of nurses at a children's hospital and to identify factors affecting infection control performance. The subjects of the study were 160 nurses from five children's hospitals, and data collection was conducted from February 19 to 29, 2024. Data analysis was performed using frequency, percentage, mean, standard deviation, and difference analysis of variables using t-test, ANOVA, Pearson's correlation analysis, and multiple regression analysis. As a result of the study, infection control performance was positively correlated with infection control organizational culture (r= .610, p< .001) and infection prevention environment (r= .586, p< .001), and as a result of multiple regression analysis, infection control organization The influencing factors appeared in the following order: culture (β= .369), infection prevention environment (β= .312), medical institution accreditation evaluation experience (β= .165), and infection control education experience (β= .137), and the overall explanatory power was It was 50.8% (F=41.966, p< .001). Based on the results of this study, to carry out infection control in children's hospitals, integrated management including the will and effort of individual nurses, support and policy from medical institutions and the government is needed, and the development of an infection control education program that takes into account the special characteristics of children's hospital nurses. Application is necessary.

  • Purpose: This study was conducted to identify influencing factors on the performance of healthcare-associated infection control and microbiological hand contamination among caregivers at a tertiary hospital. Methods: The participants of this study were 59 caregivers woring at a tertiary hospital. Data were collected from July 1 to 30, 2018. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffé test, Pearson's correlation coefficients and stepwise multiple regression by SPSS 23.0 Win program. Results: Multiple regression analysis revealed that factors influencing performance of healthcare-associated infection control were awareness (β=.63, p<.001) and the experience of infection-related education (β=-3.40, p=.042). Regression equations describing the performance of healthcare-related infection control were found to be appropriate (F=27.29, p<.001) and accounted for 68% of variance. Factors affecting the degree of microbiological hand contamination were work experience (β=-0.28, p=.026) and healthcare-related infection performance (β=-0.28, p=.029). A regression equation describing the microbiological hand contamination was appropriate (F=6.10, p=.004) and accounted for 42% of variance. Conclusion: The findings of this study suggest that it is necessary to increase performance of healthcare-associated infection control by caregivers. Also, educations for preventing healthcare-associated infection and guidelines for increasing compliance with healthcare-associated infection control are recommended to improve performance of healthcare-associated infection control.

종합병원 간호사의 감염관리지식, 간호근무환경이 감염관리수행도에 미치는 영향 (Effect of Infection Control Knowledge and Nursing Work Environment on Infection Control Performance among General Hospital Nurses)

  • Purpose: The purpose of the study was to investigate the effects of infection control knowledge and nursing work environment on infection control performance among general hospital nurses. Methods: A total of 138 nurses from four general hospitals located in G and P cities participated in this descriptive study. The collected data were analyzed using t-test, ANOVA, Pearson's correlation coefficients, and multiple regression. Results: The average mean score was 21.41±1.92 points out of 25 for infection control knowledge, 2.53±0.33 points out of 4 for nursing work environment, and 4.62±0.40 points out of 5 for infection control performance. In the final analysis, the variables that had the greatest influence on infection control performance were the nursing work environment (β=.32, p<.001), working department (β=.19, p=.014), and knowledge (β=.19, p=.016). Conclusion: This study showed that nurses with a better nursing work environment, employment in general department, and more infection control knowledge delivered superior infection control performance. Therefore, to improve infection control performance, it is necessary to enhance the nursing environment and develop practical education programs for augmenting infection control knowledge.

임상간호사의 병원감염관리에 대한 중요성 인지도, 실천정도 및 만족도에 관한 연구 (A Study on Clinical Nurses Level of Perception of Importance, Performance and Satisfaction in the control of Nosocomial Infection.)

  • Hospitals accomodate patients who have a high risk of infection due to reduced immunity as well as people who require surgical, medical or other treatments. Consequently, the role of clinical nurses, who come into close contact with these patients is very important in the control of nosocomial infection. This study was done to investigate and compare the level of perception of the importance of the control of nosocomial infections as well as the level of actual performance, and the level of satisfaction with the control of nosocomial infection by the clinical nurses. Thus, the purpose of this study is to contribute basic data for improving policies and educational programs to control nosocomial infection. A summary of the survey results is as follows. 1) The means of scores on all categories of the inquiry were 4. 51 for awareness 4.42 for actual performance, and 3.20 for satisfaction, of a possible high score of 5.00. 2) Correlations of the level of perception of importance between characteristics of nurses and hospital control of nosocomial infection differed significantly according to the type of hospital establishment type (p=.005), age(p=.000), career(p=.000), position (p=.002), and regular conferences on infection control in working departments(p=.003), Correlation of the level of actual performance between characteristics of nurses and hospital control of nosocomial infection diffesed significantly according to type of hospital(p=.000), hospital size (p=.009), working department(p=.000), age(p=.000), career (p=.000), school career(p=.040), position (p=.000), education experience on nosocomial infection(p=.020), and regular conferences on infection control in working department(p=.000). Correlation of degree of satisfaction between characteristics of nurses and hospital control of nosocomial infection also differed significantly according to the type of hospital establishment (p=.003), working department(p=.000), age (p=.000), and regular conferences on infection control in working department (p=.000). 3) Correlation between clinical nurses, level of perception of importance and actual level of performance for the control of nosocomial infection was relatively positive (r=.57, p=.000). Correlation between clinical nurses degree of satisfaction and level of actual performance for control of nosocomial infection was relatively positive (r=.47, p=.000). Correlation between clinical nurses, level of perception of importance and degree of satisfaction degree with the control of nosocomial infection was also relatively positive (r=.27, p=.000).

노인요양병원 간호사의 병원감염관리에 대한 인지도 및 실천도: 융복합적 접근 방안 모색 (Perception and practice of hospital infection control in nurses of geriatric hospital : for convergent approach)

  • 본 연구는 융복합적 접근을 위해 노인요양병원 간호사의 병원감염관리에 대한 인지도와 실천도를 파악하고, 제변수간의 관계를 조사하는 연구이다. Lee(2011)가 사용한 병원감염관리 설문지를 이용하여 2012년 10월 29일부터 11월 3일 까지 자료를 수집하였고, 수집된 자료는 t-test, ANOVA 및 Correlation 으로 분석하였다. 병원감염관리 실천도의 평균 점수는 인지도 평균 점수 보다 통계적으로 유의하게 낮았다. 영역별 인지도와 실천도는 손씻기 영역, 호흡기감염관리 영역, 소독 오염물품관리 영역, 감염성 폐기물관리 영역에서 통계적으로 유의한 차이가 있었으며 실천도가 인지도보다 낮았다. 인지도는 연령, 학력, 감염관리지침서 유무에 따라 차이가 있었으며, 실천도는 감염관리지침서 유무에서만 차이가 있었다. 병원감염관리 인지도와 실천도는 통계적으로 유의한 정적상관관계가 있었다. 따라서 본 연구결과는 노인요양병원 감염관리에 대한 인지도와 실천도를 높이기 위한 제도적 지원과 더불어, 지속적이고 다양한 교육방법을 통해 체계적이고 전문적이며 실제적인 교육을 제공하는 것이 필요하다는 것과 병원감염관리 능력을 증진시키기 위한 프로그램을 개발할 필요를 제기한다.

  • Purpose: Emergency room(ER) is the first place to enter a hospital where patients who might have been infected with contagious disease. Therefore, ER should be designed with infection control in mind. Researchers examined hospital ER layouts to identify layout design that support infection control. Methods: This study analyzed the hospital ER layout of Korean and other hospitals abroad. Researchers focused on route of incoming patients who potentially have infectious disease. Crossing of this route with other routes such as for imaging and testing should be avoided for infection control. Results: There were certain hospital ERs with better control of infection related incidents. ER floor plan layout is analyzed about allocation of key functions with movement routes for each role such as patients and medical staff in mind. To identify layout strategies for ER functions researchers simplified the routes in ER into diagrams. Layout options show that bypassing infection suspected routes over other routes is possible. Implications: Hospitals can control infection easier when they adopt strategic ER layout identified in this study.

일반 병원감염, MRSA 및 VRE 감염관리에 대한 간호사의 인지도와 수행정도 비교연구 (A Comparative Study of Nurses' Recognition and Practice Level of General Nosocomial Infection, MRSA and VRE Infection Control)

  • Purpose: The purpose of this study was to describe nurses' recognition of, and practice level in management of general nosocomial infections, and methicillin resistant staphylococcus aureus (MRSA) and vancomycin resistant enterococci (VRE) infections. Method: A self-administered questionnaire was used to collect data. Data were collected on June, 2003 from 190 nurses in one university affiliated hospital located in Suwon. Result: The mean score for nurses' recognition of general nosocomial infection control was 3.57, MRSA control was 3.54, and VRE control was 3.86. The mean score on practice for control of general nosocomial infection was 3.19, for MRSA control, 3.20, and for VRE control, 3.63. There were statistically significant relationships between the recognition level and practice level for general nosocomial, MRSA, and VRE infection control. According to the general characteristics of the nurses, the mean scores for both recognition and practice were higher for those nurses who had had infection control education, for those who had worked longer in nursing, and for those who worked in the ICU. Conclusion: It is suggested that appropriate hospital infection control programs should be developed through continuous education and practice to improve nurses' level of the practice in general infection control, and especially in MRSA and VRE infection control.

코로나-19 지식수준과 감염관리 피로도 및 스트레스가 감염관리 수행도에 미치는 영향 (Effects of COVID-19 Knowledge Level and Infection Control Fatigue and Stress on Infection Control Performance)

  • This study investigated small and medium-sized hospital workers' knowledge of COVID-19, the infection-control-related fatigue and stress levels, and the factors that impacted their performance during the COVID-19 pandemic. Using a cross-sectional design, data were collected from 137 hospital workers from four small and medium-sized hospitals in S and C cities. Data were analyzed using descriptive statistics, a t-test, analysis of variance(ANOVA), Pearson's correlation coefficient, and multiple linear regression analysis. Small and medium-sized hospital workers' knowledge of COVID-19 revealed a correct answer rate of 66%. An average of 3.00 points out of 4 for infection control performance and 3.32 and 3.17 points out of 5 for infection-control-related fatigue and stress, respectively, were found. The level of knowledge regarding COVID-19 significantly differed by age and marriage (p<0.05), while infection control performance significantly differed by age, marriage, and infection control education experience (p<0.05). Infection-control-related fatigue and stress differed depending on the COVID-19 control education experience, economic and social prejudice, and fear of discrimination. There were positive correlations between knowledge and infection control performance, and infection control performance and infection-control-related fatigue and stress (p<0.001). This study showed that the factors that affected small and medium-sized hospital workers' performance included knowledge of "environmental management," "difficulties due to new role requirements," "presence or absence of infection education experience," "job type," and "age." These results suggest that practical and detailed education programs should be systematically developed and implemented for effective infection control.

  • This study provides an application of datamining approach to CQI(Continuous Quality Improvement) using the discharge summary. First, we found a process variation in hospital infection rate by SPC (Statistical Process Control) technique. Second, importance of factors influencing hospital infection was inferred through the decision tree analysis which is a classification method in data-mining approach. The most important factor was surgery followed by comorbidity and length of operation. Comorbidity was further divided into age and principal diagnosis and the length of operation was further divided into age and chief complaint. 24 rules of hospital infection were generated by the decision tree analysis. Of these, 9 rules with predictive prover greater than 50% were suggested as guidelines for hospital infection control. The optimum range of target group in hospital infection control were Identified through the information gain summary. Association rule, which is another kind of datamining method, was performed to analyze the relationship between principal diagnosis and comorbidity. The confidence score, which measures the decree of association, between urinary tract infection and causal bacillus was the highest, followed by the score between postoperative wound disruption find postoperative wound infection. This study demonstrated how datamining approach could be used to provide information to support prospective surveillance of hospital infection. The datamining technique can also be applied to various areas fur CQI using other hospital databases.

요양병원 간호사의 임파워먼트, 감염관리 조직문화, 감염관리 인지도가 감염관리 수행도에 미치는 영향 (Effects of Empowerment, Infection Control Organizational Culture and Infection Control Awareness on Performance among Nurses in Long-Term Care Hospitals)

  • Purpose: The purpose of this study was to examine the influence of empowerment, infection control organizational culture, and infection control awareness on infection control performance among nurses in long-term care hospitals. Methods: A descriptive survey study was conducted with 125 nurses as subjects who had been working for at least six months in four long-term care hospitals located in Busan metropolitan city and Gyeongsangnam-do Province. Data were collected from September 30 to October 28, 2021 and analyzed using t-test, one-way ANOVA, Pearson's correlation coefficients, and stepwise multiple regression with SPSS/WIN 26.0. Results: The results showed that infection control performance had significant correlations with empowerment (r=.36, p<.001), infection control organizational culture (r=.51, p<.001), and infection control awareness (r=.75, p<.001). Multiple regression analysis for infection control performance revealed that the most powerful predictor was infection control awareness (β=.70, p<.001). Empowerment, infection control awareness and awareness of infection control guidelines explained approximately 60.0% of the variance in infection control performance. Conclusion: Findings indicated that various factors are related to the infection control performance among nurses in long-term care hospital. Based on the results of this study, further development and application of the programs to enhance empowerment and infection control awareness are needed in order to improve the infection control performance of nurses in long-term care hospitals.
